We have repeatedly reported to both the manufacturer and the dealership that there are multiple screws and staples protruding throughout the RV, creating an ongoing safety hazard. ... t portion of our ownership since purchasing it in summer of 2025. On April 11, 2026, during only our second night using the RV, I was walking from the main sitting area down the stairs toward the kitchen/bedroom area when my sock became caught on a protruding screw sticking out of the stair trim/flooring strip. This caused me to lose my balance and fall down the stairs. I landed forcefully on my hands and knees, resulting in pain and visible bruising to my knees and feet, and required medical evaluation. This incident was directly caused by a known and previously reported defect that had not been properly corrected. Given that: these hazards were previously reported/documented, the dealer & manufacturer was aware of the risk, they both knew that I have mobility issues affecting my balance/walking and a family with young children are using it, the defects are widespread throughout the RV, and the condition has now resulted in a personal injury. This represents a ongoing to both myself and my children. The vehicle remains unsafe for normal use. Due to the repeated nature of these defects, prior repair attempts, and now the resulting injury. I am requesting that the manufacturer take appropriate action to ensure safety. This includes a full evaluation of the vehicle and consideration of removal from service, replacement or repurchase if the defects cannot be permanently corrected. I am also requesting that this issue be reviewed for potential broader safety implications.Read more

I am reporting a safety defect on a 2022 Forest River Rockwood Mini Lite travel trailer, model 2516S, This RV is equipped with Dexter axle #1 and #9 from a total batch of #288 axles that Forest River recalled on July 27, 2022 under NHTSA recall 22V-558. The RV was purchased on July 15, 2022, twelve days before the recall. Of the 244 recalled RVs with those axles, only the single-axle tent campers
... were recalled because the defect was visually obvious on those units. On tandem-axle RVs such as this one, the incorrect installation was not visually apparent. This travel trailer was manufactured with brake axle defects and buyers of these units were never contacted. It does not take an engineering analysis to conclude that if axle #1 and #9 from this batch are defective and 244 RVs were recalled, then all 288 axles should have been inspected and included in the recall. The brakes are significantly ineffective in forward motion, causing complete brake failure under normal braking. This is an active safety hazard. There is serious concern that the remaining 44 axles from this batch were placed on tandem-axle RVs and released into commerce without notification, despite the fact that I have not changed my phone number, address, or email since purchasing this trailer. This is a one-owner, low-mileage unit that has never been serviced or altered. Attempts to resolve this issue through Forest River representative dealerships, Dexter Axles, and Forest River have been ongoing for three months with no resolution. No repairs have been made. The trailer is currently being towed over 1,300 miles to Montana for work with defective brakes. Multiple photos have been attached clearly showing the incorrect brake installation. Please investigate this latent manufacturing defect and consider expanding the recall to include these excluded axles.Read more

I own a 2024 Alfa Romeo Tonale. The rear passenger seatbelt became internally jammed and would not retract or function properly. The belt appears to have twisted within the retractor mechanism during normal use. The condition is not user-correctable and requires removal of interior trim and access to the retractor assembly to resolve. This indicates that the seatbelt mechanism allowed the belt to
... become internally tangled and locked. There is no visible damage, fraying, or evidence of misuse. The vehicle is new, and the seatbelt has only been used under normal conditions, including use with a child passenger. The dealership diagnosed the issue as a “twisted/tangled belt” and denied warranty coverage, stating it was caused by external influence, but did not provide evidence of misuse or component damage. This raises a safety concern because a seatbelt should not become internally jammed to the point that it cannot retract or function without disassembly. A failure of the retractor to properly manage belt alignment during normal use could impair occupant protection in the event of a crash. I am reporting this as a potential defect in the seatbelt retractor system.Read more

The vehicle has a recurring P0420 code (“Catalyst System Efficiency Below Threshold Bank 1”). A boroscope inspection at Ricardo Caballero Kia Santa Isabel (invoice #[XXX] dated December 5, 2025) confirmed internal disintegration of the catalytic converter, with physical pieces of the catalyst falling out when the exhaust pipe was removed. This failure appears related to Recall 23V534 (SC276) a

(official Kia distributor in Puerto Rico) has ignored my warranty claim since December 2 and December 22, 2025 (more than four months). Motorambar is now refusing to authorize a courtesy loaner vehicle at the dealer Ricardo Caballero Kia Santa Isabel, claiming the tests will take less than 5 days. This leaves me without transportation. The catalytic converter is fully covered under the Federal Emissions Warranty (8 years / 80,000 miles). I request that NHTSA investigate this as a potential safety and emissions defect and require Kia / Motorambar to: 1. Immediately repair or replace the catalytic converter under warranty at no cost. 2. Provide a courtesy loaner vehicle during the repair. 3. Reimburse previous diagnostic costs of $449.17. The vehicle suddenly stopped and died while driving. The mechanic confirms that the engine is gone, contact Hyundai. Ourisman of Rockville and Hyundai Motor America (HMA) refuses to replace the engine for free. The vehicle has now developed a severe engine knock consistent with the well-documented defect affecting Hyundai’s Theta II engines. Hyundai has publicly acknowledged this issue and i
... ssued a major recall covering approximately 470,000 vehicles with the following specifications: • Model Years: 2011–2012 • Engines: 2.0L and 2.4L • Manufacturing Period: December 11, 2009 – April 12, 2012 Hyundai and federal regulators identified the safety concern as manufacturing debris that could restrict oil flow to rod bearings. This defect can lead to engine knocking and eventual engine failure. Critically, Hyundai has acknowledged that engine failure may stall the vehicle while driving, significantly increasing the risk of a crash. Hyundai’s official remedy for this defect required dealerships to: • Inspect engines for bearing noise • Replace the engine if necessary • Honor Hyundai’s extended engine warranty of 10 years / 120,000 miles for affected vehicles. This issue has also been the subject of significant litigation, including the class action lawsuit Case No. [XXX], filed January 23, 2018, against Hyundai Motor America (HMA), Kia Motors America, Inc. (KMA), Hyundai Motor Company (HMC), and Kia Motors Corporation (KMC). Despite these well-known issues, Ourisman of Rockville failed to disclose any of the following when selling my daughter's this vehicle: 1. The widespread engine defects associated with the 2011 Hyundai Sonata. 2. The recall and safety concerns related to debris restricting oil flow to rod bearings. 3. The extended Hyundai engine warranty covering affected vehicles up to 120,000 miles. This omission is particularly concerning because this defect presents a serious safety hazard and a clear and present danger. Incident date: Oct 17, 2025
I am submitting a follow-up complaint regarding a previously reported front axle weight and capacity issue on this motorhome (original ODI complaint #11398920). At the time of the original complaint, the front axle was already operating at or above its rated capacity under normal conditions, even after removing factory-installed components and redistributing weight rearward. Despite removing appr
... oximately 820 lbs from the vehicle, the front axle weight was only reduced by about 200 lbs, indicating a systemic weight distribution problem rather than a loading issue. Since that time, the issue has escalated into a safety failure. We recently experienced a blowout of the front left tire while operating the vehicle under normal conditions. Based on prior weight measurements and ongoing inability to bring the front axle within safe limits, I believe this failure is directly related to the overloaded front axle condition. This appears to be a known design limitation. Forest River revised subsequent model years of this same unit with increased front axle and tire capacity, suggesting awareness of insufficient capacity in earlier models. Additionally, numerous owners of similar units have reported the same front axle weight issues. The vehicle cannot be operated within safe front axle limits without removing essential functionality (such as fuel or standard equipment), which is not practical or intended use. Given the progression from a documented weight concern to an actual tire failure event, I am requesting that this matter be formally reviewed or reopened as a safety defect investigation. I can provide supporting documentation including CAT scale tickets, prior correspondence, and details of the tire blowout incident upon request.Read more

We received a recall on this 2025 Toyota Sienna hybrid (NHTSA Recall No 25V-668 / Toyota Recall No 25TA12) for the seat rails that may come unwelded in a high-speed accident in December, 2025. When we asked our dealership about it they acknowledged it, but said that there was no fix. Because we regularly take 2-4 passengers besides ourselves in our van, we asked for a replacement vehicle. They off

How hard is it to take the rails out, weld them properly, and put them back in? If the safety of my family was not in jeopardy with this recall, I likely would have ignored it, especially after it's taking so long to get this repair taken care of.Read more

I grab the fire extinguisher and climbed the ladder onto the roof and began put

My 2017 Chevy Volt is having Battery Energy Control Module (BECM) issues. I receive the occasional error message of "propulsion is reduced" and engine fault codes of P1E00, U2619, U2620, U2621, U2622, U2623, and U2624. GM issued a Special Coverage bulletin (N232432680) extending the warranty for the Battery Energy Control Module (BECM) on 2016-2018 Chevy Volts to 15 years/150,000 miles. My Chevy

Volt is within this year and mileage range, but for some reason my Chevy Volt VIN is not included in this Special Coverage (N232432680), which means that this special coverage for some reason doesn't apply to my 2017 Chevy Volt, even though it has the same issue. Is there a way that my 2017 Chevy Volt VIN can be added to this Special Coverage bulletin (N232432680) so that I can get warranty repairs? Thank you.Read more
