My 2014 Ford Focus with a DPS6 dual-clutch transmission began experiencing severe shuddering and harsh shifting. A Ford dealership confirmed the concern and diagnosed DTC P07A3 — Transmission Friction Element A Stuck On — using Ford's prescribed pinpoint test. Recommended repair exceeds $4,000. Vehicle is available for inspection. This is a recurring failure on this specific vehicle. Ford's ow
... n warranty history documents six prior DPS6 repairs between 2016 and 2023, including two full clutch replacements, three TCM/PCM reprogramming sessions, and a full TCM replacement under program 14M02 — all at Ford dealerships. The current failure occurs just 35,000 miles after Ford's most recent transmission repair. Prior repair orders document dangerous safety events including sudden loss of drive while in motion, no-start conditions, and complete loss of throttle control on the highway — consistent with the known DPS6 failure pattern. Ford issued six Customer Satisfaction Programs covering this transmission over eleven years — 14M01, 14M02, 15B22, 19N07, 19N08, and 14M02 Supplement 7 — acknowledging a persistent design defect. Despite this documented repeat failure history on this specific vehicle, Ford has denied goodwill assistance. I am requesting NHTSA investigate Ford's continued denial of repair assistance on vehicles with documented repeat DPS6 failures and the ongoing safety implications of this transmission's known design defects.Read more

This vehicle has had the same recall for longer than a year now that Ford doesn't seem to have any kind of remedy for. They just keep "inspecting" and "reprogramming." How many fires need to happen under the hood of these vehicles before Ford is forced to either replace the fuel injectors or compensate their customers in some way? EGR cooler failed while driving on May 29, 2026. Towed truck to Greenville Chrysler that day. EGR cooler, melted intake manifold and melted wiring harness are being replaced. Service Advisor says "Safety Recall VB1 / NHTSA 19V-757 Diesel EGR Cooler" was accomplished in 2021. Per the Recall, the EGR cooler is replaced with a new part number. Accomplishment of this recall clearly did not e

I have requested that the dealer provide failed parts upon completion of reparis. Myself and others were put at risk when my truck lost all power on IH-30 in traffic before daylight. Dealer has verbally indicated that the egr cooler failed and melted intake manifold and wiring harness. No inspection by anybody except dealer service shop. The vehicle is suffering from a chronic, cascading powertrain and fuel management defect that has resulted in a critical safety hazard: sudden engine stalling. Over the last 17,000 miles, the vehicle has experienced three severe engine/emissions failures that the manufacturer has been unable to permanently resolve. At approximately 30,000 miles (on 29-Jun-2025), the engine suffered a severe Cylin

At approximately 39,000 miles (on 02-Jan-2026), the unresolved fuel issue caused a failure of the Catalytic Converter (Code P0420) and an Evaporative Emission System leak (Code P0456). At 47,000 miles (Current Event), the fuel delivery system failed completely. The engine began running excessively rich (Code P2178), causing the engine idle to drop dangerously low (Code P0506). This defect caused the vehicle to suddenly stall out, resulting in a complete loss of motive power and leaving me stranded. Because the manufacturer's authorized repair facilities have only addressed the symptoms rather than the root cause of the fuel system failure, the vehicle repeatedly loses power and stalls without warning. This poses a severe safety risk to myself and other motorists, especially when operating the vehicle in traffic or at highway speeds.Read more

Vehicle: 2024 Tesla Model Y Date of Incident: XXX Location: Southbound XXX Express Lanes near the [XXX]/[XXX] exit split in [XXX]. At the time of the incident, Tesla Full Self-Driving (FSD) Supervised was engaged. I was traveling in the Express Lanes and intended to remain in the Express Lanes. I was not attempting to take any exit. As the roadway approached a lane split/gore area, the vehicle
... began steering toward the striped boom barriers that prevent drivers from going into oncoming traffic, rather than remaining in the travel lane. There were not impending impact alerts or lane deviation alerts. There were no unusual weather conditions, and I did not observe any road conditions that would have made the lane path unclear to a human driver. As soon as I saw the vehicle veering into the boom barriers, I intervened by taking control of the steering wheel. The system disengagement alert/chime occurred only after I took over. Despite my intervention, the vehicle made contact with the boom barrier assembly. The impact caused significant damage to the windshield and driver's side door frame. Glass entered the passenger compartment. My infant child was in the vehicle at the time. Fortunately, no injuries were sustained. Tesla Dashcam footage of the incident was preserved and is available. A claim was opened with Tesla Insurance, and I was advised to submit a vehicle malfunction report for review of vehicle telemetry and FSD operation during the incident. After the collision I maneuvered the vehicle back into its lane, exited the toll highway and parked at a nearby shopping center to assess our injuries the vehicle. While there, another Tesla parked near me, and the driver had experience the same exact situation while his Tesla was in autodrive. I am requesting investigation into whether the vehicle's driver-assistance system improperly tracked toward the boom barriers rather than remaining within the intended travel lane. Request for Goodwill Assistance – Turbocharger Failure (Special Coverage XXX) Dear General Motors Customer Assistance, I am writing to request a one-time goodwill repair for my 2018 Buick Encore (VIN: [XXX]). Approximately two months after purchasing this vehicle, the check engine light illuminated and diagnostic testing identified code P0299, indicating turbocharger failure. This issue appea
... rs to align with General Motors' Special Coverage Program XXX, issued in February 2024 for certain 2017–2018 Buick Encore vehicles equipped with the 1.4L engine due to known turbocharger failures. Unfortunately, my local GM dealership has informed me that they are unable to perform the repair under the special coverage because the vehicle carries a Pennsylvania reconstructed (R) title. While I understand the vehicle's title status, I respectfully ask GM to consider the unique circumstances. In Pennsylvania, reconstructed vehicles must undergo a rigorous Enhanced Vehicle Inspection process before they can be legally registered and operated on public roads. My vehicle successfully passed all required inspections and is legally certified for highway use. Because this special coverage was issued to address a defect that may lead to significant engine damage and, in some cases, a potential engine compartment fire, I am requesting a one-time goodwill repair or other assistance from General Motors. The concern involves a known condition affecting this vehicle platform and presents a potential safety risk during both local and highway driving. I have been a loyal GM customer and would greatly appreciate your review of this situation. Given that the vehicle has been properly repaired, inspected, and approved for road use under Pennsylvania law, I respectfully request that GM reconsider eligibility for assistance despite the reconstructed title designation. Thank you for your time, consideration, and any assistance you may be able to provide. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(BRead more

While driving down the highway I suddenly heard a loud bang and what looked like smoke came out from the area by the front passenger seatbelt. The seatbelt had suddenly retracted fully and locked. Thankfully the seat was unoccupied at the time, so nobody was injured by this. When calling the local Ford dealership we were told we needed to bring vehicle in for inspection before parts could be order
... ed. When we called back to schedule the inspection we were told the recall was paused so we needed to wait to even have the inspection. After contacting Ford corporate we were finally able to bring in for inspection 06/01/2026 to confirm what we already knew about the broken seatbelt retractor. The dealership relayed this info to Ford, and Ford told them not to fix it at this time. When we called the dealership 06/08 they relayed that info to us. We then requested that Ford provide us a loaner vehicle. The dealership called us back 06/09 to let us know that Ford would not be providing a loaner vehicle. Our front passenger seat is occupied about 95% of the time, and that seatbelt is completely unusable. We have not been given any kind of timeline for when it can be repaired, and have no other option but to drive it the way it is. It is illegal and we are forced to put the life of one of our family members at risk because we do not have any extra seats or the money to buy yet another vehicle or pay for a rental as we are still paying for this vehicle that we bought less than a year ago.Read more

The front light **What component or system failed or malfunctioned, and is it available for inspection upon request?** The front passenger-side (right) headlight failed completely on my Land Rover Defender. The headlight assembly, including the bulb and/or associated electrical components, ceased functioning without any prior indication of a problem. The vehicle is available for inspection upon re
... quest. **How was your safety or the safety of others put at risk?** The sudden loss of a front headlight significantly reduced visibility while driving at night, creating a serious safety hazard for myself, passengers, and other road users. Reduced lighting also diminishes the vehicle's visibility to oncoming traffic, pedestrians, and cyclists, increasing the risk of a collision. **Has the problem been reproduced or confirmed by a dealer or independent service center?** Yes. The failure was confirmed by an authorized Land Rover dealership, which inspected the vehicle and verified that the front passenger-side headlight had failed. **Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others?** The vehicle was inspected by an authorized Land Rover dealership, which confirmed the headlight failure. No inspection has been conducted by police or insurance representatives at this time. **Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear?** There were no warning lamps, dashboard messages, flickering, dimming, or any other symptoms prior to the failure. The headlight failed suddenly and without any advance warning, providing no opportunity to anticipate or respond to the malfunction before it occurred.Read more

"While driving a 2025 Hyundai Ioniq 6 at approximately 10,000 miles, the vehicle experienced a sudden, catastrophic Integrated Charging Control Unit (ICCU) failure with no advance warning. I was holding my breath and have to call AAA road side emergency assistance, only when an external battery is connected I could move the car in "Turtle Mode" to a safe spot. The manufacturer has replaced the ICCU, but the total failure of critical emergency safety and braking indicators poses a fundamental safety defect that needs federal investigation."Read more

When the vehicle is parked outside on hot days, the driver's side seatbelt retraction system locks up and will not release until the vehicle internal temperature drops significantly. I own a 2020 Ford F-150 with the 5.0L V8 engine and have under 60,000 miles on the vehicle. The engine is consuming excessive amounts of oil — approximately 3 to 4 quarts every 5,000 to 6,000 miles under normal driving conditions with no visible leaks or blue smoke. I brought the vehicle to a Ford dealership and opened a formal case with Ford Motor Company. Ford performed an oil consumption tes

I do not find this acceptable for a relatively new engine and the owner’s manual does not indicate that adding oil between changes is normal or expected. This level of oil consumption poses a potential safety risk as inadequate lubrication can lead to premature engine wear and failure. I am requesting that Ford acknowledge this as a defect and provide a remedy under the powertrain warranty. Other F-150 owners have reported the same issue and I believe this warrants a formal investigationRead more
