While driving a 2021 Honda CR-V Hybrid, the vehicle has intermittently and without warning lost the ability to accelerate. When this occurs, depressing the accelerator pedal — even fully to the floor — produces no response, and the vehicle simply coasts and loses speed. This has happened on three occasions — January 13, 2025; December 16, 2025; and June 10, 2026 — including at highway spee
... d. Each occurrence is documented by a Honda dealer service invoice. In each instance, the check engine light began flashing immediately before the loss of power. The only way to restore normal operation was to bring the vehicle to a stop, turn the ignition off, wait, and restart. Restarting restores power but also clears the check engine light and any stored diagnostic trouble codes. Safety risk: Losing motive power without warning, particularly at highway speed or while merging in traffic, creates a serious risk of a rear-end or loss-of-control collision, because the vehicle cannot accelerate out of danger. The affected systems appear to be the power train and hybrid propulsion system, possibly originating from the engine given the flashing check engine light, which typically indicates an active engine misfire. The exact failed component has not been confirmed (UNKNOWN). The problem has NOT been reproduced or confirmed by the dealer during inspection. The vehicle was taken to an authorized Honda dealer for each occurrence, but because restarting the vehicle clears the fault codes, the dealer found no stored codes and could not duplicate the failure during a test drive. The vehicle has not been inspected by the manufacturer, police, or insurance. The vehicle is available for inspection upon request. This matches NHTSA Preliminary Evaluation PE25016 regarding intermittent loss of motive power in MY 2020–2022 Honda CR-V Hybrid vehicles.Read more

... an back to get my baby out of her car seat and to safety. I was at my local YMCA and staff ran out to I assist me and had a fire extinguisher. YMCA called the police and fire department. Both were dispatched to the scene. I called my husband for help while we waited for police & fire to arrive. My husband popped the hood and we saw large blue flames coming out from the transmission or the engine and we notice that transmission fluid started to leak under the vehicle. My husband put the fire out with the fire extinguisher. A police report was made. I contacted the dealership immediately. I contacted my auto insurance agent the next morning. The fire dept. used heat guns to test the heat level in the engine area. Prior to this, I did not have any issues with the vehicle. I believe this to be some type of mechanical or electrical malfunction or failure, based on the white smoke and blue flames. My daughter and I were put at extreme safety risk. If my husband and the YMCA staff did not assist, I feel that we would have lost our lives. With the large amount of smoke rolling into the air, children entering & leaving the YMCA were exposed to smoke/oder. Police had the vehicle towed to a tow lot near by to await Mercedes to pick up the vehicle and tow it to the dealership that I got the car from, so that it can be inspected. That was on 06/01/26. It is now 06/11/26 and Mercedes has not picked up the vehicle from the tow lot as they had confirmed that would be taking care of it.Read more

As I was driving 75 mph down the highway my collision alert flashed all my lights turned off, including my headlights and all interior lights as it was dark time and they flashed back on a couple of times until they finally stayed on. I pulled over and as I came to zero, speed, the vehicle completely shut down. It did start back up, but this is not the first time I get a light on my dash that says
... stability track and then everything resets and I don’t have any lights on at all after that this has been happening off and on I also have three coworkers that have the same make, and year that experienced the same problems ! My situation on the highway was very dangerous. It was pitched dark outside and I lost vision due to my lights going off at 75 mph and my steering wheel also locking up. The next time the vehicle does it I will take a video for proof. I have heard that this problem is very common in these vehicles and believe something should be done because that is a dangerous situation if I was going around a corner and my steering wheel locked up, I could easily flip my car. I have small children that’s why I have the big space but we need safety for our family.Read more

2025 Triumph Speed 400 — Moisture-Triggered Limp Mode and Loss of Turn Signals My 2025 Triumph Speed 400 enters limp mode and displays a check engine light and wrench warning whenever ridden in rain or high humidity. When this occurs, turn signals and hazard lights also stop functioning, leaving me without signaling capability in traffic. Both issues clear only after the bike dries out. The bat
... tery is disconnected and reset has not worked in the past, and continues not to help the issue. At my break-in service (~150 miles ago), I reported the issue. The dealer applied dielectric grease to the speedometer connectors and checked the battery terminals, but the fix was unverified as it didn’t rain until today. On 6/10/26 after riding to work in the rain, the bike entered full limp mode when I tried to leave — power was severely restricted and turn signals and hazards were completely inoperative. The simultaneous loss of power and signaling in traffic is a serious safety hazard with no advance warning to the rider. This is a documented issue across multiple owner forums affecting 2024 and 2025 Speed 400s, and Triumph is aware but has issued no recall or formal remedy. I am requesting NHTSA investigate and compel Triumph to issue a fix.Read more

The steering when turning especially to the left and more so at highway speeds(although I have noticed it just driving and at city speeds making micro adjustments while going straight) stops having a responsive feel and as I come out of turns and start to straighten the steering wheel back up it does not move and then all of sudden feels like it lets loose and moves a few degrees in the direction
... I am turning it aback to and then feels like it catches again. I do notice it even just moderately adjusting my line (but it does not fall or move as many degrees as when actually tuning) and once again more noticeable at highway speeds. This seems to line up with the service bulletins and recalls the later year models. I believe it is the same problem but due to the model year they would rather distance themselves from the responsibility. I have been noticing something with it for about a week before the date listed but I wanted to make sure and it has actually started showing up more and worse. I am afraid this is going to cause a major issue.Read more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? The vehicle experienced a failure related to the steering wheel assembly that caused the "Travel Assist Not Currently Available" warning to appear while driving. The issue affected the operation of the vehicle's driver assistance systems, including Adaptive Cruise Control and Travel Assist functi
... ons. An authorized Volkswagen dealership diagnosed the vehicle and confirmed that the steering wheel assembly requires replacement. The vehicle is currently available for inspection. 2. How was your safety or the safety of others put at risk? The failure caused the vehicle's driver assistance and safety systems to become unavailable while the vehicle was being operated on public roadways. The unexpected loss of Adaptive Cruise Control and Travel Assist functionality could increase driver workload and create a potential safety risk, particularly during highway driving where these systems are designed to assist with vehicle control and spacing from other traffic. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. An authorized Volkswagen dealership inspected the vehicle, retrieved diagnostic fault codes, and confirmed that the steering wheel assembly is defective and requires replacement. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has been inspected by an authorized Volkswagen dealership. The dealership diagnosed the fault and determined that the steering wheel assembly requires replacement. The dealership has submitted a goodwill assistance request to Volkswagen because the issue first occurred before the vehicle exceeded the warranty mileage limit. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? NHTSA COMPLAINT — SHORT VERSION I am reporting a persistent, unresolved, safety-related steering defect in my 2025 Hyundai Ioniq 6 (leased 10/12/2024). Beginning in September 2025 at approximately 15,412 miles, the vehicle exhibits: (1) uncontrolled steering wheel shifting left and right at highway speeds; (2) hard pulling to the left when accelerating or decelerating; and (3) the vehicle swayin
... g or jerking within its lane without driver input. I described the issue to the dealer as "pretty dangerous to drive" — this language is documented in Repair Order #XXX. Despite 6+ documented repair attempts over 9 service visits from September 2025 through May 2026, the defect remains unresolved. Repairs attempted include: two 4-wheel alignments, road-force balancing, steering angle sensor calibration, front camera calibration, new front view camera installation, and a complete steering gear box (R-MDPS) replacement. The vehicle was out of service for 129 documented days. CRITICAL ADMISSION: Dealer Repair Order #XXX(11/5/2025) explicitly states that Hyundai's Technical Service Bulletin procedures for this steering defect only apply at speeds of 40 MPH and below, and that "above that speed there are too many variables." The defect occurs at highway speeds above 40 MPH. This is a written admission that no manufacturer-approved repair procedure exists for the defect as experienced. Hyundai Techline Case #XXX was opened in January 2026. After Techline involvement and new camera installation, the technician's own repair order states "concern still present" after road testing. Hyundai Motor America corporate case #XXX was opened, and Hyundai has now stated they cannot process a buyback or lemon case. Without in-vehicle connectivity, automated crash alerts / location information is unavailable. There's NO indication there's a problem unless certain information screens are visible, and only then, the indication is the lack of any connectivity signal level bars (a missing very small and dim icon) Has the problem been reproduced or confirmed by a dealer or independent service center? Yes via live phone assistance with Hyundai bluelink support. They suggested the manual reset and observed the re-establishment of connectivity. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not yet. I had the car in for a recall, but the service department said there was nothing they could do until Bluelink provided a fix. There's no fix announced. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? My vehicle previously received Safety Recall 20V-437 related to the sliding door handle cable assembly. After the recall repair was completed, the driver's side power sliding door has again begun exhibiting abnormal behavior. The driver's side sliding door does not consistently open fully and has failed to close properly on more than 20 occasions. The problem is reproducible and has become increa
... singly frequent. On multiple occasions, the power sliding door has also begun closing while children were entering the vehicle, requiring intervention to prevent contact. This condition creates a safety concern because occupants may believe the door has opened or closed properly when it has not. It also creates a risk of injury when the door unexpectedly changes direction or begins closing during entry or exit. The affected component appears to be the driver's side power sliding door system and related latching/closing mechanism. The vehicle has not yet been inspected for this recurrence because Honda advised that a dealer diagnosis is required to determine whether the current condition is related to the prior recall repair. There were no warning lights or dashboard messages associated with the condition. Because this vehicle previously received a safety recall repair addressing sliding door operation and latching concerns, I am reporting the recurrence of similar symptoms for review.Read more

The vehicle was subsequently inspected by another authorized dealer, which was unable to identify a definitive cause of the recurring condition. The check engine light was cleared, but it returned less than a week later with the same symptoms. The failure creates a safety concern because the vehicle may enter reduced-power or limp-home operating modes without warning, potentially limiting acceleration and the ability to maintain traffic speed. This is especially concerning when merging onto highways, crossing intersections, towing loads, or operating in heavy traffic. The recurring nature of the failure has reduced confidence in the reliability and safe operation of the vehicle. The condition has been reproduced multiple times and has been documented by authorized service facilities. Warning lamps and fault codes have repeatedly reappeared following repairs. I am reporting a potential premature transmission failure in my 2019 Toyota Highlander AWD. At approximately 89,000 miles, the vehicle developed a noticeable and persistent whining noise coming from the transmission/drivetrain area. The noise continued despite transmission service and has progressed to the point where the vehicle requires a transmission replacement. I have had the vehicle evaluat
... ed by three separate repair facilities, including transmission specialists. All three independently determined that the transmission has failed and requires replacement. Repair estimates range from approximately $7,200 to $12,000. A complete transmission failure at approximately 89,000 miles is unexpected and raises concerns about a possible design or manufacturing defect. A transmission failure while driving could cause loss of power, inability to accelerate, reduced control of the vehicle, or create a dangerous situation while merging, entering traffic, or traveling at highway speeds. The vehicle has been maintained and used normally. I am submitting this complaint because I believe this issue may affect other Toyota Highlander owners and should be investigated for a potential safety-related defect.Read more

I am trying to get the fire department to amend report to record stated belief of fire origin (vehicle) which is not today evidenced in the report and as per the stated opinion by one of the fire fighters. I am further insisting that the insurance company assign a forensic expert in evaluating the origin and cause of the fire. Nissan has not been notified to date. I am imploring NHTSA to take this apparent prior documented safety issue further beyond what has been concluded in order to prevent any future safety issues should it so be judged by NHTSA as the authority in these matters.Read more

I was driving on the freeway at a high rate of speed (65-70mph) and a piece of my roof rack broke off my car and flew off into the road. This could have hit a car behind us or harmed a person inside the car. Nothing hit my car to cause it to come off. I showed it to the dealer and they claimed that pieces of cars don't just fall off for no reason. They said they would file a claim for it to be cov

Factory Ford 20” wheels with Factory Installed Michelin 275/65 R20 LTX A/T2 tires. Tires have just under 30,000 miles on them and still have more than 50% tread. Tread wear is even. Tires have been rotated. Truck has 73,000 miles but had different wheels and tires for first 43,000 miles. I stored these wheels and tires inside climate controlled building. When my aftermarket tires wore out, I put
... these back on. They had 10 miles on them when I took them off. I have had them on since April 2022. On Friday June 5th, 2026 I was traveling about 60 MPH and had a catastrophic blow-out of the passenger side rear tire. The tires blew completely apart and the wheel hit the ground and the back side of the wheel broke off, before I could get over to the side of the road the tire beat the bed and running board to pieces and even damaged the exhaust and bumper. The wheel was ruined. Thousands of dollars in damage. All the tires are in good condition, not dry rotted, no cracks, the truck has been indoors in a garage. They hold air. TMPS works and I check my pressure all the time. I had just set the air pressure early that morning before leaving. The tires are less than 10 years old which is the date for replacement. They have more than half their tread. No cracks. They hold pressure perfectly. They are worn evenly. No reason for the tire to have blown out other than a manufacturing defect. When I got stopped there was a huge hole in tire and no tread on it at all.Read more

While driving at normal speeds. Car completely stopped working. Dashboard flashed to get to safe area. I could not shift the car neutral. Steering became stiff. The lights and infotainment was still on but the car had no power to accelerate. Had this been on the highway I would have been in a collision since my car went to the right very quick. Once car was stopped could not shift to neutral.
