The Incident: While operating the vehicle under high load, the driver received an explicit "Service Engine Soon" powertrain malfunction alert stating: "The powertrain control module lost communication with the vehicle active grill shutter module for a period of time." Immediately following this warning, the engine experienced a severe loss of power, followed by loud metallic banging from the engin
... e bay. The vehicle stalled completely and lost all motive power, creating an imminent highway safety hazard. The Physical Damage: Subsequent mechanical teardown by the dealership revealed terminal block destruction, including deep vertical cylinder wall scoring, heavy unburnt liquid gasoline residue in the engine oil pan, and a blown head gasket.Relationship to Active Safety Defect: This catastrophic engine failure mirrors the progressive failure profile of NHTSA Campaign 25V455 / Ford Safety Recall 25S75 (Low-Pressure Fuel Pump Failure). In this EcoBoost architecture, the Active Grill Shutter (AGS) module shares critical voltage paths, grounding blocks, and wiring looms with the engine management and fuel delivery systems. The sudden network communication loss is diagnostic proof of a severe voltage drop or electrical short caused by the high-friction fuel pump degradation detailed in Recall 25S75. This circuit failure instantly starved the engine of fuel under turbo boost, inducing an extreme lean-out condition. The resulting pre-ignition forces violently shattered the internal piston rings (the loud banging), which then gouged the cylinder walls, wiped out compression, fouled the crankcase with fuel residue, and blew the head gasket.Request for Action: Ford's current VIN parameters for Recall 25S75 have not yet captured my specific vehicle despite it matching the exact production window, component setup, and catastrophic failure sequence. I am requesting that the NHTSA log this case as definitive evidence of consequential engine destruction originating from the 25V455 fueRead more

As is clearly a very common defect, my exhaust manifold bolts broke and the manifold warped. This is the second time this has happened. RAM knows about this problem. In fact, it is so well known that it is called "the HEMI tick". The fact that such a shoddy part has continued to be put on millions of trucks and other vehicles should warrant a recall.

My new vehicle, with under 4,000 miles, has already suffered multiple chewed wires due to rodents attracted to the soy-based wire coatings used by the manufacturer. This is not a minor inconvenience — it is a serious safety issue. Damaged wiring can cause loss of power steering mid-drive, ABS and braking system malfunctions, airbag deployment failures, engine stalling on highways, and electrical
... fires — any of which can result in injury or death. The manufacturer knows soy-based coatings attract rodents, the consequences are safety-critical, and proven mitigations exist (capsaicin-infused coatings, hardened sheathing, metal conduit) yet are not implemented from the factory. If vehicles are engineered to withstand foreseeable environmental hazards like water intrusion, heat, and road debris, there is no justification for leaving critical wiring defenseless against a well-documented and predictable threat. A material choice known to invite damage, paired with insufficient physical barriers, is not an external cause — it is a design flaw.Read more

While operating the vehicle under load, the vehicle suffered a sudden and severe loss of engine performance, followed immediately by loud metallic banging from the engine bay. The vehicle lost all power and was immediately disabled (LIMP MODE), presenting an imminent highway safety hazard. The Physical Damage: Subsequent mechanical teardown revealed deep vertical cylinder wall scoring, heavy raw fu
... el residue in the engine oil pan, and a blown head gasket. Relationship to Active Safety Defect: This catastrophic block failure perfectly mirrors the progressive failure profile of NHTSA Campaign 25V455 / Ford Safety Recall 25S75 (Low-Pressure Fuel Pump Failure). Internal component degradation in the fuel tank delivery module dropped fuel pressure to the engine. Under turbo boost, this starvation induced an extreme, instantaneous lean-out condition. The resulting low-speed pre-ignition (LSPI) exploded and shattered the internal piston rings (the metallic banging), leading to mechanical wall gouging, unburnt fuel wash into the lower crankcase, and a secondary blown head gasket. Request for Action: Ford's current VIN parameters for Recall 25S75 have not yet captured my specific vehicle despite it matching the exact production window, component setup, and catastrophic failure sequence. I am requesting that the NHTSA log this case as clear evidence of consequential engine destruction originating from the 25V455 pump defect, and evaluate whether Ford's recall scope must be expanded to protect other consumers from highway engine failures.Read more

On Friday, June 12, 2026, the check engine light illuminated on my vehicle. I took it to my local Ford dealer on Saturday, June 13, 2026. The dealer diagnosed the issue as a defective Gear Shift Module (GSM) and quoted approximately $1,000 for replacement. The service advisor informed me that failure to repair this component could result in the vehicle becoming unable to shift into Park or unable
... to shift out of Park. This defect poses a serious safety risk, as it could prevent the truck from being properly secured when parked, potentially allowing it to roll away, into traffic, or cause injury to persons or property. Ford is aware of this issue, as a Technical Service Bulletin (TSB) has been issued for it. However, the repair is currently being treated as a customer-pay item rather than a warranty or safety-related repair. This condition represents a significant safety concern that could affect many other 2024 F-150 owners. I urge NHTSA to investigate this matter promptly to determine if a recall or mandatory safety fix is warranted. I am available to provide additional details, including the dealership repair order, diagnostic information, and any supporting documentation. Thank you for your attention to this important vehicle safety issue.Read more

At no point did any warning lights illuminate or any indication appeared on the dashboard to alert me of a problem before these dangerous events occurred. The truck was taken to a Ford dealership where it was diagnosed with an electrical fault in the transmission solenoids. The final diagnosis is a full Transmission R&I and overhaul at only 44,000 miles. This issue is covered under Ford TSB 21-2315 which acknowledges that the 10-speed automatic transmission may exhibit harsh engagement, delayed shifts, and shifting problems due to known defects. My 5yr/60,000-mile powertrain warranty expired 3 months ago by time only. I am well under the mileage limit at 44,000 miles. Ford has agreed to cover only $2,097.06 of the $5,242.65 repair, leaving me responsible for $3,145.59 for a known manufacturing defect. When I contacted Ford requesting full goodwill warranty coverage, I received poor customer service a manger (Luis) promised to call back multiple times with additional coverage options and never did. A full transmission overhaul at 44,000 miles is not consistent normal wear and tear and is clearly the result of a manufacturing defect that Ford has already acknowledged through TSB 21-2315. The sudden and unexpected nature of these failures with no warning, combined with the serious safety risk posed to myself and my child, makes this matter extremely urgent. After installing the recent software update (version 5.0.10) that was intended to fix the rear camera not appearing when shifting into reverse, my vehicle has continued to experience the same problem, now with significantly increased frequency. Nearly every day--sometimes multiple times a day--the infotainment system/center screen will fail to boot up before shifting into reverse and the camera vi

Occasionally, the center screen will remain black, with no indication that the system is booting up, for up to 30 seconds after starting the vehicle, even though the screen should wake up immediately upon starting. The issue first occurred the day after installing the update (April 10, 2026), but has recurred nearly every single day since then, up until and including the day I submitted this report. The issue has been exacerbated by the increased summer heat in Arizona, leading me to believe the computer--besides lacking required processing power & memory--is not adequately cooled. The earliest available appointment to have the issue checked out was July 14, 2026, so the vehicle has not yet been inspected. No warning lamps or related symptoms. My 2017 Chrysler Pacifica Hybrid entered limp mode and was taken to Central Florida Chrysler Dodge on XXX. The service advisor, [XXX], identified three recalls, which needed to be completed, after claiming they had completed them, the issue persisted. I authorized a coolant flush without being informed it involved the battery cooling system. Further diagnostics revealed a failed hybrid battery.
... However, Chrysler denied the battery replacement claim, citing contamination from “incorrect” coolant. I dispute this reasoning, as I was not properly informed about the coolant issue and believe there is no proof that it caused the battery failure. Under the Magnuson-Moss Warranty Act, a warranty or recall claim cannot be denied without clear evidence linking the fluid to the failure. I have opened a case with Chrysler (Case #XXX) and am requesting that the battery replacement be honored under the recall, especially since recall guidelines indicate that unresolved issues after software updates may require component replacement. After a days of back and forth, not the dealership claims the PIM is not the original part of the vehicle and for that reason it would be covered under warranty. However the law is clear and Chrysler is obligated to replace it since this part is also part of the recall they claimed was completed. Now the dealer wants US$ 5K to replace the PIM. My safety, as well as the safety of my children and family, is under constant, severe threat. Furthermore, the manufacturer has failed to provide a comparable, safe, or adequ
... ate utility rental vehicle to mitigate our loss of use while we wait indefinitely for these remedies. There were no warning lights or prior symptoms before the recall. This vehicle is actively subject to three major, concurrent manufacturer safety recalls carrying critical safety risks: NHTSA Campaign 25V-741 (Stellantis 68C) for high-voltage hybrid battery fire risk; NHTSA Campaign 25V-766 (Stellantis 78C) for engine casting sand contamination causing catastrophic engine failure; and the recently issued June 04, 2026 recall, NHTSA Campaign 26V-363 (FCA 21D), for an Electro-Hydraulic Power Steering Pump (EHPSP) electrical short circuit and fire risk. The combined safety risk of these defects is severe, including spontaneous vehicle fire while driving or parked, and sudden, catastrophic engine seizure leading to an unrecoverable loss of propulsion at highway speeds. The vehicle is currently available for inspection upon request and has been evaluated by an authorized manufacturer dealership. The dealer confirmed that the safety recalls apply to this specific vehicle. Out of an abundance of caution, the manufacturer has issued strict, overlapping instructions for both the battery and the EHPSP defects, advising owners to park the vehicle outside and away from structures or other vehicles until a remedy is obtained. Furthermore, for the engine sand contamination defect, the manufacturer has explicitly stated that a definitive repair remedy is not yet available, leaving the vehicle with a permanent risk of sudden mechanical failure or an engine-compartment thermal event.Read more

At the end of the ramp, I noticed I was unable to get the truck to accelerate from a stopped position either. Only then did I notice the dial t
... ransmission had moved to N from D. I did not initiate this move while I was on the highway, nor was I aware of it, nor do I believe I bumped the dial. This is the second time this has happened. Previously it occurred some time in late 2025 in a store parking lot. At highway speeds, this is extremely dangerous.Read more

Vehicle went to turtle mode 3 separate times at highway speeds forcing a stop. Each time it was towed to the dealer. First two times little was done per the report and was returned. On the third time it has spent 7 weeks for a repair on the transmission not a replacement. The dealer also needed to order a replacement exhaust clamp that took 5 plus days to receive and install. Service departme

The driver-side front door and related body fit/sealing system appear to be defective. The door is visibly uneven and misaligned with the surrounding body panels, resulting in excessive door gaps and significant air intrusion into the cabin while driving. The condition is available for inspection upon request, and photographs and videos documenting the issue are available. The excessive air intrus
... ion creates substantial wind noise and driver distraction while operating the vehicle at highway speeds. The misaligned door also raises concerns regarding proper door sealing, structural fit, weather protection, and occupant safety in the event of an accident. A vehicle of this age and condition should not exhibit such defects. The condition is consistently reproducible during normal driving, particularly at highway speeds where excessive air enters the cabin through the driver-side door area. (If applicable, add: "The vehicle has been presented to an authorized Chevrolet dealer for inspection.in FH dailey chevrolet San leandro, they replaced weather strip, but air is more then what it was before, they declined my service and told me to take the vehicle where I purchased from in epic chevrolet Sacramento. No vehicle is not inspected by anyone. No warning lamps or diagnostic messages were displayed. The primary symptoms were visibly uneven door alignment, inconsistent door gap measurements, and noticeable air intrusion and wind noise while driving. These symptoms were first observed shortly after taking delivery of the vehicle and have persisted since that time.Read more

When checking the oil, realized that there was no antifreeze in the reservoir. I went ahead and filled it to the max line, as the antifreeze didn't even come up to the minimum line of the reservoir tank. My gut was telling me that I need to every so often check the antifreeze, there was no apparent leak, there was nothing on my garage floor to indicate that there was a leak. Every time I checked
... the antifreeze, it was below the minimum line. I had also been noticing that my car was shifting hard. the rpms were randomly jumping higher than normal when I was maintaining a normal speed. When I go to start my car, the whole car shakes as though the engine is misfiring. When I took the car in to a mechanic, they looked it over and I was told that I needed to replace the long engine block due to a coolant intrusion into the cylinder. The car will sometimes show the check engine light and sometimes it just goes off. I had the check engine light come up before and was told after having it hooked up at Auto Zone that I just needed a tune up.Read more
