My vehicle has been in the shop 7 times for misfire in all cylinders over a 2 1/2 year span Kia dealer refused to replace the engine, 8 months after I threatened to sue the dealership for saying they fixed the misfire in all cylinders issue and 8 months later I’m getting misfire in 4th cylinder. As the engine light came on again and I have a high end car diagnostic tool attached to the car whic
... h is giving me that code and previous codes of misfire in all cylinders. First misfire in all cylinders started at 16,000 miles bought car brand new and it continued all the way to 37,067 miles . The Kia dealership keep saying it’s this or that and it’s been fixed but the lights come on the previous 7 times after a week or less after taking it home from the dealer and then saying it’s fixed last time I wanted a new engine a Kia representative said it doesn’t need a new engine. I talked to a technician there about reading about this issue in alot of same vehicles he said I was right but Kia doesn’t want to pay for a new engine so they give excuses or just reset or clear the codes which doesn’t resolve the problem , Kia has changed the fuel tank, changed the spark plug wires, the spark plug cables the assembly, ecm which never resolved the problemRead more

2022 Honda Accord Infotainment “Ghost Touch” Defect I am reporting a recurring safety defect involving the infotainment touchscreen in my 2022 Honda Accord. The touchscreen intermittently registers false inputs (“ghost touches”) without any user interaction. During operation, the screen randomly changes radio stations, switches applications, exits navigation, opens menus, changes setting
... s, and becomes unresponsive. The issue occurs while driving and forces the driver to divert attention from the roadway in an effort to regain control of the system. This defect creates a significant distraction and increases the risk of a crash, particularly when navigation is being used in unfamiliar areas. In some instances, the screen becomes completely unusable for extended periods. My understanding is that many other 2021–2022 Honda Accord owners have reported identical symptoms, including screen replacements that failed to permanently resolve the problem. Numerous public complaints describe the same malfunction and indicate that the issue may be widespread. I believe this defect warrants investigation because it affects a driver interface that is relied upon during vehicle operation. Honda has not provided a permanent repair solution, and affected owners continue to experience recurring failures across multiple Honda models and trims. I respectfully request that NHTSA investigate this defect and determine whether a recall or other corrective action is warranted. Vehicle: 2022 Honda AccordRead more

I had a flat tire. When I recently bought the Hyundai Santa Fe Hybrid, I did not think to ask about a spare tire, they have been standard on every car I bought in the past. The vehicle was serviced for NHTSA Recall 18V735 (Canister Purge Valve/Fuel Tank stalling hazard). Immediately upon pickup from the dealership, the vehicle began severely stumbling and stalling while driving on the highway. It died while driving on the street. The dealership's diagnostic paperwork confirmed an 'O2 sensor logic error causing the vehicle to stall' right after they performed the mand

The dealership is refusing to verify if their technician damaged the adjacent wiring harness or glitched the software flash, leaving the vehicle an active highway stalling hazard. As for my experience, The car died in Clear Lake by Baybrook Mall. Extremely high traffic, almost got hit. (Yelled at by several drivers). Constant stalling happened in Baytown. Scariest incident was going over the very tall Fred Hartman bridge. My 10 year old daughter and I felt sure it was about to die and we were going to roll backward down the bridge becauseit kept dropping power. XXX - approx 9:00pm - departed Copper and Sage restaurant enroute to the WigWam. After about 1/2 mile at 30-35mph, I slowed down to make a right turn. After completing the turn I pressed the accelerator to continue and the Palisade had absolutely no power. I pressed the pedal to the floor with no success. I was finally forced to pull over and stop the vehicle and place it in park. After doing so,
... the Palisade was able to return to operation with the pedal and for fear of another incident we drove home. 2nd Incident - 2 Mar 2026 - approx 1200pm. We were traveling west on XXX from [XXX] to [XXX] and began the ascent through the mountains going west towards XXX. After experiencing a steady grade upward approximately 6-7% at speed of 55-60mph, I used the hand shift paddle to downshift one gear to assist in the upward climb. Upon doing so, the Palisade once again died with no power at all. I pushed the accelerator pedal to the floor with no success of power. This incident could have been devastating as we were in the far left lane with many other vehicles behind us. I had no place to pull over to the left so I began to make my way to the right shoulder over 3 lanes very slowly under only previous velocity motion. We narrowly avoided being hit from behind numerous times and finally made it over to the right shoulder. After finally stopping the Palisade, I put the vehicle in park and it returned to operation with the pedal once again. We then carefully pulled out off the shoulder and the Palisade operated normally until we arrived at our destination. Check engine light solid/engine failure while driving. 2018 Sonata SE purchased new in fall 2018. Around 50,000 miles started noticing oil level drop and had to add more before next scheduled change. Dealership service center said I just had to shorten the mileage between changes which didn't seem appropriate for a vehicle with 50,000 miles but I did. Continued to have oil consumption problem with
... no signs of leak, no burning oil smell or exhaust issues. Dashboard display would go off and when I immediately checked dipstick there was barely any oil so I'd add more then go in for full change. At 67,000 or so miles in June 2026, without oil display going off the engine light was now solid. Got to reputable service center near my home and oil was under a quart left yet the warning lamp hadn't gone off. They ran the engine error code P0304 and was misfire #4 cylinder and after compression test, #3 was very low and #4 very high. NOT drivable at that point then towed to dealership so yes, available for inspection by NHTSA at the dealership. For now they claim a 'tune up' is needed for spark plug replacement, valve cover gasket replace and fuel induction service that's my cost to incur which seems to just be fixing the damage of an engine oil consumption problem not fixing the issue that caused the damage. The engine seized/stalled without warning while I was driving the vehicle (granted at low speed in my neighborhood when the engine light turned on). I would not say the engine warning lamp occurred prior to the engine fail, it happed at the same time. This should not happen for a vehicle with my mileage and regular maintenance. I looked up NHTSA for potential recall or other helpful information and started to see extremely similar scenarios to mine and wanted to add complaint as well. This is getting very expensive outside of whatever cost the dealer wants as a fix with rental car needed and tow services. Thank you for your consideration of this engine safety issue.Read more

The contact owns a 2016 Tesla Model S. The contact stated that approximately six days after purchasing the vehicle, while driving approximately 45 MPH, the vehicle drove over a bump in the road, and a grinding sound was heard coming from the vehicle. The contact stated that after driving over a bump in the road, the vehicle felt different while driving. The vehicle was taken to an independent mech

The independent mechanic advised the contact not to drive the vehicle due to safety concerns and indicated there might be structural damage to the vehicle. The contact stated that after the suggestion, the contact continued operating the vehicle. The following day, the vehicle was taken to the dealer where the vehicle was purchased, and the contact was advised that the failure was associated with worn brake pads. The dealer replaced the brake pads. The contact believed that additional repairs may have been performed but was not advised of any other work completed. The vehicle had not been modified and had not been inspected by Tesla. The contact believed that the vehicle may have an inaccurate odometer reading; however, no evidence was available to support the concern. The contact remained concerned that the corrosion and possible structural damage might exist within the suspension system even though the brake pad was replaced. The failure mileage was approximately 108,000.Read more

I am writing to file a formal safety complaint regarding the extreme delay in the recall remedy for my vehicle. Despite the recall being issued months ago, Ford Motor Company/Lincoln has failed to resolve this critical safety issue in a timely manner. Here are my vehicle details: Year/Make/Model: 2016 Lincoln MKX VIN [XXX] NHTSA Recall Number: 25V544 Manufacturer Recall Number: 25S87 Compone
... nt Affected: Rear Flexible Brake Hoses Description of the Safety Concern: The recall notice explicitly states that a failure in the rear flexible brake hoses can lead to a loss of brake fluid, resulting in increased brake pedal travel and significantly increased stopping distances, which greatly elevates the risk of a crash. Recently, I have started experiencing a noticeable softening in the brake pedal ("increased pedal travel"), which makes driving the vehicle extremely hazardous. When I contacted my local Lincoln dealership, I was informed that they do not have the necessary parts in stock and could not provide a definitive timeline for the repair. As a consumer, I am being forced to drive a vehicle with a known, severe braking defect because the manufacturer has failed to supply the remedy parts. This is a direct threat to my life, my family’s safety, and the safety of others on the road. Furthermore, this prolonged delay is causing me significant financial hardship and stress. I urgently request NHTSA to intervene and hold Ford Motor Company accountable for expediting the parts distribution for this critical safety recall. My 2016 Kia Optima equipped with the 2.4L DOHC I4 GDI engine began to experience progressive and severe oil consumption during the latter part of my ownership. The oil warning light began activating repeatedly, even after shortly getting the oil changed. Several certified mechanics and oil change service stations told me this was an ongoing issue with this car and engine and that there was nothin
... g I could do but monitor it and keep it filled. At their instruction, I monitored and had it changed more frequently, often topping it off at the oil change station and at home when needed. Approximately 2 weeks after an oil change upon putting the car in reverse it started to shake uncontrollably. The car was towed to a mechanic who stated I needed an engine. I then towed it to an authorized Kia franchise dealership where the technician documented the oil consumption complaint, confirmed a Cylinder 2 misfire code, performed a borescope inspection, and found the Cylinder 2 exhaust valve burnt with a chunk missing. Engine replacement was recommended by the Kia dealership technician. Prior to my purchase, the engine had already been replaced at approximately 80,000 miles under recall-related circumstances. The replacement engine installed by a Kia dealership subsequently failed, exhibiting symptoms consistent with the documented widespread defect affecting the 2.4L DOHC I4 GDI engine in 2016 Kia Optima vehicles, including severe oil consumption, cylinder misfire, and catastrophic internal engine damage confirmed via borescope at a Kia franchise dealership. Kia America denied the claim based solely on mileage despite the damage being confirmed by their franchise technician. My vehicle became unsafe to drive due to this engine instability shortly after starting before becoming fully inoperable. My vehicle has been non-operational for approximately 5 months. Kia has not provided a substantive written determination despite a formal written demand submitted in March 2026.Read more

... ng the same engine family. Kia has since backtracked on prior findings, and both Kia and the dealership have stopped responding to my requests for clarification and documentation. The cylinder damage and related engine failures appear to be symptoms of a larger engine defect that poses a safety risk due to sudden loss of power and stalling while driving. I request that NHTSA investigate these recurring engine failures and Kia’s handling of these claims.Read more

Fuel system leak. Strong odor of gasoline fuel both around and inside the vehicle. No noticeable fuel product on the ground or underneath the vehicle. I am greatly concerned that this is a very significant fire and vehicle operator/occupant safety hazard. I have also found many other C6 generation Corvette owners online that are experiencing the same thing to their C6 Corvette.

Its becoming more frequent and a huge safety issue. When you're driving the car, it feels like the emergency parking brake automatically engages. And when you push the brake pedal, it almost feels like it's dead, meaning there's no pressure back against your foot, which is really unusual for this vehicle, period. You don't necessarily lose control of the car, but you certainly don't have complete
... control of the braking system, period. Then you are in a period of time to get off the road safely and turn the vehicle off and restart it. Then the system seems to correct itself, and you can drive away. But I'm concerned about this from a safety perspective,Read more

My SRS light being on continuously is a MAJOR Safety concern because it disables the airbags--so in the event of an accident, the airbags will not deploy. I contacted Honda Corporate and they created a case--Case number 16432964 for this issue--but they told me it would be possibly months to get anything done and I would still have to pay out of pocket first. My 2018 Volvo XC90 T8 Plug-In Hybrid experienced a catastrophic, premature component failure of the Electric Rear Axle Drive (ERAD) propulsion system while driving. The ERAD unit suffered a total failure of internal bearings and sensors, completely disabling the electric rear wheel propulsion system and suddenly forcing the vehicle into a reduced-power "limp mode" on the road. This sudden loss of
... hybrid propulsion creates an immediate highway safety hazard, removing expected acceleration and vehicle control in traffic. Volvo has documented this exact engineering flaw internally under Technical Journal TJ 36665, recognizing a manufacturing vulnerability in these specific hybrid components. However, the manufacturer is refusing to provide remedy or financial cost-sharing to repair this vital propulsion and safety defect, leaving a hazardous, compromised vehicle on the road.Read more

The Incident: While operating the vehicle under high load, the driver received an explicit "Service Engine Soon" powertrain malfunction alert stating: "The powertrain control module lost communication with the vehicle active grill shutter module for a period of time." Immediately following this warning, the engine experienced a severe loss of power, followed by loud metallic banging from the engin
... e bay. The vehicle stalled completely and lost all motive power, creating an imminent highway safety hazard. The Physical Damage: Subsequent mechanical teardown by the dealership revealed terminal block destruction, including deep vertical cylinder wall scoring, heavy unburnt liquid gasoline residue in the engine oil pan, and a blown head gasket.Relationship to Active Safety Defect: This catastrophic engine failure mirrors the progressive failure profile of NHTSA Campaign 25V455 / Ford Safety Recall 25S75 (Low-Pressure Fuel Pump Failure). In this EcoBoost architecture, the Active Grill Shutter (AGS) module shares critical voltage paths, grounding blocks, and wiring looms with the engine management and fuel delivery systems. The sudden network communication loss is diagnostic proof of a severe voltage drop or electrical short caused by the high-friction fuel pump degradation detailed in Recall 25S75. This circuit failure instantly starved the engine of fuel under turbo boost, inducing an extreme lean-out condition. The resulting pre-ignition forces violently shattered the internal piston rings (the loud banging), which then gouged the cylinder walls, wiped out compression, fouled the crankcase with fuel residue, and blew the head gasket.Request for Action: Ford's current VIN parameters for Recall 25S75 have not yet captured my specific vehicle despite it matching the exact production window, component setup, and catastrophic failure sequence. I am requesting that the NHTSA log this case as definitive evidence of consequential engine destruction originating from the 25V455 fueRead more
