This is a follow-up to NHTSA Complaint #11744977 regarding the Occupant Detection System on a 2024 Subaru Outback. The passenger airbag indicator showed "off" twice, on separate drives, with two different adult passengers seated normally during regular highway driving. Shifting the seat made the light come back on both times. Since filing the original complaint, the dealer kept the vehicle for mul
... tiple weeks, opened a national technical case with Subaru, and performed a "zero-point calibration" of the ODS sensor. Subaru of America opened Case #260618-1500443. A Subaru of America specialist then told us a technician took one test drive, could not reproduce the issue, found no mechanical problem, and concluded the vehicle is "operating as designed." They suggested the original incidents may have been related to "passenger positioning," but did not identify or test any specific position. Subaru has closed the case on this basis. We do not believe a single test drive that failed to reproduce an intermittent fault proves there is no defect. The known failure mechanism in related Subaru ODS recalls — degraded sensor connections and moisture-related circuit damage — is intermittent by nature and would not reliably show up on demand. This vehicle has an unresolved passenger airbag issue that was recalibrated, not diagnosed. The case was closed without identifying a root cause. We are filing this update so the pattern is part of the record.Read more

Horn on vehicle consistently goes out. Either goes out entirely or the volume of the horn is below a safe level. This is a known Hyundai issue and Hyundai refuses to acknowledge it on this specific model, but has acknowledged it on different models. I Have had multiple repairs by Hyundai directly, and the horn consistently keeps going out. There are sometimes in dangerous situations where I need
... to utilize the horn to avoid an accident and warn other drivers of danger, and I am unable to do so, and it causes dangerous driving situations. This is a known defect on multiple Hyundai vehicles in this year, but the Tucson is one of the few vehicles they will not call a voluntary recall on and NHTSA needs to be notified about this and act accordingly as a regulatory body to protect consumers and people on the road.Read more

Component or system that failed/malfunctioned: The Electric Power Steering (EPS) Gear / Steering Rack Assembly. The component has not yet been removed from the vehicle and is available for physical inspection upon request. How safety is put at risk: When navigating low-speed turns or accelerating from a stop where the steering wheel is rotated to approximately 45 degrees, the steering gear mechan
... ically binds and catches. The wheel completely fails to self-center or return to a straight position on its own. The driver must apply sudden, manual physical force to yank the wheel back to center. This creates an immediate loss-of-control and lane-departure hazard during low-speed maneuvers. Because this is a progressive mechanical wear issue, there is a severe risk of a total steering lockup at higher highway speeds. Confirmation by dealer/independent center & manufacturer inspection: The vehicle has not yet been inspected by a dealer, independent service center, or the manufacturer due to the high out-of-pocket cost of the repair. The owner is operating the vehicle with extreme caution while awaiting a formal safety recall regarding this documented steering gear defect. Warning lamps, messages, or prior symptoms: No electronic dashboard warning lights or error messages have illuminated. The primary prior symptom was a subtle, rotational scraping/friction noise heard coming from the front driver-side wheel well when passing roadside walls or barriers, indicating ongoing mechanical friction within the steering assembly. The 45-degree steering binding first appeared several months ago and has progressively worsened.Read more

I own a 2017 Lincoln MKZ equipped with the 2.0L EcoBoost engine. At approximately [MILEAGE] miles, the vehicle began experiencing loss of coolant, rough running/misfire, shaking/rough start, and reduced engine performance. The vehicle was diagnosed by a Ford/Lincoln dealership as having coolant intrusion into the engine/cylinders and/or engine block failure, requiring engine/long-block replacement
... . The estimated repair cost was approximately $8000. This creates a safety concern because coolant intrusion can cause misfires, overheating, engine damage, and possible loss of power while driving. Ford/Lincoln has issued Technical Service Bulletin 19-2346 and superseding bulletin 22-2229 for 2017–2019 Lincoln MKZ vehicles equipped with the 2.0L EcoBoost engine. These bulletins describe low coolant, white exhaust smoke, rough running, misfire codes, overheating codes, and coolant intrusion into the cylinder, with the prescribed repair being long-block engine replacement. Despite this known documented condition, Lincoln/Ford denied assistance because the vehicle is outside warranty. I am reporting this as a potential safety defect because the failure can occur without adequate warning and may cause engine overheating, misfire, or loss of power while the vehicle is being operated.Read more

I perform all maintenance on my vehicles, I have previously been a ASE Certified Master Technician, so I know engines and vehicles very well. Vehicle is outside of current engine recall window due to the engine build date being July 15, 2024. ... engine, installed a magnetic drain plug and installed Filter Mags on oil filter. Ran 5W30 oil for 2 oil changes, then moved to 0W40 oil. Oil analysis performed for each oil change. Results were looking acceptable up until the next to last oil change at 13,801 miles. Changed engine oil and filter on 6/12/2026 @17,732 miles. Found a heavy build up of metal particles on the magnetic drain plug and when oil filter was cut open there was a heavy build up of metal particles where the Filter Mags were located and the filter element had fine metal particles in entire engine oil filter element. The engine has made no noise and is still running, but based on the evidence the engine is currently failing and if I would have continued to drive it, the engine would likely lock up on the highway. CLEARLY GM DID NOT solve the crankshaft problem July 1, 2024 and CLEARLY using a higher viscosity oil like 0W40 will not keep these engines from failing. If I would have continued to drive the vehicle and if I would have not changed oil and found metal in the oil, this engine would have absolutely failed on the highway. This vehicle has over 80% highway use.Read more

2018 Chrysler Pacifica with the 3.6L V6 engine had a check engine light turn. At same time a vibration started, which was most noticable when the car was in idle. Also, a foggy exhaust started at the same time. No temperature or other warnings were occuring. After driving it to the repair facility the car was not able to be turned on. The findings are that there is a headgasket leak and radiator
... failure that resulted in coolant entering into the cylinder. That resulted in either hydrolock or engine overheating, causing engine failure. Again no temperature warning or other indication of a major issue was provided. The likely repair is a full engine and radiator replacement. It appears that many other Chrysler Pacifica vehicles have reported similar issues with the 3.6L Pentastar engine. No recall is currently available for the 2018 Chrysler Pacifica for this issue, however, it looks like the older versions (~2011-2013) have had a recall for similar issues. The shark fin antenna seal on my BMW failed, allowing water to enter the Telematics Control Unit (TCU). This caused a complete failure of the vehicle’s Emergency SOS / BMW Assist crash notification system. The failure occurred without any warning messages or prior indications that the system was compromised. Because the TCU controls the vehicle’s automatic crash notification and emergency call
... functions, this failure disabled a critical safety system. This defect placed me and my family at risk, as the vehicle would not be able to automatically contact emergency services in the event of a collision or other serious incident. The failure first occurred in April 2026. I immediately contacted my BMW dealership’s service center, where the service advisor acknowledged that the shark fin seal had issues causing this to happen (water intrusion). However, instead of treating this as a safety defect, I was told to file an insurance claim. I also contacted BMW Customer Service, but both the dealership and BMW declined responsibility and refused to cover the repair. The shark fin antenna seal is a manufacturer‑installed, non‑serviceable component. Its failure directly disabled a federally regulated safety system without warning. BMW’s refusal to address this issue leaves the safety defect unresolved and exposes other owners to the same risk.Read more

At every vehicle startup, multiple warning messages appear on the dashboard indicating failure of the driver assistance safety system. The warnings include Collision Mitigation System Problem, Lane Keeping Assist System Problem, and Auto High Beam System Problem. The issue occurs immediately upon ignition while the vehicle is stationary and resets each time the vehicle is turned off and restarted.
... The problem is persistent and has not resolved over time. The vehicle was inspected by an authorized dealership and diagnosed with a diagnostic trouble code indicating an internal failure of the forward-facing camera unit used for the driver assistance system. The repair recommendation was replacement of the camera module and calibration. This malfunction affects the vehicle’s advanced driver assistance safety systems, which are designed to support collision avoidance and lane keeping. The failure may result in reduced or non-functioning safety features while driving, increasing the risk of unsafe operation without full driver assistance functionality. I was barely able to get to the shoulder . We were then stuck there within inches of traffic moving at high speeds until a toow truck could be sent. That took hours while we stayed seatbelted into our truck in 96 degree weather waitng for a wrecker. Even the slightest bit of distarction by one of the driver coming by us cold have been catastrophis. We count oursekves lucky we weren't hit. I am filing this formal complaint to request immediate NHTSA intervention regarding Nissan North America’s bad-faith denial of critical safety recall repairs on my 2023 Nissan Rogue. ... ecall Campaigns 26V-080 and 26V-081. By forcing me to drive a vehicle that exhibits violent engine shaking and severe, abnormal mechanical noise, Nissan is placing my safety and the safety of the motoring public at immediate risk. They are insisting that I continue operating a compromised vehicle until a catastrophic, total engine failure occurs on the highway. Detailed Timeline of Events and Defect Manifestation June 2025 (Vehicle Under Factory Warranty): The vehicle began exhibiting abnormal, excessively loud engine noises. I presented the vehicle to the authorized Nissan dealership for diagnosis. The service department dismissed the issue, claiming they "could not replicate" the noise, and released the vehicle back to me without repair. I captured digital video and audio recordings of the defect at this time to preserve empirical evidence. February 2026: NHTSA and Nissan announced recalls 26V-080 and 26V-081 specifically targeting the 1.5L and 2.0L VC-Turbo engines for main bearing failure, which causes violent engine shaking, severe noise, knocking, and sudden loss of motive power. [XXX]: As the engine noises worsened and progressed into violent engine shaking, I brought the Rogue back to the dealership service department. They admitted the vehicle was shaking excessively but claimed it was a "secondary issue" caused by a failing Exhaust Gas Recirculation (EGR) valve. Manufactured/dealership explicitly stated that because they did not yet find metal shavings in the oil pan, they would not authorize the recall remedy. At approximately 93,000 miles, my vehicle experienced a sudden transmission malfunction. About one week before the failure, intermittent warning messages appeared for Emergency Braking Malfunction and Forward Collision Warning Malfunction. These warnings appeared and disappeared several times over multiple days. About three days before the major transmission issues began, the vehicle was taken to
... an authorized Toyota dealership in North Brunswick, New Jersey, for a routine oil change. At that time, the vehicle was operating normally, and no transmission-related issues were reported. 3 days later, the warning messages returned, and the check engine light came on. Around the same time, the vehicle began experiencing serious transmission problems, including delayed shifting, harsh gear changes, and RPM flare-ups without corresponding acceleration. These issues created a serious safety risk because the vehicle could hesitate or fail to accelerate properly when merging into traffic, turning at intersections, or attempting to avoid hazards. The unpredictable shifting and loss of power increased the risk of a collision. After the check engine light appeared, the vehicle was returned to the same Toyota dealership. The dealership performed diagnostic testing and identified internal transmission and transmission control system faults, including solenoid-related failures and torque converter clutch issues. The dealership determined that the transmission requires full replacement and provided a repair estimate of approximately $12,000. The vehicle has no known prior accident damage related to this issue. there are numerous consumer complaints involving similar transmission problems in 2018–2019 Toyota Camry vehicles equipped with the 8-speed automatic transmission. I am filing this complaint because this failure occurred at relatively low mileage for a Toyota vehicle and presents a significant safety concern due to unpredictable power delivery and loss of acceleration.Read more
