I am writing to formally raise a serious concern regarding the functionality of Tesla’s Full Self-Driving (FSD) system. On May 11th, while the vehicle was operating under FSD, I experienced an unexpected disengagement of the system, which then reengaged on its own without any input from me. This is a significant safety concern, as if this were to happen in stop-and-go traffic on a highway, the car would be unable to accelerate to highway speeds. On 6/20/26, I took the car to Lia Honda in Brewster, NY for diagnosis. They reported a failed Electric Power Steering Control Unit/Steering Rack (trouble code U0420-68). They explained that this single electrical component failure also caused all other components to report failures because they are connected in a ring. Estimated repair cost $4,595.98. I have filed a complaint with Honda to request out-of-warranty coverage. Car is ≈3 years old, 3 months out of warranty, and only lightly used at 17,484 miles. Note that this car falls under NHTSA recall 24V-744, which pertains to a manufacturing defect in the worm gear of the Electric Power Steering. We had the recall service completed on 12/26/24, but it seems possible that continued friction in the worm gear may result in overloading of the EPS control unit. Tires were purchased April 2026. Vehicle was then driven in June 2026 for off-road use, minimal usage prior to June due to mechanical issues). A structural failure occurred on 3 new truck tires (Size: 275/60R20, DOT: 0225) within 24 hours while driving under 10 MPH on a basic off-road trail. ... suffered a 3-4" sidewall tear. Vehicle was at 15 PSI (standard trail pressure). 10 companion trucks ran identical terrain without issue, isolating the failure to this tire batch. Swapped to spare. Day 2: Remaining tires aired up to 20 PSI to mitigate risk. Despite higher pressure and slow speeds, the exact same failure mode recurred on the replacement tire in the same rear driver location, causing another 3-4" sidewall tear. Trail patches failed. Borrowed a spare from another vehicle. 15 minutes later, a 3rd identical failure occurred on the rear passenger tire, causing a blowout and tearing on the inside sidewall. Vehicle was stranded, requiring a 2nd borrowed spare to exit. Tires had only 300 miles of total use. Failed components include the tire sidewalls and internal casing structures. All 3 failed tires are preserved and available for material inspection. Rapid, consecutive loss of multiple tires severely compromised vehicle stability, leaving me repeatedly stranded. Had these structural failures occurred at highway speeds rather than 10 MPH, a catastrophic loss of control or rollover could have occurred. The selling dealer has explicitly refused to file a warranty claim or inspect the failed casings. No prior warning lamps, pressure alerts, or symptoms occurred before the sudden, catastrophic failures.Read more

While driving at highway speeds, the vehicle experienced a sudden and total loss of engine power and stalled in active traffic. I lost motive power and had to safely coast through traffic to the shoulder. The vehicle would not restart and had to be towed to a repair facility. ISSUE 2 — Auto stop/start shuts engine off while coasting The stop/start system incorrectly kills the engine at low speed without driver input. On restart, the engine hesitates and idles roughly. Unexpected engine shutdown reduces power steering and braking assist, creating a hazard in traffic. No warning precedes the event. Dealer is aware; no repair order on file. ISSUE 3 — Transmission valve body failure; hard downshift persists after repair Abnormal transmission behavior required three dealer visits to diagnose. The valve body was replaced. The vehicle still exhibits a hard 5-to-4 downshift after repair. Abrupt downshifts during deceleration reduce predictable vehicle control. Repair orders are on file at Mercedes-Benz of McKinney. I am filing this complaint to formally report two separate incidents of DCDC converter failure in my Land Rover Range Rover Sport — the identical defect described in NHTSA Safety Recall 26V248 — and to request that NHTSA investigate why my vehicle has been excluded from that recall despite exhibiting the same failure mode. INCIDENT 1 – [XXX] While driving, my vehicle displayed a red “Stop
... Safely Electrical Fault Detected” warning message and subsequently lost motive power, coasting to a stop in Bellevue, WA. I was unable to safely move the vehicle out of active traffic. I called 911 and emergency responders were dispatched to block the lane behind my vehicle to prevent a collision. The vehicle was towed to Jaguar Land Rover Bellevue, an authorized Land Rover dealership, where the DCDC converter was diagnosed as the cause of failure. Repair documentation is on file. INCIDENT 2 – [XXX] The same failure recurred. My vehicle again displayed the “Stop Safely Electrical Fault Detected” warning and lost motive power while in motion in Chicago, IL. The vehicle was towed a second time to Jaguar Land Rover Chicago, where the same DCDC converter defect was documented and addressed. Repair documentation is on file. In both incidents, my vehicle came to a stop in an active traffic lane with no ability to self-recover, creating a direct risk of a rear-end collision that could have seriously injured or killed me, my family, and other road users. When I check my VIN against the recall, my vehicle does not appear as an included unit. I have received no owner notification letter from JLR despite the recall report stating owner notifications were planned for on or before June 12, 2026. I am requesting that NHTSA determine whether my vehicle meets that criteria and was incorrectly excluded, or whether the recall population should be expanded. Frequently and randomly I get errors on my dash for the front collision sensor and slippery roads warning. This seems to occur for no reason as all while I am driving. When this happens my RPM's drop and my car will stall at red lights or stop signs. In a few instances, my car has refused to accelerate because it believes the car is obstructed. ... rous occasions. I need to turn my car off and on multiple times in order to get to my destination. Once the car is left alone for awhile, the issues completely goes away and the car drives like normal until it inevitably randomly occurs again. This appears to be an issue with the Kicks model across the board, as loads of other people online have had the same issue and tried multiple things to fix it to no avail. Mechanics have no idea what is causing it or how to fix it. It is also impossible to replicate the issues since it happens for seemingly no reason and then goes away for no reason. 3 images are attached to show proof at different miles (however this has occurred more times than I can count). Vehicle: 2012 Kia Forte Recall: NHTSA 23V-652 (Kia SC284) – HECU/ABS fuse recall My vehicle underwent this safety recall in 2025. The dealership replaced the HECU fuse. ... the warning condition returned. Kia was aware the warning lights returned immediately after recall completion. I was advised if the vehicle later entered limp mode again, to contact Kia roadside assistance for towing support per prior guidance. The warning condition was not resolved by the recall repair. The brake warning light remained present, additional warning indicators later appeared, and the vehicle ultimately entered limp mode in January 2026, resulting in loss of reliable transportation and safety concerns. Due to this condition, I was forced to rely on a rental vehicle for an extended period at significant personal cost totaling several thousand dollars. I'm a single parent with a child requiring ongoing medical appointments, making reliable transportation essential. Kia only approved a limited "goodwill payment" of $191.40 for an earlier rental period, while refusing reimbursement for the substantially larger rental costs incurred after the recall failure and progression to limp mode. When the vehicle entered limp mode, I had to arrange towing because Kia assistance was not available as they closed out the recall. Kia is now requiring out-of-pocket diagnostic payment before determining whether the issue is related to Recall 23V-652, despite the warning condition returning immediately after recall repair and later progressing to limp mode consistent with the original failure symptoms. I am reporting a potentially ineffective recall remedy and unresolved recurrence of the safety condition addressed under Recall 23V-652.Read more
