While driving my 2024 Toyota Grand Highlander Hybrid AWD at highway speed with only 16,755 miles on the vehicle, multiple warning messages suddenly appeared, including warnings related to the engine, hybrid system, AWD system, and other vehicle functions. The dealer diagnosed a punctured radiator caused by a rock or similar road debris that allegedly passed through the front grille and struck the radiator. The dealer confirmed that coolant loss from the puncture triggered the vehicle's protective reduced-power mode. This failure created a significant safety risk. A sudden loss of power while traveling at highway speed increases the risk of rear-end collisions and other accidents, particularly in heavy traffic where drivers may not expect a vehicle to rapidly decelerate. There were no warning lights, messages, leaks, drivability concerns, or other symptoms before the incident. All warnings appeared suddenly at the time of failure. Toyota has declined warranty coverage, classifying the damage as a road hazard. However, I believe this may indicate a design defect. I have identified numerous reports from other GH owners describing nearly identical failures involving rocks penetrating the radiator through the front grille, often at relatively low mileage. These reports suggest the radiator may be inadequately protected from ordinary roadway debris. My concern is not simply radiator damage, but that a small piece of road debris encountered during normal driving can disable a critical component and cause the vehicle to enter a severe reduced-power mode at highway speed. This happened 3 times earlier so I had the car in to Curtis Ryan Honda for service one week prior and they claimed the battery was defective and put a new one in. The only way to eliminate the issue is for the vehicle to be turned off after rear passenger exits during a drop off. As a parent of three active children, I make multiple drop-offs each day at school and sporting events. The repeated and prolonged alarms have become a distraction while driving and have contributed to what I would describe as “alarm fatigue.” My concern is that when a legitimate seat belt safety issue arises, the effectiveness of these alerts may be diminished because of their frequency in routine, non-risk situations.Read more

... n. The dealership advised me that the vehicle had coolant intrusion involving cylinder 4 and that the engine would need to be replaced. Because this was a serious diagnosis, I then took the vehicle to another reputable automotive repair shop for a second opinion. That shop also advised that the engine needed to be replaced due to the coolant intrusion issue. This creates a safety concern because coolant intrusion can lead to misfires, engine damage, overheating, loss of power, stalling, or engine failure while the vehicle is being driven. I was more than 250 miles from home and had to avoid driving the vehicle because of the risk of further engine damage or possible failure on the road. The vehicle is available for inspection upon request. The issue was inspected by the dealership and by an independent automotive repair shop. No crash, fire, injury, or death occurred. Seconds later it felt like if someone was in my passenger seat and just yanked my steering wheel to the right hitting the guardrail. My whole tire and the assembly was off but everything was still intact to the tire. The tie rods, axle, ball joints, the whole assembly.

I was told by GM that the force of the impact was what ripped off my tire but it was not enough impact or damage to deploy the airbags. I was on crutches for my left leg, bruising and scar now from a huge abrasion to my right arm. Still having right shoulder pain. I sent all this to the attorney General and they tried to mediate but Afton from GM told Mike that he contacted me 3 times and I did not return her calls. I called her a left her multiple messages and she has not returned my calls. I only had the car for a few months with 5,000 miles on it. I put $2,500 down then had to turn around and get a used vehicle, put another $2,000 down and making the same payment due to the hit on my credit for a 2023 plain vehicle. ... do with the recall. I took my truck to another garage and they found that the check engine light was indeed caused by the fuel pump running at 52psi - the recommended is 51-85psi for my truck. They said the new part would be in the following week to fix it for $1200. When I relayed all of this information to Ford National they told me they weren't able to help me and they were still investigating. They told me that they also weren't responsible for damage to my truck as my powertrain warranty expires next month. They refused to help in any way or offer a timeline of when I could expect a fix. Ford has left me stuck with no options to prevent a potential crash if this faulty piece makes my truck stall on a highway - I can either pay $1200+ for a part from another garage they are legally supposed to fix, or continue to drive a potentially unsafe vehicle based on Ford's negligence. Sioux Falls Ford and Ford National, based on a year timeline with several calls and text chats with both, are refusing to comply with their legal responsibility to remedy this safety recall.Read more

I belive the manual of the Graco Extend2fit 3-in-1 Trueshield and possibly other Extend2fit models is in need of revision-specifically in the section that instructs how to measure for one inch of appropriate head clearance within the shell. ... hic in the manual seems to show that one should measure for one inch of head clearance starting from the blue line I drew onto my photo. If you compare the fabric stitching in the graphic to the fabric stitching in the photo and identify the line relative to the stitching, the part you measure from appears to match the blue line I drew in my photo. I have always measured from the blue line I drew and I've owned the seat for 6 years. But according to Graco, I have been measuring wrong all this time, which means my child has been unsafe all this time. After talking with CPSTs at Graco, they are insisting that you are supposed to measure from the red line I drew onto my photo. This doesn't make sense because: 1.) That does not match the graphic provided in the manual. 2.) The red line's only physical marker is the fabric cover. The cover can shift, as mine does, exposing more of the headrest adjustment handle underneath, which would alter the measurement point since, according to Graco, the only point of reference is apparently the fabric cover. This will lead to inaccurate measurements everywhere. Fabric covers are not consistent reference points. I have spoken with many respected CPSTs outside of Graco, and none of them know for sure which line to measure from, the red or the blue line I drew. Many are convinced it is the blue line, as I am. If CPSTs are confused, I can guarantee most people are confused. Which means many people are measuring from the blue line, and according to Graco, that is incorrect. Incorrect fits lead to unsafe children, so this needs to be addressed.Read more

Most recently, while driving in a rainstorm, the vehicle suffered a complete failure of the windshield wipers, a total loss of the high-beam headlights, a total loss of the horn, and a total loss of electronic communication to the electronic parking brake system. Most critically, the vehicle experienced an uncommanded propulsion hazard where it hard-locked itself in the 'Drive' gear and absolutely refused to shift into 'Park' or 'Neutral,' forcing the driver to physically maintain heavy hydraulic brake pressure to keep the vehicle from creeping forward. Because the horn failed and the parking brake was disabled, the driver was trapped in the driver's seat holding the brakes and was forced to call 911 for emergency first responders to come and physically block the wheels so the driver and infant could safely exit. Tesla Service has diagnosed the issue as a failed central Body Control Module (Part #1010906-00-D), but the cascading network failure modes present an extreme, life-threatening hazard regarding vehicle control and child entrapment.Read more

My Nissan rogue is experiencing issues with the throttle body leading it to mal function lights warning see owner manual at 27k miles which also the air mass flow sensor code is coming on P101 at very low mileage. In which those parts are suppose to last for years more mileage I’m concerned for the safety of this vehicle as it’s being very costly and defective and the issues are appearing same

However, this remedy completely failed to resolve the underlying manufacturing defect, and the hazardous steering behavior persists, making the vehicle unsafe to operate. Because I live on an isolated island with only one authorized Acura franchise facility, I have no alternative dealership options. The local dealership management and the visiting Acura Field Technical Specialist are formally refusing to diagnose, evaluate, or replace this defective steering rack. They are using the presence of an unrelated, aftermarket air suspension modification—which was installed after the failed recall attempt and does not physically obstruct the steering gearbox assembly—as a pretext to deny federal safety recall resolution and standard warranty repair. The manufacturer is failing to provide a successful remedy for an active, critical safety hazard. Furthermore, the local franchise facility has ordered me to remove this unsafe vehicle from their property, creating a severe safety risk. PCS system has failed. This causes the vehicle to be unable to charge and can lead to loss of power if unable to get to a supercharger before the battery is depleted.
