This week, I was driving my Tesla at approximately 65 mph in the middle lane of a six-lane freeway. Autopilot was engaged. Traffic was moving normally, and I was not aware of any obstacle or hazard that would have required emergency braking. Without any warning, the vehicle suddenly applied the brakes and rapidly decelerated until it came to a complete stop in the middle travel lane of the freeway
... . I was not pressing the brake pedal. The event occurred unexpectedly and created a potentially life threatening safety hazard. my vehicle came to a stop in a high-speed traffic center lane! After I disengaged Autopilot and pressed the accelerator, the vehicle responded normally. Immediately following the incident, the display showed the message “Autopilot unavailable.” That message later disappeared, and no additional warning messages remained on the screen. I contacted Tesla to report the incident. Tesla reviewed the vehicle’s logs remotely and later discussed the results with me by telephone. During that conversation, I was told that the event was considered an “anomaly.” I was also told that the vehicle logs reflected the event approximately one hour from when it actually occurred and that the logs indicated the vehicle was not operating in Autopilot at the time of the incident. This information was inconsistent with the event, because Autopilot was engaged when the sudden braking occurred. I could not even move to berm. Tesla did not request that I bring the vehicle in for a physical inspection. Instead, I was told there was no reason to have the vehicle examined. During the same conversation, I was encouraged to consider subscribing to Tesla’s Full Self-Driving (Supervised) system, which requires an additional monthly subscription, rather than continuing to use autopilot as a result of a question I asked about two plans. Tesla closed my service ticket without performing an inspection indicating there was no benefit spending $$ to essentially receive no additionalRead more

The wiring in my trunk harness was severed due to the repeated opening and closing of my trunk. This caused my reverse lights to fail and my trunk latch to stop working. This issue is part of a known recall: Campaign 21V-00D. Tesla attempted to fix the issue in 2023 but the poor repair did not work. I took my Tesla to the dealer and they refused to remedy the issue unless I would pay $1300. I pers

Vehicle: 2018 Ford F-150 3.0L Power Stroke Diesel I am submitting this complaint because my vehicle's transmission exhibits repeated and potentially dangerous operating behavior that has not been corrected despite service by a service shop. The transmission unexpectedly downshifts while driving at highway speeds without driver input. It is also consistently slow to shift into appropriate gears d
... uring normal acceleration, causing delayed power delivery followed by abrupt and harsh gear engagement. The transmission frequently shifts aggressively, resulting in severe jolts that force occupants forward in their seats. The shifting behavior is unpredictable and significantly impacts safe operation of the vehicle. I brought the vehicle to an authorized mechanic for diagnosis and repair. During the inspection, I was informed that the transmission fluid level was approximately 75% below the proper level. The diesel automotive shop performed a transmission software update and returned the vehicle to me. Despite these repairs, the transmission problems have continued and appear to have become more severe. The ongoing symptoms include: - Unexpected downshifts at highway speeds. - Delayed gear engagement and hesitation before shifting. - Harsh, abrupt shifting accompanied by violent jolts. - Unpredictable transmission behavior that reduces driver confidence and control. These conditions create a significant safety concern, particularly at highway speeds, where unexpected downshifts or delayed power response can increase the risk of a collision. Because the issue persists after dealership service and a software update, I believe there may be a defect affecting the transmission or its control system that warrants investigation. I respectfully request that the National Highway Traffic Safety Administration investigate this issue to determine whether it represents a broader safety-related defect affecting other vehicles of the same model and model year.Read more

Oil burns too fast causing oil changes to be required by every 3000 miles
Incident date: Mar 1, 2026
Our 2018 Pacifica has had problems similar to what many others have described in the ongoing NHTSA Investigation (PE25009), but recently while driving highway speeds around a curve the steering failed to respond briefly which caused us to drift towards the oncoming lane before the steering started to respond. We stopped at a Pep Boys repair shop and had them to inspect the steering components but
... they were uncertain of the cause. I was a certified mechanic many years ago as a young man, and this is the most serious safety problem I've ever experienced. Even when this steering problem is brief it can be enough to cause a driver to oversteer and lose control as the steering delay catches up to a driver's input. The possibility of crossing into oncoming traffic is frightening to say the least, so YES it put my family and others safety at risk! There haven't been any warning lights. I suspect it is the rack & pinion system since it has an electric motor rather than a power steering pump. If it's due to the electric motor there should be a warning lamp that lets the driver know if it's failing.Read more

We purchased a Rhino USA Adjustable Trailer Hitch with locking hitch pins on Amazon. On its very first use, while towing our empty 1,600-pound Forester camper, the steel pin fractured where it enters the locking head. We heard the pin snap while making a slow turn about a quarter mile from our home. The locking head detached and was later found on the roadway. Fortunately, the pin remained in the
... receiver long enough for us to get home, but this could easily have resulted in the ball mount sliding out of the receiver and a trailer separation. We have retained both pieces of the failed pin and have photographs. Because the locking head detached and was found on the roadway, while the remaining pin stayed in the receiver only by chance, this failure had the potential to allow the ball mount to exit the receiver during travel which could be deadly to those on the highway. We have reported this to the manufacturer, but have not heard from them yet.Read more

I am the owner of a 2017 Ford Fusion with 61,000 miles on the odometer. Please note that I have always taken good care of my car, having it serviced promptly when required. In recent weeks, I have had to replace my complete steering rack (cost $4,500) and two weeks later, my 1.7-liter Ecoboost engine due to a coolant leak into the number two cylinder at a cost of $6,700. Ford has known about both

Regarding the engine, Ford issued Customer Satisfaction Program 21N12, Technical Service Bulletin TSB 22-2134 and Customer Satisfaction Program 19B37 - Supplement #4 to replace the car's engine short block. This covered cars for 7 years of service or 84,000 miles - with, as quoted in the Ford memo sent to dealers on June 9, 2022, "coverage automatically extended to subsequent owners," for presumably the life of the car. Since this is clearly a manufacturing error (as Ford admits in its June 9, 2022 memo to dealers) and not due to negligence on my part, I should be reimbursed for my expenses. The NHTSA should also open a recall case on this matter for owners like myself.Read more

One week ago (6/19) while driving the main screen went black. I pulled over and restarted engine 3x with no luck so drove to dealership. Another startup there it was still black. I was told by service manager “I cannot have you drive it like that” and I was offered a rental which I accepted. Just prior to claiming the rental I again started my Murano and the screen was normal again. I took
... my vehicle but made an appt to have things checked on Monday. At that time I was told that it just needed a software update. That was concerning to me because how would I know in the future it needed an update prior to another incident of the screen going black. Response was that I wouldn’t. Yikes! Additionally, the google assist has repeatedly been of concern with audible requests being met by incorrect responses (different radio station than requested, or static); temperature setting set to 3 degrees higher than requested. Audio will cut out randomly that will only reset after the car is shut off for period of time. Talk to text malfunction and looping. During one errand shortly after taking ownership the backup camera froze and stayed like that for a 25 minute drive. The dealership did a factory reset yesterday (6/26) in an attempt to resolve several issues but with no luck. I also got an oil change. My receipt did not reflect the reset so I requested that be added to my service record.Read more

My independent repair shop has just diagnosed a coolant leak weeping from the vacuum line connection at the water pump; they referred to this problem as “coolant migration”. As documented extensively by Audi in Technical Service Bulletin 19-23-45 (2070349/2), this indicates an internal seal failure of the water pump, causing pressurized engine coolant to migrate directly into the vehicle’s v
... acuum system. While I understand that my vehicle is technically past its 4-year factory warranty timeline, a mechanical failure of this nature at just 43,000 miles is highly premature and points to a known manufacturer design defect rather than standard wear and tear. More importantly, this defect poses an immediate, severe safety risk. Because the vacuum system directly services the power brake booster, the migration of coolant into these lines risks catastrophic contamination of the braking system. This results in a stiff brake pedal and compromised vehicle stopping ability. I am deeply concerned about operating this vehicle with an active, manufacturer-acknowledged safety hazard affecting my brakes. At this time, I have not had this problem confirmed by the dealer, but plan to in the next week. The vehicle it available for inspection upon request.Read more

We were getting in our car to drive home from a restaurant. We attempted to start our car like any other day, and the car started. However, our car was stuck in gear, it was in parking but also not in parking. The car was not moving, the gear was in “park”, the dot light for park was not showing, and then we had several warnings appear on our dashboard. At first it was three, then it went to s

We had: check speed limit system, check high beam assist, check driver attention warning, check blind spot safety system, check forward safety system, check rear cross-traffic safety system appear in 20 seconds of car being on. This car just hit ~15,500 miles on it. To add to the strangeness, we open the Kia Access App and tried “lock” and “unlock” (while being in the car”, then turn the car off. We started the car again and it worked as if this never happened (no immediate warnings like above). Yet we did recieve a check engine light and a vehicle diagnostic on the instrument (dashboard) saying “follow-up check required: 2 (Engine Management System and Transmission Control System.)” The car allowed itself to recognize it in park gear and drive off. The Kia Access app said we experienced a UOO01F0 code tied to the Engine Control System. We never seen anything like that before. We did, however, experience a problem with the driver seat powertrain at 11,000 miles where the seat wouldn’t move. The seat was fixed by the dealership though. Incident date: Jun 10, 2024
To the National Highway Traffic Safety Administration (NHTSA), I am writing to formally submit a safety complaint regarding my 2023 Kia Seltos, which exhibits a recurring, dangerous defect involving the engine stalling without warning. This defect results in a total loss of power while the vehicle is in operation or attempting to accelerate, posing a critical safety risk. The failure manifests in
... two primary scenarios: * **Remote Start Failure:** The vehicle frequently stalls immediately after utilizing the remote start feature when shifting into drive. * **Auto Start/Stop Instability:** The Auto Start/Stop system contributes to the engine shutting down unexpectedly when lifting off the brake or attempting to accelerate from a stop. When this occurs, the dashboard illuminates multiple warning lights (e.g., battery, oil, "key not in vehicle"), and the vehicle loses all forward propulsion. On inclines, this defect causes the vehicle to roll backward, forcing the driver to rely solely on emergency braking to prevent a collision. ### Evidence of Widespread Concern My research indicates this is not an isolated incident; numerous Kia owners report identical, dangerous symptoms: * Drivers across various models report the engine shutting down and the vehicle rolling backward on hills, requiring a full power cycle to resume operation. * Some owners report losing all ability to accelerate at traffic lights, accompanied by critical dashboard warnings. * Others report the vehicle shutting off on its own while driving or refusing to start despite the dashboard indicating it is running. I have attached evidence documenting these experiences: **image.png**, **image_2.png**, **image_3.png**, **image_4.png**, and **image_5.png**. Given the frequency of these reports, I urge the NHTSA to launch a formal investigation into this apparent software or engineering failure. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The rear view camera has a recall. I took the vehicle into a Ford dealership on 6/23/26 for this recall. This is a risk to myself and others because backup cameras are a federally required safety feature. This is my 2nd attempt for Ford to fix this recall at two separate dealerships. Ford does not care about their recalls and the safety of Ford customers! I almost hit another car that was co
... ming out due to the blind spots on that vehicle. My young daughter also drives the car and this is not the 1970’s as young drivers depend on that feature. Ford said that they could not find the problem with this issue and further diagnosing must be done on my part for $198. This is not fair to customers as this was a recall issue. If Ford is not properly training their technicians to diagnose this issue, it should not be the responsibility of the customer to pay for their error in producing this car. The problem has been reproduced and confirmed by a Ford dealership. There were no warning lights or issues that were displayed. Ford only sent out letters stating that a recall was issued on the problem. In May of 2026 my backup camera and trunk was not working. Ford inspected and replaced the parts on the vehicle that they assumed was part of the recall. For a 2016 Mustang, a common cause is not just the camera itself but also wiring/connectors in the trunk area or the APIM/radio module that displays the image. If the dealer simply performed the recall software update or replaced one component and the screen is still blank, the underlying problem may not have been fully diagnosed. This the 2nd attempt to have a recall fixed by Ford but to no avail (2 separate dealerships)Read more

Year/Make/Model: 2025 Cadillac XT6 AWD Sport Mileage at time of incident: ~12,233 COMPONENT: Service Brakes, Hydraulic / Brake Booster Vacuum Sensor DESCRIPTION OF THE PROBLEM: While driving on a main road 1 mile away from the dealership, the brakes failed without warning. The vehicle was driven directly to the dealership (Les Stanford Cadillac, Dearborn, MI) for emergency inspection. This is the
... same defect that has now caused brake failure twice in approximately 5,500 miles: a brake booster vacuum sensor failure that previously triggered a brake assist warning and was "repaired" on 10/07/25 at 6,747 miles, then recurred and was "repaired" again on 05/29/26 at 12,233 miles. Both repairs replaced the brake booster vacuum sensor assembly, performed by the same dealership and same technician, yet the underlying defect has continued to recur. This is a serious safety defect that creates an unreasonable risk of loss of braking ability while driving, with potential for a crash, injury, or death to the driver, passengers, or other motorists. WHEN DID THE PROBLEM FIRST OCCUR: Originally October 2025; recurred June 2026 with loss of brakes on a main road. WHAT WAS THE OUTCOME: Vehicle driven directly to dealer for inspection following loss of brake function 1 mile away.Read more

My vehicle has suffered catastrophic electrical failure due to water intrusion directly into the rear cargo compartment. This water ingress has destroyed vital electronic control modules that were designed and placed in low-lying, unsealed body cavities—an engineering layout heavily documented as a design vulnerability in the E70 chassis. My safety was put at risk as this happened on the highw
... ay and threw my SUV into limp mode. I could not drive the speed limit on the highway and had to drive home with my hazard lights on while cars drove around me and almost collided with me on the highway. BMW has previously acknowledged the severe liabilities of placing sensitive electronics near drainage paths, as seen in the Catalano v. BMW of North America litigation. This is not a wear-and-tear issue; it is a design failure that leaves critical safety components swimming in water during normal rain events.Read more
