This creates a safety risk of being stranded with no ability to add fuel, including the risk of running out of fuel on the road or being unable to refuel in an emergency. The failure appears to worsen in cold weather. This is a widely reported problem on 2022-2025 Lexus NX and TX hybrid vehicles and has been attributed to a faulty electronic fuel-door lock actuator (referenced by owners and parts suppliers as OEM part number 77030-F6020). Owners are frequently forced to use a manual emergency release located behind an access panel in the cargo area in order to refuel. FUEL TANK CANNOT BE FILLED TO CAPACITY: On a near-empty tank, the fuel pump nozzle shuts off after only approximately 8 gallons, despite the tank's rated capacity of approximately 17 gallons. Adding additional fuel requires repeated, prolonged attempts and often only a few more gallons can be added. This significantly reduces usable driving range relative to what the vehicle's fuel gauge and range estimate imply, creating a risk of unexpectedly low fuel and reduced range in situations where refueling options are limited. Other owners have reported the same behavior, and in at least one documented case a Lexus Field Technical Specialist diagnosed the cause as a deformed/defective fuel tank requiring a redesigned replacement. Both conditions are present on my vehicle. I am reporting them so that they are documented at the federal level. No crash, injury, or fire has occurred. INCIDENT 1 (MAY 15, 2026): While maneuvering at low-speed idle into my residential garage, the engine un commandingly surged . I was tapping the brake to go cautiously. Simultaneously, the brake pedal became rigid and physically impossible to depress. The vehicle crashed forward into my tool benches and drywall. INCIDENT 2 ( on or about May 22,2026): Under the identical conditions, the vehicle without command surged forward with total brake rigidity. Fortunately, this time it caused no damage. INCIDENT 3 (June 10, 2026) During a slow speed idle parking maneuver , the vehicle violently surged forward with total brake rigidity and struck shelving. To cut propulsion I immediately shifted out of Drive. Instead of neutralizing the event the car the car underwent a severe bidirectional runaway, violently accelerating in REVERSE for approximately 35 feet across my driveway crashing into my woodpile . I ended up moving forward and stopped somehow. TECHNICAL EVALUATION for INVESTIGATORS: The recurring rock hard brake pedal serves as empirical pneumatic proof that the throttle body valve uncommandedly opened wide while my foot was actively modulating the brakes, instantly destroying the engine manifold vacuum reserve. The vehicle was taken back to a dealer, and the entire pulley system was replaced, and the display screen was re-flashed, but months later the failure recurred and became worse. This component is suffering from an intermittent failure that creates an unpredictable and immediate strand risk, compromising driver safety by leaving the vehicle unable to start or powe

Manufacturer American Honda Motor Co. is fully aware of this systemic defect, having issued Technical Service Bulletin TSB 20-116 (Diagnostic Trouble Code DTC B12C5) specifically addressing ignition start/stop switch failure. Despite this being a documented engineering flaw admitted by the manufacturer, American Honda Case Management (Case 16436837) is actively enforcing predatory financial barriers to prevent remediation. They are refusing to issue a conditional diagnostic waiver to verify the code, choosing instead to force an upfront out-of-pocket dealer diagnostic fee onto the consumer with no guarantee of goodwill coverage. Furthermore, corporate representatives handling this file on July 1, 2026, actively refused to provide their names or employee identification numbers when requested. I request that NHTSA review this case file and investigate American Honda’s bad-faith deployment of cost barriers to intentionally depress the reporting and remediation of a documented, safety-critical manufacturing defect.Read more

This directly induced severe physical trauma, muscle spasms, and an immediate loss of lower extremity neurological sensation while operating the replacement vehicle. The contact stated that the entire vehicle burned with the exception of the rear exterior lights and the rear passenger's side tire. The fire was extinguished by the Fire Department, and a report was filed. A Police report was filed. There was no property damage. The contact was transported to the hospital by ambulance. The contact sustained chest pains, bruises on the chest cavity, several contusions and bruises, and difficulty breathing. The contact was undergoing breathing therapy. I was driving the vehicle, when I started to fell a " Shuddering " sensation on acceleration. I let off the gas pedal and the Shuddering went away. A few miles down the road the check engine light came on and was flashing. I immediately pulled and shut the vehicle off. After sitting for 10 minutes I restarted the vehicle and the check engine light was no longer on, but the " Shuddering " sensation
... was still present. I took it to Dupratt Ford in Auburn,Ca. and they diagnosed it as a Cylinder 1 mis-fire. Upon further inspection they found water in the Cylinder 1 spark plug well. After doing some research I found out the Ford issued TSB 23-2011for this exact issue, but did not order a recall to fix the faulty windshield cowl which is the problem. I found thousands of complaints and numerous Ranger owners who have had the exact same issue and had to pay out of pocket to get the issue and damaged fixed. Meanwhile Ford knew about the issue and did absolutely nothing, did not even alert the owners of the issue. I was lucky and did not have to replace the cylinder head, but others have not been so lucky. I had to pay over $1800 to have the mis-fire issue fixed and had to do my own preventative measures to seal the windshield cowl. I contacted Ford and they were only willing to reimburse me $725 dollars of my repair bill which was 100% their fault.Read more

While driving my 2025 Jeep Wrangler 4xe in normal conditions, the vehicle unexpectedly loses electric drive power. Power is reduced in stages — to approximately 50%, then 25% — then electric mode is disabled and the dash displays "Electric Mode Temporarily Unavailable," after which the vehicle forces into hybrid operation after a delay. This has occurred at 41%, 73%, and 81% battery state of charge during normal driving, with no wide-open-throttle demand — conditions under which the manufacturer's specification states electric power should continue. This is a loss of drive power. The manufacturer's own recall A7C (NHTSA campaign 25V710000) identifies a software error that may result in loss of drive power in these vehicles, and states that a loss of drive power increases the risk of a crash. My vehicle exhibits this condition but is not included in that recall population, which raises a question of whether the recall scope covers all affected vehicles. Safety risk: unexpected loss of drive power during normal driving — including when merging, crossing intersections, or pulling into traffic — creates a delay in which the vehicle does not respond to the accelerator as expected. Reproduced by the dealer: dealership service personnel personally experienced the loss of electric propulsion during a test drive, and the condition was also reproduced on a second 4xe vehicle at the dealership. In all cases, no diagnostic trouble codes were present, no repair was performed, and the vehicle was returned as "operating as designed." A neutral arbitrator in the manufacturer's warranty arbitration found that a nonconformity exists with a propulsion issue on the vehicle. The condition has not been properly diagnosed because I am unable to obtain the diagnostic measurement I have requested.
