On XXX I retrieved my 2023 Tesla Model Y (VIN: [XXX]) from Tesla Motors NV, Inc. ([XXX]) after their third attempt to correct a severe panel and component misalignment following a collision repair (Invoice #XXX). The facility replaced major front-end structural components, including the Front Bumper Beam (1687601-00-E), Front Subframe Crush Can (1109193-00-C), and Cast Front Underbody Left Hand Cr
... ush Can (1608453-00-D).Tesla's official guidelines require a mandatory 3D structural measurement test whenever these specific cast aluminum underbody and subframe parts are replaced. The repair facility verbally claimed the damage was minor/cosmetic and that only a manual tram measurement was performed. However, a manual tram gauge cannot detect 3D frame twists or height deviations in cast aluminum components, making it wholly insufficient. As a direct result of this unverified structural alignment, the vehicle's front end remains warped. After the first repair, the headlamp shifted and protruded on the right side. After subsequent attempts to fix it, the left headlamp is again protruding outward. Because the structural frame is misaligned, the forward-facing cameras are out of calibration. Under Autopilot, the vehicle exhibits dangerous behavior, including misidentifying a standard bike lane as an extended vehicle lane. Furthermore, the repair facility has since removed or hidden the conversation logs regarding these complaints, and I can no longer find the messages in the Tesla app. Operating a vehicle with unverified structural alignment and compromised driver-assist vision systems poses an immediate, severe safety risk. This near-miss was an immediate, severe life-safety crisis not just for the occupants but also to others on the road. 2. Symptoms & Alert Codes: There were zero warning symptoms prior to the incident. Immediately upon the power collapse, the touchscreen displayed critical faults. Active diagnostic telemetry confirmed alert codes: DI_a137 (No Capable Drive Units) DI_a063 (System Graceful Power Off) 3. Manufacturer Confirmation & Inspection Status: The failure was confirmed in writing by the manufacturer’s service center. The diagnostic invoice explicitly states: "The rear drive inverter experienced an unintended reset due to an error. Sudden power reduction and shutdown while driving." The vehicle is currently held at the manufacturer's facility and is fully available for inspection. 4. Engineering Assessment: As a mechanical engineer, my assessment is the surge through the AEB event to the inverter’s DC-link was unable to damp this load, forcing a hardware logic reset. Crucially, the software logic failed to partition this fault. In a dual-motor architecture marketed as having built-in powertrain redundancy, a localized rear inverter reset should be isolated, leaving the front motor operational to safely navigate out of traffic, this did not happen. Component: Lane Support System — Emergency Lane Keeping (ELK) Part of standard Autopilot safety suite (not FSD) activated without valid lane detection data and pushed the vehicle into the oncoming lane of traffic on Ohio State Route [XXX]. The system then aborted mid-intervention (LSS_STATE_ABORT), leaving the driver in the oncoming lane. The resulting head-on collision killed both drivers — t
... he [XXX] Tesla driver and the driver of the oncoming vehicle. Two people died because of this ELK malfunction. Tesla's own telematics data confirms the following sequence: 5At 23:51:17.420 UTC, the Lane Support System activated Emergency Lane Keeping (LSS_STATE_ELK). 6At 23:51:17.652 UTC (232 milliseconds after activation), the system logged ALC_UNAVAILABLE_NO_LANES — confirming it had no valid lane detection data during the intervention. 7At 23:51:17.755 UTC (335 milliseconds after activation), the system logged LSS_STATE_ABORT — the system cancelled itself mid-intervention. 8During the 335-millisecond intervention, the vehicle's steering angle reached -16.79 degrees leftward and lateral acceleration measured +1.200 m/s² leftward — both confirming the vehicle was pushed into the oncoming lane. 9The driver responded with the largest steering torque (-5.08 Nm rightward) and largest rightward steering angle (+19.50 degrees) recorded in the entire telematics file — confirming he was actively attempting to recover. 10 The head-on collision killed both him and the driver of the oncoming vehicle. We requested additional telematics data from Tesla through the Tesla app. A forensic expert has examined the vehicle's ECU motherboard— missing critical fields including continuous yaw rate, high-resolution steering data, ELK steering commands, and lane detection confidence scores. A subsequent request for additional data produced an error message and Tesla has not provided further data. This crash is the exact behavior under PE25012. My 2023 Prius Prime cannot be charged normally in hot weather without triggering a thermal-protection fault on its high-voltage charging system. Avondale Toyota diagnosed it in writing: "CODE P1CE996 CHARGER COOLING FAN REVOLUTION SENSOR COMPONENT INTERNAL FAILURE. THE ELECTRIC CHARGER ASSEMBLY HAS A INTERNAL OPEN CIRCUIT CAUSING IT TO BE INOP. RECOMMEND REPLACEMENT ADD PN# G9090-47061." Quote: $2
... ,516.99 plus ~$200 diagnostic. The vehicle cannot verify its high-voltage charger's cooling fan is operating. In Phoenix heat (100F+), normal charging repeatedly sets the fault and check engine light. I verified by controlled testing: reduced-current charging in cool hours avoids the fault, but any normal charge in heat triggers it — in factory configuration, no obstruction. I am forced to restrict charging to nighttime reduced-current sessions to avoid operating the charger with unverified cooling. A plug-in vehicle that cannot safely complete a normal charge in summer conditions is a safety concern. This is a documented defect: Toyota TSB T-SB-0054-23 (8/7/23), "MIL ON With DTC P1CE996," states the charger assembly "has been modified to reduce the possibility of this condition," superseding part G9090-47060 with G9090-47061 — the part my dealer recommended. The TSB covers VINs before XXX; mine ([XXX]) is in range. Toyota denied coverage at all levels (Case XXX), citing third-owner status and declined unrelated services. A Brand Engagement supervisor acknowledged this identical repair would be fully covered under the PZEV emission warranty if registered in [XXX] — conceding it is a warrantable emissions defect — while denying it for Arizona registration. I request NHTSA review whether a recall or warranty extension is warranted for affected VINs. Hot-climate owners bear full cost of a documented defect on a high-voltage component. What component or system failed or malfunctioned, and is it available for inspection upon request? (On our 2021 gasoline suburgan, the rear wheels locked up at free-way speeds, which nearly caused a possibly fatal accident. The lock released but it appears the strain caused rear differential gear teeth to break and the rear differential now needs to be replaced for the vehicle to operate properly.
... ) How was your safety or the safety of others put at risk? (The sudden rear wheel lockup at freeway speeds resulted in temporary loss of control of the vehicle and significant surprise to the driver, who, after swerving into the adjacent lane was able to gain control of the steering and stop the vehicle. Had there been another vehicle in the other lane the MVA could have been fatal for 9 people in the suburban and any other vehicle involved.) Has the problem been reproduced or confirmed by a dealer or independent service center? (The rear differential damage was confirmed by a dealer.) Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? (No.) Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? (No warning or messages. This has happened 3 times over 3 years, always on extended trips 9-12 hours in duration. The first two times nearly caused an accident but no differential damage. This third time, the differential was immediately damaged due to the incident. We have since learned that this is an issue with 2022 suburbans, for which a recall has been issued. A recall has also been issued for 2021 DIESEL suburbans. However, no recall has been issued to gasoline 2021 suburbans, but apparently needs to be.)Read more

... at completely consumed the vehicle. The local fire department responded to the scene to extinguish the blaze. The vehicle is a total loss. This safety incident occurred exactly one week after I received the official manufacturer safety recall notice by mail for Hyundai Recall 301 (NHTSA Campaign Number: 26V308000), which outlines an overheating transistor defect within the Hybrid Power Control Unit (HPCU) capable of causing a vehicle fire. The vehicle caught fire due to this exact un-remedied defect before I could realistically bring it to a dealership for the required safety software update. However, information concerning this conversion was not shared with the federal government as the vehicle VIN still lists it as a Promaster 1500. This caused me to miss notification for NHTSA Campaign Number 25V792000 involving a potential trave

While driving my 2015 Infiniti Q50s, the transmission suddenly suffered a catastrophic electronic failure, causing the vehicle to immediately drop into "limp mode." This resulted in a sudden and dangerous loss of acceleration while operating in traffic. A diagnostic scan revealed trouble codes P0500 and P0720 (Output Speed Sensor circuit). However, following Infiniti's official Technical Service
... Bulletin (TSB ITB19-040), the actual failure was the Transmission Control Module (TCM), which is permanently integrated into the transmission control valve body inside the transmission. Because the speed sensor cannot be replaced independently of the integrated module, I was forced to pay out-of-pocket to replace the entire transmission control valve body assembly and have a dealership program the new unit. This is a widely documented, premature failure on this vehicle platform that creates an immediate highway safety hazard when the vehicle drops power without warning.Read more
