I am writing to report a serious safety concern involving our 2023 Ford Transit Connect (VIN [XXX]) van that resulted in an unintended vehicle movement and collision. While the incident resulted in only minor property damage and a minor injury, we believe the circumstances warrant review due to the potential for a much more severe outcome. The vehicle had been exhibiting operational issues prior
... to the incident. Employees reported that it was running and shifting noticeably rough and, after being parked, would not move when placed in either Drive or Reverse. The engine RPM would increase to approximately 2,000 RPM with throttle input but the vehicle remained stationary. The following witness statements document the sequence of events: Witness #1: [XXX] started the van to verify whether the issue from the previous day remained. The van still would not move. He placed the transmission in Neutral, turned the engine off, and we were able to push the van by hand. Based on this, he concluded the brakes were not preventing the vehicle from moving. He then reached into the van and restarted the engine. Immediately upon startup, the van accelerated forward unexpectedly at a high rate of speed, jumped the curb, and struck the hangar wall. [XXX] attempted to stop the vehicle, and I also attempted to intervene, but the event occurred so quickly that neither of us was able to prevent the collision. Witness #2: I went to the van to diagnose the reported issue. I had been informed the van had been running and shifting roughly before it was parked and would no longer move afterward. I started the vehicle while it was in Park and attempted to engage both Reverse and Drive, confirming that it would not move. The engine RPM increased to approximately 2,000 RPM with throttle input and remained elevated. I then shut the engine off and placed the transmission in Neutral with the ignition in the Run position to determine whether the vehicle would roll. On April 17, 2026, while driving my 2022 Hyundai Palisade with approximately 167,000 miles, the vehicle suddenly lost engine power and was unable to maintain normal driving speed. The sudden loss of propulsion created a safety risk because the vehicle could no longer operate normally in traffic. I was able to move the vehicle out of traffic, but it would not restart. I also observed white smoke co
... ming from the engine compartment. The vehicle was towed to Matt Bowers Hyundai Slidell, an authorized Hyundai dealership, where it remains and is available for inspection upon request. The dealership has inspected the vehicle and advised that the engine needs to be replaced. The exact internal component that failed and the final cause of the engine failure remain under review by Hyundai. I do not recall any warning lamps or messages before the sudden failure. The timing of any prior symptoms is UNKNOWN. The vehicle had been maintained through authorized Hyundai dealerships. On December 29, 2025, a Hyundai dealership replaced the oil filter housing because of an oil leak and performed a synthetic oil and oil filter change. On March 11, 2026, approximately five weeks before the failure, the dealership performed another oil and oil-filter change and a multipoint inspection. I am reporting this incident because the sudden loss of engine power while driving, the inability to restart, and the smoke from the engine compartment created a serious safety concern. I am filing this complaint regarding my 2021 Kia Sorento, which has suffered a serious transmission failure. The vehicle is displaying diagnostic code P0741, indicating a torque converter clutch malfunction. The transmission slips, shifts harshly, hesitates during acceleration, loses power unexpectedly, and has become unsafe to drive. The vehicle is available for inspection upon request. This defec
... t has put my safety and the safety of others at risk. On several occasions the vehicle failed to respond properly when accelerating, including while entering highways and driving in traffic. The hesitation and sudden loss of power created fear and anxiety because I could not trust the vehicle to move when needed to avoid a dangerous situation. The problem has been reproduced and confirmed by a repair facility, and the symptoms are consistent with a transmission torque converter clutch failure. I also contacted Kia and provided the vehicle information. Kia informed me that they would not repair the vehicle because it has exceeded 100,000 miles and that the recall or warranty extension had expired. However, my vehicle is experiencing the same transmission-related symptoms reported by many other Kia owners.I never received any recall notification because I purchased the vehicle from a dealership as the second owner. Had I been properly notified, I would have taken the vehicle in for inspection much earlier. It is extremely frustrating and financially devastating to be denied assistance solely because of mileage when the failure appears to be related to a known transmission defect. Before the failure, the check engine light illuminated and code P0741 appeared. Prior to that warning, the vehicle had already shown delayed shifting, slipping, hesitation, rough gear changes, and loss of power, which became progressively worse. This situation has caused significant emotional stress, anxiety, and financial hardship. I am requesting a full investigation and a fair resolution from kia.Read more

On four separate occasions while driving anywhere from 35 to 55 mph the engine light begins to flash and I lose all power. I am given just enough time to pull to the side of the road. I turn off the engine, wait a few minutes, and turn the engine back on, which seems to remedy the problem. At that point, I am able to drive off like nothing happened. I am on a CRV forum and I’m seeing more and m
... ore people discuss the same issue! Because there’s no code that comes up, and I am not able to replicate the scenario at the mechanics… There is nothing that can be done. It’s quite scary to be going 55 miles an hour and have to pull over on the side of the freeway. One time there was not a shoulder of the road that I was on, luckily a gas station was just up ahead and I was able to pull in there to reset the engine.Read more

On or about XXX, my 2021 Acura RDX, with approximately 121,078 miles, suddenly displayed multiple dashboard warning lights and system-failure messages while I was driving. These included the check-engine light, Road Departure Mitigation System Problem, steering, stability-control, braking-assistance and other driver-assistance warnings. Earlier, the tire-pressure monitoring system had also indicat
... ed a malfunction even though the tire pressures were correct. The vehicle initially continued to drive and the conventional brakes appeared to function normally. However, it then began hesitating when I attempted to accelerate. The vehicle would eventually gain speed, but the initial hesitation created a serious safety concern when pulling into traffic or attempting to accelerate. The check-engine light remained solid while the vehicle was stopped but flashed whenever I began driving and accelerating again. No crash, fire or injury occurred. The vehicle had undergone a full diagnostic inspection approximately two weeks earlier, and I was informed that everything checked out properly. There were no known drivability problems before this incident, and the vehicle has been maintained according to the manufacturer’s recommended service schedule. The vehicle was subsequently inspected by Elite Acura, an authorized Acura dealership in [XXX]. The dealership confirmed a head-gasket failure and a problem with the rear differential left-side actuator. The dealer recommended replacing the head gasket, four spark plugs, coolant, oil and filter, as well as replacing the rear differential left-side actuator and changing the differential fluid. The estimated head-gasket repair was $5,391.46, and the estimated rear differential actuator and fluid service was $813.85. The dealer also recommended rear brake pads and rotors. The head-gasket and rear differential actuator problems were confirmed by the authorized Acura dealership. My 2021 Volkswagen ID.4 Pro S has unexpectedly lost propulsion and come to a complete stop while being driven on at least three separate occasions. When this occurs, the vehicle jolts, stops moving, and cannot be placed into Drive or Reverse. I am only able to shift it into Neutral. After the first incident, I took the vehicle to a Volkswagen dealership the following morning. The dealership state
... d they could not reproduce the problem or identify any faults. The same failure occurred twice in one day this week. During one of those incidents, the vehicle lost propulsion and stopped in the middle of a busy street, creating a serious safety hazard because I was unable to move the vehicle out of traffic. Immediately before and during the failure, the vehicle displayed multiple warning messages, including: Error: Drive system. Please safely stop the vehicle. Electrical system not working correctly. Please service vehicle. Error: Electronic Stability Control (ESC). Front Assist is not available. Lane Assist is currently not available. Rear Traffic Alert is currently not available. Park Distance Control is not available. Error: Auto Hold. These warnings appeared at the same time the vehicle lost propulsion. The vehicle stalled without warning while driving and became undriveable. This is extremely concerning because it can occur suddenly in traffic, placing the driver, passengers, and surrounding motorists at risk of a crash. The vehicle is currently at the Volkswagen dealership for inspection, and I am awaiting their diagnosis. Because this dangerous condition has now occurred multiple times, I am concerned there is a serious defect affecting the vehicle's drive system or electrical system.Read more

On Sunday, July 12, 2026, the 6.2L V8 (L87) engine in my 2023 Chevrolet Silverado 1500 ZR2 seized and locked up completely, resulting in total loss of propulsion. I was driving at 65 MPH when it felt like the transmission was slipping and I lost power and coasted the truck to the side of the road. COMPONENT: Engine (6.2L V8, RPO L87) — connecting rod/crankshaft failure consistent with Safety Re
... call N252494001 (NHTSA Recall 25V-274). The vehicle is available for inspection. It is currently at Cable Dahmer Chevrolet, where it was towed on July 13, 2026, and remains inoperable. SAFETY RISK: The engine seized during operation, causing sudden and complete loss of propulsion in traffic. This is the exact failure mode identified in Recall 25V-274 as increasing the risk of a crash. CRITICAL DETAIL: This vehicle previously underwent inspection under Safety Recall N252494001 and PASSED on Feb 3rd 2026 at VAN Chevrolet in Kansas City. GM deemed the engine non-defective and issued extended warranty coverage of 10 years/150,000 miles under the associated special coverage program. Despite passing GM's recall inspection, the engine suffered catastrophic failure. This appears directly relevant to NHTSA's open recall query regarding the effectiveness of GM's recall remedy. DEALER CONFIRMATION: A technician at Cable Dahmer Chevrolet has verbally confirmed the engine is locked up/seized. Formal diagnosis is pending master technician sign-off and submission to GM, expected the week of July 20, 2026. The manufacturer has not yet inspected the vehicle. WARNING SIGNS: No warning lamps, messages, or symptoms appeared prior to the failure. The engine failed without warningRead more

While driving in the dark, the car's headlamps extinguished multiple times for 1-2 seconds over the course of about 15 minutes. The headlamp switch was in the "Auto On-Off" position. I eventually turned the headlamp switch to the "On" position and the incident did not repeat for the next 45 minutes while driving home. The next day I examined the ambient light sensor located at the rearview mirro
... r console behind the windshield. The glass was clean and without water or bug debris. I tested the "Auto On-Off" function by placing a towel over the ambient light sensor, and it correctly controlled the on-off function of the headlamps. I noticed a loud rattle when starting my 2018 Ford Expedition, at first occasionally and then on every cold start. The Ford Dealer's service department diagnosed a cam phaser issue. There were no warning lights or messages available to me, but the service department's diagnosis showed several timing faults. I was told the cam phaser repair is $4000 - $5000 and continued use puts the engine and tra
... nsmission at risk. I contacted Ford customer service and was told that my vehicle was not eligible for assistance with these repairs and that my level of loyalty also did not make me eligible for assistance. I have maintained this vehicle with oil changes, fluid changes, tire and brake maintenance for the life of the vehicle. I would like the NHTSA to investigate Ford for a recall of the cam phasers and effected parts since this is a known and prominent issue that also impacts road safey.Read more

Key will not fully turn back and release, keeping the accessories running. After many start, shift, move a little and waiting for a period of time it eventually turns to normal to remove. I’ve seen articles on if it says “shift to park” on the dash lights and it does not. Bought used with 70,000 10/24 and continues to get worse.
