My 2026 Toyota Grand Highlander exhibits abnormal fluttering/vibration of the hood at speeds above approximately 25 mph. I have identified that this is the same defect documented in Toyota's own Technical Service Bulletin T-SB-0111-24 (issued December 4, 2024), which addresses this condition on 2024 model year Grand Highlander and Grand Highlander Hybrid vehicles, with the same complaint widely re
... ported by 2025 model year owners as well. Despite this being a known, documented Toyota defect for three consecutive model years, I have been unable to get it resolved through Toyota's official channels: I contacted Toyota's PATT Lab (Parts and Technical Assistance) regarding the vibration under Dealership Task Number DC261970283. PATT Lab informed me that other 2026 model year vehicles exhibit the same condition, characterized it as "intended behavior," and closed the task without applying the known TSB remedy. I was directed to Toyota Brand Engagement for further assistance. This was opened as Case Number 260722000822. Toyota Brand Engagement stated they are not mechanics and that the issue must be resolved at the dealership level, but has provided no directive or authorization to the dealership to perform the known TSB repair. As a result, I am caught in a loop between Toyota corporate departments, none of which will authorize or perform the fix that Toyota's own engineering team has already documented and issued a bulletin for. My vehicle has a known manufacturing/design defect that Toyota has acknowledged in writing for prior model years, yet I am being told simultaneously that it is "normal" and that it's a dealership matter — while no dealership has been given authorization to apply the remedy. Requested Action: I am requesting that Toyota Motor Sales, USA be required to apply the existing Technical Service Bulletin remedy (or its 2026 model year equivalent) to my vehicle at no cost, consistent with how this defect has already been addressed for 2024 and 2025 modRead more

I have a 2021 Silverado with the 6.2L engine. I had received the recall notice, but it took several months before I could get the truck checked. By the time I was able to get it inspected, the truck had over 100,00 miles, with the symptoms associated with the 6.2L engine failure. It was said to have passed inspection, but I had my doubts. Low and behold, my engine failed not once but twice. T
... he first time I was told it was not due to the recall and paid out of pocket. The most recent failure, (same exact failure as before, and nearly caused an accident) happened last week. I towed it to a different dealership than where it was inspected and where the last repair occurred, and was told it needed a new engine. I called and was told by the dealership that a new engine was ordered, and they were waiting to hear back from GM on if they would cover it or not. I was also told they were very confident GM would cover it, and that is why they ordered a new engine. A few days later, I get a call and find out GM is in fact not covering it and the repair is around $12,000. I am beyond frustrated at GM for not taking accountability, and the dealership for giving me false hope that this would be covered.Read more

My 2020 Volvo XC90 T8 plug-in hybrid has been out of service for 78+ days due to a hybrid battery defect covered under an active manufacturer warranty (Claim Type XXX, valid through XXX). The vehicle was towed to Volvo Cars Arrowhead (Glendale, AZ) on XXX after a hybrid battery failure. The dealer confirmed in writing on XXX that multiple hybrid components require replacement and that parts are on
... extended backorder, and that Volvo Cars USA had authorized a rental vehicle because a loaner could not be offered "for an indefinite period of time." On approximately XXX, my service advisor advised me by phone that more than 700 vehicles nationally are waiting on the same backordered part and that the repair has no estimated completion date. I sent a formal written demand to Volvo Car USA ([XXX]) on XXX requesting repair by a firm date, a replacement vehicle, or repurchase. Volvo did not respond within the stated 14-day deadline or at any point thereafter. The defect appears related to NHTSA's existing recall action on 2020–2022 Volvo plug-in hybrid vehicles for high-voltage battery thermal runaway risk. I am filing this complaint to add to the federal record on this recall and to document Volvo's failure to repair an affected vehicle within a reasonable time or provide any remedy despite formal demand. I own a 2021 Audi e-tron with approximately 58000 miles. Beginning in March 2026, the vehicle developed intermittent Virtual Cockpit/instrument-cluster problems with blank display. I reported the condition to Audi dealer of Princeton NJ. The vehicle subsequently experienced two dead 12-volt battery incidents and became inoperable. The second 12 volt battery was new. Following the most recent inci
... dent, the Audi dealer diagnosed the Virtual Cockpit/instrument cluster as causing a drain on the 12-volt battery and recommended replacement at a cost of approximately $3,500. The replacement part is unavailable, and Audi has provided no estimated delivery date. The vehicle has remained out of service since July 03rd 2026. This condition presents a potential safety concern because failure of the instrument cluster may prevent the driver from reliably viewing speed, warning indicators, vehicle status, range, and other safety-related information. The recurring 12-volt battery discharge may also leave the vehicle unexpectedly disabled. The vehicle is additionally showing an indicated range of only approximately 160 miles at 100% charge. Audi has not established whether this is related to the instrument-cluster failure, high-voltage battery condition, battery-management system, software, or another electrical defect. Audi has not provided documentation of a comprehensive high-voltage battery health and capacity evaluation. Audi issued a recall or service campaign affecting other 2021 Audi vehicles for substantially similar Virtual Cockpit/display problems. However, Audi states that my VIN is excluded and has not provided a technical explanation identifying a different component, part number, software version, supplier, or failure mechanism. I request that NHTSA evaluate whether the recurring instrument-cluster failure and 12-volt battery discharge constitute a broader safety defect and whether the existing Audi recall or service-campaign population should include this car.Read more

The vehicle automatically initiated the emergency-call sequence but announced words to the effect of, “Your call cannot be completed.” The Montgomery County Police Department investigated the collision under Incident Number XXX. Fortunately, a passerby witnessed the collision and assistance was available. Without that person, emergency assistance could have been delayed, increasing the risk of serious harm. An injured, disoriented, trapped, or unconscious occupant may not be able to locate a phone and call [XXX]. The vehicle had a prior history of recurring GPS, SOS, and connected-services failures. It was presented to authorized Volkswagen dealers four times while under warranty, and a Volkswagen TAC case was opened, but no root cause or permanent repair was documented. Shortly after Volkswagen was notified that a lemon-law filing would proceed, the GPS, green SOS indicator, and some connected functions appeared to return for about two days without any explanation of a repair, software update, reset, remote action, or configuration change. The GPS then became inaccurate again, while the SOS indicator remained green. I request that NHTSA investigate why the indicator remained green when the automatic crash call could not connect, whether Volkswagen verifies actual voice and location functionality before displaying the green indicator, and whether affected vehicles require a safety recall or other corrective action. SAFETY DEFECT REPORT: Sudden Power Loss & Falsified Records 1. INCIDENT & 5-HR TOWING (XXX): On XXX at ~16:30, while executing a low-speed (<10 mph) left turn in Cambridge, MA, the vehicle had a sudden, total high-voltage failure mid-turn, losing all motive power and power steering without warning or error codes. Photos/videos were taken on-scene confirming ZERO physical impact or undercarriage c

With zero power, the driver and several helpful bystanders had to use extreme physical effort to manually push the dead car out of the road. AAA arrived at 18:00 but could NOT tow because total power loss completely locked all 4 wheels/brakes (no Tow Mode). AAA dispatched a 2nd specialized flatbed with dollies (arrived 21:00). Driver was stranded on-site for over 5 hours. Car reached Tesla Dedham Service at 22:00. On XXX at 10:00 AM, driver reported the driving power loss in person at Dedham, handed over key, and got a loaner. 2. WRITTEN NOTICE (XXX): On XXX, explicit written notice was sent to Tesla Service Manager Taso, re-confirming the exact mid-driving power loss hazard. 3. TESLA DIAGNOSIS & REPAIRS: Invoice #XXX confirms a severe high-voltage failure: "A hardware overcurrent event caused small high voltage battery brick imbalance and rear inverter failure." Tesla replaced Rear Drive Unit Inverter (3DU) and Pyrotechnic Battery Disconnect (Pyrofuse). 4. FALSIFICATION OF SAFETY ISSUE: Despite 5-hr towing from wheel lock-up, XXX in-person report, and XXX written notice detailing mid-turn power loss, Tesla falsely recorded on invoice: "customer states: vehicle will not charge." This is deliberate falsification and deceptive practice by Tesla to downplay a critical road hazard into a benign charging issue. While driving at approximately 35 mph, the center display screen failed to disengage the rear view camera. The persistent, high-visibility flashing of moving traffic behind me in my peripheral vision completely obscured my forward safety alerts and navigation, creating an immediate visual hazard. This system glitch distracted me at a critical moment, causing an impact with a curb. Given that this incident was precipitated by a verified malfunction of a federally regulated safety system (FMVSS 111), I requested that Hyundai corporate review this case, investigate the failure of this component, and cover the cost of the $3,337 structural repairs. They have denied my request. I am aware that NHTSA Recall 26V400000/Hyundai Recall 304 is pending complete entry of all affected vehicles (vehicle was built at the Montgomery plant). Hyundai issued Technical Service Bulletin TSB 24-BE-001H for 2022–2026 Tucsons. It officially documents that these infotainment systems suffer from "freezing, lagging, unresponsive touchscreens, and intermittent blank screens". The bulletin attributes this to a loose internal coaxial cable and software bugs. I feel strongly that the camera freeze is a direct byproduct of this documented hardware/software defect. Hyundai explicitly stated to the federal government: "An instrument panel display that fails increases the risk of crash".Read more

I bought the vehicle new in November 2025. 2026 Toyota Tubdra TRD SR5. In the first week I noticed a lot of jerking and shaking. I brought it to the dealer and was told it was a break in period and it should go away. I’ve had the truck in the shop 5-6 times since and the jerking and shaking hasn’t gone away. I’m at 6300 miles now. The dealer won’t even respond to me anymore. The only way t
... o make the jerking go away is to step on the gas pedal harder but this has become extremely dangerous because it’s forcing me to go faster than I would like, especially in school zones. The dealers say that it’s the truck running very low RPM’s to maximize fuel efficiency and also because there are 10 gears but is that’s the case than why isn’t everyone experiencing it. It’s worse on cold mornings (40 or below) but it’s almost like the truck needs an hour or two to warm up. Even then it’s still jerking. My guess is that the truck doesn’t know what gear it’s supposed to be in but 4th or 5th gear are the worst where the truck hesitates and jerks. I’ve been told to run it in Sport mode but that doesn’t help. I’m finding more and more complaints about this online with the Toyota Tundra SR5’s and I’m part of numerous groups. Someone needs to step in because no one should have to go through this and it’s very unsafe to have to push the gas pedal down harder to make the jerking go away.Thank you for your time.Read more

Incident date: May 1, 2026
Vehicle was purchased new in June 2026. Within approximately six weeks of ownership, I am on my third windshield. Two windshields have fractured after minor impacts from small pebbles or road debris. Incident 1 occurred on or about June 11, 2026. The impact was not audible and was not noticed at the time it occurred. The damage was discovered later as a spider crack approximately the size of a co
... in, originating from a single point of impact. The crack subsequently propagated well beyond the initial impact point. The windshield was replaced with OEM glass. Incident 2 occurred on July 21, 2026. Light road debris struck the windshield during normal driving, again producing an initial spider crack roughly the size of a coin at the point of impact. This crack likewise spread beyond the initial damage. The windshield again requires replacement. In both cases the initiating impact was minor - in the first instance, not forceful enough to be heard from inside the vehicle - yet each produced a spider fracture that propagated rather than remaining localized. Two windshield failures from low-energy debris strikes in six weeks is not consistent with normal glass performance. The windshield is a structural component of the vehicle and serves as the mounting point for the forward-facing ADAS camera. Glass that fractures and propagates this readily from routine road debris presents an unreasonable safety risk: it obstructs the driver's forward view, compromises occupant retention in a rollover or frontal collision, and requires repeated ADAS recalibration. No crash, fire, or injury has occurred. This failure pattern appears consistent with numerous existing NHTSA complaints regarding Hyundai Palisade windshields across multiple model years.Read more

I then attempted to schedule service with AutoNation Acura, an authorized service center. Over approximately one month I left more than 25 voicemails on both the reception and service department lines. I received no return call and no acknowledgment. I have been unable to secure a service appointment. On [DATE] I sent written correspondence to Acura Corporate Relations documenting the failed scheduling attempts, restating the safety nature of the failure, and requesting referral to an alternate authorized dealer. [No response as of this filing.] The effect is that a safety-critical driver assistance system has been intermittently non-functional since at least April 2, 2026, and the manufacturer has neither acknowledged the defect nor made its service network available to diagnose it. The vehicle remains in service. Failures occur without warning during normal operation, disabling collision mitigation braking, lane keeping, road departure mitigation, and adaptive cruise control at once. The vehicle retains its original factory windshield, never replaced, modified, or damaged, eliminating a common cause of camera misalignment. Mileage is low for the model year. Other Acura and Honda owners report comparable camera failures with similar codes. I renew my request that NHTSA evaluate whether this constitutes a safety-related defect and whether a broader pattern exists across vehicles using the same camera module and AcuraWatch/Honda Sensing architecture.Read more

The starter/starter solenoid and related engine oil sealing have failed three times. Most recently on 6/18/2026, and previously in 2024 and 2025, the vehicle would not start: the dashboard illuminated but the engine would not crank. The engine then began smoking and a burning electrical smell entered the cabin. My wife and young child were in the vehicle. The vehicle was rendered completely inoper
... able and had to be towed each time. Safety risk: an engine that smokes and emits a burning electrical odor upon attempted starting presents a fire hazard to occupants. On the most recent occurrence the failure happened at a fuel station with my family present, over 1,000 miles from home, stranding us for seven days. Dealer confirmation: yes. On the two prior occasions a franchise dealer diagnosed a defective starter solenoid and replaced the starter. Both repairs failed and the identical failure recurred. On the most recent occurrence, a different franchise dealer found that a plug sealing an unused opening on the engine block was missing, allowing engine oil to spray onto and saturate the starter, destroying it. The starter and the plug were replaced. All failures were documented on dealer repair orders, which are available on request. Inspection: the vehicle has been inspected and repaired by franchise dealers on all three occasions. No police or insurance inspection. The manufacturer has been notified through its customer care department, which has open case records on this vehicle. Warning indicators: none. There were no warning lamps, messages, or symptoms prior to any of the three failures. The vehicle simply would not crank, followed by smoke and a burning electrical smell. This vehicle has spent approximately 60 days out of service since purchase and has had persistent electrical problems documented since 2023. Despite repeated dealer repairs the same failure mode continues to recur. I am reporting this as an unaddressed safety defect.Read more

I own a 2026 Toyota Corolla purchased in November 2025. The recurring problems began on December 8, 2025, when multiple warning lights appeared on the dashboard along with the message “System Malfunction – Visit Your Dealer.” The warning indicators involved several vehicle safety and driver-assistance systems. The vehicle was taken to an authorized Toyota dealership for inspection and repair
... . On June 16, 2026, the vehicle experienced another serious malfunction, including a “Transmission System Malfunction” warning. The electrical and operational problems continued despite dealership visits and repairs. The battery was also replaced, but this did not permanently resolve the recurring failures. During a later incident while the vehicle was being driven, multiple warning lights appeared and the vehicle lost power or shut off. There was no crash or injury, but the unexpected malfunction while driving created a serious risk to the driver, passengers, and other motorists. The dealership has also addressed an issue involving the shift lever position sensor and corrosion or water intrusion. I do not know the exact component responsible for all of these failures. However, the problems have occurred repeatedly since December 2025 despite multiple dealership visits. Toyota was notified and corporate Case No. 260630000224 was opened. I have not received a permanent explanation or resolution, and I am concerned that the vehicle could unexpectedly malfunction or shut off again while being driven.Read more

My vehicle's infotainment system both center console and dashbboard went completely black 2 weeks after I purchased the car in August of 2025. The vehicle was in the shop for a week and a half, and they finally fixed the issue. The vehicle at the time was bought used with about 2,200 miles on it at time of purchase. Now in July of 2026, exactly one year from the day i bought the car, with only 9,5
... 00 miles on it, we are having the same issue again with both the dashboard and center console going completely dark, not allowing us to see any critical information such as MPH, gas fuel, tire pressure, etc. Other than that the car seems to drive just fine. I noticed there was a mass recall of 144,000 cars because of this same issue with Mercedes Benz. The GLC-class was in fact listed on this list of eligible recall vehicles. However my VIN # was not eligible for some reason, even though it is the exact issue that the recalls were happening on, and fits within the recall class type and year of manufacture (2024-2026). I am simply requesting to be put on this recall list because it seems like this issue will continue to persist and we do not want to be turning this in every year and also risk our lives driving a car that does not give us very critical information in order to drive safely.Read more

I purchased the truck at the end of March 2025. Within a few weeks I was noticing different issues in the transmission. It would occasionally shudder, have jerky or long shifts, would jump gears and lose power. The shudder was random but mostly on the highway, the jerky or long shifts would mostly occur when the egjne was cold and more often in cold weather. The lose of power and jumping gears wo

I took the truck in three times to get diagnosed but they couldnt replicate the issue or find a code. I finally decided to pay for a flush with the fluid that was recommended by a TSB. After the flush the issues were then replicated by the service techs. I have an extended warranty. The representative then inspected the truck and found the same issues and approved a transmission replacement. I took the truck in on June 22nd 2026. As of July 21 the dealership is still waiting for the replacement transmission to be shipped from GM.Read more

While accelerating to pass a vehicle that was signaling and operating well below the speed limit, my vehicle experienced sudden and continued unintended acceleration. I was able to bring the vehicle to a stop while fighting against the engine within an intersection but the engine continued building revs, bouncing off the limiter while my foot was entirely removed from the accelerator pedal and ful

During this time I had moved the gear selector to neutral, only furthering the engine's revs, and attempted to place the vehicle into park without success, before having to reach beneath my dashboard, foot still on the brake, to pull the accelerator pedal up. I verified both visually and by feel that the pedal was stuck in it's position entirely on it's own, there was no outside object or floormat inhibiting it's ability to return.Read more

On June 14, 2026, while driving approximately 70 mph on a Michigan freeway, the 6.2L V8 engine began shaking violently. The vehicle lost oil pressure, warning messages appeared on the dashboard, and the vehicle completely lost propulsion and shut down while still in motion. My spouse and child were inside the vehicle. The sudden loss of power at highway speed created a serious risk of a collision
... , although I was able to bring the vehicle to safety. No collision, bodily injury, fire, or property damage occurred during this incident. An OnStar/Cadillac notification reported a critical engine and transmission system problem and diagnostic trouble code P0016. The vehicle was towed directly to an authorized Cadillac dealership. The dealership opened the repair order on June 15, 2026, at approximately 46,640 miles. The dealership determined that the vehicle required a replacement engine. The replacement engine later arrived, but additional required parts, including a seal kit and gasket, were still outstanding. General Motors advised in writing that there was no estimated arrival date for those parts and no confirmed repair-completion date. The vehicle and failed engine components have been in the possession of the authorized Cadillac dealership and should be available for inspection. General Motors confirmed that the engine failure and repairs are covered under warranty. I am reporting this incident because the engine failure caused a complete and unexpected loss of propulsion at highway speed, placing the vehicle occupants and surrounding motorists at risk.Read more

The biggest concern that I have is what I have learned is referred to as “Sticky steering” when I'm driving especially 55mph. It's almost like it catches or hangs up on something before making a small correction. The with the steering wheel doesn't settle back in the center of the lane as it should. It tends to drift toward the left side. I try and correct it but it's a constant back and forth
... fighting the steering wheel just to stay in my lane. I'm afraid not only is it seriously dangerous but it may appear as if I am driving erratically. It seems to be getting progressively worse. I must always have both hands on the wheel constantly making corrections just to keep the car within the lane I am in. On the same subject I have noticed is that there is a popping or clicking noise whenever I try and correct it or turn the wheel especially towards the right. I've searched for any recalls and discovered that it is a consistent issue with newer models. What I don't understand is why Honda hasn't addressed this if the problem is serious enough to issue a recall in the 2022 and 2023 civics why would they not investigate the same issues in earlier models such as my 2019. This is not just a inconvenience it is a major safety concern. I hope Honda will take this seriously before someone gets hurt.Read more
