The vehicle is available for inspection upon request. This creates a significant safety concern because nighttime visibility is reduced with only one functioning low-beam headlamp. It also makes it more difficult for other drivers to see my vehicle, increasing the risk of a crash, especially on dark roads or during poor weather conditions. The warning message first appeared before the headlamp permanently failed. The problem was intermittent at first but has progressively worsened until the passenger-side low beam no longer functions. I have learned that numerous owners of higher-trim Ford Explorers equipped with Adaptive LED Headlamps have reported experiencing the same issue. I understand that similar complaints have already been submitted to NHTSA. The replacement cost is extremely expensive and often requires replacement of the entire adaptive headlamp assembly, leaving owners to pay out of pocket for what appears to be a widespread defect. I respectfully request that NHTSA investigate this issue to determine whether a manufacturing or design defect exists and whether Ford should issue a recall or other corrective action. The unpredictable nature of this acceleration profile makes controlled low-speed maneuvering extremely difficult and unsafe. 3.I brought this safety concern to my authorized Mercedes-Benz dealership service department. The service advisor and technicians verified the issue and confirmed that this lag is a widespread, inherent design issue across the CLE 300 line—reproducible in other identical customer models and dealership loaner vehicles. According to the service team, the delay stems from factory programming coordinating the 48V mild-hybrid system, drive-by-wire throttle mapping, and Eco Start/Stop function in default Comfort mode. Because the vehicle automatically reverts to default Comfort mode every time the engine is started, drivers are continuously exposed to this safety risk unless manually overridden on every single drive. The dealership indicated there is currently no official factory software recall or bulletin available to fix the programming. 4. The vehicle had to be towed to an authorized Cadillac dealership for inspection and repair. After diagnosing the problem, the dealership informed me that the entire transmission required replacement. This is a major component failure on a nearly new vehicle with relatively low mileage. A complete transmission replacement on a new vehicle, combined with a sudden failure at highway speed, raises serious concerns about the safety and reliability of this model. A transmission should not fail unexpectedly under normal driving conditions, especially in a new vehicle. (They changed gearbox in 14 days) I am reporting this incident because I believe it represents a potential safety defect that could place other drivers and passengers at risk. I respectfully request that NHTSA investigate whether similar transmission failures have been reported in the 2025 Cadillac XT4 and determine whether this issue warrants further investigation or a safety recall. In addition to the highway transmission failure, I have experienced another serious safety concern. When shifting the vehicle from Park into Drive or Reverse, the transmission sometimes does not engage immediately. Instead, there is a delay of several seconds, followed by a sudden and unexpected engagement that causes the vehicle to lunge forward or backward. I am concerned that the restraint system does not properly accommodate shorter adult drivers. While the seat belt remains fastened, the fit is poor enough that it creates a distraction during vehicle operation and raises concerns about occupant protection in a crash. ... nge on its own. During the lane change, the FSD system misjudged the lane boundary and steered the vehicle into the lane divider/barrier. The vehicle struck the divider on the front right side. The failed system is the FSD/automated steering and lane change function. No forward collision warning sounded and no automatic emergency braking activated before impact with the fixed barrier. There were no warning lamps, alerts, or symptoms prior to the failure; the system initiated and executed the lane change without indicating any problem. The impact damaged the front right body panel, front right bumper area, and the right headlight. My safety and the safety of vehicles in adjacent lanes was put at risk, as the collision occurred at roadway speed during an automated maneuver I did not initiate. Had the divider been a vehicle or a pedestrian work zone, the outcome could have been far more severe. All FSD telemetry, camera footage, and vehicle data from the incident should be available from the manufacturer, and I have requested the vehicle data report from Tesla. The vehicle is available for inspection upon request and is being taken to a Tesla-certified collision center for repair. An insurance claim is being filed with my carrier.
