There is a known issue about exhaust manifolds for my year model truck. A ram technician said in 2019 mopar created a new exhaust manifold for my truck that would be less likely to create soot buildup and warping in the exhaust manifold. In 2021 my EGR was replaced because of a recall but now my exhaust manifold has failed and needs to be replaced out of pocket even though it’s been a known issu

I purchased a 2024 GMC SIERRA 1500 Denali Crew Cab Diesel 3.0L After several times bringing the truck to the dealership I purchased from and conduct all service related work. I am reporting a safety-related defect with a 2025 Hyundai Palisade SEL. Within days of purchase (December 14, 2024), I began experiencing abnormal noise and vibration from the rear of the vehicle. The issue consistently occurs at highway speeds (45–55 mph) and over bumps. The problem has been present since the first week of ownership and has worsened over time. Multiple inspections confirmed
... the issue: •Selling dealership dismissed it as “normal tire noise” without proper diagnosis •Second Hyundai dealership confirmed severe abnormal wear (cupping) on all four tires •Independent inspection (Firestone) confirmed uneven tire wear, alignment out of specification, and ongoing vibration Despite early reporting, Hyundai Motor America has refused to provide a root cause diagnosis or take responsibility. They denied tire replacement, attributing the issue to tire wear. This explanation is not valid. Tires do not develop severe cupping shortly after purchase without an underlying defect such as improper alignment or suspension issues. This condition creates a safety risk: •vibration reduces vehicle stability at highway speeds •uneven wear increases risk of tire failure The issue affects all four tires and was present almost immediately after purchase, indicating a likely manufacturing or setup defect. The problem has been confirmed by multiple service providers but remains unresolved. The vehicle is not safe to operate under normal driving conditions. I request immediate investigation into a potential manufacturing defect affecting vehicle safety. Hyundai has been aware of this issue since the first week of ownership and failed to take corrective action. Despite multiple documented inspections confirming abnormal tire wear and vibration, no root cause diagnosis or repair has been provided. This raises serious concerns about a potential systemic defect affecting vehicle safety.Read more

The cylinder head cracked, causing a coolant leak, which then lead to the car overheating. My car was taken into Mazda dealership because there is a warranty extension for this specific part CSP111. The Mazda dealership, corporate, and warranty department all stated the known widespread defect cracked cylinder head will not be replaced on their behalf due to an overheating code. Mazda is using a s

This defect part and Mazda’s unwillingness to provide a proper solution caused my car engine to fail, posing a safety risk. No symptoms other than low coolant were present, which my car was then parked, not driven, and towed to the dealership, in hopes the warranty would be honored.Read more

I am filing this complaint because this vehicle has had multiple intermittent safety-related electrical/software and driver-assistance problems, and the vehicle is available for inspection upon request. On multiple occasions, the vehicle has powered on but would not shift out of Park into Drive. The most recent time, I was leaving work and the vehicle would not let me shift into Drive. I was stuck
... there for about two hours and had to call for a tow. When the tow truck arrived, the vehicle suddenly allowed me to shift into Drive again and I was able to drive home. This is very concerning because the vehicle can leave someone stranded without warning. The vehicle has also falsely detected hazards or forward-collision situations several times while driving, even though there was no nearby vehicle, pedestrian, object, or anything else that should have triggered a warning. On multiple occasions, the vehicle has abruptly applied automatic emergency braking and slammed on the brakes for no apparent reason while traveling at 30-50 MPH. I am very concerned this could cause a crash, especially if another vehicle is behind me. The vehicle has also had charging and range display problems on multiple occasions. For example, last week it was charged to about 80% and then driven about 50 miles, but the displayed range/state of charge did not properly decrease or update until the vehicle was turned off and restarted about an hour later. These issues have continued despite prior dealer updates and recall work. The manufacturer has been notified, and the vehicle is being brought back to an authorized dealer for inspection. I do not know yet whether the dealer or manufacturer has been able to reproduce or confirm the problems. The issues appear to involve the electrical/software systems, shift/park function, battery/range display, forward collision warning, and automatic emergency braking systems.Read more

After the car dealership completed the first tire rotations, at 30,000 miles I rotated the tires to find severe inner tread wear on the tires where the inside of the tire was wore to the sidewall while outside portion was intact. I took the vehicle to an independent shop and the right rear suspension/tire was out of specifications on camber causing said tire wear. Toyota did not manufacture the ca
... r so that camber could be adjusted. An aftermarket kit was purchased to be able to compensate for the lack of adjustment. This is a safety concern as if a person does not rotate every 5,000 miles it will lead to tire failure at highway speeds. This is a common problem while researching forums. I have contacted the manufacturer and was given a generic response. There are reports with documentation with tire failure as early as 11,000 miles. Toyota needs to be held accountable and make a remedy for this instead of offering consumers a run around and blame hitting pot holes or on tire manufacturers, when it is clearly a suspension limitation. Thank you.Read more

Driving on the freeway, my transmission made a loud bang jerked the truck and then completely failed. Towed to aamco who inspected and diagnosed as the CDF drum failed and had to have the entire transmission rebuilt. No warning light and no indication that it was having any problems before the failure. My daily commute is around 40 miles one way, I was coming home from work, and I was approximately 8 miles from my home. To Whom It May Concern at NHTSA, I am filing this safety complaint regarding a defective ABS Hydraulic Control Unit (HCU) on my 2017 Ram 1500 Rebel 4x4 5.7L Hemi, VIN: [XXX] , with approximately 88,000 miles. DEFECT DESCRIPTION: The ABS HCU has failed, resulting in complete loss of anti-lock braking, traction control, electronic stability control, and cruise control. This is an identical failure
... mode to the defect addressed under NHTSA Recall 24V-896, which covered 2017-2018 Ram 2500-5500 trucks. My 2017 Ram 1500 uses the same or substantially similar HCU component and is experiencing the identical failure. SAFETY RISK: The loss of ABS, ESC, and traction control significantly increases the risk of a crash, particularly in adverse weather conditions. This is not a minor inconvenience — it is a serious safety hazard that FCA/Stellantis has already acknowledged in heavier-duty Ram platforms. MANUFACTURER KNOWLEDGE: FCA was aware of this defect as early as 2016 based on warranty claims and customer complaints. Despite issuing Recall 24V-896 for heavier Ram trucks with the same HCU failure, FCA has failed to extend recall coverage to the 2017 Ram 1500, leaving thousands of owners without remedy. FINANCIAL IMPACT: Repair estimates range from $1,200 to $2,000. The part (OEM #XXX) remains on backorder or engineering hold, making timely repair impossible through normal channels. REQUESTED ACTION: I respectfully request that NHTSA: 1. Open a formal investigation into the 2017 Ram 1500 ABS HCU failure 2. Compel FCA/Stellantis to expand Recall 24V-896 or issue a new recall to include the 2017 Ram 1500 3. Require FCA to reimburse owners who have already paid for this repair out of pocket I am happy to provide any additional documentation, repair invoices, or diagnostic reports upon request. Systemic Hydraulic Defect & Safety Recall Circumvention I am reporting a critical safety defect in the clutch hydraulic system of my 2013 Fiat 500 Abarth, which directly mirrors the failures described in NHTSA Recalls 16V-302 and 20V-124. Diagnostic measurements confirm a 1.23mm hydraulic over-travel in the clutch release bearing/slave cylinder assembly. This over-travel causes the diaphragm spr
... ing to be over-extended, leading to the catastrophic failure of the pressure plate and subsequent inability to disengage the clutch. This poses an immediate safety risk of vehicle surge or inability to shift gears while in motion. Despite this objective engineering data, Stellantis (FCA US, LLC) Executive offices (specifically Mike Macdonald) have formally refused to perform the safety recall repair, citing my status as the "5th owner" as a justification for exclusion. This represents a willful circumvention of a federal safety recall, as the hydraulic over-travel is a latent design defect present since manufacture and is entirely unrelated to ownership history or vehicle maintenance. The vehicle is currently available for inspection to verify these measurements.Read more

This vehicle safety complaint is regarding my 2019 Toyota Highlander XLE, VIN [XXX] . A Toyota-authorized dealership, Sansone Toyota in Avenel, New Jersey, documented that the front driver axle is prone to freezing in the differential. This is a serious safety concern because if the axle seizes while the vehicle is in motion, it could cause sudden loss of control. This vehicle is used regularly a
... s our family vehicle, including to transport our family of 6 with our young children. I am extremely concerned about continued operation of the vehicle while this documented condition remains unresolved. In addition to this safety-related axle issue, this vehicle has a broader history of serious mechanical problems. On May 13, 2022, after a recent service visit at DCH Brunswick Toyota, I reported that when I pressed the gas the vehicle was making an unusual noise like an old car, that the car delayed to speed when I pressed the gas, that I had to press the gas all the way in order for the car to move faster, and that it was not like this prior to service. At approximately 25,000 miles, I also asked that the brakes be checked because when we drove the vehicle and used the brakes, the car made a startling sound. In 2023, I also reported hard/heavy steering and jerking while driving. I reported transmission-related abnormal noises and drivability concerns while the vehicle was still within Toyota’s powertrain warranty period, but the condition was not properly diagnosed at that time. The transmission has since been confirmed by authorized Toyota dealerships to require full replacement. The vehicle also has a documented engine oil leak, leaking shock absorber, and prior recall history involving a stalling condition. I've filed this complaint with NHTSA due to the axle-related safety risk. Toyota is now on written notice of that condition. Mileage: approximately 101,000 Documenting safety issue: Sansone Toyota, Avenel, NJ Please review this potential safety defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more

I had this problem and it clearly states the year, make and model of my truck in the recall, but because my vin wasn't included I'm out of luck? I will attach all the info i have on this matter. Any help would be greatly appreciated. Thanks [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While driving at highway speeds on the interstate, the B-pillar exterior trim suddenly detached from the vehicle. The trim remained tethered only by the keypad wiring, causing it to flail in the wind and create an immediate road hazard. Simultaneously, the vehicle’s instrument cluster triggered a "Liftgate Open" warning, despite the liftgate being secure. I was forced to make an emergency pull-o
... ver onto the shoulder of the interstate—a dangerous situation—to prevent the trim from completely detaching and striking other vehicles. Upon reaching my destination, the vehicle suffered a significant electrical short. The interior lights remained on indefinitely due to the false "liftgate open" sensor reading, and the door keypad became completely inoperable. To prevent a total battery drain and potential fire risk from the short, I had to manually disconnect the battery. The dealership confirmed the failure but stated that current B-pillar recalls (such as Recall 25S53) currently only apply to 2016–2017 models, despite my 2019 model using the same defective clips and adhesive. This failure is identical in nature to Recall 24S02 (A-pillar detachment). The detachment of this trim is a documented projectile hazard. Furthermore, the resulting electrical short in the door harness has compromised the integrity of the vehicle’s sensor system. I am requesting that NHTSA investigate the expansion of the B-pillar recall to include the 2019 model year to prevent further highway incidents.Read more

The 2.0L Nu GDI engine in this vehicle is experiencing terminal internal failure. Following a P1326 KSDS warning event, a borescope inspection at an authorized dealership documented severe cylinder wall scoring in all four cylinders. Despite this documented internal block destruction, the manufacturer has denied the mandated settlement warranty repair based on a software-based proxy test (BCT). Th

The factory white paint on my 2014 Toyota Corolla has been peeling and delaminating in large sheets down to the primer and bare metal. This appears to be a paint adhesion failure, not normal wear or surface chipping. The vehicle has not been in any accidents, repainted, or subjected to improper care. The problem developed gradually and has continued to worsen over time. Large sections of paint det
... ach from body panels such as the hood, roof, trunk, and sides. Paint has come off while driving and during routine car washing. This condition presents a safety concern because pieces of paint can separate at driving speeds and become road debris, which could strike other vehicles or create hazards for nearby drivers. In addition, exposed metal body panels are vulnerable to corrosion, which may affect the long‑term safety and integrity of the vehicle. No warning lights or messages were present prior to the paint failure. A Toyota dealership visually confirmed the peeling paint and indicated this issue has been observed on other Toyota vehicles with white paint. No repair was offered without full repainting at the owner’s expense. The affected areas are visible and available for inspection upon request.Read more
