The control arm bearing appears to have failed without warning causing loss of steering control. This occurred at low speed on a street in [XXX] last night near a gas station, so the driver was able to drive to a safe location easily without risk to life. Had the failure happened at speed a possibly fatal collision would have been likely. The vehicle has been used for the past 8 months to drive t
... o work and has not been subject to any stress. It has not been modified. The tow truck driver said he has seen a number of other control arm failures recently, not necessarily in the same vehicle type. It may be due to bad pot holes this year in [XXX] , they are bigger and deeper than normal. This information is based on assessment by tow truck driver last night. Vehicle is at Jim Shorkey in Murrysville for repairs INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am reporting a safety concern following an over-the-air (OTA) recall update (Hyundai Recall #296) on my 2026 Hyundai Palisade Calligraphy AWD (software version 263Q.LX3.USA.ccNC.001.001.260326). I understand this recall was issued in response to a fatal incident involving the second-row seat system, which is why this issue is especially concerning. Since the update, the second-row walk-in seat f
... unction used for third-row access is not operating correctly. When I press the button near the headrest, the seat does not move fully forward, and I have to use the manual slide forward/back button to move it the rest of the way. When returning the seat, it does not go all the way back, and the seatback remains tilted forward at about a 70–80 degree angle instead of returning to its normal upright position, requiring manual adjustment using the recline button. In addition, during one occurrence when the second-row seat was activated, the front passenger seat moved slightly on its own without any input. This unexpected movement of another seat is especially concerning. This behavior began only after the recall update and appears to be a malfunction, possibly related to seat calibration or software coordination between seat systems. This creates a potential safety issue involving unintended seat movement and improper seat positioning.Read more

In November 2025 a new transmission was installed Curry Honda in Atlanta GA on my 2020 Honda CR-V. The new transmission came with a 3 year/36,000 mile warranty. In March 2026 my vehicle broke down and was towed to Tameron Honda in Daphne Al where a speed sensor was replaced at my expense of 388.44. The adaptive cruise control was not working prior to this repair. Within two weeks the vehicle ex
... perienced further issues and had to be driven in a compromised condition back to Curry Honda in Atlanta where the transmission wire harness was diagnosed as corroded and replaced at a cost of 1696.99. My vehicle has been garage-kept its entire life except when in the care of a dealership. Corrosion on a recently replaced transmission in a garage-kept vehicle cannot be attributed to environmental exposure or owner neglect. It points directly to a defective part of improper installation at Curry Honda. These failures occurred within months of a new transmission installation with no prior history of issues. I believe this is a safety defect as the vehicle broke down unexpectedly and had to be driven in a compromised condition posing a safety risk.Read more

I am filing this safety complaint on behalf of myself and my wife, [XXX] , the primary driver of our 2021 Ford Escape PHEV, with active under Recall 25SC4 (no remedy available). The vehicle has experienced 8+ sudden complete shutdowns during normal driving since February 2026, always during EV-to-hybrid mode transition, always without prior warning. The only warning is a brief sound like air escap

At shutdown: engine cuts completely, steering wheel locks rigid and cannot be turned, brake pedal locks hard and cannot be depressed, gear selector locks in Drive and cannot be changed. The driver has zero ability to steer, brake, or move the vehicle. Must activate hazards and wait 1-5 minutes for restart while immobilized in an active traffic lane. Incidents occurred: (1) February 2026: multiple times, dealer retained vehicle 5 days, found nothing. (2) Early April 2026: multiple times, including one witnessed by a police officer who directed traffic around immobilized vehicle. Dealer kept 3 days, blamed the original Ford-supplied charger. Replacement did not fix defect. What component or system failed? The inner brake pad material on the front wheel of the door side and off door side separated. The material did not come unbonded from the pad plate, the material itself separated. I have the pads and pictures of the failure. How was your safety put at risk? The material could have become jammed in the brakes. This could have caused the single brake to lock up (pos
... sible loss of control), cause a brake to overheat to failure, or catch the tire on fire. The material could have totally come loose. This could have caused a brake line to be severed causing loss of brakes or, due to loss of pad material, a reduction in braking capability. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, the technician working on my vehicle stated he has seen this brake pad material failure on Kodiak pads multiple times. Has the vehicle or component been inspected by the manufacturer? Not yet. ISSUE 1 – VOLVO ON CALL / VCM "SERVICE REQUIRED" MESSAGE: My vehicle has displayed a persistent "Volvo On Call Service Required" error message in the Driver Information Module (DIM) for over one year. This is directly covered by: • Volvo Technical Journal TJ 36916.1.0 (NHTSA ID: MC-10253788), issued April 2024 • Fault code: VCM-U105000 (Vehicle Communication Module) • TSB explicitl
... y covers 2018 S90 vehicles • The TSB states the required software packages are free of charge I contacted Volvo Cars USA at [XXX] , who acknowledged the issue and arranged a complimentary diagnostic and fix with the Manasquan dealer. But, the dealer would not perform the diagnostic at no charge, leaving me unable to confirm the fault code or proceed with the TSB-specified remedy without paying out of pocket. I have had this unresolved warning message for over one year now. The dealer told me that if I did not pay for the repair my car could lose power and stop suddenly. ISSUE 2 – PARK ASSIST SYSTEM FALSE ALERTS: My Park Assist System sensors generate false proximity warnings for objects more than 20 feet away — far beyond the system's normal detection range. Cleaning the sensors did not resolve the issue. This is directly covered by: • Volvo Technical Journal TJ 31558.8.0 (NHTSA ID: 10161789), issued April 2019 • Root cause: Water ingress into park assist sensors and connectors due to a defective splice in the cable harness • Volvo introduced a harness splice correction in production starting June 2018 — meaning vehicles built before that correction may contain the defective harness • TSB covers 2016–2019 Volvo vehicles A malfunctioning park assist system is a direct safety concern — drivers rely on accurate proximity alerts when maneuvering, and persistent false alerts undermine confidence in the system and may cause drivers to disregard genuine warnings. What component or system failed or malfunctioned, and is it available for inspection upon request? As I attempted to leave a grocery store, less than 5 minutes away from home, the steering wheel was extremely difficu
... lt to turn. I didn't see any leaks under the hood nor on the ground underneath my car. Nor was there any smells nor odors. On 5/1/2026, I did hear a slight noise when I turned the steering wheel to back into the driveway at home. How was your safety or the safety of others put at risk? Steering wheel became difficult to turn right or left. I barely made it home to park it, as the longer I drove to get home, the more difficult it became to make turns. I parked it immediately as I didn't want to make too wide of a turn on a road, risking any accidents. Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet. I will need the car towed to a local Ford/Lincoln dealership for assessment. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Not yet, in the process of contacting my auto insurance about it. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The message on the dashboard was "Service Power Steering" and it appeared on 5/2/2026 for the very first time. Also, the power steering icon showed in a stable amber color. This message did not appear before.Read more

4/26: I was in the turn lane and SLOWLY approaching the vehicle in front of me. I was at least 12 feet back when the emergency brake engaged causing me to fling forward in my seat. A pedestrian was passing at a

There two incidents have occurred within 2 weeks of purchase of my car. There is a feature to temporarily turn off automatic brake, but resets with each restart of the engine making it inconvenient to apply. Also, it is easy to forget to apply each time you start the car. I’ve also read since my incidents other reports of Hyundai drivers saying the disable feature does not always work and automatic brake engages. I feel that the automatic braking is a HUGE safety risk for driver, passengers, pedestrians and other vehicles on the road. I frequently drive to another state via highway to visit my mother and a now terrified to drive that distance considering all of the accelerations and decelerations involved with highway travel. I feel completely unsafe in my new vehicle. Please review and encourage a software recall that completely removes automatic brake or allows drivers the option to disable and enable at will.Read more

Failed Head Gasket in the engin affected fuel injectors and spark plugs to fail or not perform normally and started misfiring in the engin. Upon extensive search on internet and social media , it has been found that there are hundreds of victims who own the same year vehicle & model ( or same engine types ) and faced same issues and paid too much money to repair even though the Honda is aware of the issue and not taking care of it. NHTSA Complaint SummaryVehicle: 2017 Ford Escape SEEngine: 1.5L EcoBoostVIN: [XXX] Defect: Engine Block Design Flaw / Coolant IntrusionTechnical Description of the Issue:My vehicle is experiencing a well-documented manufacturing defect specific to the 1.5L EcoBoost engine. The engine block was designed with a "v-groove" between the cylinders that is prone to structural failure. This allows engine
... coolant to leak into the combustion chambers (coolant intrusion) and allows engine oil to contaminate the cooling system. My vehicle currently exhibits "milky," tan-brown sludge in the coolant reservoir, confirming that oil and coolant are cross-contaminating due to internal engine failure.Extreme Safety Hazards and Dangers:This defect creates several critical safety risks for the driver, passengers, and other motorists:Sudden Loss of Power at High Speeds: The mixing of oil and coolant destroys the engine’s ability to lubricate internal bearings. This can lead to a catastrophic engine seizure while the vehicle is in motion, causing an immediate loss of power and power steering, which creates a high risk of collisions on highways.Engine Fire Risk: When coolant enters the combustion chamber, it can cause severe engine misfires and overheating. Overheated oil or pressurized coolant spraying onto a hot exhaust manifold during a mechanical failure presents a significant fire hazard.Hydro-locking: If coolant floods the cylinder while the vehicle is in operation, the engine can hydro-lock, causing the engine to stall instantly and potentially throwing metal debris onto the roadway, endangering other drivers.Visibility Hazards: Excessive white smoke from the exhaust (caused by burning coolant) can suddenly obscure the driver’s rear-view visibility and the visibility of trailing motorists. Conclusion:Ford Motor Company was aware of this defect, as evidenced by Customer Satisfaction Program 21N08 and TSB 19-2346. Vehicle E-Clutch system malfunctioning. When riding the e-clutch warning lights will flicker and cause the bike to jult uncontrollably for about 3 seconds. It happens at random, no one situation is the same, it could be in the middle of a turn, it could be at a stop light, it could be getting up to speed, etc. This has happened over 10 times while riding and after taking it to the dealership they
... were unable to reproduce the warning lights and the issue didn't show up when running the codes. After picking it up for the second time, the malfunctioning started again within the first 20 minutes of riding it. This is not just a user error, or a operating error like the dealership tried to tell me it was. This is a bigger issue. This almost cost me my life and I won't let this be brushed under the rug. This specific incident was the worst of the bunch. It was about 6 hours after I picked the bike up from the dealership the 1st time. I was out riding with a friend making frequent stops at resturaunts and gas stations. The E-clutch light had already been on at this point but per the dealership they told me it should still be safe to ride. Obviously not. After our last stop of the day, the bike decided (as I was getting up to speed) to junk uncontrollably for about 3 seconds, shut the engine off, then turn the engine back on and accelerate. At this point I hit the kill switch and glided to a safe spot to pull over and assess the issue. This could have easily costed my life if I had been at a higher speed. The repair order attached is from the 2nd repair. The first repair was able to get covered by warranty I guess so I didn't get any paperwork.Read more

I am filing a safety complaint regarding my vehicle following service performed at a Kia dealership. My vehicle was returned to me on April 25, 2026, after being at the dealership for approximately one week. When it was returned, it had missing bolts/screws that were not missing prior to the service visit and were not related to the reason I originally brought the vehicle in. The missing compone
... nts create a safety concern, and I do not feel safe operating the vehicle in its current condition. This issue was reported immediately. My husband left a voicemail with the dealership (Evans Kia) reporting the problem, and I reported the issue to Kia America that following Monday. Since then, I have made multiple attempts to resolve this issue through Kia America. I opened a case on April 17, 2026, and have followed up several times. The case has been escalated multiple times (including April 22 and April 28). I was informed that Kia America also reached out to the dealership, but despite this, I have not received any direct response from the dealership or instructions to bring the vehicle in for repair. Despite clearly stating that the vehicle may be unsafe to drive, there has been no urgency in correcting the issue. I am requesting that this matter be reviewed as a potential safety concern involving improper vehicle servicing and release of a potentially unsafe vehicle. I can provide photos and documentation showing the missing bolts/screws if needed.Read more

I purchased a 2025 Rivian R1S Adventure (VIN: [XXX] ) new from Rivian. Since the first week of ownership, the vehicle has exhibited persistent pulling, drifting, and handling instability requiring constant steering correction. I have brought the vehicle to Rivian Tampa for five service visits over five months. The defect has never been resolved. What I am requesting: Rivian's own technicians have
... twice attributed this vehicle's handling problems to the factory-installed tires, and confirmed via wheel-swap testing that tire pull is the root cause. These are factory-installed tires I did not select or modify. I am requesting: (1) replacement of all four tires with a highway-appropriate set at $0 cost; (2) a four-wheel alignment after installation at $0 cost; and (3) reimbursement of $61.81 charged for a tire rotation that could not address the confirmed defect. Rivian has acknowledged enough of a problem to waive $717 in labor and convert the alignment to warranty — yet refuses to cover the tires that their own technicians confirmed are defective. That is not an acceptable resolution. I have complete documentation including all work orders, invoices, and service records supporting every claim above.
