This vehicle is a 1 year old (2025) Honda Odyssey with roughly 11,000 miles. On two separate occasions now, the auto start-stop feature has caused the engine to die altogether. On both occasions, the vehicle stopped at a red light in a busy intersection, the auto start-stop feature turned off the engine (intentionally), but when the light turned green and my foot came off the brake and onto the ga
... s pedal, the vehicle died entirely. A number of lights and an error message illuminated on the dash board. From my online searches, it seems as though this is a very common issue. 1. What component or system failed or malfunctioned, and is it available for inspection upon request? Auto start-stop / engine. No not available. 2. How was your safety or the safety of others put at risk? Having your vehicle die at a busy intersection or stop sign is a huge risk. It can cause others to try and swerve around you, or it can cause someone not paying attention to rear end you. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle will be brought to the dealer in the coming days. Unfortunately this is my only vehicle and I cannot afford to lose it for an extended amount of time, therefore I have to prolong inspection and servicing. The issue has happened to me twice, and has happened to countless others online. According to others on the internet, the dealers are typically slow to address this issue, and even when "addressed" the issues seem to continue. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No, see #3. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warnings prior to the engine cutting out altogether. Only a message after the failures. In both cases I do not remember what the messages said because I was scrambling to restart my car in a busy intersection.Read more

Over a year ago, my vehicles engine began ticking. On 12/14/2024, while driving the vehicle, it developed a rod knock, went into a limp mode and was towed back to our house. I eventually took the vehicle to Honda in Beaumont, Tx where they did an inspection of the vehicle and verbally told me it was the crankshaft, but would not provide me any documentation stating that. According to the lady at t

I was quoted almost $10,000 to replace the engine with a salvage yard engine with almost as many miles on it. Hello, I has problems driving my car like by brakes braking out of nowhere. There was even a couple times I felt like the car swerved, even though I did not do anything to the steering wheel. Then I found out I had a recall that said not to drive the car because it was dangerous so I parked it in my driveway and I have not driven it for many months as I’ve been getting rental cars. Eventually, I
... brought it into Antelope Valley Chevrolet and they told me they were going to fix it but then they actually told me that I had to pay around $1000 for a part that I found out later only cost under $100 and takes three minutes to put in. This is apart from Chevrolet and not some knock off from China that I found for under $100 online. So I’m a little upset that I came in for a recall and they’re trying to get me to do a repair for almost $2000. They never even offered me a rental car and they’re threatening me saying that if we don’t move forward with it, they’re gonna charge me $50 a day for storage. I feel like they’re intimidating me to try to make me traded in for a new one, but I am not ready to get a new one. I just want to get my car on the road again.Read more

LIFTGATE MALFUNCTION
Incident date: Feb 23, 2026
See attached document for complaint. The undersigned is legal counsel representing XXXXXXXXXXX in the above-mentioned matter. Please be advised that XXXXXXXXXX purchased a new Kawasaki 2024 motorcycle model ZX636JRFBN on May 9, 2024 with total purchase price of $13,981.92. On or about April of 2025, XXXXXXXXXXX received a recall notice with follow up notice in June of 2025 NHTSA Recall No. 25V376
... “Crankshaft Bearing Seizure” causing the engine to potentially “lose engine power unexpectedly due to a seizure of the #5 crankshaft bearing” that may lead to an increased risk of a crash. Given the nature of the recall, the notice is marked with a “DO NOT RIDE” advisory until the repair is made and that the customer is to contact a dealer for such repair. Sometime on or about late August of 2025, the motorcycle was inspected at a Kawasaki dealership in an effort to address the recall. This was the earliest that the dealership could address the recall nearly three months after getting the “DO NOT RIDE” notice. The mechanic who examined the motorcycle indicated that it did not appear that the bike had the defect. In November of 2025, an additional recall notice “Crankshaft Bushing Seizure” was issued under Recall # 25V798 since the notice acknowledges that a prior repair on the vehicle may not have addressed the defect. A second letter was issued in November of 2025 under the same recall notice indicating the parts needed for the repair are not available. Finally, another notice was issued under the same recall, 25V798. Again a “DO NOT RIDE” warning is issued on this notice. The letter advises that the “method used to torque the crankcase bolts will be different than the one used in the prior recall “and that the recall applies to “vehicles already remedied under recall campaign MC25-08 (NHSTA Recall #25V376). Once again there are instructions to contact a dealer for repairs. It is presumed that parts are now available to attempt another repair. I am submitting this complaint regarding Recall 78C (NHTSA Recall 25V-766) affecting my 2024 Jeep Grand Cherokee 4xe. This recall has now been open for approximately 120 days without a remedy available. Stellantis has confirmed that engineers are still

Since purchasing this vehicle in May 2024, it has been subject to five recalls within approximately 18 months of ownership: Recall 78C – NHTSA Recall 25V-766 Recall 68C – NHTSA Recall 25V-741 Recall 73C – NHTSA Recall 25V-576 Recall 28C – NHTSA Recall 25V-472 Recall D5B – NHTSA Recall 24V-944 Due to the severity of the defect described in Recall 78C and the number of recalls affecting this vehicle, my family has lost confidence in the safety and reliability of the vehicle. My wife is currently unwilling to ride in the vehicle because of the risk described in the recall. With a newborn child arriving next week, this situation has placed significant strain on our household because we are effectively operating with only one vehicle. Earlier this week, the vehicle began shaking and intermittently accelerating and decelerating on its own while in operation(I have video proof of this) This behavior poses a serious danger to me and to others on the road. Due to these ongoing issues, I filed a formal complaint with Stellantis and was assigned case number [XXX]. I have been working with Stellantis representative Steve Stander; however, communication has been extremely difficult. Unfortunately, Mr. Stander has not been responsive to phone calls or emails in a timely manner, which has made resolving this matter challenging. At this time, the vehicle is once again in the shop, leaving me without transportation. During our last discussion, Mr. Stander acknowledged that Stellantis understands the vehicle is not dependable and offered $10,000 in down payment assistance along with employee pricing on a new Stellantis product. However, I currently still owe approximately $3,400 on the lease. Given the serious safety issues and repeated failures of the vehicle, I am requesting Stellantis’ assistance in grounding the lease and covering the remaining $3,400 balance so that I may exit this lease. Incident date: Feb 3, 2025
I am reporting a premature engine failure on my 2019 Mazda CX-5 with approximately 55,000 miles. The vehicle developed a significant engine oil leak that was diagnosed by a repair facility as a casting failure in the cylinder head behind the timing cover. Due to the location of the leak and the nature of the defect, the entire cylinder head assembly had to be replaced. The total repair cost was $
... 5,262.32, which I paid out of pocket. The repair documentation indicates that Mazda has issued a Technical Service Bulletin addressing this condition, but there is currently no recall or warranty extension covering the issue for my vehicle. A cylinder head casting failure at such low mileage appears premature and not consistent with normal wear and tear on a properly maintained engine. An engine oil leak of this nature could potentially create safety risks, including oil contamination of engine components or the possibility of oil leaking onto hot engine parts. Because this issue appears to be documented by Mazda through technical service guidance and has resulted in a costly major engine repair at relatively low mileage, I believe it should be investigated to determine whether a broader manufacturing defect exists that could affect other vehicles. I am submitting this complaint so that the issue is documented and can be evaluated for potential safety concerns or a future recall if similar failures are occurring in other vehicles.Read more

The car had a piston rod failure and is available for inspection at the Springfield Honda dealership. The car was traveling at 63 mph when the check engine light started flashing along with 6 system errors including the power steering. I had to make an emergency stop on the side of the road. The dealership has confirmed and recommended an engine replacement. They have a video recorded of the sou
... nd and error codes. The only person to have inspected the car is the dealership. We were told that it wouldnt be covered because our VIN didnt fall with in the affected vehicles for the piston rod recall. The car was driving normal and had no service lights on prior to the check engine light coming on during the incident.Read more

Vehicle Safety Complaint - Tesla Cybertruck Steering System Failure During FSD While driving my Tesla Cybertruck with Full Self-Driving (FSD) engaged, the rear steering system failed without any prior warning. FSD immediately disengaged, and a red steering system ERROR appeared on the instrument cluster. The rear axle steering was completely disabled, turning radius became severely limited, and ve
... hicle speed was restricted. This occurred while actively driving in traffic and was extremely dangerous. I accessed the vehicle’s Service Mode and documented three fault codes that triggered simultaneously at 17:48:14 on March 4, 2026: 1.RSAM_a260_monitorTripDetected ∙Monitor trip detected, system degraded ∙Potential Impact: Steering system performance degraded, vehicle speed limited ∙Only clears on next power-up 2.RSA_a212_activeFault ∙Rear Steer Actuator (RSA) stopped at current position ∙Internal condition detected, defaulted to safe state ∙Fault Reason: CONTROLLER_IRRATIONAL ∙Previous State: RSASTATE_ACTIVE_STEERING (was actively steering when failure occurred) ∙rsamControllerRational = false 3.PEPS_a182_rearSteerDegraded ∙Primary Electric Power Steering (PEPS) detected RSA not functioning as expected ∙Reason: RSA_UNHEALTHY ∙rsaUnhealthy = true ∙rearSteerCommand = -0.59 All three faults were set at 17:48:14 and cleared at 17:50:55. Additionally, an earlier alert — EGGRIGHT3_a203_airwaveLeftLateralUnavailable — was logged on March 2, 2026 at 21:11:08, which may indicate a related or recurring issue. The simultaneous failure of the rear steering actuator and disengagement of FSD while driving at speed represents a serious safety hazard. The RSA controller was flagged as “IRRATIONAL” while the system was in active steering mode, meaning the steering hardware failed during operation — not at startup or in a parked state. I have photographic documentation of both the dashboard steering ERROR warning and all Service Mode diagnostic screens showinRead more

I own a 2022 Jeep Grand Cherokee that is experiencing unintended automatic braking while driving. The crash detection/forward collision warning system shows a constant warning light on the dashboard. While driving under normal conditions with no obstacles present, the vehicle will suddenly brake on its own. This has occurred multiple times and now happens almost daily. The problem first occurred o
... n November 26, 2024, the same day I purchased the vehicle used with approximately 34,158 miles. The issue has continued as the mileage has increased to approximately 51,500 miles. This creates a serious safety risk because the vehicle can suddenly brake in the middle of traffic without warning, which could cause another vehicle to collide with the rear of my vehicle. I am concerned about the risk of a crash or injury. The problem has been reported to the dealership multiple times since purchase. The vehicle has been in the shop for repair attempts, but the issue has not been permanently corrected. The warning light remains on and the unintended braking continues to occur during normal driving. The vehicle is currently available for inspection.Read more

I am reporting a suspected defect involving the front suspension or steering system of my 2026 Tesla Model Y AWD. The vehicle produces loud popping, knocking, and rattling noises from the front end when turning the steering wheel at low speeds, particularly when the steering approaches full lock in either direction. The issue is most noticeable when entering or exiting uneven surfaces such as slo
... ped driveways or parking lot entrances where the suspension is under load. During these conditions, the front suspension produces distinct mechanical popping or knocking sounds that appear to originate from the front wheel or suspension area. The noise does not occur while driving straight. It becomes significantly more noticeable when turning left or right at low speeds and worsens as the steering approaches full lock. In addition to the noise, a noticeable mechanical jolt or feedback can be felt through the steering wheel when the popping or knocking occurs. This issue began suddenly without any warning signs. One moment the vehicle was operating normally, and the next the popping and knocking noises began. Since that time the issue has continued consistently during turning maneuvers. The vehicle has not been involved in any collision and has been driven under normal conditions. Numerous other 2026 Tesla Model Y owners have reported similar front suspension symptoms, suggesting this may represent a broader pattern of potential suspension or steering component issues. Because the noise and feedback occur through the steering system while the suspension is under load, this raises concern that a suspension or steering component may be shifting, binding, or prematurely wearing. This condition raises safety concerns because abnormal movement or failure of steering or suspension components could affect vehicle control during turning or emergency maneuvers. I request that this issue be investigated for a possible defect trend affecting Tesla Model Y vehicles.Read more
