Vehicle produced symptoms over time: Loss of Ford Pass connectivity, Inconsistent turn signal function, occasional dashboard alarm for Open Driver Door while driving, unwarranted security alarm triggers when still at home. Second to last incident was an unwarranted security alarm that triggered, and was unable to be stopped by use of the security controls (lock/unlock, alarm key button). Horn "w
... ore out" as if fuse burned. Disconnected battery to stop. Two days later, after reconnecting the battery I drove the vehicle, Horn was not functional, and I was unable to stop the engine. Once started, the vehicle did not turn off, even with key completely removed from ignition switch. This was repeatable, every ignition cycle, thereafter. I drove to the dealer, that morning. The dealer mechanic was required to shut off the engine. Dealer mechanic diagnosed failed Battery Junction Box. After Repair, Mechanic diagnosed faulty Body Control Module. Dealer mechanic also recommended replacement of full "Main" wiring harness, interconnecting the two. The cause for all failures was corrosion and electrical shorts due to water ingress. I am aware Ford has issued numerous TSBs and recalls for faulty BCMs, in the same age range as this vehicle. Also, TSBs for water intrusion into the BCM. The mechanic took photos of corrosion on both the BCM and main wiring harness. My prior vehicle was a 2011 Ford F-150, with the same 6.2L engine. No such faults were experienced up to Nov. 2023 sale. Photos of the corrosion on main wiring harness and BCM attached. Dealer has possession of the vehicle. Repairs in progress. Ford has declined support for faulty components; Recalls not associated with this vehicle. Estimated repairs to date on 6 year old vehicle on critical electronics in excess of $7,000.Read more

To the Office of Defects Investigation National Highway Traffic Safety Administration I am submitting this complaint to formally report a serious and recurring safety defect involving the automatic braking and collision detection systems in our Mercedes-Benz GLC. License Plate: 9RHN710 Dealer Servicing Vehicle: Mercedes-Benz of Stevens Creek (AutoNation), San Jose, California The vehicle has re
... peatedly experienced unexpected and unprovoked braking events, including situations where the automatic braking system engages despite there being no vehicles, pedestrians, or visible obstructions present. The issues observed include: • Sudden automatic braking activation without obstruction • Unexpected vehicle deceleration during normal driving conditions • Repeated dashboard alerts indicating “Speed Severely Restricted” • Repeated Attention Assist / “Coffee Break” warnings during normal local driving • Sudden brake engagement when parking, accompanied by a loud mechanical noise These events create a dangerous driving condition, as unexpected braking could lead to a rear-end collision from vehicles traveling behind the driver. During one incident, the abrupt braking caused a severe jolt to the driver, resulting in whiplash symptoms that required medical evaluation and MRI imaging, which identified cervical strain. The vehicle has been brought to the authorized dealership multiple times for inspection and repair attempts. However: • The dealership has not been able to diagnose or repair the issue • Service visits have largely been documented as general inspections • No ADAS (Advanced Driver Assistance Systems) diagnostic or calibration report has been provided, despite the fact that the malfunction appears directly related to radar, camera, or collision prevention systems. We respectfully request that the Office of Defects Investigation review this complaint and determine whether further investigation into this issue is warranted.Read more

See attached document for complaint. I purchased a certified pre-owned Nissan Leaf SV Plus on [XXX] specifically for the larger battery capacity and the ability to use a DC fast charger, as this would allow me to drive my electric vehicle wherever I needed to go, including regular trips to Seattle from Mount Vernon. I was notified by Nissan in September 2024 that, due to the possibility that the
... battery could catch fire while using a DC fast charger, that my vehicle’s battery had been recalled and was instructed not to use a DC charger. They stated a remedy would be available in October 2024. When the date came and went, I contacted Nissan about the issue and the date was pushed to November 2024. Then December 2024, then spring 2025, then fall 2025. After numerous contact attempts to replace my battery, I initiated a buyback with Nissan on 10/16/25. I unexpectedly required medical treatment in October and contacted the Nissan rep on 10/28/25 to inform them of my medical issue and asked they contact my husband. My husband also attempted to contact the rep w/ no response. After coming back home, I found out that my claim was closed—Nissan said I failed to respond. Now, my doctors want me to receive further medical treatments in Seattle 3-5 times per week. At this time, my vehicle cannot transport me to the required appointments since I cannot use the fast charger. My medical treatment is delayed until Nissan resolves this! My teenage daughter regularly drives this vehicle. The vehicle began experiencing significant oil consumption, oil leaks, and intermittent loss of power. During an early inspection at an independent repair facility, a check engine code P1326 was identified. Due to the symptoms and diagnostic code, the repair facility advised that the vehicle could be heading toward engine failure and recommended t
... hat the vehicle be taken to an authorized dealership because potential engine issues may fall under manufacturer warranty coverage. The vehicle was then towed to an authorized dealership. During the first visit, the dealership retained the vehicle for an extended period of time, approximately two months. During that visit, several recommended services were completed, including replacement of spark plugs, ignition wires, and replacement of the valve cover gasket along with other recommended maintenance. During that visit, it was also stated that the vehicle has a known issue with excessive oil consumption. Because of this, the oil level is checked weekly and oil is added as needed. I also informed the dealership that the independent repair facility had identified code P1326, but I was told that the dealership diagnostic codes were different. After these repairs, the vehicle experienced another incident where it lost power and began idling roughly while being driven on a busy highway. The vehicle was taken back to the dealership. During that visit, it was stated that the fuel injector harness and oil pressure sensor needed to be replaced. The dealership also indicated that the spark plugs needed to be replaced again, even though they had been replaced at the same dealership less than a year earlier. At that time, I asked if the vehicle would be safe to drive for approximately one week because I needed to travel out of town and my daughter would need the vehicle. The dealership advised that it was safe to drive during that time. The vehicle was then brought back to the dealRead more

I was not aware the recall existed at the time of my accident, and the recall remedy was not completed on my vehicle until February 12, 2026 — nearly six months after the incident. I filed a claim with Toyota Motor North America. They inspected the vehicle on February 11, 2026, found the camera functioning at that time (after the recall had since been remedied), and denied my claim. They cited owner's manual language advising drivers not to rely solely on the backup camera, but did not address the fact that the recall was unrepaired at the time of the accident. Additionally the parking collision warning and automatic braking should have engaged. The rock impacted the car right near one of the car cameras....so the collision warning and braking should have engaged. I am filing this complaint because I believe the unrepaired recall directly caused my inability to see behind my vehicle, and that Toyota's delay in notifying me and completing the remedy contributed to this incident. I am requesting that NHTSA review whether Toyota's notification and remedy timeline for Recall 25TA13 was adequate, and that this incident be added to the record of complaints associated with that recall.Read more

My 2019 BMW X3 (G01 chassis) displays a persistent "Emergency Call System Malfunction" warning on the instrument cluster that cannot be dismissed or reset by the owner. The warning appears every time the vehicle is started and remains on the dashboard throughout operation. This is a known defect caused by failure of the Telematics Communication Box (TCB) module, which is commonly triggered by wat
... er intrusion through a defective seal on the roof-mounted shark fin antenna housing. BMW has acknowledged this issue through Technical Service Bulletin SIB B65 12 22 (Roof-Mounted Antenna Housing Seal Not Adhering), yet has not issued a recall or offered a no-cost repair to affected owners. This defect presents the following safety concerns: 1. The persistent, non-dismissible warning light desensitizes the driver to critical dashboard warnings, undermining the purpose of the vehicle's warning system. When drivers are conditioned to ignore a permanent warning, they are more likely to overlook new, actionable safety alerts. 2. The emergency call (eCall) system itself is rendered non-functional, meaning the vehicle will not automatically contact emergency services in the event of a serious collision where the driver is incapacitated. 3. The warning creates a visual distraction on the instrument cluster during driving. BMW's only offered remedy is full replacement of the TCB module at a cost of $1,500–$2,500 to the owner, despite this being a manufacturing defect (faulty antenna seal adhesive) and not a wear item. The defective component is located under the headliner and is not accessible for owner maintenance. This issue is widespread across 2017–2023 BMW models equipped with the shark fin antenna, including the X3, X4, X5, X6, X7, 3 Series, and 5 Series. A class action lawsuit has been filed against BMW of North America regarding this defect. Numerous complaints have been submitted to NHTSA and documented extensively on BMW owner forums.
