The contact owns a 2013 Kia Optima. The contact stated that while driving 75 MPH, the vehicle lost motive power. The accelerator pedal was depressed; however, the vehicle eventually stalled on the side of the road. The key, battery, oil, and engine warning lights were illuminated. There was an abnormal ticking sound coming from the vehicle. The contact stated the failure was related to NHTSA Campa
... ign Number: 23V652000 (Service Brakes, Hydraulic). The vehicle was towed to an independent mechanic, where it was determined that the engine had seized. The engine failed to turn over. The mechanic had previously performed a routine oil change. The vehicle was towed to the local dealer, where it was diagnosed with low voltage. The spark plugs were checked, there were no oil leaks, and the vehicle was diagnosed with catastrophic engine failure. The vehicle was not repaired. The manufacturer was contacted, but no assistance was provided. The failure mileage was approximately 111,862.Read more

My 2017 Subaru Forester Touring (VIN: [XXX] , 118,200 mi) has a recurring EyeSight failure where the system becomes disabled and will not complete camera registration or calibration. Subaru of Sterling (VA) diagnosed active code B28A0 “Vehicle Model Judgment” and noted that the EyeSight stereo camera would not accept vehicle model registration. The technician documented: “Error for camera ad
... justment incomplete. When attempting to relearn UI for Eyesight camera, system is not taking vehicle registration; suggests camera replacement and perform recalibration.” This occurred after a confirmed DCM/STARLINK parasitic draw had caused repeated low‑voltage conditions until the dealer reset the DCM to factory mode (per the RO: “No further draw present at this time”), suggesting EyeSight registration may have failed due to prior voltage instability rather than true hardware failure. Subaru's service flow for B28A0 indicates this code can occur when the model write/registration is incomplete, interrupted, or mismatched—not necessarily when a camera has failed. Subaru lists causes such as: CAN communication issues, incorrect model registration, incomplete write due to ignition timing, or power/fuse issues during aiming (EyeSight diagnostic manuals). I am reporting this to NHTSA because EyeSight is a safety-critical system that governs adaptive cruise, lane keep assist, and pre-collision braking. My dealer is recommending camera replacement without documenting that Subaru’s required pre‑replacement steps (model registration inspection, CAN/LAN re-scan, voltage and fuse verification, genuine-part verification, etc.) have been completed. I request ODI review because EyeSight should not fail to register unless hardware is defective or a procedural/voltage issue exists. I want this documented in case this represents a broader pattern of EyeSight registration/calibration failures on Forester models using the same stereo camera platform. I am submitting this complaint regarding a safety concern with my Lexus IS 250. The dashboard in my vehicle is melting and has become extremely sticky, shiny, and reflective. As the material deteriorates, it creates significant glare on the windshield when sunlight hits the dashboard. This glare interferes with my visibility while driving and creates a potentially dangerous situation for myself, m
... y family, and other drivers on the road. This issue is not merely cosmetic. The deteriorating dashboard material directly affects safe vehicle operation by impairing the driver’s ability to clearly see through the windshield, especially during daylight conditions when sunlight reflects off the dashboard surface. In addition, the dashboard material appears to be chemically breaking down. The surface remains sticky and continues to degrade over time, raising concerns about possible chemical exposure inside the vehicle cabin for my family I contacted my local dealership and also reached out to **Lexus corporate customer service. I was informed that Lexus previously had a Customer Support Program addressing melting dashboards, but the program expired in September 2021. I was told notification letters were sent in March 2019, but I never received one. Had I received the notice, I would have addressed the issue because it directly affects driving safety. They said there was nothing else they can do. Given the ongoing safety risk, I respectfully request that the **National Highway Traffic Safety Administration review this defect and determine whether further investigation or action is warranted. Engine with leas than 40,000 miles is consuming one (1) quat of oil every 2,000 miles. GM dealership showed me the internal GM service update stating the accepted rate of oil consumption for engines uses in the vehicles referenced is 1qt per 2,000 miles. That may seem acceptable because GM put it in a notice to dealerships but I assure you the end consumer would never buy this engine if this was
... explained prior to purchasing. There is a reason new cars to not consume oil (at least not at a level you need to add oil between oil change intervals) at this rate. Long term wear and tear on the engine - note Gm offered a warrantee covering 100,000 miles, which is nice but the problems will become more severe over time and miles. I consider this borderline criminal as it will surely cause engines to lock up.Read more

I own a 2021 Tesla Model Y with approximately 46,500 miles. The vehicle is currently displaying battery error code BMS_a079, and the battery will only charge to approximately 30%, which severely limits the driving range and makes the vehicle unreliable for normal use. I reported this issue to Tesla on March 6, but the earliest service appointment available to me is March 17 at the Tesla service c
... enter in Fayetteville, Georgia. Because Tesla service centers are not widely available, I must travel to another city to have the vehicle inspected. The limited battery charge creates a difficult situation because I must determine how to get the vehicle safely to the service center with very restricted range. I may have to arrange towing or attempt to drive a vehicle that cannot fully charge. This issue has also created a hardship for my household. I have a toddler and normal daily responsibilities, including errands and transportation needs, but the vehicle cannot reliably be used due to the charging limitation. Being without dependable transportation for an extended period of time places families like mine in a very difficult position. This problem appears to be related to the vehicle’s battery management system and is not the result of driver misuse. Consumers should not be placed in situations where a major battery fault leaves them with limited transportation options while waiting extended periods for service, especially when service centers are not easily accessible.Read more

My car was just fine, then I noticed white smoke coming from the exahust pipe. It started to smell like antifreeze. Then the next day my antifreeze level got really low fast and over heated. While driving on the highway at 65 miles per hour, my Atlas shut itself off. I had to put it into park and restart the ignition. It turned on after 2 attempts but just barely. It turned on and gave enough power to move forward very slowly but virtually no propulsion. It turned on briefly, I pressed the gas and then it shut off but had enough to cruise to get to the shoulder across the highway. T
... here was no shoulder next to the lane I was in. Luckily the cars around me saw that I put my hazzard lights on immediately and did the same. Once at the shoulder, tho the system had shut down again... the Atlas suddenly engaged and sped up on its own. The control panel of the car was "resetting" itself still from the initial shutdown. The system appeared to reboot itself after putting into park and pushing the ignition button. And was still "processing" while I attempted to get it to the side of the road while it shutdown again. Once it was "rebooted" that's when the system appeared to "catch" and the car "sped up" by itself. Once all that happened, probably in the span of 5 minutes or so, I was able to drive 15 miles away safely to a gas station, turned off the car, got gas and it re-started like nothing had happened and drive fine home, about 2 miles further. The instrument panel including map and "driving screen" has shut down before while driving a few times, but never anything with the engine. While stopped at a light a few times, the car has previously "shut itself off" and restarted fine after being put into park and restarting ignition. This is by far the worst and most dangerous experience I've had that could have resulted in a fatal collision, had the cars around me not also been paying close attention and helped me out by slowing behind me and creating awareness for others approaching at 65-75 MPH as well.Read more

Vehicle: 2026 Chevrolet Silverado 1500 Incident Description: While driving at highway speed, the transmission fluid line failed and transmission fluid rapidly leaked from the vehicle. There were no warning lights, alerts, or messages before the failure occurred. As the fluid was lost, the vehicle began shutting down while still moving in traffic and stopped in the middle of the highway with vehic
... les behind the driver. The truck attempted to place itself into Park before it had fully stopped, and once stopped in the roadway it could not be shifted out of Park. The driver had to move the disabled vehicle off the roadway while traffic was approaching from behind. Safety Concern: The vehicle experienced a sudden loss of transmission fluid and shut down in an active traffic lane without any warning, creating a serious safety hazard for the driver and surrounding vehicles. Photos show the truck stopped in the middle of a busy highway with traffic passing while the owner documented transmission fluid pouring from underneath the vehicle. Additional photos show the dealership repair report stating the transmission cooler line was left unclipped from the fan shroud assembly, allowing it to contact the A/C compressor clutch and wear a hole in the line, causing rapid transmission fluid loss. The service manager also stated that other trucks have arrived from the factory with the transmission cooler line not properly clipped or secured. Videos available as needed. Since this incident and repair, the vehicle has continued to exhibit abnormal drivetrain behavior including shuddering and harsh or unexpected downshifts during acceleration and when using cruise control. The vehicle has been returned to the dealership multiple times for inspection, but no confirmed cause or documented successful repair has been provided.Read more

Further inspection revealed the sunroof wind deflector frame slides and shoes were broken, and cables were loose and out of position. Keep in mind, the sunroof has been used only a handful of times. After doing some research and learning Palisades had a history of exploding sunroofs, I worried that continuing to drive the ca
... r in a compromised condition (even a covered film to keep wind and rain out) would trigger the sunroof to do the same, putting my grandchildren in danger. After taking it to the dealer and having them confirm a TSB issued by Hyundai to address this very issue, I also learned it would not be a covered expense. Repair would run $3,000, a hefty consumer expense for a recognized flaw and safety hazard by the manufacturer.Read more

This vehicle does not recognize that the gear is on park. Due to the issue, it has left me stranded twice. The first time it was at a very remote area up in the mountains. I was stuck for hours due to battery dying. The second time being a very dangerous situation Because it was a very dangerous area late at night.

I was driving imin the dark and suddenly,no warning etc my car says low engine oil pressure . I had kids
... in the car, on the highway (80mph) when my car suddenly stops at not exactly a slow decline . It is dark , it’s a long high way. Luckily we made it home . But now the dealership I hit it from and Luther family ford of Fargo nd do NOT WANT TO TOUCH MY CAR. Ford company said they have reached out to Luther to tell them to tow my car and inspect etc but Luther ford keeps hanging up in them . We owe 12k still on this car and insurance. We can’t not drive it, I have to uber and use DoorDash . My hubby goes out of town for work and I work night shift so we can not use his beater of a work car. We also scheduled roadside assistance today for 330pm and THEY DID NOT COME TO GET THE CAR. I rescheduled again for the am and hopefully so but I feel they black listed my vin or any recalls with this car. It is unsafe to drive , their is no replacements thought they keep promising year after year I have read . It’s is stressful, it is unsafe and it is wrong to leave ppl hanging when I am doing all I can on my part . We can’t not afford a car payment on a broken car that is dangerous and doesn’t drive . Where is our rights? Who cares about my safety? MY KIDS SAFETY? That night on the highway ? Thankfully a lady in a truck helped us get home . This is so cruelRead more

2019 Kia with approximately 64,000 miles. Engine failed without warning. Prior to failure, vehicle had been experiencing intermittent stalling on multiple prior occasions and had been brought to a mechanic for inspection before the final failure occurred. Vehicle stalled while being driven on a highway at night on [XXX] between mile markers 67-71 in Maryland, creating a dangerous situation for th
... e driver and other motorists. The oil warning light appeared immediately prior to engine failure. Vehicle was towed to an independent mechanic who inspected it and confirmed the engine had failed. The mechanic noted that the engine was well maintained, ruling out neglect as a contributing factor. Mechanic stated this is a known issue with Kia engines from this era. Vehicle is available for inspection. Problem has been confirmed by an independent service center. Engine replacement cost is approximately $6,000. Kia's powertrain warranty does not fully transfer to subsequent owners, leaving this vehicle owner without coverage despite low mileage and what appears to be a manufacturing defect. Owner was advised by mechanic that Kia has covered similar failures as goodwill replacements due to the known nature of this defect. The upper radiator hose on some huge percentage of these trucks fail (mine has failed at just over 20K miles). You can check the Colorado Chevy forum, youtube, etc., et al, and find plenty of instances where this has failed. As this is the cooling system, it can cause the engine to overheat and potentially ruin the engine. The flaw seems to be an engineering issue with the rubber seal they use (in
... stead of a high quality o-ring) to seal the upper hose to the radiator. Additionally, since this can cause engine failure it should be addressed immediately. Instead, my local Chevy dealer, since its not a recall item, said they could schedule a time next week for me to LEAVE the truck and then they would get to it in 7-10 days. This is extremely unfair to the consumer who depends on these vehicles. This should absolutely be a recall item. You can find 1000's of examples online and if you can pull GM repair data, I'm sure you'll find 1000's more.Read more

To Whom It May Concern, I am writing to file a formal safety complaint regarding my 2019 Mercedes-Benz CLA250 involving a leaking fuel rail that caused the vehicle to shut down and become unsafe to operate. The failure of the fuel rail resulted in a fuel leak and engine shutdown. A failure within the fuel delivery system presents a serious safety concern, as it can cause sudden loss of power whi
... le driving and poses a potential fire hazard if fuel comes into contact with hot engine components. During the repair process, I was informed that the necessary replacement component, Fuel Rail Part Number 2700701995, is no longer being produced by Mercedes-Benz. Because this critical component is unavailable, the vehicle cannot currently be repaired and is therefore undrivable. It is deeply concerning that a safety-related fuel system component for a 2019 vehicle is no longer available. Without access to this essential part, the vehicle is effectively disabled. Additionally, based on the nature of this failure, I believe this fuel rail defect should be included in the recall list for this vehicle. A leaking fuel rail capable of shutting down the engine represents a significant safety risk that may affect other vehicles of the same model. I have attempted to contact Mercedes-Benz Customer Care at [XXX] on seven separate occasions and have left messages requesting assistance, but I have not received a return call. Because this issue involves both a serious fuel system failure and the inability to obtain a critical replacement part, I respectfully request that the National Highway Traffic Safety Administration investigate whether this defect affects other 2019 Mercedes-Benz CLA250 vehicles and determine whether a recall is warranted.
