The systems that failed included the shifter, which causes the car to lock so that it cannot shift out of park . This is most likely due to a shifter assembly and/or sensor malfunction. Several months prior to this event occurring (i.e., the car would not start, despite a new battery and consistent scheduled maintenance), this operator of the vehicle complained to the dealership that the car start
... ed to feel tremulous, along with the gas gauge reading 30-40 miles below the expected fill on a full tank. The dealership "checked the sensors" and determined nothing was wrong. The driver has been noticing shaking and continued problems with this vehicle shifting in and out of lower gears (1st through 3rd in an automatic V6 engine). The culmination is now this car will not start, and states both 'service shifter' and 'brake sensor service' lights illuminate upon attempting to start the engine. From reading, it is likely to have a transmission range sensor failure, a faulty shifter module, or wiring corrosion. Also, Stellantis (software manufacturer for these sensors) has admitted fault of these sensors in the past, and had extended warranties for some, but not all vehicles. Unable to operate a nonfunctioning vehicle now stuck with towing to a dealership I've not had good results with since the purchase of this vehicle.Read more

Complaint Description I am filing this complaint with the National Highway Traffic Safety Administration regarding a safety issue with my new Mercedes Benz Sprinter 2500. I purchased this vehicle brand new from Mercedes Benz Manhattan. From the first day of ownership, I began experiencing multiple serious problems with the van. The main issue is that exhaust smoke and/or crankcase gases enter the
... cabin while driving. In addition, the vehicle experiences strong vibrations and the transmission frequently kicks or shifts abruptly. On several occasions this issue has caused the engine to stall while driving. (Traffic light stop) These problems create a potentially dangerous situation and make the vehicle unsafe to operate. The dealership, Mercedes Benz Manhattan, has attempted to repair the vehicle many times, but the problems persist and have not been resolved. Because of the ongoing safety risks, I believe this issue should be investigated.Read more

The cooling system was pressure tested and held pressure externally, indicating the coolant loss is occurring internally through the exhaust heat recovery system rather than from an external leak. This condition can cause coolant loss and overheating while driving. If the engine overheats while the vehicle is in operation, it could create a safety risk due to sudden loss of power or engine damage. The dealership confirmed the diagnosis after inspection and testing and provided a repair estimate to replace the affected exhaust heat recovery system component. The overheating warning message was the first symptom observed prior to diagnosis. The problem was confirmed by the dealership after inspection and testing. I purchased my vehicle from a certified used car dealership in Connecticut in 2020 with a little over 1800 miles. Since purchasing the vehicle, it has been very reluctant to down shift and had slight transmission whine. I was notified of a recall for I believe the TCM a few months after purchasing it, which i had remedied by the dealership. Over the years, my car has been maintained completely to
... Toyotas manufacturer standard. Around 50,000 miles i started experiencing shuddering during shifts and slipping during shifts and a louder whine Around 89,000 miles my transmission symptoms worsened. Sporadic shifting, obvious whine, grinding and clunking during shifts and shifting between gears, and clicking from a stop. At 97k i serviced the transmission fluid which seemed to help a little bit but did not get rid of the symptoms fully. The fluid was dark brown, which was serviced 3,000 miles before Toyotas recommended interval. I then replaced the driver side cv axle shaft which also did not remedy any symptoms and i was notified by my shop that it was premature transmission failure. Now at 100,998 miles, my transmission grinds and clunks during all lower gear shifts, will shift through gears while maintaining steady speed, and whines audibly from the exterior and interior. My car is still running and driving for now, but after research this is a very common issue with the UA80E transmission and Toyota has knowingly sold these cars with these production errors putting people at risk of injury and financial nightmares.Read more

My 2021 Jeep Wrangler Rubicon (3.6L eTorque, automatic transmission) has had recurring drivetrain instability since about 2,000 miles while under factory warranty. It intermittently has high RPM flare during acceleration, delayed shifting, shuddering under load, and abrupt engagement. At low speeds (about 10–5 mph), it harshly downshifts, grabs suddenly, RPMs jump up and down, and it can feel li
... ke it will stall; it has stalled intermittently. Braking feels inconsistent, and at times it feels like the vehicle resists braking or surges before slowing. The forward collision warning/automatic emergency braking has activated with no obstacle present, and the power steering assist has been lost intermittently. There is also an engine tick and a high-pitched whining noise from the engine/drivetrain area. In January 2026, the transmission catastrophically failed and required replacement of the transmission, torque converter, cooler, and control module. The vehicle was out of service for over 24 days. After installation of a new Mopar remanufactured transmission and torque converter, the same symptoms returned. AAMCO has now found that the new torque converter failed, with metal shavings throughout the transmission, in the fluid, and at the bottom of the pan. I was advised that the vehicle should be sent to a Jeep dealership because no codes are currently present. I was also advised that the dealership may replace only the torque converter rather than the entire transmission, despite recurring failure and metal contamination. This presents a safety concern due to loss of acceleration, unpredictable torque engagement, stalling, unintended braking activation, power steering loss, engine/drivetrain noise, and repeated drivetrain failure. I have all the records if requested, file is too large to send in the upload area.Read more

I am the owner of a 2026 GMC Sierra 1500 Denali Ultimate equipped with the 6.2L V8 L87 engine (VIN: [XXX] ), purchased new approximately three months ago. I am filing this complaint proactively. I have experienced no engine failure or symptoms to date. However, I am filing based on a documented pattern of L87 engine failures in 2025 and 2026 production vehicles, confirmed by multiple GM dealership
... technicians, which suggests that the manufacturing defect underlying NHTSA Recall 25V-274 may not have been fully resolved by GM’s stated production improvements dated June 1, 2024. NHTSA Recall 25V-274 (April 2025) covered 597,630 vehicles from model years 2021–2024 for catastrophic L87 engine seizure caused by connecting rod bearing failure and crankshaft defects. GM excluded 2025 and 2026 model year vehicles based on a claimed manufacturing fix. However, multiple 2026 production vehicles have since experienced the same bearing failure at extremely low mileage. Open NHTSA Investigation EA25007 is already examining vehicles outside the original recall window, and Recall Query RQ26001 is investigating the adequacy of GM’s remedy. I am requesting that NHTSA consider expanding the investigation scope to include 2025 and 2026 model year vehicles. As a 2026 owner of a $90,000+ truck with the identical engine configuration, I believe my vehicle warrants inclusion in any expanded investigation or recall action. I am reporting a safety concern involving a 2018 Dodge Journey SE (VIN: [XXX] ) related to a failure in the vehicle’s emissions control system, specifically the catalytic converter and oxygen sensors. On January 13, 2026, the vehicle began producing a strong sulfur or “rotten egg” odor while driving, and shortly afterward the check engine light illuminated. Diagnostic testing revealed troubl
... e code P0420 (Catalyst System Efficiency Below Threshold – Bank 1). This code indicates that the catalytic converter is not properly converting exhaust gases, which can result in increased emissions and potential exposure to exhaust fumes. The vehicle was inspected by an authorized Chrysler, Dodge, Jeep, and Ram dealership in Maurice, Louisiana, which confirmed that the catalytic converter and oxygen sensors are failing and must be replaced. The vehicle remains available for inspection upon request. This vehicle was purchased for our growing family and is regularly used to transport our children, including a [XXX] infant and a [XXX] child. Since the emissions system issue began, occupants have noticed persistent exhaust-like fumes and strong sulfur odors while the vehicle is operating. Our infant has experienced recurring respiratory issues, and our [XXX] began experiencing respiratory symptoms after we purchased this vehicle. While we cannot definitively determine the cause, we are concerned that exposure to exhaust fumes from the malfunctioning emissions system may be contributing to these health issues. In addition, adult occupants have reported feeling unusually tired or fatigued after longer car rides in the vehicle, which raises additional concerns about potential exposure to exhaust gases inside the cabin. Because catalytic converter failures can allow harmful exhaust gases such as carbon monoxide and other pollutants to pass through the exhaust system improperly, we believe this issue presents a potential safety risk to vehicle occupants, p INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 URead more

I am filing a complaint regarding a suspension defect on my new Tesla Model Y Juniper. Based on information from other Tesla owners, this appears to be a recurring issue affecting multiple Model Y Juniper vehicles. A persistent suspension defect on a new vehicle may pose a safety risk and should be investigated. I request that NHTSA review this issue and investigate whether there is a manufacturing or design defect affecting the suspension system of the Tesla Model Y Juniper.
