Incident Description: On multiple occasions in January and February 2026, while operating a 2024 Mazda CX-5 in reverse on low-friction surfaces (snow/ice) with a slight incline, the vehicle suffered a total systemic failure. ... multaneously, the electronic gear selector (shifter) became non-functional, preventing the vehicle from being placed into 'Park' to stop the uncontrolled rearward slide. Safety Risk: The vehicle became a "runaway" object on several occasions. In one instance, the vehicle narrowly avoided crushing a pedestrian against another parked car, stopping only when the tires reached a different surface texture. In a separate incident, the vehicle slid uncontrolled down a residential hill, striking snowbanks. There is a grave concern regarding whether the Supplemental Restraint System (airbags) remains functional during these engine-stall events, as the dealer could not confirm occupant protection status during a total power loss. Inspection & Reproduction: The vehicle was presented to an authorized Mazda dealer on March 3, 2026. The service department attempted to replicate the issue by reversing over soapy water on a gravel surface. However, due to the high friction of the gravel, they were unable to induce the specific tire-slip/low-traction conditions required to trigger the malfunction. No error codes were found during an informal scan, and no warning lamps or messages appeared prior to or during the failures. Component for Inspection: The vehicle is available for inspection. Summary of Issues: 1.On March 3, 2026, I contacted my service manager regarding my inability to open the driver’s side door. I also reported persistent rattling sounds coming from the interior panels and driver’s side door, extending toward the rear of the vehicle that leads to the gas cap door. I expressed concern that the issue could be electrical in nature; however, I was advised that new door locks were needed. 3.On March 10, 2026, my vehicle was returned to me after being serviced, with the explanation that both front door locks had been replaced. I questioned why the passenger-side lock was replaced, as I had no prior issue with that door. 4.After picking up my vehicle, I traveled and left it parked at Austin Airport until 3/15/2026. Upon my return, I experienced the exact same issue-the driver’s door would not open. 5.On March 16th, 2026, I returned the vehicle to Lexus service department for further evaluation. This repeated inconvenience has been time-consuming and frustrating. As this is a luxury vehicle. 6.On March 18, 2026, I was notified that my vehicle was ready for pickup and was told the issue was related to the door bezel. Upon retrieving my vehicle, I immediately noticed that the driver’s door lock was visibly missing. I returned to the service department and documented the issue with photographs. I was then informed that the driver’s door panel would need to be replaced. Later that same evening, while attempting to refuel my vehicle, I encountered difficulty opening the fuel door. I again heard the same rattling and popping sound extending toward the gas tank. At this point, I am concerned for my safety and do not feel comfortable operating this vehicle.Read more

This omission is particularly concerning given that I have previously owned two Jaguar F-PACE vehicles, both of which were equipped with fuel tanks of approximately 21–22 gallons and provided an average driving range of close to 400 miles. Within days of taking ownership, I noticed a substantial reduction in driving range. I raised this concern with the salesperson within a few weeks; however, no action was taken. Over time, it became increasingly clear that the vehicle’s limited range significantly impacts its usability, particularly given my profession as a real estate agent, which requires frequent and long-distance driving. The current range makes even routine trips inconvenient and impractical. Due to this issue, I have limited my use of the vehicle, which is reflected in its unusually low mileage of approximately 13,000 miles. After the delay, the vehicle may surge forward unexpectedly, increasing the risk of collision. The issue has been reported to multiple authorized Toyota dealerships on several occasions within the first 15 months of ownership. The vehicle has been inspected by dealership technicians, but the concern has not been diagnosed or repaired. Dealerships have stated that the behavior is normal, despite the repeatable nature of the issue. No warning lights, error messages, or diagnostic indicators have appeared at any time. No diagnostic trouble codes have been reported. I wouldn't say my safety or others safety was put at immediate risk, but could have been since the car did eventually completely shut down right as I was able to get home only about 1-2 miles away. Vehicle was seen by dealership a few days later and as far as I'm aware are the only entity to do an inspection on the vehicle. No prior warning of any kind before the ICCU failed, and car had complied with all recalls that were supposed to prevent or at least reduce chances of ICCU issues.Read more

Made it to local shop and code P129F was found for fuel pump module. I see there is a recall on this but not for my particular vin. Recommend expanding recalls.

To the best of my knowledge… My 2020 GMC Sierra Denali with a 6.2L engine and 74,000 miles had a complete engine replacement. One lifter on bank 2 was stuck, and the push rod in the cylinder was bent. The engine made a popping noise when starting. I am filing a safety complaint regarding a progressive engine defect. Beginning as early as 2019 (approximately 84,000 miles), I repeatedly reported excessive oil consumption and related engine concerns to authorized Audi dealerships. These complaints continued over several years. At no point was I informed that these symptoms could be associated with a known defect pattern or progressive engine
... failure. The condition has now escalated to a severe engine malfunction. The vehicle began exhibiting engine misfires, flashing check engine light, EPC warning, and unstable operation. The dealership confirmed misfire fault codes and oil contamination affecting the ignition system. Engine replacement was initially recommended. Excessive oil consumption leading to internal contamination and misfires presents a significant safety hazard. Loss of engine power or sudden failure while driving can create dangerous conditions, particularly at highway speeds or in traffic. Despite the long history of reported symptoms, Audi of America has declined to provide assistance, citing only warranty status and failing to address the defect progression. The vehicle has now been inoperable at an Audi dealership for approximately 60 days (nearly two months), with no resolution. This situation raises concerns regarding a safety-related defect, failure to disclose a known issue, and a potential broader pattern affecting similar vehicles equipped with this engine. I am requesting that this issue be reviewed for potential defect investigation. The dealer provided diagnostic findings with Failure Codes via a temporary link that is no longer accessible. I own a 2022 Audi e-tron Sportback that experienced multiple serious issues involving the high-voltage battery system. During service at Audi South Atlanta, the dealership documented diagnostic trouble code P29FF00 – Hybrid/EV Battery Thermal Runaway Detected. This code is related to battery overheating and potential thermal runaway, which is a critical safety condition in electric vehicles. D
... espite identifying this safety-related fault, the dealership did not perform any repair or replace any components. Instead, they cleared the fault codes and returned the vehicle to me stating it is operating as designed. Prior to and during this issue, the vehicle experienced: •Severe range reduction (approximately 60 miles at 60% charge) •Charging interruptions and warning messages •Regenerative braking malfunction •Abnormal energy consumption •Mechanical drag and braking irregularities These issues are consistent with a high-voltage battery or power system defect. The dealership’s decision to reset the fault code without addressing the underlying cause raises serious safety concerns. A thermal runaway-related fault should not be cleared and ignored, as it may indicate a risk of battery failure or fire.
