Incident date: Mar 22, 2026
While driving, the vehicle experienced a sudden catastrophic failure involving the engine and turbocharger, resulting in an immediate loss of power and the vehicle becoming disabled in an active roadway. The failure occurred without warning and required a response from the San Diego Police Department due to the vehicle being immobilized in traffic. This created a hazardous situation for the drive
... r and surrounding vehicles. The vehicle had approximately 26,000 miles at the time of failure. The vehicle was later inspected by an authorized BMW dealership, which acknowledged the failure but attributed it to a non-OEM oil filter without providing any technical explanation or evidence showing causation. A sudden loss of power in a moving vehicle presents a significant safety risk, especially when it occurs without warning and leaves the vehicle stranded in traffic.Read more

Component/System: Fuel System – Fuel Filler / Left-Side Radiator and Cooling Fan Assembly (subject to NHTSA Recall 25V536). What happened: On June 15, 2024, I was refueling my 2023 Chevrolet Corvette Z06 at a BP gas station in Phillipsburg, New Jersey. As I removed the fuel pump nozzle from the vehicle, a violent explosion occurred. I was immediately engulfed in a fireball. My clothing caught fi
... re and was burning me. I jumped clear of the flames and extinguished the fire on my clothing. I saw that the vehicle remained fully engulfed in flames with the pump nozzle still lodged in the filler. I ran to the adjacent pump island where a station attendant was attempting but was unable to open a locked fire extinguisher cabinet. I broke the glass with both of my bare hands to remove the fire extinguisher. I sustained a severe laceration to my forehead that was approximately 3 inches long and required plastic surgery of over 50 stitches to close. I then ran to the car that was burning with approximately 10 foot flames and I used the fire extinguisher to put out the fire on the vehicle and pump. I then pulled the pump handle from the fuel filler opening and called the police. The police and fire department arrived and confirmed the fire was extinguished. The flames singed the hair on my head and legs. Component availability: Documentation including photographs, police report, and medical records are available. Safety risk: A fuel-fed explosion at an active gasoline pump created an extreme and immediate risk of death or serious injury to me the station attendants and others. Dealer/manufacturer confirmation: GM subsequently issued NHTSA Recall 25V536, acknowledging the defect — fuel spillage during refueling can contact the left-side radiator cooling fan, which remains active even with the ignition off, causing ignition. This recall directly confirms the defect that caused my incident. Insurance inspection: The police and Geico inspected the vehicle. No warnings at all.Read more

2020 Volkswagen Passat SE 2.0L VIN: [XXX] Complaint Summary: I am reporting a persistent and unresolved fault involving the secondary air injection system (DTC P0491 – insufficient airflow) that has resulted in repeated, conflicting diagnoses across multiple repair facilities. This raises concerns about a potential systemic defect affecting this vehicle model. Detailed Description: The issue
... began prior to March 2025 while the vehicle was still under warranty. The vehicle displayed a Check Engine Light associated with diagnostic trouble code P0491 (secondary air injection system insufficient airflow). On April 3, 2025, a repair facility diagnosed the issue as a failed secondary air injection pump and replaced the component. However, the Check Engine Light returned within 24 hours with the exact same fault code, indicating that the repair did not resolve the issue. A second independent diagnostic evaluation was performed later in April 2025. This inspection confirmed that: The secondary air injection pump was functioning correctly Airflow through the system was present Electrical controls and PCM commands were operating properly The fault was attributed instead to a mechanically stuck secondary air injection combination valve Despite this revised diagnosis, the issue has remained unresolved. As of 2026, a dealership evaluation has now attributed the same fault condition to a potential failure within the cylinder head assembly, indicating a significantly more serious underlying condition. Safety Concern: This issue involves the emissions and engine air management system and has resulted in: Persistent Check Engine Light Repeated failure of emissions-related components Conflicting and escalating repair diagnoses across multiple qualified service providers I am requesting that the National Highway Traffic Safety Administration review this issue for potential investigation into a systemic defect. Hyundai has declined to escalate or properly address the issue and has advised that they will continue repair attempts without further resolution. Component(s) Involved: Electrical System, Visibility/Wiper, Structure: Body: Door: Power Access/Locks. Description of the Defect: The front and rear center console "Command Centers" (touchscreens) have suffered a total electrical failure and are completely black. On the 2025 Escalade, these screens have replaced nearly all physical buttons. Consequently, there is no manual override or physical
... backup for the safety and visibility systems hosted on these modules. The vehicle's Global B (VIP) electronic architecture uses a shared high-speed data bus; a failure in these primary command modules creates a high risk of a "cascading failure" that could spread to the 55-inch pillar-to-pillar main instrument cluster, potentially disabling the speedometer and safety warnings while the vehicle is in motion. Describe the Safety Risk (How this could cause an accident): The total failure of these center consoles creates the following immediate hazards: Loss of Visibility (Defroster/Defogger): There is no physical button to activate the windshield defogger. If the windshield fogs over due to humidity or temperature changes while driving, I have no way to clear it. This creates an immediate "driving blind" situation, which is an extreme safety risk at any speed. Occupant Entrapment & Security: The controls for power door locks, window lockouts, and child safety locks are digital and hosted on these failed screens. In an emergency, I cannot verify or manage the locking status of the vehicle, posing a risk to passengers (especially children in the rear). Driver Distraction & System Instability: The failure of these core modules causes significant system instability. Because these nodes are linked to the main 55-inch driving display via the serial data gateway, an electrical short or data loop in the center consoles could cause the main speedometer and driver assistance alerts to freeze or black out without warning.Read more
