I am writing to file a formal safety complaint against Tesla, Inc. regarding a software update failure that has disabled legally required vehicle functions on my Tesla for over two years. For more than 26 months, my vehicle has been unable to complete a software update through Tesla's over-the-air (OTA) update system. This prevents me from accessing my speedometer, turn signals, and other legally required safety features, rendering the vehicle non-compliant with federal and state motor vehicle safety standards during this period. I have made multiple documented attempts to resolve this issue through Tesla's customer support channels over the course of these two years. Despite these efforts, the issue remains unresolved. Tesla has now informed me that resolving the defect will require a paid service visit — despite the fact that the failure originates within Tesla's own software update infrastructure, not from any action or negligence on my part. I am requesting that NHTSA investigate this matter on the following grounds: 1. The software defect disables legally required vehicle safety functions, including the speedometer, turn signals, hazard lights, and more. 2. The defect has persisted for over two years despite repeated good-faith efforts by the vehicle owner to resolve it through the manufacturer. 3. Tesla's OTA update system is manufacturer-controlled infrastructure. A failure within that system constitutes a manufacturer defect, not a maintenance issue. 4. Tesla is requiring payment from the consumer to correct a defect in their own systems. I believe this issue may not be isolated to my vehicle and could affect other Tesla owners experiencing similar OTA update failures. FCA indicated that NHTSA-25V882 was not applicable to my VIN despite my VIN production date in April '25; within your suspect period. Original issue remains, FCA STAR case generated however cannot replicate error condition for FCA engineers. The next scheduled visit to FCA is to replace a telematics module for possible link to issue with the Uconnect5 system. This safety issue was first noted before 800 miles and continues through 2768 miles thus far. As this issue pertains directly to a vital Air Bag sensor issue and the inability to call for a SOS assistance, I expect the NHTSA to raise this concern with FCA USA and to determine the cause of this safety defect and issue a new Recall Notification for repair and/or replacement of affected parts.Read more

The vehicle was towed to Audi Beverly Hills for inspection. The dealership diagnosed the fault as: Control Module Main Processor Performance, Symptom Code 20079, per Technical Service Bulletin TSB 2074484/3. Per that TSB, the dealer was instructed by Audi of America to create a PSS (Pending Service Solution) record, erase the fault memory, and release the vehicle to me with no repair performed. The stated guidance from Audi of America to the dealer was to inform me that this is "an extremely infrequent fault" that is "being reviewed." No fix exists. No timeline for a fix has been provided. The vehicle was released to me with the underlying defect unresolved. This is a known, manufacturer-acknowledged defect with no current remedy. As a fully electric vehicle, the Q4 e-tron's braking and all safety systems are electronically dependent. A recurrence of this fault while driving presents an unreasonable safety risk. I am requesting NHTSA investigate whether TSB XXX /3 and Symptom Code 20079 warrant a formal safety recall. The vehicle was driven to Sweets Auto Repair, where suspension repairs were performed. The contact stated that since 2023, the front and rear wheel hub assembly, the rear driver's side upper control arm, the front driver CV axle, the front stabilizer, and the front driver lower control arm and ball joint assembly had been replaced. Additionally, the sway bar link was replaced three times, all four tires were replaced due to premature wear on one occasion, and the front tires were replaced at 4,000 miles due to premature wear on another occasion. The front-end alignment was performed; however, the failure persisted. The vehicle was taken to another certified independent mechanic, where it was diagnosed that the front passenger's side struts were loose and nearly detached, the front driver's side struts were finger tight, the front and rear sway bar links were internally worn out, and all four tires were prematurely worn out and needed to be replaced. He then filed a claim with State Farm Insurance which was required to get his car towed to a Kia repair location. On 1/9/25, [XXX] [XXX] t Castrucci Auto Shop in [XXX] [XXX] he car was totaled. I talked with [XXX] who was [XXX] between the auto shop and State Farm, told me that once the repair exceeded the value of the car, he was instructed by State Farm it was totaled and there was no need to continue. On or around 3/4/26, Kia sent engineers to assess the damaged vehicle. Kia determined it was an engine fire and the cause was undetermined due to the condition of the engine department. ... rial between cylinders is very tiny, any minor disturbance will cause coolant to leak into the cylinder. Usually failures are happening between cylinders 2&3 where the material is the thinnest. 2) Honda is really trying to get a lot of power out of this small engine with the inclusion of the turbocharger. Higher octane fuels are needed to prevent premature detonation / knock, especially with forced induction (turbo charger). You can still run the car using 87 octane, but the chances of problems like a blown head gasket happening increase. 3) The head bolts that keep the engine head to the engine block are not strong enough. These bolts are torque to yield at around 50 ft lbs (if that). There have been instances where these bolts don't have enough clamping force to withstand the pre-detonation. This force causes the head to jump and the bolts to stretch. The heat from the combustion in the cylinder is no longer contained, which warps and damages the head gasket over time, leading to its failure. It was suggested to use ARP studs which we were told are a higher quality than the OEM Honda head bolts and have a stronger clamping force. Our son ended up stranded 3 states away with this issue that we then learn is VERY common yet there are no recalls.Read more

This issue is especially concerning because these seat functions are located in areas of the vehicle intended for passengers and children. We have an infant who will be occupying these seating areas, and this defect presents a direct safety risk. Since purchasing the vehicle, I have also become aware that this defect has been linked to multiple injuries and the death of a young child. Additionally, Hyundai has issued a stop-sale on affected vehicles, yet current owners are still expected to operate them without a confirmed repair available. At this time, no remedy has been completed on my vehicle, and I do not believe it is safe to operate given the nature of the defect and its location within the passenger seating area.
