I am submitting this complaint regarding my 2025 Mazda CX-90 PHEV, leased in April 2025. Since early in my lease, the vehicle has had ongoing drivability issues that have worsened over time and now present a serious safety concern.From the beginning, the vehicle exhibited jerking and hesitation while driving. This creates a risk of collision, especially when merging or at intersections.Approximately six months into my lease, the radiator failed and had to be replaced. More recently, while driving, multiple warnings appeared at once: Hybrid System Malfunction, Battery System Malfunction, Oil Monitoring Malfunction, and i-ACTIVSENSE partially disabled. These warnings were documented.Two weeks prior, the dealership reported no diagnostic codes. However, on my most recent visit, I was told via video that the active air shutter is now the issue. The required part is on backorder until August. I was told I could either remain in a non-comparable loaner while still making full lease payments, or continue driving the vehicle in its current condition until the part arrives. I find this unacceptable given the safety concerns.I expressed that I do not feel safe driving this vehicle, but the service advisor stated this is “not a safety concern.” I disagree. A vehicle that jerks, hesitates, or stalls while driving, combined with multiple system malfunctions and disabled safety features, presents a clear safety risk.I also disclosed prior to leasing that I do not have access to charging. I was told this would not be an issue. I am now being told that not charging the vehicle may be contributing to the problem. Service advisors have also indicated these vehicles have known hybrid battery issues.I no longer feel safe driving this vehicle and am concerned for my safety and my family’s safety.Read more

Subject: Safety Complaint – Loss of Drive Power Prior to Recall Resulting in Crash (2025 Honda Accord Hybrid) I am submitting a safety complaint regarding a 2025 Honda Accord Sport Hybrid (VIN: [XXX]) involved in a crash on [XXX], which we believe is related to a recall issued later in January 2026. ... eed limit, on an empty road. The driver suddenly experienced a loss of control, the vehicle became unresponsive, veered out of its lane, and did not respond to steering correction. The vehicle ultimately collided with an electric pole and was declared a total loss. At the time of the crash, no recall or warning had been issued. We reported the incident to Honda on October 23, 2025 (Case #[XXX]), describing the loss of control and unresponsiveness. However, no mention was made of any potential defect or safety concern. In January 2026, we received a recall notice stating that if the ICM CPU resets while the vehicle is in operation, it may cause loss of drive power and increase the risk of a crash. This description closely matches what occurred during our accident. We also informed the dealership, Scott Honda (Vineland, NJ), but our concerns were not investigated. Instead, the dealership attempted to sell us another vehicle without addressing the safety issue. We believe the defect existed prior to the recall and may have directly contributed to the crash. We are also concerned that the manufacturer did not warn us despite our earlier complaint describing similar symptoms. This incident resulted in the total loss of our vehicle and ongoing financial hardship, as we have been without a replacement vehicle for over six months. We respectfully request that NHTSA investigate the timing of the recall, the manufacturer’s awareness of the defect, and how pre-recall complaints like ours were handled. This resulted in a delay while I attempted to determine why the tailgate would not open and then had to return to the driver’s seat to raise the rear window before accessing the cargo area. During this time, my child was left standing in a parking lot longer than necessary. While the vehicle was stationary, the inability to quickly access the rear compartment created an avoidable safety exposure. Why This Is a Concern: The system does not account for vehicle state (e.g., parked, engine off, zero speed). A more appropriate design would allow the tailgate to open when the vehicle is stationary or automatically raise the rear window when the tailgate is activated. Instead, the current behavior enforces a strict lockout regardless of context, which can delay access in situations where timely access is important for safety. Another person used their vehicle to jumpstart the jeep with jumper cables. The dash showed there was a “coolant issue” and to check the engine. The jeep was inspected today and serviced with no issues to the battery, it was not the cause. -Previously on multiple occasions the front screen for the radio would not turn on, has glitched and flickered, would not connect to cords plugged in for carplay, chargers will not charge the phone/connect inconsistently (multiple different chargers, worked in other vehicles/outlets), screen would be black but radio would play and unable to turn off/adjust the volume, screen “buttons” will not work/respond to touch often. I have video of this happening, unable to upload footage to this complaint. Can provide if necessary. -Per the service dept at Tyrone Ford Dealership, the O2 sensor is on along with several other codes regarding this issue. This is a manufacturer’s defect, not a user error. It is a safety concern and is not something the consumer should be held accountable to pay for/fix. The notice stated this would be corrected by a dealer of my choice making a software update, and that the work would take a maximum of one-half day. ... urchased the vehicle new. On that date, it had approximately 34,000 miles on it. The service adviser stated the corrective work would take about 4 hours, but later I received a call from him saying there was a problem downloading the software update and they would need to keep my vehicle overnight. Thereafter, the dealer made repeated attempts to download the update with no success. I was informed they had contacted the Audi technical group, but their response was slow and their recommended fixes had not worked. The dealer says they then contacted more senior members of the Audi technical group, but that has not resulted in resolution of the problem. I have communicated with the southern regional manager of Audi's customer care division to no avail. He only sends polite emails basically just saying "we're working on it." I have now been without my vehicle for over a month with no end in sight. I understand this upgrade download problem has occurred with other Audis subject to the recall, but the fix has never taken this unreasonable amount of time I have experienced. In my opinion Audi USA has totally failed to provide the necessary technical support to the local dealer or provided the necessary resources, e.g., sending qualified technicians to the dealership or taking my vehicle where it can be fixed. This is the fifth new vehicle I have purchased from Audi Richmond, and up to now the quality of the vehicles and servicing by the dealer has been first rate. There is a sticker on the window of the side sliding door that gives instructions for unlocking the door (from the inside) in case of an emergency. ... nly have been used on RV's on a Ram Promaster chassis as that is a standard function of that van. The Ford Transit does not have that feature. An employee at Thor acknowledged to me that the sticker was placed in error. I know that it was placed on at least one other (and possible many other) Ford based RVs. There has been nothing sent out to owners to warn them of this issue. In the event of a fire or accident this could delay or prevent a passenger from being able to exit the vehicle. The dealer has NOT been able to reproduce or confirm the transmission issue despite multiple documented visits over the past year. However, AutoZone confirmed diagnostic code P0304 Cylinder 4 Misfire Detected on March 19, 2026 via OBD-II scan. The vehicle has been inspected multiple times by the Hyundai dealership over the past year. Hyundai Motor America has an open case on this vehicle and has escalated it to their buyback department. The Check Engine Light illuminated during the highway incident then self-cleared. Transmission slipping, shuddering at low speeds, high RPMs at low vehicle speeds, and failure to shift properly have been reported to the dealer for over one year. This vehicle contains the identical DCT transmission covered under Hyundai Recall 236 but my VIN was excluded despite identical symptoms. Car has been asleep after parking and will not wake up leaving all electrical components dead. GM has not been able to repair and unfortunately this has occurred 5 times including twice the month of March, 2026.
