I am reporting a possible safety defect in my 2024 Toyota Sienna Hybrid AWD, VIN [XXX] . The vehicle intermittently surges, lurches, or creeps forward while I am pressing the brake pedal and attempting to slow or stop. It feels as though the vehicle briefly adds power or braking force momentarily decreases even though my foot remains on the brake. The sensation is not normal creep alone; it feels
... like a brief unintended forward push while braking. The problem is intermittent, which makes it difficult to predict, but it creates a safety concern because it can happen when slowing, stopping, or maneuvering at low speed. I am concerned this could increase stopping distance or cause the vehicle to move forward unexpectedly. I am requesting investigation of possible defects involving the braking system, hybrid regenerative braking transition, brake actuator, pedal sensing, or related control software. It happens daily. I was merging onto I-85 with my sister was in the front passenger seat, and my son was behind me on the driver's side. I heard a very loud bang/pop and looked to my right and immediately saw the curtain airbag deploying on the passenger side. I did not feel anything to indicate I hit something, no noise, no beeping from my vehicle safety features, no shake of the steering wheel to indicate I veere
... d off the road. Nothing of that nature. I was able to get to the shoulder of the highway safely, despite not being able to see out of the passenger side of the vehicle due to the deployed airbags. It was confusing and disorienting because I knew we were not involved in a crash, but the airbags still deployed. I called 911 and started assessing any injuries for my son or my sister. The police arrived a short time later and opened the front passenger door and immediately cut the airbag to assess the situation. The police officer said there was no visible damage to the van and no other vehicle nearby to indicate a collision. I asked if the tire blew and he said no, he couldn't see any damage. This was an incredibly dangerous situation that could have ended with serious injury or worse. We were traveling on an interstate and the airbag deployment could have injured me and prevented me from getting off the road. In addition, the airbag deployed right where my younger son's car seat is. He would have suffered serious injury had he been in the vehicle when this happened. The shop doing the repair ran computer diagnostic testing and I instructed them to keep all parts that are being replaced. The van has been inspected by a Honda body shop manager as well as an American Honda technician. There were no warning lights or indications before the airbag deployment.Read more

My 2024 BMW X5 experienced a complete megatronics failure at 54,500 miles. The megatronics unit controls critical vehicle functions including transmission and engine management. This failure caused the vehicle to become completely inoperable — posing a significant safety risk. This failure occurred just 4,500 miles beyond the factory warranty on a barely 2 year old vehicle. A 2024 mo
... del year vehicle should not experience such a critical safety-related failure at this mileage. BMW Corporate has been unresponsive for 7 days despite opening case number XXX and promising a 3-5 business day response. I am requesting NHTSA investigate whether this is a widespread defect that may affect other 2024 BMW X5 owners. Incident Description: "Vehicle suddenly entered limp mode while driving at highway speed on XXX in [XXX] . At speed, my wife was driving and without warning was pulled into oncoming traffic. Thank goodness, no accident. A completely failed and broken rear control arm (photo attached). I was told Nissan offered an extended 10 year warranty for this. We were 10 months beyond that. This is no exaggeration, people will be killed because of this. The part literally separated in 2, causing the wheel to immed
... iately slant at a near 45-degree outward angle and shoot ther car forcefully across the lane. One day prior she was on the expressway in [XXX] . I can’t imagine the outcome there. To make this an extended warranty item and not a recall to me is this agency not looking out for the citizens of this country. Nissan is protected, but that should not be this agencies mission. This should be a recall. I can’t imagine how many thousands of people are out there this very day with this suspension part right at the breaking point. We received no notice about this extended warranty, no notice that would have prompted me to have it checked out. It was an accident-bomb waiting to explode. The repair was $1500, and despite having a part Nissan themselves admit is prone to failure, they are dodging the financial ramifications of their inadequate part. The power liftgate system appears to be malfunctioning. The vehicle intermittently displays a warning that the liftgate is open when it is fully closed and secured. When this occurs, the liftgate will not open using the exterior handle, interior button, or key fob. Additionally, the false “liftgate open” alert triggers the vehicle’s alarm system to activate unexpectedly without any attempt t
... o access the vehicle. The issue occurs randomly and without warning. This malfunction creates a safety risk because the liftgate cannot be opened when needed, which could prevent access to emergency supplies, child safety items, or the spare tire in an urgent situation. The false alert also causes the vehicle alarm to activate randomly, which can startle the driver and others nearby, potentially causing distraction while driving or in traffic areas. Additionally, the system incorrectly indicates that the liftgate is open, creating uncertainty about whether the vehicle is secure and safe to operate. A malfunctioning liftgate could present a hazard if the vehicle incorrectly detects an open door while in motion. Yes, the issue has been reproducible at Bluebonnet Ford in New Braunfels, TX. Vehicle is a 2024 Jeep Grand Cherokee 4xe (PHEV) subject to Safety Recall 78C (25V-766), issued November 2025. The recall states the engine may be contaminated with sand from the casting process, which can cause catastrophic engine failure, vehicle fire, and unexpected unrecoverable loss of propulsion. No remedy has been developed or made available by the manufacturer as of April 2026 — more tha
... n five months after the recall was issued. The manufacturer has confirmed in writing that an active stop-sale and stop-production order is in effect on all affected 4xe models due to this unresolved defect. Despite this, the manufacturer has simultaneously deemed the vehicle "operable" and denied a buyback request, requiring the consumer to remain in possession of a vehicle that cannot legally be sold due to its unresolved safety status. The manufacturer's written target window for the remedy was February–March 2026. That window has passed with no repair available and no updated timeline provided. The consumer has been without full use of the vehicle since January 2026 and has been paying out of pocket for rental transportation. The defect has not been confirmed by a dealer because no remedy exists for dealers to perform. No crash, fire, or injury has occurred, but the consumer has chosen not to operate the vehicle due to the known fire and catastrophic failure risk documented in the recall notice itself.Read more

The impact completely shattered the windshield, obstructed 100% of forward visibility, and caused structural damage to the hood and hinges. I was able to safely navigate to the shoulder by relying
... on side-view mirrors and lane-keep assist, but the incident constituted an immediate and life-threatening safety hazard. Failure of Safety Redundancies: Lack of Digital Warning: At no point prior to or during the drive did the vehicle’s software provide an "unlatched," "open," or "warning" notification on the primary display or via audible alert. The vehicle’s sensors failed to detect a compromised latch state. Secondary Latch Failure: The mechanical secondary safety catch, designed to prevent the hood from fully opening if the primary latch fails, did not engage or hold. The hood bypassed all mechanical and digital failsafes. Spontaneous Activation: The vehicle had not been manually accessed in the frunk area immediately prior to this trip, suggesting either a mechanical fatigue of the latch assembly or a software-commanded release error. Year/Make/Model: 2025 Tesla Model Y License Plate: [XXX] Previous Recalls: This failure appears identical in nature to NHTSA Recall 24V-554, which affected 2021-2024 models. It appears the remedy or part quality issues addressed in that recall may persist in the 2025 model year production. Safety Consequences: The sudden loss of visibility at high speeds on a congested highway created an extreme risk of a high-speed collision, multi-vehicle pileup, and potential fatality. The force of the hood striking the glass sent shards into the cabin. I am requesting an immediate investigation into the 2025 Model Y hood latch assemblies and sensor logic to determine if a new or expanded recall is necessary to prevent loss of life. I am writing to notify you of an Integrated Charging Control Unit (ICCU) failure on my 2023 Hyundai Ioniq 5, purchased in October of 2023. While driving home on March 4 2026, the dashboard displayed the warning "Check vehicle electrical system," and then a "pull over safely" warning. I had to leave the vehicle in a parking lot and walk home. It was towed to a dealer on March 5, 2026 to diagnose an
... d repair the problem. The car has just under 17000 miles on it. I was told the dealer was swamped with Ioniqs with this problem and had no ETA for parts. Fortunately they will be able to repair our car next week. Hyundai claims the replacement part has been redesigned to address the problem. Interestingly, I'm reading that Hyundai has issued a recall for some vehicles of this make and model year for this very serious issue; mind bogglingly we received no such notice.Read more

The Auto Idle Stop system on my 2025 Honda Odyssey fails to restart the engine when the brake pedal is released quickly after a full stop. When this occurs, the vehicle stalls completely in traffic and must be manually restarted by the driver. This has happened multiple times and is reproducible. The failed component is the Auto Idle Stop/engine restart system. The vehicle is available for inspect
... ion. My safety and the safety of others has been put at risk multiple times because the vehicle stalled at traffic lights in active traffic, leaving me unable to accelerate through an intersection. The problem has been confirmed by Honda Marysville dealership. The dealer stated they could not repair the issue and advised me to modify my driving behavior as a workaround. No repair was performed. The vehicle has been inspected by the selling dealership. A message appears during the failure telling me to restart the car. The problem first appeared within the first year of ownership on a brand new vehicle with basically zero miles at purchase. The cause appears to be a defect in the Auto Idle Stop restart system that fails under specific braking conditions.Read more
