I am submitting a vehicle safety complaint regarding a vehicle purchased on March 5, 2026. Within days of purchase, the vehicle exhibited multiple safety-related issues despite reportedly passing a Virginia safety inspection on February 27, 2026. The safety concerns identified include: - Driver-side headlight partially inoperable, which resulted in a traffic stop on March 15, 2026 - Front hood not
... properly secured/latching, creating a risk of opening while driving - No engine oil present upon inspection shortly after purchase - Coolant reservoir completely empty - Check engine light illuminated shortly after purchase Diagnostic testing following the check engine light revealed code P0420 (Catalyst System Efficiency Below Threshold). A mechanic advised that this condition is not a new issue and would have developed over time, indicating that it likely existed prior to the sale of the vehicle. At the time of purchase, the vehicle had an outstanding manufacturer safety recall identified as NHTSA Recall No. 23V651000 (Recall #251), dated April 3, 2024. This recall had not been completed prior to sale and was only addressed on March 24, 2026, after I took the vehicle to a Hyundai dealership. Additionally, the vehicle was subject to a manufacturer campaign identified as Campaign #P33, dated December 15, 2025. This further indicates that the vehicle required manufacturer-related service that had not been fully addressed prior to sale. An independent inspection conducted on March 16, 2026 confirmed conditions inconsistent with a vehicle that had recently passed a safety inspection. These issues raise serious concerns about the safety, inspection integrity, and roadworthiness of the vehicle at the time it was sold. I am requesting that this matter be reviewed for potential safety-related concerns and any related violations. I am available to provide supporting documentation, including inspection reports, diagnostic results, service records, and photographic evidence.Read more

The contact owns a 2017 Chevrolet Malibu. The contact stated that while driving at various speeds, the message "Engine Power Reduced" was displayed, and the check engine warning light was illuminated. Due to the issue, the vehicle went into LIMP Mode, and the contact was forced to pull over and turn off the vehicle. The contact waited approximately 30-40 minutes before restarting the vehicle and r

The mechanic informed the contact that the failure could be caused by the floormat being under the accelerator pedal. The contact had the floormat removed; however, the failure persisted. The dealer was notified of the failure, and another appointment was scheduled to service the vehicle. The manufacturer was not notified of the failure. The vehicle was not repaired. The failure mileage was unknown.Read more

On April 5, 2026, my 2017 Toyota Highlander was involved in a side‑impact (T‑bone) collision on the front & rear passenger side. I was seated in the front passenger seat. The impact was hard enough that my insurance company declared the vehicle a total loss. During the crash, I struck my head, but no airbags deployed (front, side, or curtain). The vehicle is being released to Liberty Mutual In
... surance. There were no warning lamps, messages or other symptoms of the problem prior to the failure. I am concerned that the airbag/SRS system may not have functioned properly in a crash severe enough to total the vehicle and cause a head injury to a front passenger. I am requesting that NHTSA record this incident and consider whether there may be a safety defect related to side or curtain airbag non‑deployment in this vehicle. Crash date: April 5, 2026 Approximate speed of other vehicle: [unknown] Location of impact: front passenger side (T‑bone) Result: vehicle declared total loss, head injury to front passenger, no airbag deployment.Read more

The infotainment head unit in my 2018 Subaru Outback has experienced internal adhesive delamination, resulting in frequent "ghost touches" and unpredictable system malfunctions that pose a severe safety hazard. Most critically, the backup camera display intermittently disappears or freezes while the vehicle is in reverse, causing a total loss of rear visibility during low-speed maneuvering. While
... the vehicle is in motion, the unit executes unauthorized commands without driver input, including making random outgoing phone calls to my contacts, switching screens away from the GPS navigation during active routing, and cycling through radio stations and apps at high volumes. These uncommanded actions create extreme driver distraction and a loss of access to essential vehicle safety interfaces. The failure is persistent and prevents the reliable use of mandated safety equipment and navigation.Read more

I am reporting a serious safety defect involving a 2011 Buick LaCrosse CXL. The vehicle has repeatedly shut off while being driven, creating a dangerous situation. The issue first occurred in January 2026. The vehicle was later towed for repair in February 2026, and I was told the issue had been fixed and picked up the vehicle 3/20/2026 and gave $1600. However, on April 9, 2026, the vehicle shut

When the vehicle shuts off, it creates a loss of power and poses a significant safety risk in traffic. I am concerned that this may be a broader safety issue and am requesting that this defect be documented and investigated.Read more

I am writing to formally file a complaint regarding my 2021 Kia Sorento and the repeated lack of resolution from the dealership where the vehicle was purchased and serviced. I have taken my vehicle to the dealership multiple times to address ongoing and serious safety concerns. Despite having active warranties on the transmission and powertrain, I have continually been told that there is nothing t
... hey can do, and my concerns have not been properly addressed. The vehicle is experiencing persistent issues, including: The transmission jerking and jarring when shifting gears The dashboard and center display screen intermittently go completely black while driving These issues do not occur constantly, but they happen frequently enough to pose a significant safety risk. I regularly have my grandchildren in the vehicle, and their safety, as well as my own, is my highest priority. Each time I have contacted the dealership to report these problems, I have been met with excuses and resistance to even scheduling an appointment. Due to the lack of cooperation, I felt I had no choice but to drop off my vehicle on April 13, 2026, without an appointment in order to have it addressed. I am extremely disappointed in the level of service and treatment I have received. As a loyal customer—this being my third vehicle purchased from the same dealership—I have never experienced this type of disregard. I also feel that I have not been treated fairly and, at times, have been dismissed in a way that is concerning and unacceptable. I am requesting the following: A full and thorough diagnostic of my vehicle, specifically addressing the transmission and electrical issues Proper repair under warranty Clear communication regarding findings and next steps Immediate attention to these safety concerns I purchased this vehicle with the expectation of reliability and safety. At this time, I do not feel safe driving it, and I am requesting that Kia take this matter seriouslyRead more

My 2024 Ford Ranger XLT 2W has been suddenly losing power or stumbling upon initial acceleration. This has been occurring intermittently. My truck now has approximately 20,000 miles. Several times, the truc

I have taken my truck to a local dealer who attempted to diagnose the problem and they didn’t offer a solution since they were unable to reproduce the problem during their road test. It has happened many times since then and I am scheduled to return the vehicle to the same dealership to attempt to diagnose and fix the problem again soon. The problem seems to occur, regardless of which drive mode the truck is in and also occurs with Electronic Stability Control both enabled and disabled. The contact noticed he had sustained bumps on the head and neck from hitting the steering wheel, but did not feel sore at the time. The contact woke up dizzy the next day and sought medical assistance. The contact underwent X-rays and was informed that he had also sustained torn chest muscles. A police report was filed. The vehicle was towed to a tow lot. The insurance company towed the vehicle to another lot the following day, and the vehicle was totaled. The contact related the failure to NHTSA Campaign Number: 16V651000(Air Bags, Seat Belts), which was previously performed on the vehicle. Additionally, the contact stated that the manufacturer had also issued NHTSA Campaign Number: 17V437000(Air Bags, Seat Belts) for vehicles repaired under the previous recall, and the recall information stated that the affected vehicles may not have received the complete software update necessary to remedy the recall condition. However, the second recall had not been performed on the vehicle. The dealer was not contacted. while operating my 2026 Honda Civic, I experienced a sudden and catastrophic tire blowout with absolutely no prior warning. At no point before or during the incident did my Tire Pressure Monitoring System (TPMS) alert me via the dashboard that my tire pressure was low or compromised in any way. Three critical failures occurred in this incident: 1. TPMS System Failure — My federally mandated TPMS
... system, required under FMVSS 138, failed to detect and alert me of dangerously low tire pressure prior to the blowout. I had zero dashboard warning at any point. 2. Tire Integrity Failure — The tire experienced a catastrophic sidewall blowout on a brand new vehicle. No road hazard, debris, or obstacle was struck prior to the failure. The structural integrity of the tire failed without cause. 3. Vehicle Safety System Communication Failure — The vehicle’s onboard diagnostics failed to detect and communicate a critical safety condition to me as the driver. On a 2026 model year vehicle, multiple redundant systems should have caught and communicated this danger, giving me the opportunity to respond and prevent the incident. These three failures created a dangerous chain of events that left me with no ability to take preventive action. The incident caused significant emotional distress. I have retained the damaged tire as physical evidence and have documented the incident with photographs. I am requesting a full investigation into whether this represents a pattern of TPMS and tire failures in the 2026 Honda Civic, and whether a safety recall is warranted. This complaint is a follow-up update to my previously filed complaint, NHTSA Complaint No. 11726321, filed on March 23, 2026, regarding two open safety recalls on my 2025 Hyundai Palisade SEL Premium, VIN [XXX] . I am filing this updated complaint to report that a SECOND failed repair attempt has now occurred on the seat belt recall (NHTSA Recall No. 25V607 / Hyundai Recall No. 283). UPDATED STA
... TUS OF SEAT BELT RECALL (25V607): First repair attempt: January 19, 2026 at Liberty Hyundai, Mahwah NJ (Invoice No. [XXX). Technician confirmed open recall. No correction made. Parts specially ordered (RC283-S8500-QQH Seat Belt Buckle Kit). Parts were not available. Second repair attempt: April 2026 at my servicing Hyundai dealer. Technician again confirmed open recall. Seat belt buckle parts remain on order and are still not available. No correction made. Two documented failed repair attempts now on record. UPDATED STATUS OF AIRBAG RECALL (26V034): As of the date of this complaint, Hyundai has still not developed a remedy for the third row airbag FMVSS 226 violation. No repair has been attempted or completed because no fix exists. This recall remains completely unresolved. SUMMARY: I purchased this vehicle on [XXX] from Maxon Hyundai, Union NJ. This is my primary and only family vehicle. I have children who regularly use the third row. After two separate dealer visits spanning more than three months, neither safety recall has been repaired. Hyundai has been unable to provide parts for the seat belt recall and has no remedy developed for the airbag recall. I am formally requesting that NHTSA: 1. Note this updated repair history in connection with Complaint No. 11726321 2. Escalate pressure on Hyundai Motor America to expedite parts availability for Recall 25V607 3. What happened? We own a 2025 Jeep Wagoneer S (EV). Since delivery, the software has been unpredictable and dangerous. On day one (18 miles), delivery was delayed for a frunk latch failure requiring a total factory computer reset. The most terrifying event was when the brake and gas pedals "swapped" actions while my husband was driving our three sons. The car sped up when braking and stopped when
... hitting the gas. The vehicle has also "bricked" (lost all propulsion) twice in traffic with zero warning, nearly causing collisions. Charging fails frequently; it once sat for 12 hours with zero charge gained despite "green" indicators. We deal with constant "ghost" issues: the car jolts unexpectedly, the "Creep" setting glitches (causing it to roll into traffic or roll backward on hills), and the parking brake slams on by itself while reversing. Recently, the backup camera went black while backing out, and sensors beeped at phantom objects. The power trunk even closed on its own, nearly hitting my head. Other failures include McIntosh audio failure, door lock malfunctions, and Uconnect/Wi-Fi errors. Safety Risk: Our family is at constant risk. Between pedals malfunctioning, dying in traffic, and unpredictable movement, this EV is a major hazard to us and other drivers. Dealer/Technical Info: Prestige Jeep has seen the car 8+ times, once for 45 consecutive days. Dealers often "cannot duplicate" because glitches clear on restart, but they return. These symptoms align with STAR Case S2508000058 (unintended torque) and TSB 08-077-25 (IDCM/electrical shutdowns). Components are available for inspection. Warnings: Symptoms began at 18 miles. We frequently see a "Red Wrench," "Red Turtle" (Limited Speed), and "Service Park Brake" messages. Frequent software failures make this vehicle unsafe for the road.Read more

**What component or system failed or malfunctioned, and is it available for inspection upon request?** The vehicle experienced a critical failure immediately on the same day of purchase, rendering it not functioning or inoperable. It was deemed to be preexisting from the dealership I bought it from. The battery is malfunctioning affecting the driving capacity and range. --- **How was your safety
... or the safety of others put at risk?** The failure created a significant safety concern, as the vehicle was not operating reliably immediately after purchase. This placed me at risk of being stranded and potentially exposed to hazardous conditions, including loss of transportation in unsafe or uncontrolled environments. I was notified of a recall on my vehicle related to door trim. I contacted my authorized Kia dealership to have the recall repaired and was told the required parts are unavailable, with no estimated timeline for resolution. I have been on a waitlist for over one year with no update. The unrepaired defect causes excessive noise that makes the vehicle unsafe and unsuitable for highway travel. I have

2025 Cadillac Escalade IQ GM issued a Customer Satisfaction Program (NHTSA ID 11017089, Manufacturer Communication N252492830-01) for 2nd and 3rd row outboard head restraints that may not lock in the upright position on 2025 Escalade IQ vehicles. My vehicle has this exact defect—both the 2nd and 3rd row head restraints were stuck in the down/forward position and could not lock upright. The saf
... ety consequence is serious: head restraints that cannot lock upright provide no head or neck support in a rear-end collision for any 2nd or 3rd row passenger. The defect was identified by Chevrolet Cadillac of Santa Fe, NM on Nov 12, 2025 (RO# 0081837). The dealer confirmed the recall applied and ordered replacement parts from GM. The parts were not available. No ETA was provided. I was forced to take the vehicle back with the recall incomplete because the wait was indefinite. The dealer's own notes state: "2nd and 3rd row headrest still malfunctioning." The replacement head restraints did not arrive until late March 2026. The recall was finally completed on March 30, 2026 (RO# 0086689)—over 4 months after the defect was identified and the parts were ordered. For those 4+ months, I had a vehicle with a known safety defect that GM had already acknowledged via a manufacturer communication, but GM could not supply the parts to fix it. Any passengers in the 2nd or 3rd row were at risk of serious head and neck injury in a collision. This is not an isolated parts delay. The same vehicle waited 3.5 months at a body shop (Apr-Jul 2025) for routine OEM bumper and sensor parts, and 2.5 months at a dealer (Nov 2025-Jan 2026) for a radar module. GM appears unable to support the Escalade IQ with replacement parts, leaving owners with vehicles that have known safety defects and no available remedy.Read more

The failure involves the rear suspension system, specifically suspected rear control arm / suspension linkage play, which caused severe misalignment (rear toe out of specification). ... ithin less than 7,000 miles. This condition increases the risk of tire blowout, loss of traction, and potential loss of control at highway speeds. The issue has been confirmed by an independent service center (Les Schwab), which documented suspension play in the rear control arm area and alignment out of specification (rear toe approximately +1.20° total). The alignment could not be maintained, indicating an underlying mechanical issue. Tesla inspected the vehicle but stated that no suspension components required replacement and attributed the issue only to alignment. However, alignment does not go out of specification without a mechanical cause. Despite multiple requests, Tesla refused to provide detailed documentation regarding the inspection or repair of the suspension system. They replaced only one rear tire and did not address the root cause. The other rear tire had already worn completely bald. After approximately 5,386 miles, the replacement tire has again worn down close to the minimum safety limit, demonstrating that the underlying issue was not corrected and continues to affect vehicle safety. There were no warning lights or system messages prior to the failure. The first symptoms were abnormal tire wear, increased energy consumption, and unstable driving behavior at highway speeds, which began shortly after vehicle purchase. This appears to be a repeat safety-related defect that has not been properly diagnosed or repaired.Read more

I was told by my mechanic I have a head gasket leak. Turns out there is a class action lawsuit for my 2.3 ecoboost motor for this coolant intrusion problem. It is a known problem. Hello. I got a recall and fixed in July 2021 for Blind Spot Sensors/Modules on my 2016 GMC Acadia SLT. Now 3.5 years later Im having the same issue with these sensors that are located in the rear bumper of the car, they get water damage, corrosion, and then fail. A very poor design. And seems like the recall didn't fixed the issue either. This must to be a common issue for this car, lots of c
... omplaints online even after the recall. And the car dealership wont cover it because it was already fixed at that time and my warranty on my car expired not to long ago. They said it would be about $2000 to fix. I feel like this prematurely failed and it was recalled once for the same issue but now failed again is crazy after 3.5 years. This shouldn't be a on going thing especially after a recall. I shouldn't have to pay for this outta my own pocket for a flawed design and would like my repairs for this to be covered again. Please help. Attached is a invoice from July of 2021 recall that was repaired. I only got a verbal estimate from the dealership (text messages) which I do have. saying that it would be a $2000 fix this time. I didn't go through with it because I feel like it should be covered. Attached pictures of sensor/module in rear bumper that is corroded. Lights on both side mirrors will come on and stay on. Won't shut off. Messages on dash saying "Service Side Dectection System". Blind zone setting won't shut off.Read more

The radar cruise control system on my 2017 Toyota Highlander is not functioning properly and creates a safety hazard. When initially setting the radar cruise control, the system works as expected. I can accelerate to a desired speed, press the “set” control, and the vehicle maintains that speed correctly. The problem occurs when I try to increase the set speed. After setting an initial speed (
... for example, 45 mph), I accelerate to a higher speed (such as 65 mph) and press the “set” control again to update the cruise setting. Instead of setting the new speed, the system reverts to the previously stored speed. When this happens, the vehicle rapidly decelerates and applies braking to return to the lower speed. This sudden and unexpected braking creates a dangerous situation, especially on highways, as it can put surrounding drivers at risk of a rear-end collision. I have tested this multiple times and experience the same result each time. This issue only occurs when using radar cruise control and does not occur with standard cruise control. note this has been an ongoing issue since the day I purchased my vehicle in January 2017. I notified the dealership about the issue and they stated they would open a case with ToyotaRead more
