I am writing to report a critical safety failure regarding my vehicle’s display systems. While driving, the left side of the infotainment system (the digital instrument cluster) went completely dark. I lost all visibility of my speedometer, odometer, and critical vehicle warnings. I immediately pulled over and attempted to power-cycle the vehicle, but the display remained unresponsive. Shortly a
... fter, the right side of the infotainment system also failed, resulting in a total loss of control over the HVAC (AC), radio, and integrated garage door opener. This is a recurring issue documented by other owners of this model, and given the loss of the speedometer while in motion, I consider the vehicle unsafe to drive until repaired.Read more

I want to include an important detail about the accident involving my 2025 Ford F-150 (VIN: [XXX] ). Immediately before the collision, I experienced a sudden change in braking. The brake pedal became very hard, and the vehicle did not slow as expected even with increased pressure on the pedal. This condition lasted for a few seconds and significantly reduced my ability to stop. The auto braking f
... eature also did not activate. There were no clear warning indicators at the time, and the event happened very quickly. Based on how the vehicle responded, it felt like a loss of brake assist. My vehicle collided with a semi truck, totalling the front end. I was very lucky to walk away alive. Thankfully airbags and my seat belt saved me. I am reporting this as part of the incident record in case it is relevant to the cause of the accident. What component or system failed or malfunctioned, and is it available for inspection upon request? The front windshield (Acoustic Glass) failed due to a spontaneous stress fracture. The crack originated from the edge of the glass without any external impact. The vehicle has been repaired, but the service records and high-resolution photos of the crack are available for inspection upon request. Ho
... w was your safety or the safety of others put at risk? The spontaneous crack occurred suddenly during a heatwave, significantly obstructing the driver’s forward visibility. Furthermore, the crack expanded rapidly during a subsequent rainstorm and temperature change, compromising the structural integrity of the windshield and potentially interfering with the front-facing ADAS cameras (Autopilot/Safety features) located behind the rearview mirror. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The problem was formally inspected and confirmed by the Tesla Burlingame Service Center. They issued a written evaluation report stating the damage was a "Stress Impact" (a manufacturing/structural defect) rather than a rock chip. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes, the component was inspected by the manufacturer (Tesla Service Centers in both Burlingame and San Francisco). Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps or messages appeared. However, this vehicle was previously subject to a Heat Pump recall (22V045) related to defrosting issues, which may have contributed to long-term thermal stress on the windshield structure prior to this failure.Read more

The transmission (powertrain) developed a delay when shifting into reverse, followed by harsh engagement. The condition worsened over time and resulted in a complete loss of reverse. The vehicle will not move when shifted into reverse. This creates a safety risk when parking, backing out of spaces, or maneuvering in traffic, as the driver cannot safely control vehicle movement. The issue occurred
... at approximately 89,000 miles under normal driving conditions. No warning lights were present prior to failure. The problem was reported to a dealership and diagnosed as a transmission failure. The condition is reproducible and ongoing. I was also informed that multiple vehicles of the same model were experiencing similar transmission issues at the same time. The failure limits safe operation of the vehicle and increases risk in situations where reverse movement is required.Read more

Unjustified Denial of Engine Warranty Coverage - 2015 Kia Sportage "I am filing this complaint because Kia is denying warranty coverage for a known engine defect (rod bearing failure in the Theta II GDI engine) on my 2015 Kia Sportage. ... officially performed this update at an authorized Kia dealership (Hendrick Kia of Cary) on April 17, 2026. Despite my vehicle now fully complying with the recall requirements, Kia continues to deny the warranty claim by retroactively applying a 'missed update' penalty. This is a clear attempt to evade their obligations under the class-action settlement regarding this engine defect. The manufacturer is holding the current owner responsible for maintenance history they cannot control, even though the vehicle is now technically compliant. I request that the NHTSA investigate this practice, as it prevents consumers from accessing the safety-related warranty protections they are entitled to." Tips for your submission:Read more

What happened? The vehicle was delivered to an authorized dealership for a federally mandated "Stop-Drive" Safety Recall (V01 - Passenger Airbag Inflator). During this service visit, the dealership attempted to charge an illegal $275.00 diagnostic fee for the airbag warning light, which is directly related to the recall system. The manufacturer’s corporate office confirmed via email that as o
... f April 23, 2026, the recall remains "OPEN" and incomplete in federal records. However, the manufacturer also issued a contradictory statement claiming the repair was "concluded" based on dealer reports. Most critically, while in the dealer's custody for this safety repair, the vehicle was lost or released to an unauthorized party. A manufacturer-arranged "Reunite" transport service arrived on-site to return the vehicle, but the dealership could not produce it, claiming it was no longer on the lot. The dealer has failed to provide any signature, identification, or surveillance footage to prove a legal release of this "Stop-Drive" recall vehicle. The Supplemental Restraint System (SRS/Airbag). The system is not available for inspection because the dealership lost the vehicle while the recall was still marked as "OPEN" and incomplete. How was safety put at risk? The vehicle is under a "Stop-Drive" advisory because the airbag inflator may explode, resulting in serious injury or death. By failing to perform the repair, attempting to charge for diagnosis, and losing the vehicle, the dealer and manufacturer have left a high-risk safety defect unaddressed and the vehicle's location unknown. Has the problem been confirmed by a dealer or independent service center? Yes. The manufacturer’s own database and the NHTSA/Mopar websites confirm the recall is "Incomplete/Open". Furthermore, a third-party towing company confirmed the dealership could not locate the VIN on their lot during a scheduled pickup. Read more

On April 17 2026 while driving my 2022 Acura MDX at very low speed in a parking lot, the vehicle suddenly accelerated forward without warning and surged into bushes before I could stop it. I did not mistake the gas for the brake pedal. This felt like a sudden unintended acceleration event. I was preparing to park in a stall and the vehicle lunged forward unexpectedly. I applied the brake, but t
... he braking did not immediately stop the forward surge. Earlier, I experienced another unusual event in which the vehicle abruptly auto-braked, jerked, and made a warning sound while I was slowing for a red light. I noticed the car felt as if it was surging a bit on residential streets and was about to mention it during the next service. This incident has caused me to lose confidence in the safety of the vehicle. I contacted Niello Acura who has been very cooperative. I was told the issue would need to be duplicated or that they could only inspect it if a code had been stored once the car is repaired. The car is currently being repaired with close to 7,000 in front end damage. I am reporting this because a sudden unintended acceleration presents a serious safety risk and could easily have caused a more severe crash or injury. I did research on this issue and have found various reports of unintended accelerations in the Acura MDX forums from various years but was unable to find any recalls or outcomes with these reports My concern is this will be dismissed as "operator error" or a phantom incident that cannot be reproduced.Read more

The mechanic has noted the vehicle requires a full transmission replacement. This is a known defect with the Ford PowerShift DCT transmission affecting multiple model years. ... ependent mechanic. When they system failed numerous error lights/warning went off and continued to cycle thru (plus beeping). I wanted to report this in hopes of continued pressure on Hyundai to add this model/year to their recalls.Read more

Vehicle: 2024 Toyota Corolla Cross (gas‑powered, non‑hybrid) System/Component: Electrical System → Start/Stop System → Driver Information Display Issue Type: Design Defect / Driver Distraction I am reporting a recurring issue with my 2024 Toyota Corolla Cross (gas‑powered, not a hybrid). The vehicle frequently displays a “Battery Charging” message on the dashboard whenever I come to
... a stop at traffic lights or stop signs. This message appears even when the vehicle is operating normally and there is no actual charging system problem. According to Toyota documentation and owner discussions, this message is triggered when the auto start/stop system decides not to shut off the engine due to the 12‑volt battery’s state of charge. However, the message is unnecessary, cannot be disabled, and provides no actionable information to the driver. It functions as a non‑critical status notification but is presented with the same prominence as a warning. This creates a driver distraction, as the message repeatedly draws attention away from the road for a condition that is not a fault, not a safety issue, and not something the driver can address. The message appears frequently during normal driving and cannot be suppressed. I believe this is a design flaw in Toyota’s start/stop implementation and dashboard messaging. The system should not display a warning‑style message for normal operating behavior, especially when it cannot be corrected by the driver. I am requesting that NHTSA log this issue as a potential driver‑distraction concern and encourage Toyota to revise the software to eliminate or suppress this unnecessary message.Read more
