The front camera lens cover — located in the grille below the Chevy bowtie logo — has developed a spiderwebbed/shattered crack pattern. There is no impact damage to the surrounding grille or bodywork. The vehicle was garaged and not in use when the damage was discovered. The camera appears to still be working and parts of the image are crystal clear but the cracks in the cover distort the pict

I own a 2016 Ram 1500 EcoDiesel, VIN [XXX] . The vehicle has been in and out of Rydell Jeep & Ram dealership over 3 times in the last few months for fuel pump issues. It has been at the dealership for over a month total (in and out). Each time I have had to pay for towing and rental cars. I have not been reimbursed for any rental or towing costs. On April 11, 2026, immediately after picking up the
... truck from the dealership (where it had just been serviced for a previous fuel pump issue), the vehicle suddenly lost all power while I was driving on the freeway and would not restart. It was towed back to Rydell. On April 23, 2026 at 9:30 AM, Bruce (service advisor) and Mike (mechanic) at Rydell verbally admitted to me that their original diagnosis was wrong. They had initially said a gear inside the engine that turns the CP4 fuel pump had come loose or failed. They now admit that diagnosis was incorrect, but they refuse to provide that admission in writing. They are now trying to charge me for the repair. I believe the actual root cause is that the CP4 high-pressure fuel pump seized internally. That seizure would have placed extreme resistance on the engine's drive gear for the pump, causing the gear or its retaining bolt to fail as a secondary consequence. The pump seizure is a known safety defect in these EcoDiesel vehicles. Because the dealership has admitted the original diagnosis was wrong, and because this failure happened immediately after fuel pump-related service, I am requesting that NHTSA investigate this as a safety defect. The sudden loss of power at highway speeds is extremely dangerous. I also request that any existing recall or investigation (such as NHTSA Campaign 22V406000 a INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am filing a safety complaint regarding my 2025 Chevrolet Tahoe Premier for repeated brake assist/braking failures. Most recent incident: While driving, the check engine light came on. I turned the vehicle off for approximately 10 minutes and restarted it, but the check engine light remained on. Approximately 20 minutes later, multiple warnings appeared on the dash including: “Brake Assist Fai
... lure Press Hard to Brake,” “Speed Limited,” and “Service ESC.” At that point, braking ability was severely reduced. I was close to my destination and approached a stop sign. I intended to roll through the stop sign because the brakes were not functioning normally, but another vehicle suddenly approached. I pressed the brake pedal with both feet. The brakes were stiff and hard to press. The vehicle still did not stop properly and came to a stop in the middle of the road past the stop sign. After arriving at my destination, the vehicle sat for approximately 1–2 hours. When restarted, the brakes appeared normal again, but the check engine light remained on. In the MyChevrolet diagnostics app, it then showed warnings for Engine and Transmission System, Transmission System, and Four Wheel Drive System. While driving to the dealership afterward, the vehicle also began shaking. This same issue previously occurred on April 16, 2026. I was driving on the interstate when the brakes lost assist and the dash displayed “Service ESC,” “Brake Assist Failure Press Hard to Brake,” and “Speed Limited.” After reaching my destination, I was told by someone at the dealership to turn the vehicle off for 10 minutes to see if that would fix things, so I turned the vehicle off for about 10 minutes. When restarted, all warning lights were gone and braking returned to normal. I still drove the vehicle to the dealership that same day for service. The vehicle is experiencing persistent dashboard warnings including "Brake performance may be reduced," "Low Speed Braking Control problem," and "Parking sensor problem." These issues have been presented to your authorized dealer, Napleton Honda in Morton Grove, on three separate occasions with no resolution: September 29, 2025: I arrived at the dealer bay with active warning lights. The service w

I provided cell phone photos of the active dashboard warnings, yet the dealer reported "nothing found." April 20, 2026: The vehicle was again brought in for the same issues. I again provided cell phone photos as proof of the defect, but the dealer reported "nothing found." Despite the dealer's inability to diagnose the cause, my photographic documentation from 9/29/25, 10/25/25, 4/20/26, and 4/25/26 proves these defects are real, recurring, and significantly impair the safety and value of the vehicle.Read more

Attached with my photos is a screenshot from a Facebook post with over 300 comments of people having the same issue and also a service bulletin that was put out a while back. I was driving home from the farmers market with my two young kids. I was turning left at a busy intersection when the engine died and an engine warning flashed on the dash (I don't recall the exact message; it sounded catastrophic). There were no "check engine" or any other warning messages prior to the incident. I was able to restart the car immediately and give it a lot of gas to get out of t
... he intersection - thank goodness - but didn't feel it was safe to drive the full 1 mile home, as there are several inopportune locations for an engine to die along the route, so I parked it in a residential neighborhood and walked home with my kids. My instinct there seems correct as it would not start again thereafter. I got it towed on the Monday and the mechanic called me to tell me that a piston had broken off in the engine and there is complete engine failure. I've attached a photo he took. This is a car we've had for 5 months with only 47,000 miles on it, regular service according to Carfax, and full/clean oil. No error messages or warnings prior. It was really scary to be in a busy intersection with a dead engine and young kids in the car.Read more

On [XXX], my 2022 GMC Yukon Denali XL (VIN [XXX] ) experienced a complete loss of propulsion while I was driving on [XXX] in El Paso, Texas, with my wife and two young children in the vehicle. The transmission locked, and the vehicle became inoperable in the middle of the roadway with traffic passing on both sides. OnStar SOS was activated to communicate the unsafe location. The El Paso Police Department was requested to divert traffic. A city tow vehicle arrived after approximately fifteen minutes; my family was assisted across the roadway. Roadside assistance did not arrive for approximately three hours. This was the second loss-of-propulsion event on this vehicle in less than twelve months. The first occurred in March 2025 and resulted in engine replacement under NHTSA Safety Recall 25V274 (L87 Engine Loss of Propulsion). The [XXX] failure occurred after the recall remedy had been applied and the engine was replaced. The vehicle was towed to the dealer who replaced the throttle body, intake air duct and a service item titled "Repair open ground circuit". I am filing this report to document a second loss-of-propulsion event on a vehicle that already received the remedy under Recall 25V274, and to ensure NHTSA has visibility into the potential pattern of recurring or inadequate remedy on the L87 platform. While driving my 2017 Honda CR-V, I experienced sudden, unexpected steering behavior consistent with a defective electronic power steering (EPS) gearbox. The steering wheel momentarily became difficult to turn in either direction, with a "sticky" or binding feel followed by a sudden release of resistance, causing oversteer beyond my input. When the wheel was turned to or near full lock, the EPS ap
... peared to apply assist opposite my input, briefly pulling the vehicle unexpectedly to one side. The failure occurred early on most noticably at highway speeds going down windy roads downhill. It became progressively worse in other driving situations over time. This unintended steering behavior reduced vehicle maneuverability and created a serious risk of loss of control, including coming dangerously close to unavoidably crossing the double-yellow line into oncoming traffic. I took the vehicle to Tony Honda in Kona. The dealer diagnosed a defective EPS steering gearbox and replaced the gearbox assembly, performed a four-wheel alignment, and completed the steering angle neutral position learning procedure. The diagnosis and repair exactly match Honda Service Bulletin 18-103 / NHTSA Recall 18V-663 (Honda codes C2N / P2O), covering 2017–2018 Civic and CR-V vehicles for an improperly secured torque sensor magnet that shifts within the gearbox and causes unintended steering input. However, my VIN was not included in the recall, and I paid $2,000 out of pocket for a repair Honda performs free of charge on recalled vehicles with the identical defect. The recall covers only 54 vehicles total, despite multiple owner complaints reporting the same failure on 2017 CR-Vs outside that VIN range. I request that NHTSA investigate expanding Recall 18V-663 to include additional 2017 CR-V VINs with the same defect, and that affected owners who paid for the repair be eligible for reimbursement.Read more

Multiple recalls issued, we were informed by Cochran Nissan there were “no fixes for the these recalls.” Yet this vehicle left us stranded along highways and busy roadways on numerous occasions. The final repair involved engine replacement and other
... extensive repairs. Only to have the engine leak oil less than a week after these extensive repairs. When the dealership could not fix the vehicle or determine the cause, h why blamed us and claimed the warranty was voided. Months of ongoing issues, repeated visits and incorrect diagnoses caused entirely by improper and inadequate repair attempts. Despite bringing the vehicle in multiple times, the repair issues were never properly resolved. Instead, the dealership repeatedly placed temporary fixes on the problem, each time making the situation worse. The technicians failed to identify or repair the root cause of the issues, resulting in additional damage and subsequent repairs that should have been covered under warranty. Instead of acknowledging these ongoing failures, the dealership blamed us for the damage and claimed that the warranty had been voided—an accusation that is both unacceptable and inaccurate given that the damage stemmed directly from their own missed diagnoses and improper repairs. Because of the dealership’s unprofessional conduct, lack of accountability, and sub-par workmanship, we ultimately no longer have the vehicle. This situation has caused significant stress, financial loss, and inconvenience to our family.Read more

I am creating this complaint because my vehicle has not been included in the NHTSA recall # 25V-767 on the V35A 3.4L twin-turbo V6 in my 2023 Sequioa. My vehicle was produced on the same manufacturing line and likely had the same engine debris. Toyota gives the reason for not including the hybrid because it is not a safety issue as the hybrid motor can provide power if the engine fails. However ow
... ners who have encountered this issue in a hybrid tundra or Sequioa has said that power-steering and control systems failed creating a safety issue. Additionally we use this vehicle to do long road trips that often put us long distances from any service center or roadside assistance. This creates an additional safety hazard for our family being stranded far from potential help due to this issue. Since this is a known issue covered under a recall for other models with the same issue I am asking that my vehicle be included in the recall so that we can have confidence to use this vehicle as intended. We bought a Toyota and a lifetime Toyota owners due to the reputation of reliability that we could depend on the vehicle to get us across the country in safety. Not including our vehicle in this recall is breaking the trust that we have in the brand to do the right thing to cover their mistakes. I am requesting that the recall be consistent to cover all the engines impacted by manufacturing debris including the hybrid variants of the motor.Read more

Severe structural corrosion was discovered on a low-mileage 2019 compact SUV affecting critical suspension components, including the front subframe, lower control arm mounting points, and structural attachment areas. The issue was identified during inspection at a dealership. The technician documented that suspension components were severely rusted, subframe bolts were rusted and cracking, and lo
... wer control arms were deteriorating due to corrosion. The technician advised that the vehicle should not be driven and recommended towing for further structural inspection. These components are part of the load-bearing suspension system and may pose a safety risk if failure occurs. This level of corrosion is unusual given the vehicle has approximately 27,000 miles and has been operated in a region without exposure to road salt or severe winter conditions. Independent mechanics who inspected the vehicle also expressed concern about the severity of the corrosion. The problem was confirmed by both a dealership and an independent repair facility. The vehicle was not inspected directly by the manufacturer. No warning lights or messages were present prior to discovery of the issue. Photographic evidence shows rust perforation, material loss, and metal delamination along structural seams, indicating corrosion progressing internally. This condition may compromise suspension mounting integrity and increase the risk of failure while driving. The affected components were repaired and are available for inspection upon requeRead more

The passenger airbag system failed, specifically the occupant sensor and the pad beneath the passenger seat. These components are responsible for detecting whether a passenger is present and ensuring the airbag deploys correctly in a crash. I've also been told that I may be at risk of not having my airbag deploy in an accident. My vehicle is available for inspection upon request. I am a single mo
... ther. My child rides in the vehicle regularly, and my elderly parents occasionally ride in the passenger seat . Because the occupant sensor has failed, the passenger airbag may be suppressed entirely in the event of a collision. No passenger in the front seat would have airbag protection in an accident, and it's possible I would not either. We have been driving under these conditions for an extended period of time without a remedy. A trusted independent mechanic inspected the vehicle and confirmed the fault. They diagnosed the issue as a failure of the occupant classification sensor and seat pad, and noted that recalibration was attempted. The recalibration did not resolve the problem, and the airbag warning light remains on. I have not yet taken the vehicle to a Nissan dealership for diagnosis. It has not been inspected by Nissan, law enforcement, or insurance representatives. The airbag warning light on the dashboard has been illuminated continuously for many months. The light is solid, not flashing, which my mechanic indicated reflects a confirmed, persistent fault in the system rather than an intermittent issue. There were no prior symptoms before the light appeared.Read more

Summary of the Safety Problem: A 2026 Ford Bronco rolled out of its parked position with no occupants, despite being left in Park, and collided with another vehicle. Full Description of the Incident: The Ford Bronco was placed in Park, was not running, and had no occupants. I exited the vehicle several times to reinstall the removable roof panels. The vehicle remained stationary during this proce
... ss. After completing the roof panel installation, my wife and I closed the doors and trunk and we walked toward the building. A witness (the owner of the other car that was involved in car collision ) confirmed that the Bronco was still parked where we left it. A short time later, we learned that the Bronco had rolled out of its parking spot and struck a parked car. The Bronco had not been touched, driven, or moved by anyone after we walked away that we were aware of. There were no warning signs that we were aware of. The police were at the scene and filed a report. This was a Enterprise Car Rental Vehicle with approx. 1,300 miles. I am unsure if this vehicle has been inspected by Enterprise. Damage / Injuries: •Collision with a parked car. •No injuries reported. •Police report filed (attached). •Photos available (attached). Why This Is a Safety Defect: A vehicle that rolls away while in Park violates the intent of Federal Motor Vehicle Safety Standard (FMVSS) 114, which aims to prevent accidental rollaway. This Bronco should not have moved under any circumstances once placed in Park. What You Request NHTSA to Investigate: •Whether the 2026 Ford Bronco has a defect in its Park mechanism, parking brake system, or electronic controls that can cause unintended rollaway. •Whether similar complaints exist for this model year. •Whether a recall or inspection campaign is warranted to prevent future incidents. Twice I have been driving on the highway at 60 mph and the transmission had suddenly shifted into low gear and subsequently went into limp mode. The dealer is unable to find an issue with the vehicle. There were no warning lights, before, during, or after. After I pulled onto the shoulder of the road and turned the Jeep off and turned it back on it operated normally.

This is a report of a Failed Remedy for Safety Recall N321 (Recall #19V293). Records confirm that this vehicle had the recall repair performed on 12/09/2020. That repair has now failed. A certified technician at the dealership has confirmed via video that the windshield bond has detached, leading to significant water intrusion. This flooding has ruined safety-related electronic modules (cameras/au
... dio) and caused significant mold growth in the interior, which is a biohazard health hazard. The manufacturer is admitting the failure but is refusing to provide a 100% covered remedy for this failed safety recall, offering only a 40% 'goodwill' contribution. The vehicle is currently dismantled and unsafe to drive. A failed windshield bond compromises the structural integrity of the vehicle in a rollover and prevents the proper deployment of the passenger airbag. Furthermore, the active electrical failures in the safety-related camera systems and the respiratory risk from mold growth make the vehicle unsafe for occupancy Technician Diagnosis Summary Technician Name: Skyler (Jaguar/Land Rover Certified) Date of Diagnosis: March 23, 2026.Dealership: Land Rover Parsippany, NJ Key Findings from Skyler's Video: Direct Admission of Failure: "So what we have going on is the windshield is leaking. The windshield is leaking so we’re going to have to get the windshield resealed for you." Extent of Water Damage: "Inspect the carpets and they are very wet all the way around on the vehicle... the vehicle does have like a mildewy smell." Safety/Electrical Failure: "Initially when I pulled the vehicle in, the surround sound cameras were not working... it's possible that one of the modules had water in I opened a case with Land Rover Corporate (Case #[XXX]), which they have since closed after refusing to provide a 100% covered remedy for this safety failure. They offered only a partial 40% 'goodwill' contribution and refused further assistance despite the confirmed safety hazard. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMRead more

URGENT SAFETY DEFECT - Repeated front hub overheating/fire risk (2025 Western Star 47x) I am submitting a formal safety complaint regarding my 2025 Western Star 47x The front wheel hubs have over heated to the point of failure on 6 separate occasions, resulting in a fire on two of those occasions. The issues have been documented with Freightliner and accompanied by photo/video evidence. Each inc
... ident required the vehicle to be towed to an authorized dealership, and repairs were performed under warranty. Despite these repeated repairs, the issue has not yet been resolved. The hubs are currently overheating again, indicating a persistent and unresolved defect. This is no longer a routine maintenance or warranty issue. This is an ongoing safety defect that presents a serious fire hazard and risk of injury, property damage, or loss of life. Key Concerns - - Six separate over heating and/or fire incidents involving the front hubs - Multiple warranty repairs with no permanent resolution - Recurrence of the same failure condition - Within days or weeks of warranty repair, the vehicles becomes unsafe to operate under normal working conditions due to over heating hubs, which break down the lubricating gear oil, furthering the problem, causing more heat with thinner oil, which eventually leaks onto the brakes, causing a fire. During the times where I was fortunate enough to catch it before a fire started, the fluid was so hot that it melted the plastic hub cap to the point of blowing up like a balloon before eventually rupturing. These instances were video documentedRead more

The vehicle first experienced a serious malfunction on October 12, 2025, while driving on an expressway at 65 mph, and the same issue recurred on March 8, 2026, while driving on a 6-lane, busy highway at 45 mph. The condition involves the vehicle unexpectedly shutting down, rendering the acceleration pedal useless and causing the car to glide to a complete stop in traffic, creating a significant s

There is fluid accumulation under the engine air filter assembly.
Incident date: Apr 24, 2026
On [XXX], while my vehicle was at the dealership for a federally mandated engine replacement recall (Campaign PI2107Y/Z), the service department attempted to obstruct the completion of the safety repair. The dealership informed me that unless I paid a 264.40 fee for maintenance and fluids, they were required by the manufacturer to drain the old, contaminated coolant from my failed engine and reuse
... it in the brand-new replacement engine. This threat of intentional contamination poses a severe safety risk, as reusing old fluid from a failed engine can lead to premature engine failure or overheating, potentially creating another hazardous situation on the road. I contacted the manufacturer corporate office (Case [XXX]), and they confirmed that no such requirement to reuse old coolant exists and that the dealership had not even attempted to file a claim for fluids. The dealership misrepresented manufacturer policy to monetize a free safety recall and threatened to sabotage the integrity of the new engine to force a sale. This issue was confirmed to be a dealership-level fabrication by the manufacturer’s corporate customer service line.
