Hello, I am having issues with the infotainment system on my accord. It is suffering from the ghost touch issue. When this happens the car constantly makes an alert beep and then randomly starts going through the menus on the system. Here are a few of the issues that can result. Navigation changes or setting change, radio can change or even turn on or off or change inputs, the cars setting can cha
... nge, it can start calls to any of your contacts including emergency #s. The only way to stop this from happening is to turn off the screen. However if the radio is on, you can not turn it off. This results in the driver constantly having to divert their attention to the radio /infotainment system from the road. It is extremely distracting and very unsafe. This is a known issue and there is no active recall. As far as I can tell this has not been fixed in replacements, simply a new version of the same radio is put into the car. I would expect that this has or will create accidents if is nor recalled. As these cars age, this will get worse for all of them. I did reach out to honda and they informed me there is no recall. Please help keep people on the road safe!Read more

Our 2024 Carnival has had a recurring engine oil leak that has required the vehicle to be left at the dealership 4 times for the same issue. Each time we get the van back the repair lasts about 1-3 weeks (depending on how often it's driven). Oil continues to leak from the engine area, creating risk of engine damage or a potential fire hazard if oil contacts hot components. The issue has not been r
... esolved despite repeated repair attempts under warranty. The Carnival has 38,092 miles on it, and has been in the repair shop over 4 times. The 1st time it was put in the shop the van has 34,420 miles on it. Each time it has been left at the dealership for 9 - 17 days while the repairs were made. We have brought it to the same dealership each time and they have been very sympathetic and helpful. Not only does this issue cause the risks mentioned above, but it has ruined the concrete in my garage as well as added oil to the roadway which could cause an accident. I have all the repair orders from the first 3 repairs that I will attach. However I do not have thee current one because the vehicle is currently being repaired for the 4th time and I haven't received the finalized repair order documents. I can update this claim once the repairs are completed. I will be contacting Kia tomorrow and wanted to have a claim number started.Read more

2021 Toyota Highalnder UA80F Transmission Whine / Known Defect - Potential for Sudden Failure I own a 2021 Toyota Highlander Platinum V6 AWD (UA80F 8-speed automatic transmisstion) purchased in July 2021. Current milage is 78k. Starting 3 days ago I noticed a faint whine noise coming from the transmission. The pitch changes with speed and when the throttle is depressed (It's louder under load/ac
... celeration). This matches the exact symptoms described in Toyota TSB T-SB-0008-21 for whine/grind noise caused by the front carrier assembly pinion shaft issues in 2021 Highlanders with UA80 transmissions. On April 27, I went to a Toyota dealer to confirm the source of the noise and diagnose. As per their diagnosis report "Tech took vehicle on road test to duplicate concern, after confirming complaint vehicle was then lifted up and inspected while under a load, tech as able to isolate noise to transmission at this time, fluid level and condition was inspected to ensure there were no signs of shavings or discoloration and after inspection tech found nothing abnormal during fluid level check. Tech then ran a health check to ensure there were no DTC present related to transmission concern and found no DTC present. Recommend replacing transmission and torque converter at this time". They provided a quote of $9,392. This condition represents a safety concern because the internal wear can progress to harsher shifting, loss of power, limp mode, or complete transmission failure while driving, potentially causing a crash or stranding the vehicle in traffic. I have audio/vehicle recordings of the whine and service records showing regular Toyota dealer maintenance. I am requesting the the NHTSA investigate this ongoing UA80 transmission issue as it appears to be a known manufacturing or design defect tat continues beyond the narrow TSB applicability and warranty limits.Read more

I completed the N252494003 recall inspection on my 2021 Chevy Silverado 1500 6.2L at Carl Black Buick/Chevy/GMC in Roswell, GA. They said my crankshaft was not one of the vehicles affected and sent me on my way with my free oil change as a remedy to the issue. I subsequently used that information, thinking the “bottom assembly” of my motor was good and not impaired, and proceeded to have the D
... OD/DFM deleted from the motor because it was a known fail point and this work cost me $7,000. Maybe 6 months after my inspection for the recall, I begin to hear a noise and I take it to the same dealership for service and they tell me the crankshaft is faulty. Nothing about the DOD/DFM delete and subsequent ECM tune had any impact on the crankshaft and yet, the dealership is denying fault when it was there service department hid the truth of the engine during the inspection. The warranty for the crankshaft is no longer applicable because of the modifications I did, but had they been honest about the faulty crankshaft, I would’ve never done that work to it in the first place. The engine should’ve been flagged during inspection and replaced.Read more

To whom it may concern. Our 2017 XC90 unexpectedly, spantaneously, and catastrophically caught fire under the hood and was a total loss 5 minutes after parking it on April 19, 2026. It was always parked overnight i

We do have video of the entire incident as the Volvo was parked on Federal Government Property at the time. We are happy to share any and all information if this helps future vehicles to be safer. It should be noted that there is an active recall for the 2016 XC90 for a coolant hose leak that can cause a fire. Given that the 2017 XC90 is of the same design. Upon checking the internet and Reddit, it appears that our situation is not unique and there are other instances and reports of similar fires.Read more

ICCU failed despite all prior recall updates being performed. Incident began when home circuit breaker tripped; vehicle subsequently refused Level 1 charging with "Check Electrical System" warning. Vehicle then displayed "Stop vehicle and check power supply," and was unable to drive above 40mph (with a turtle icon visible on the dash) once I pulled away from the charging station. This created a significant safety hazard while navigating traffic. Within two hours, the 12V battery was completely depleted; vehicle was unresponsive and required a jump-start for towing. Dealer confirmed ICCU failure requiring replacement. The fact that the vehicle's internal failure caused the immediate malfunction of public infrastructure is deeply concerning, as was the loss of motive power on the road.Read more

I am filing a safety complaint regarding a 2022 vehicle with approximately 47,000 miles that experienced a sudden and dangerous mechanical failure while in operation. While driving at approximately 37 mph, a dashboard warning message appeared indicating that the vehicle required attention but could continue to be driven for a short period. Shortly after the warning, the vehicle suddenly lost the
... ability to maintain proper gear engagement. At one point, no gear was engaged at all, creating a hazardous driving situation. I pulled over and turned the vehicle off. After restarting it, I was able to regain limited operation; however, the vehicle would not shift beyond second gear, significantly limiting its drivability and creating an unsafe condition in traffic. This vehicle has been consistently maintained at an authorized dealership according to all recommended service intervals, with no missed or delayed maintenance. The dealership later informed me that the failure was related to the mechatronic system and SOS system. I was also told that the repair would not be covered under warranty. This incident raises serious safety concerns due to the sudden loss of power and gear function while the vehicle was in motion. I am requesting that this issue be reviewed for potential defects that could affect vehicle safety and reliability. I am submitting this complaint so that the issue can be investigated and to help prevent similar incidents from affecting other drivers.Read more

This is a continuation for NHTSA campaign number 11711615. While an additional NHTSA complaint has been filed for the power train, this focuses specifically on design failures in the braking system. Our vehicle experienced a highway failure of the fuel pump, but due to the nature of a powertrain failure the car could not be put into neutral, only remain in park. The car was stranded in the middle
... of the road. As a AWD vehicle it similarly could not be towed as both wheels were fully locked. Tow informed us that there should be a manual release under the shifting column, however, this design safety feature had been removed in the 2024 iteration. The car was stuck on the road behind a blind turn on a somewhat busy road with vehicles typically traveling between 25-50 mph. The failure of a manual override for neutral and braking is a critical design failure that can lead to major accidents and death in the event of engine or transmission failure. There were no warnings, nor instructions or ability to get the car off the road.Read more

The vehicle continues to exhibit warning lights and unresolved performance issues despite multiple repair attempts. The dealership has not identified a clear root cause, and prior repairs including sensor and EVAP-related components have not resolved the condition. ... ngine insulation “hood blanket” component (not originally installed on the vehicle) are required to address the issue. These components were presented as necessary to correct the fault and resolve intermittent shifting behavior. The shifting issue is intermittent but occurs during normal driving and raises safety concerns due to unpredictable drivability and transmission behavior. The vehicle continues to exhibit warning lights and unresolved performance issues despite multiple repair attempts. The dealership has not identified a clear root cause, and prior repairs including sensor and EVAP-related components have not resolved the condition. Given the involvement of transmission-related drivability concerns and repeated unresolved electronic faults affecting engine performance, I am filing this complaint as a potential safety-related defect affecting normal vehicle operation.Read more
