Hello, I was driving on the highway with my family and my engine had a sudden loss of power. I only have 52,000 miles on the engine and it has been serviced the entire time. We almost got in a major accident as we had cars and trucks moving around us at a very high speed as we tried to pull over in the breakdown lane. I brought the car to Daddario GMC in Shelton, CT. They did a diagnostic test
... and said that cylinder 2 was misfiring. Upon closer inspection they saw that there was oil sitting on top of the piston head which indicates engine failure. I have since contacted GM warranty services given my car was just past the warranty date of November 2025, but still under the 60,000 miles of coverage. I did some research and have seen this is a very common with these engines as GM did a recall on 600,000 already. For some reason my VIN # is not listed on the current recall even though my engine is in the same year as the others that were. I would like GM to cover my engine 100% but was told they may only pay a % to fix it. Can you do anything to help me get them to cover the engine at 100%? I also wanted to let you know about this issue as my VIN was not under recall and this may happen to other people/families who have the same engine as mine that isn't recalled. Thank you, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Beginning in approximately May 2025, the vehicle began displaying diagnostic trouble codes identified by independent auto parts retailers and service providers. These codes indicated potential issues within the engine, electrical, and powertrain systems. The vehicle was subsequently brought to a dealership for diagnostic evaluation and remained there from approximately May 15 through May 19, 2025

However, the vehicle had recently exhibited repeatable symptoms, including entering “limp mode” and triggering diagnostic trouble codes, prior to being delivered to the dealership. Similar codes and issues were again identified after the dealership visit. The vehicle has experienced multiple incidents of entering “limp mode,” resulting in sudden loss of power and reduced drivability while in operation. This condition occurred without reliable warning and created unsafe driving situations, including the inability to maintain normal traffic flow. Due to safety concerns, the vehicle has not been driven under normal conditions since approximately May 2025 and has been limited to short, local movements only. The issue poses a safety risk to the driver, passengers, and surrounding traffic due to unexpected loss of power and inconsistent system diagnostics. The problem appears to be ongoing and unresolved. I purchased a 2020 Ford F-150 XLT with a 5.0L engine from Quirk Auto Group in Maine approximately six months prior to discovering a major engine defect. At the time of sale, I was provided documentation stating there were no known mechanical issues or prior defects with the vehicle. Shortly after purchase, I brought the vehicle back to the dealership due to a noticeable rattling noise and perform
... ance concerns. I was told the noise was caused by debris (rocks) stuck in the undercarriage, and no further action was taken. This concern was documented during that visit. Over time, the vehicle began consuming excessive amounts of oil, approximately three quarts per month which is far beyond normal operating conditions. I later brought the vehicle to a certified Ford dealership for further diagnosis. After inspection, I was informed that the engine had significant internal wear, including worn cylinder walls, and required a full engine replacement (long block), with an estimated cost of approximately $15,785. I have since learned that this engine issue is consistent with a known defect documented in Ford Technical Service Bulletins (TSBs) as well as an open class actions lawsuit against ford, and records indicate this issue had been present with the previous owner. This information was not disclosed to me at the time of sale. I have made multiple attempts to resolve this issue directly with both the selling dealership and Ford Motor Company. Ford has offered partial financial assistance toward the repair, which further indicates acknowledgment of a defect. However, the dealership has not taken responsibility for selling a vehicle with a pre-existing condition that was either known or should have been known.Read more

While driving on a surface road in Columbia, MD at approximately 40 mph, the dash display of the 2024 Ford Lightning Flash went completely blank, leaving no indication of vehicle speed, battery percentage, battery temp, motor temp, cruise control status etc. Zero information was displayed, and the dash was completely cold and dark. This failure, while traveling at speed, represents a gross safety
... hazard, especially because the same LCD displays automatic emergency braking informaion etc. Immediately, I was able to pull over, and establish an OBD-2 connection to the ECM, and pull error logs. I also took screenshots of critical components and voltages. I cleared the codes, re-checked for codes and saved another error log showing a couple of codes. I then cycled power to the truck several times, hoping for a reboot to clear the error. I also performed a manual reset of the Ford SYNC system. Using the OBD-2 virtual dash on my reader, I was able to safely display speed and battery data, and drive immediately to the nearest Ford Dealer (Koons Ford). On the way, I phoned Crossroads Ford (Apex, NC) who did a remote error check using the vehicle's modem. The dealer may have saved this data. Upon arrival to Koons, service writers and technicians were able to witness and verify inoperability of the dash unit, and cycle the vehicle power, whereafter the problem persisted. They then took it back for service and evaluation. After approximately 4 hours, I was told that the "dash reset itself after a couple hours" and they could not get it to repeat the error after it came back. Before this, there were no warning signs of dash failure, nor since.Read more

This is related to the incident report number XXX I filed previously. Before, between, and after that incident and this one reported today. The vehicle performs mostly normal, the severe issues are completely random and unpredictable. On 4/28/26 I started my truck and left to take my daughter to her [XXX] pediatrician appointment. I left my neighborhood and turned left into traffic. I didn’t
... speed up much, as the red light on the street stopped me about 100 yards down the road. Then, when green light hit I attempted to accelerate. The vehicle appeared to be stuck in 6th gear and would only rev up the RPMs to about 4000 and barely move forward. I managed to drive about 50ft and get the vehicle off onto the grass shoulder infront of someone’s house. I shifted the vehicle from park to drive and turned it on and off multiple times. This did not resolve the issue and I eventually put the truck in 4x4 in an attempt to make more power and drive forward. This entire time I’m on the shoulder of morning 55mph traffic, hazard lights on, with my infant in the back seat. I managed to get the vehicle to shift into 2nd gear and drive up the road. Once it came up to operating temperature it would shift into higher gears and we made it to the pediatrician. The truck stalled out similarly at two other stopped intersections and on the parking deck before we arrived at our destination. I called my wife and had her drive our daughter home after the appointment. I drove the truck home, on the same route, and it acted as if nothing was wrong. It shifted hard between 1-3rd but otherwise performed well enough. I have scheduled a transmission rebuild with a non-Ford service center for 5/4/26. It is extremely concerning that these issues are happening to myself and others with the 10R80 10-speed and Ford isn’t being forced to recall these vehicles. My truck is a 2019 with 86000 miles now. The brake pedal is extremely soft and routinely travels to the floor during normal driving conditons. It also catches at various levels. It is impossible to tap the brakes to slow the vehicle down when rounding a corner which has led to several scary incidents and near misses. The truck is currently at Crossroads Ford in Indian Trail NC for 3 weeks. It has been there once before and 3 times at
... Town and Country Ford in Charlotte. Both dealers said this is the way the brakes are supposed to work. I was also told by a service manager that this is due to the electronic braking system and they are reluctant to spend time on troubleshooting because Ford cut their renumeration. A service Field Engineer stated that the brakes were in spec and traveled 2 3/4" toward the floor. This is a subjective observation as every person uses different foot pressure. The gas pedal is at 3" off the floor and the brakes are 5". The brake pedal passes the gas pedal and many times, my foot hits both pedals. My inspection with a ruler shows the brake pedal going 1 1/2" past the gas pedal. There were no warnings the first time this happened. I have taken the vehicle to an independent mechanic who bled air out of both rear brake lines which helped. This repair lasted 3 months and now the issue is back. I've been told by independent mechanics that the problem is in the ABS module (which pulls the pedal to the floor when activated).Read more

**Incident / Defect Description for NHTSA Safety Complaint** **Vehicle Information** - Make: Tesla - Model: Model S - Year: 2014 - VIN: [XXX] - Approximate Mileage: 148,129 miles - Date of Discovery / Failure: 04/23/2026 **Description of the Defect** This vehicle is equipped with Tesla’s Large Drive Unit (LDU / rear drive unit), which was used in the majority of 2012–early 2020 Mod
... el S vehicles (RWD and Performance models). The LDU design routes coolant through the hollow rotor shaft. Over time, the rotor seal fails and allows ethylene glycol coolant to leak internally into the motor and inverter. Evidence of this failure is easily visible by removing the rear speed sensor, where blue coolant, corrosion, and debris are present. This is a well-documented design flaw affecting thousands of vehicles. **Consequences of the Defect** Coolant intrusion causes: • Corrosion and rust in the stator windings • Washing out of bearing grease • High-voltage isolation faults • Inverter damage • Complete drive unit failure **Safety Risks** This defect presents an unreasonable risk to motor vehicle safety because: • Drive unit failure can occur suddenly and without warning while driving, resulting in total loss of propulsion. This is especially dangerous on highways, in traffic, or at high speeds. • Coolant inside high-voltage electrical components increases the risk of electrical shorts, arcing, or fire. • Reduced power or complete shutdown can lead to loss of vehicle control and potential crashes. I have experienced complete drive unit failure. Many other owners report identical issues, often between 50,000–120,000 miles. Tesla has not issued a recall despite this being a widespread, predictable design defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The vehicle suffered a recurring mechanical failure of the fuel injectors and a flooded EVAP charcoal canister, causing the engine to run lean (Fault P2177) and lose power. This defect creates an unreasonable safety risk as it can cause the engine to stall or lose motive power unexpectedly at highway speeds. Furthermore, the fuel system defect led to a flooded EVAP system, which poses a potential

This is a crush/entrapment hazard, especially for children. I presented the vehicle for this same rear-seat safety concern multiple times. Even after Hyundai and the dealer treated the recalls as completed, the condition remained unresolved and I returned the vehicle again for the same issue. Hyundai and the dealer changed and used inconsistent completion dates while continuing software/update activity and then still failed to acknowledge my real-world safety concerns after the supposed repair. I also reported additional post-update problems including fluctuating cabin air temperature, blower speed fluctuation without input, and reduced ventilated-seat cooling. Hyundai ultimately took the position that those systems were operating normally and instructed the dealer to release the vehicle. My complaint is not merely that Hyundai issued a recall. My complaint is that Hyundai treated Recall 296 as completed, changed the vehicle’s seat functionality by disabling or limiting previously available automatic features, used inconsistent completion timing, and still failed to correct or acknowledge the underlying safety defect I continued reporting. There has been an active recall on the front seat belts of Hyundai Santa Fe vehicles for almost a month. I scheduled an appointment online to have this fixed and was called by the dealership to let me know that there is currently no way to fix this. According to what I have read online, letters won't begin going out from Hyundai regarding the recall until June 2026. That is a very long and unaccep

The 68RFE automatic transmission in this 2021 Ram 2500 6.7L Cummins fails to engage reverse when the transmission reaches operating temperature. When the gear selector is placed in reverse and the accelerator is applied, the engine revs but the truck does not move. The defect is intermittent and temperature-dependent, which means it cannot be predicted or avoided by the driver. The vehicle is used
... to tow an enclosed cargo trailer weighing approximately 7,000 pounds. The safety risk: inability to engage reverse on demand creates a hazard in any situation requiring reverse motion, including backing away from a stopped vehicle, reversing out of a fuel station, backing a loaded trailer at a campground or boat ramp, or executing an evasive reverse maneuver. With a trailer attached, loss of reverse function can leave the truck and trailer stranded in active traffic lanes or at intersections. The defect was first reported to a dealer in August 2023 with diagnostic trouble code P2701 stored (Friction Element B implausible pressure, internal transmission fault). The code was logged but not addressed at that visit. Over the following 32 months the same defect was reported to the same dealer four additional times. On the fourth visit the dealer replaced the complete transmission and torque converter under warranty and certified the repair complete after a documented 77 mile dealer test drive. The same defect returned approximately 4 customer miles after the truck was returned to me. The truck is currently at the dealer for the fifth repair attempt, with the dealer's own write-up confirming that the vehicle will not move while in reverse at operating temperature. The defect has been confirmed by the dealer service department through their own diagnostic process, has been documented across five repair orders, and has resulted in a complete factory transmission and torque converter replacement that failed to resolve the issue.Warning indicators: a P2701 diagnostic trouble code was sRead more

My 2021 Model Y has about 24,500 miles began squeaking when turning the steering wheel or hitting bumps. I read online that many tesla owners had the same issue. The design on these weren't great allowing water to penetrate these parts and cause them to ware down faster. I already paid for the replacement of the upper and lower control arms.

2024 Kia EV6 leased January 2025. Vehicle is subject to NHTSA Recall 24V-867 / Kia campaign SC327 for the Integrated Charging Control Unit (ICCU). The recall remedy was performed at an authorized Kia dealer in July 2025 (software / operational logic update). The defect was not cured. Three months after the recall remedy was performed, the vehicle suffered complete loss of electrical power and woul
... d not move. It was towed to the Kia dealer. The dealer's own diagnostic recorded code P1A9096 — the exact diagnostic code Kia's recall instructs technicians to check for — and replaced the ICCU and high-voltage fuse. This is the recall-defined failure occurring AFTER the recall remedy was applied. Approximately three months later, the vehicle again suffered total loss of power and would not start in cold weather. It was towed in. The dealer replaced only the 12-volt battery and did not address the ICCU, despite the documented prior ICCU failure on this VIN. The 12V battery failure is a known downstream symptom of ICCU failure on this platform. Approximately one month after that, the vehicle returned for charging system failure ("vehicle charges briefly then stops") and a "check electrical/hybrid system" warning that appeared while driving. Diagnosed as charging port failure. Vehicle was out of service approximately 27 consecutive days. Total cumulative days out of service across four visits: approximately 35-40. Two safety-related stranding incidents resulted from these failures. Both occurred in winter conditions in Pennsylvania. In the first, the driver was stranded in a parking lot at night during a snowstorm in sub-zero conditions; three tow trucks were dispatched before a flatbed could recover the vehicle because the EV could not be towed conventionally with no power. In the second, the vehicle died in a school parking lot during a snowstorm; the first dispatched tow truck could not recover an EV with no power, and the driver had to walk multiple miles home in freezingRead more
