I am filing a safety complaint regarding my 2017 Ford Explorer XLT with the 3.5L V6 engine due to a known internal water pump design defect. The water pump is located internally and driven by the timing chain. When it fails, coolant can leak directly into the engine crankcase and mix with motor oil instead of leaking externally where it would be easier to detect. This creates a serious safety haza
... rd because the engine can suffer catastrophic damage, sudden failure, loss of power, or complete shutdown, sometimes with little or no warning. In my case, the vehicle experienced symptoms consistent with internal water pump failure, including my car overheating out of nowhere, forcing me to coast to the side of the highway in very unsafe conditions. This defect can destroy the engine and create dangerous driving conditions, especially if power loss occurs while driving with children or passengers, thankfully I didn't have any of my three children with me when this happened. This appears to affect many Ford Explorer vehicles from similar model years, and numerous owners have reported similar failures. The repair cost is extremely high because replacement often requires major engine disassembly or full engine replacement. I am currently financing this vehicle with a balance of $14,000 left to pay and have already paid close to $28,000. This is extremely unfair and the cost to repair is close to $5,000. I don't see a way for myself to afford that. I believe Ford should be required to address this issue because: The design can allow coolant contamination inside the engine without obvious external leakage. Failure can occur suddenly and create safety risks. Repair costs are excessive for what appears to be a widespread defect. Owners may not receive sufficient warning before catastrophic failure. I am requesting that NHTSA investigate this defect for safety implications and determine whether a recall or broader manufacturer accountability is warranted. Thank you for your time.Read more

A metallic clinking/rattling noise developed from the engine/transmission area while the vehicle was running. The issue was diagnosed as a failed flexplate. The flexplate is a critical component that connects the engine to the transmission. Continued operation with a failed flexplate can lead to further mechanical failure and could result in loss of propulsion or inability to move the vehicle, cr
... eating a potential safety risk, particularly in traffic or when acceleration is needed. The problem was confirmed by a dealership service center. The vehicle was still operable at the time, but the noise was persistent and indicated a failure condition. No warning lights or messages were observed prior to the issue. The noise appeared suddenly. The component was replaced during repair. It is unknown if the failed part is available for inspection. There are reports of similar flexplate failures in comparable vehicles, raising concern this may not be an isolated issue.Read more

The engine has coolant intrusion. It is available upon inspection. Safety at risk due to side effects from the intrusion such as: engine overheating, misfiring, massive coolant loss that could cause an engine fire. The problem was diagnosed as a blown head gasket, but there is evidence of coolant loss and corrosion of spark plugs etc from coolant in engine. Has not been inspected by insurance or p
... olice. Symptoms started March 2026; check engine light on showing code P0303 (misfire in cylinder 3) and code for over temperature sensor. It occasionally ran hot, rough start up, misfiring, coolant loss (without external leaks), trouble accelerating, eventual service wrench light, and sometimes flashing engine light when it misfired bad.Read more

I am reporting a serious and ongoing safety issue involving my 2015 Ford Fusion. The vehicle experiences intermittent power steering failure while driving. The steering assist will suddenly disengage and re-engage without warning, making the vehicle extremely difficult to control. This condition becomes even more dangerous in wet weather, where the vehicle is sometimes nearly undriveable. Additi

This raises concern of a possible mechanical failure consistent with known recalls involving steering wheel bolt defects that could result in the steering wheel detaching. These issues are consistent with known recalls, including: - Power Steering Failure Recall (15V340000) - Steering Wheel Bolt Recall (18V167000 / 18S08) Despite these known safety defects, Ford has refused to repair the issue, stating that the recall has expired. This presents a serious safety hazard, including risk of loss of steering control, crash, injury, or death. I am requesting immediate investigation and enforcement of manufacturer responsibility for this safety-related defect.Read more

Cam Phasers. Ford refuses to repair cam phasers despite having a recall previously and never fixed by previous owner. Ford quoted the repair at $8k. Previously recall stated safety concerns over faulty manufacturing of cam phasers, having plastic parts and exposed parts rather than metal and enclosed parts like in they install during the repair. While I understand the repair was a voluntary recall
... , it's still a safety concern as engine has rough idle, rough shift changes, sometimes will jolt forward while idling and possible engine shut off due to damage to camshaft caused by the failing cam Phasers. This should be an NHTSA ordered recall.Read more

In January 2025, Covert Ford [XXX] ) performed a federally mandated PCM recall repair via their mobile service unit. Immediately upon completion, my check engine light (CEL) cam e on. Prior to this service, the vehicle had zero warning lights, fault codes, or symptoms of any kind. The CEL appeared directly and exclusively following the recall work. The timeline is unambiguous. In March 2025, I re
... turned to Covert Ford to have the recall-related fault addressed. Rather than reviewing their own recall work, the dealership charged me a diagnostic fee upfront, followed by additional labor charges, resulting in a bill exceeding $1,100. No diagnostic data, scan results, or technical documentation was ever provided to support their findings. The fault was never resolved. An improperly performed recall can suppress additional fault codes within the vehicle's OBD system, leaving the driver unaware of developing powertrain or safety-critical issues. I operated the vehicle with an active, unresolved CEL for approximately three months — a condition caused entirely by the recall service itself. A federally mandated recall is a manufacturer's commitment to repair a defect at no cost to the consumer. When that repair introduces a new fault and the consumer is billed over $1,100 to diagnose it, the recall process has fundamentally failed. NHTSA Complaint SummaryVehicle: 2017 Ford Escape SEEngine: 1.5L EcoBoostVIN: [XXX] Defect: Engine Block Design Flaw / Coolant IntrusionTechnical Description of the Issue:My vehicle is experiencing a well-documented manufacturing defect specific to the 1.5L EcoBoost engine. The engine block was designed with a "v-groove" between the cylinders that is prone to structural failure. This allows engine
... coolant to leak into the combustion chambers (coolant intrusion) and allows engine oil to contaminate the cooling system. My vehicle currently exhibits "milky," tan-brown sludge in the coolant reservoir, confirming that oil and coolant are cross-contaminating due to internal engine failure.Extreme Safety Hazards and Dangers:This defect creates several critical safety risks for the driver, passengers, and other motorists:Sudden Loss of Power at High Speeds: The mixing of oil and coolant destroys the engine’s ability to lubricate internal bearings. This can lead to a catastrophic engine seizure while the vehicle is in motion, causing an immediate loss of power and power steering, which creates a high risk of collisions on highways.Engine Fire Risk: When coolant enters the combustion chamber, it can cause severe engine misfires and overheating. Overheated oil or pressurized coolant spraying onto a hot exhaust manifold during a mechanical failure presents a significant fire hazard.Hydro-locking: If coolant floods the cylinder while the vehicle is in operation, the engine can hydro-lock, causing the engine to stall instantly and potentially throwing metal debris onto the roadway, endangering other drivers.Visibility Hazards: Excessive white smoke from the exhaust (caused by burning coolant) can suddenly obscure the driver’s rear-view visibility and the visibility of trailing motorists. Conclusion:Ford Motor Company was aware of this defect, as evidenced by Customer Satisfaction Program 21N08 and TSB 19-2346. I have a Maverick (24 AWD XLT) and this issue has occurred twice. It happened on my vehicle the first time as I was trying to turn left, up an incline, with oncoming traffic - loss of acceleration. There was simultaneously a service light and power train malfunction message that went to my ford app. The vehicle had 5k miles on it. I took it immediate to the dealer. After testing they said the conn
... ection at the accelerator was loose (pigtail) and replaced. I just had the same failure occur yesterday (~22k miles) on my way home from work, except I lost acceleration on an inclined on-ramp trying to merge onto the highway. I had just slowed on the ramp to let a semi pass, and am thankful I didn’t try to get ahead of it. It wouldn’t have ended well. Still, I could see traffic jamming behind me, and folks were struggling to get on the highway because of my significantly reduced speed. Losing control of acceleration at a point where it is critical is very dangerous. I’ve never experienced a vehicle failure that felt like it risked my life. For this to occur twice in key moments of acceleration, with less than 25k miles is unsettling. Online there are many similar reports, not sure how this isn’t a recall yet.Read more

We had our [XXX] old and then [XXX] old who has a neurological condition and is under a neurologist for treatment of severe migraine headaches in the car with us. We had to pull over on the side of the road, then my husband started driving slowly with emergency blinkers on, we were able to get to a dealership who looked at it and ran a code and told us that the code showed a misfire in our cylinder 1 and that we needed to replace the ignition coil, which we just replaced in May 2024 and we have a rceipt to show proof of this. The car will no longer be in our possession because when my husband learned that the car was in engine failure he wanted to trade it in because we need a safe car but we do have the reports to it. The problem was confirmed by an independent service center- Christian Brothers Automotive in Crestwood, Ky. To fix the problem we have to have a total engine replacement which is priced at 12 grand. Manufacturer refuses to take responsibility for this. Engine blockhead was also found to not line up with the engine, which had to have happened at factory when car was built. The check engine light came on 2x recently, car was jerking in May 2024 and that's when the problem 1st started. Problem corrected for almost 2 years, but then same problem reappeared. Upon picking my vehicle up from the Gerald Jones Auto Group Dealership April 2026 and heading out of town I noticed that the vehicle was driving a lot rougher than I'd expected especially after an entire month of having the vehicle to repair completely, of course, I canceled my trip and immediately called the dealership and explained my concerns regarding the feel of the roughness of the vehicle & that I was not comfortable because I hadn't experienced this before. I was asked to bring it back. After waiting for the recheck I was informed that there was a hose disconnected in the rear afterwards I was advised to call the dealership with any other problems. I started home noticed that there was still a slight skipping or hesitation around the 15-50mp range I called & got voice mail & I left a detailed message about problems. I had to run an errand, on the way home as I moved onto the busy interstate the vehicle really started skipping & hesitating much worse than before & a noisy fan sound coming from the mid right side dash area that hadn't been there! I called the dealership back as instructed & left another message. I waited an entire day no one returned my call. I called again on the next day was told to bring the car in again a 2rd time. The 3rd time car was checked there I was informed it was a transmission issue not part of Recall the cost of repair would be totally my responsibility according to Manager, which is not fair!Read more
