Saturday April 4th While driving the trailer (fully loaded with equipment, no horses) back to [XXX] from [XXX] , the trailer's brakes completely failed. There was no indication of the failure until I was unable to stop at a traffic light and was pushed all the way through the intersection. Thankfully, there were no other vehicles in the other direction, so there was no accident. I was able to
... get the trailer back to a local 4 Star dealer, and the diagnostic was a failed hydraulic brake pump, resulting in a complete failure of the trailer brakes. The hydraulic pump was replaced, and the brakes worked normally afterward. The trailer was driven back from [XXX] to [XXX] on April 7th and was not used again until May 7th. Thursday, May 7th to Sunday, May 10th The trailer was driven with (4) horses to a horse show approximately 1 hour and 30 minutes away from our PA barn. The trailer performed normally with no issues. The trailer was parked in trailer parking from Thursday evening until Sunday afternoon. Sunday afternoon, the horse trailer was hooked up and loaded with (4) horses, and we started making our way back to our home barn. At the time of departure, the horse trailer was operating normally. After roughly 20 minutes of driving, we came to a traffic light and were unable to stop the trailer; the trailer brakes were not responding. Again, we were very lucky that there were no other vehicles coming in either direction. We decided to return to the horse show and made our way back, with four lanes, at speeds below 25 mph. Upon arrival, we troubleshot the problem and ultimately had a mobile trailer repair unit respond to the location. After their diagnosis, it was determined that the hydraulic brake pump had failed again, leaving the trailer brakes completely inoperable. Vehicle experienced ongoing safety-related driver assistance system failures beginning at delivery. The vehicle initially could not be delivered due to unresolved windshield and sensor concerns affecting driver assistance features. After delivery, the vehicle continued experiencing intermittent failures involving adaptive cruise control, lane assist, cameras, and related driver assistance systems
... . Warning messages remained active and the vehicle was repeatedly returned for repair. Multiple repair attempts were performed over a period exceeding 30 days, including windshield replacement, calibration attempts, and outside vendor involvement. The dealership later advised that an improper windshield/calibration issue was responsible for the failures. Because these systems intermittently malfunctioned, I lost confidence in the vehicle’s safety and reliability, especially during highway driving where adaptive cruise and lane assistance systems are intended to function safely.Read more

The vehicle experienced failure of the air conditioning system caused by an A/C compressor shaft seal leak on my 2021 Honda Pilot. The issue was diagnosed and confirmed by both an independent mechanic and an authorized Honda dealership. The dealership confirmed that the recommended repair is replacement/repair of the compressor shaft seal consistent with Honda Service Bulletin (TSB) 23-050. The ve
... hicle and failed component are available for inspection upon request prior to repair. The failure created a safety concern because the inability to properly operate the air conditioning and defrost system can reduce driver visibility due to windshield fogging and create unsafe driving conditions during warm and humid weather. Excessive cabin temperatures can also negatively impact driver concentration and passenger safety during travel. Prior to visiting the dealership, my mechanic identified the likely issue as an A/C compressor shaft seal leak and advised me to bring the vehicle to Honda because it appeared to be a known manufacturer issue. The Honda dealership later confirmed the same defect after completing a paid diagnostic inspection. The vehicle has been inspected by an independent mechanic and an authorized Honda dealership service department. A case has also been opened with Honda Corporate regarding the issue and denial of coverage assistance though was told it could take several days to weeks to be resolved. Prior to the failure, there were no warning lamps or dashboard warning messages displayed. The first symptom was the air conditioning system no longer cooling properly when first used during warmer weather around May 2026. Because the A/C system had not been used during the winter months, the problem only became apparent once temperatures increased and the system was needed again. This appears to be a known issue already acknowledged by Honda through the publication of TSB 23-050 covering 2021 Honda Pilot models.Read more

I am submitting this on behalf of my daughter, [XXX] . On Friday, May 8, 2026, [XXX] was traveling southbound on Highway 89A approaching [XXX] . Highway 89A is a narrow, winding two-lane canyon road with limited shoulder and significant elevation changes. While still on the canyon road, her 2020 Chevrolet Equinox suddenly lost all power, including power assist to both steering and brakes. A sudd
... en loss of power steering and brake assist on this road presented a serious risk of loss of vehicle control. Fortunately, she was able to get the car stopped off of the roadway. However, she was then unable to place the car into Park or shut off the ignition. Multiple warning lights were illuminated across the instrument cluster. Her roadside assistance plan towed the car to the nearest repair facility, where on Monday, technicians were unable to replicate the issue. A previously stored DTC P3055 was found in the diagnostic system. This code relates to a known and documented issue with Chevrolet Equinox vehicles, for which Chevrolet has issued multiple Technical Service Bulletins, including TSB 21-NA-139, which identifies the Multifunction Power Supply Converter (GM Part #13510484) as the known culprit for this failure mode. Upon bringing this to the attention of the service center in Fort Myers Florida, the service center manager took a drive test with me, and was able to replicate the issue, but immediately started dismissing it as "normal" and within Tesla specs. This is becoming a regular practice at that service center, where they deem warrantable issues as normal and within (their tolerance). I insisted that this issue is not to be taken likely by Tesla, because it is something that involves a safety hazard. Upon further research, I was able to find that this issue is happening to many Teslas model S and X, where the front axles bend due to the weight and high torque of the vehicles. The issue falls completely under warranty and the half shafts should be replaced, but apparently Tesla has instructed their service centers to dismiss and disregard the symptoms, and just tell the customers that no warranty replacement will occur, because it is not necessary, we just have to drive the cars like this, until the car completely fails, or an accident happens because one of the bent axle shafts breaks on a highway during normal drive. I recording this report as evidence that the issue has been reported to the highest vehicle safety authorities, and I expect a corrective action so Tesla can fix my car under the existing unexpired warranty.Read more

A similar intermittent concern was previously documented on this vehicle in 2011 (logged as "Customer Concern Not Duplicated"), establishing a recurring pattern. GM Technical Service Bulletin 07-07-30-032D (2006–2011 Cadillac CTS, automatic transmission) explicitly documents this exact condition: key sticking, Shift to Park DIC message, shifter binding, and battery drain. OEM replacement part is now discontinued. Manufacturer declined goodwill or recall coverage. This is my daily driver and constantly have my children in the car and just want the car to be safe for everyone. The transmission downshifts rough and at times seems to slip/lock up while driving. I haven't brought it to the dealership for this as of yet.Read more

My 2025 Tesla Cybertruck AWD (VIN: [XXX] ) has a recurring failure of the right rear seat occupant detection system, which governs airbag deployment and is certified by Tesla to meet FMVSS 208. The same alert (internal code RCM2_a698) persisted through two official repairs at Tesla Palo Alto: 1st repair (Invoice XXX, 8/22/2025): seat belt buckle replaced. Alert recurred 2 days later, August
... 24, 2025. 2nd repair (Invoice XXX, 9/18/2025): harness assembly replaced. Tesla then performed a Visual Quality Check, recording in the invoice: "Technician performed a visual quality check to ensure its condition meets our standards." Vehicle returned as repaired. Relying on Tesla's written certification, I reinstalled my [XXX] son's car seat in that position. He rode there daily for approximately 7 months (September 2025 - April 2026). On April 21, 2026, I tested the seat and found the sensor still actively failing - unable to detect an adult occupant. My spouse witnessed this. I have 30-second video. Alert RCM2_a698 remains active in Tesla's systems today, confirmed by screenshot. This is my family's only vehicle. My wife currently uses the right rear seat. Airbag deployment for that seat cannot be relied upon. Tesla has been notified and has not resolved this issue. While driving my Ineos Grenadier in the middle of the Arizona desert, the steering suddenly became difficult to control due to a tie rod nut coming loose. The vehicle began wandering and required constant correction to maintain control, creating a serious safety concern in a remote area far from immediate assistance. There were no warning lamps, messages, or alerts displayed inside the vehicle bef
... ore or during the incident. Just one week prior, the steering and front end components had been inspected by an authorized dealer during service, and no issues were identified at that time. Upon inspection after the incident, the tie rod hardware was found loose, which could have resulted in a complete loss of steering control if not caught in time. This appears to be a known issue within the Ineos Grenadier community involving steering and tie rod hardware concerns. The failure presents a significant safety risk to vehicle occupants and others on the road, especially during off road or remote travel where recovery and emergency services may not be readily available.Read more

They asked if I had recently mounted any new equipment on the windshield. I said I had Velcro attached and EZPASS toll reader. They asked me to remove it, because might be interfering with the cruise sensor. After I removed it, and while continuing on the interstate, with cruise activated, it again repeated the same failures. The dealer arranged to have my vehicle towed to their shop.Read more

While driving 2020 Crosstrek Hybrid, the dashboard lights suddenly flashed and then engine stalled, leaving me with no brakes and no steering. I rolled through straight through an intersection with four stop signs and I could not stop. Luckily no one was driving or walking at the time. I couldn't start car, even with a jump start. I bought a new Interstate battery with higher specs. Three days
... later the same thing happened, the car stalled while driving just about to park. The dashboard lights also flashed and I saw Hybrid System this time. Car was towed to the dealer and there was a huge amount of corrosion with the electrical system underneath the car. Previously, four times this winter the car batter was completely dead after being parked in a garage and twice in a parking lot. I was told by a mechanic who gave me a jump start that there's a parasitic drain somewhere.Read more

While driving my 2016 RAM 1500 Eco diesel, the vehicle suddenly lost power and would not accelerate beyond 40 mph.
