NHTSA safety recall 25V-586 was issued on September 9, 2025. It has now been eight months without a resolution from the manufacturer, and per the FCA recall notice (accessed at [XXX] ) "The Fuel Delivery Module (FDM) on your vehicle may contain internal parts that can be affected by heat, which can reduce fuel flow and lead to a loss of drive power while driving. A loss of drive power can cause a
... vehicle crash without prior warning." This is a critical safety issue and the lengthy delay in providing a remedy puts lives at risk. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am reporting a safety concern with BMW vehicles equipped with the B58 Technical Update (B58TU) engine, primarily in 2019–2021 model years. These vehicles use a variable-displacement oil pump containing an internal thermoplastic adjusting component that regulates oil pressure. This plastic part is prone to fracturing, leading to sudden loss or instability of engine oil pressure. Failures most c
... ommonly occur during cold starts in low ambient temperatures. When the component breaks, oil pressure regulation fails with little or no warning. Drivers often experience precursor symptoms such as inability to electronically read oil level, erratic oil pressure, or sudden drivetrain warnings. The issue can rapidly progress to engine shutdown or severe damage from oil starvation. This creates a serious safety hazard, as the vehicle can become disabled without warning while driving or shortly after startup — particularly dangerous in traffic or winter conditions when failures are most frequent, potentially stranding occupants in freezing temperatures. Oil pump replacement requires extensive disassembly and typically costs $6,000–$8,000 for the pump alone, assuming no further engine damage. Owners cannot reliably determine if their vehicle still has the original thermoplastic pump. VIN-based parts lookups now display the updated metal-component part number even when no replacement has occurred. BMW has also removed the original plastic part number from dealer systems, eliminating transparency. This issue affects multiple production years and continues to impact vehicles in the fieldRead more

Safety risk: First battery failure occurred in January at 10,000 miles while traveling in the desert with my wife and infant son. We were stranded with no Chevrolet dealer accessible. Second battery failure occurred at 16,500 miles. A failed DC/DC converter module was discovered immediately after the latest round of repairs when the dashboard lit up with multiple warning lights and a check engine light the same day I picked up the vehicle. I drive this truck daily for work and transport my young child in it regularly. Repeated unexpected failures create a serious and ongoing safety risk. Confirmed by dealer: Yes. George Chevrolet confirmed and attempted repairs on the telematics module, DC/DC converter module, and radio. All failures documented on official repair orders. Warning lamps: Yes. Multiple warning lights and check engine light appeared immediately upon pickup after the most recent repairs. ubject: URGENT SAFETY INCIDENT REPORT: Engine Seizure at Highway Speeds – Case #xxx To the Toyota Safety & Compliance Department: I am writing to formally report a critical safety incident involving my 2024 Toyota Tundra (VIN: [XXX] ). Incident Description: While traveling at highway speeds, the vehicle’s engine suffered a catastrophic internal seizure and shut down completely without any p
... rior warning. This resulted in an immediate loss of motive power in the middle of high-speed traffic. Because the engine died, I also experienced an immediate loss of power steering assist and power brake vacuum, making the vehicle extremely difficult to control. I was forced to perform an emergency maneuver to avoid a rear-end collision and struggled to steer the truck toward the shoulder while other vehicles swerved around me at high speeds. Safety Hazard & Manufacturing Defect: This failure is identical to the conditions described in NHTSA Safety Recall 24V-381 (Toyota Recall 25TA14). The dealership's diagnosis at AutoNation Toyota Weston confirmed that the bearings are discolored and "spun," which is a direct consequence of the manufacturing debris that Toyota has already admitted exists in these engine blocks. Bad-Faith Warranty Denial: Despite the life-threatening nature of this event, the dealership is attempting to blame "abuse" based on a software log from the Brake Override System. They are ignoring the fact that a metallurgical failure of the main bearings has no mechanical link to pedal input. Using a safety software record to deny a repair for a known manufacturing defect that nearly caused a fatal accident on the highway is unacceptable. Requested Action: I demand that this case be immediately reviewed by Toyota’s safety team. My vehicle manifested the exact defect described in the federal recall during a high-risk situation. I am requesting an immediate resolution to remove this dangerous vehicle from the road and to honor the terms of the INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FORead more

This is an extremely common issue plaguing Hyundai & Kia vehicles with the Theta III 2.5L Turbo engine, Such as Kia K5 GT, Hyundai Sante Fe, Hyundai Santa Cruz, & Hyundai Sonata N-Line. Its a failing ITM (integrated thermal management) unit or otherwise known as an ITMS unit. There is a rod within the ITM that prevents proper control of the flow of cooling the oil and engine coolant of the engine
... . Results in Cooling issues, such as a cold lower radiator hose while the engine is hot, or 'limp mode' during acceleration, that causes a sudden loss of power while trying to merge on the expressway. There has been other Kia models and years that have had TSB's related to fixing this issue, but Hyundai has released nothing. I have uploaded an example of one of the TSB's that do not apply to my car. Technical symptoms often include: Fans running 100% full duty (high) while the lower radiator hose stays cool. Or Fuel dilution or a 'rich' running engine. Because the engine is overcooling that keeps the car's fueling rich and in a open loop mode affecting emissions and contaminating oil. Personal Details of exactly what happened when i noticed mine was failing. I ran home on my lunch break, and while accelerating on a ramp to get up on a highway, my car started beeping and flashing "Engine Overheated" and i immediately let off the gas with preparation to pull over. The second i left off the gas, the coolant temp within 1-2 seconds went from a little over 3/4 the way up the gauge back to normal a little below half temp. I resumed driving a 3 mile drive back to work, with it showing normal temps the rest of the way. I popped the hood when i got to work to see if coolant was low, and it was completely normal, a little over halfway between the min/max lines. The car continues to run hot, often with the cooling fans running at 100%.Read more

This is a known issue caused by compressed air contamination according to Eaton Transmissions service bulletin TAIB-1006, which was first published April 2023. In the over 3 years since the service bulletin was released, Eaton, Peterbilt, Paccar, and Cummins(engine manufacturers) have failed to develop adjusted maintenance intervals, added inspections, or additional filtration to prevent contamination of the transmissions air system. They've instead passed the financial burden of repair on the customers by denying warranties. The dealer determined based on TAIB-1006 that the MTM requires replacement and the air system cleaned of contamination. I received a safety recall notice 26S28/NHTSA Recall 26V237 On the same day that this occurred in my truck. Luckily for me, I was within 2 miles of home, the only way I could get home was to drive in first gear up the mountain road to get there. My truck is not drivable. If this happens on a freeway, I most likely would be rear ended as the vehicle stops suddenly. What recourse do I have from Ford
... motor company since I can’t drive the truck that they sold me? The recall notice says a fix won’t be ready until July, and that is a long time from now. I need a vehicle to drive. I feel like not having this remedy ready yet means that it’s not ready in a timely manner.Read more

Hello, I am formally documenting a serious safety incident involving my recently purchased 2026 Dodge Ram 3500 Tradesman 6.7L dually, purchased from Towbin Dodge in [XXX] , on May 9, 2026. On May 13, 2026—only five days after purchase—we were operating the truck for our dumpster business while towing a GN trailer loaded with approximately 5 tons. We were traveling down Mount Charleston after
... a pickup when a critical brake failure occurred. While descending the mountain grade, the truck’s braking system failed to respond. A warning light appeared on the dashboard indicating an issue with the electric brake system. We had tow/haul mode and exhaust braking engaged, but neither system functioned properly. When my father pressed the brake pedal, the vehicle did not slow down. With a steep descent ahead and no effective braking response, my father was forced to steer the vehicle off the road into a ditch to avoid a direct impact with the mountain, which could have resulted in a fatal collision. After stopping, we noticed signs of extreme brake overheating, and the system appeared to be close to catching fire. I have video and other documentation of the incident. The vehicle is currently in the dealer’s possession. This is a major safety failure occurring within days of purchase, and I am requesting a full investigation into the brake system and all related components, including any recalls, defects, or electronic braking malfunctions. I do not want the truck anymore! Given the severity and risk to life involved, I am requesting immediate action and a full resolution regarding this vehicle. Worst dealership my husband and I ever dealt with. We purchased a 2019 Nissan XD Titan Diesel with warranty. Two days after purchase, the check engine light came on. I called Spencer from the service department to let them know. They took the truck the very next day; it was some issue with the gas tank, so the part was back-ordered. After getting that fixed, the check engine light still never went
... off. I questioned the dealership; all they could say was it was probably a dirty sensor. Then, not even two weeks later, I was leaving a doctor's appointment on the interstate when it threw up a code for low coolant, saying it was about to go into limp mode. So, I made it to the dealership's service station. They said it was okay and couldn't find anything wrong with it, then told me to just keep driving it until it broke down or delete it if it was them. They said, 'You know, we don't deal with a lot of diesel, so it may need to go to another dealership.' Then, two months went by, and we had more issues. We ended up back at the dealership to find out that it now had an oil leak that was quoted to us as a $14,000 job for parts and labor. We also had extended warranty and powertrain, but Spencer failed to file the warranty claim correctly the first time and filed under the wrong warranty, so they denied it. We had to call management to get them to check why, and the problem was that the service department didn't know how to file the correct warranty claim. After that was submitted, my husband asked Spencer directly and the service department if they did rentals through our warranty. They said they don't give rentals at that dealership. Note that we found out later through our warranty that they did pay, but we weren't given that chance. After picking up the vehicle, the check engine light was still on !!! there’s so much more details to the story!!!!!Read more

I had been hearing a ticking or clicking sound from the engine. When I brought it to a Honda dealership that I had used about a year ago for the 100,000 mile maintenance, I was told that the truck had a rod bearing failure and that I needed a new engine. I was told that if I kept driving it as is, that I could have catastrophic engine seizure without warning at anytime. This is a disaster waiti
... ng to happen. I saw that the same part on both the Ridgeline models of 2017 and 2019 had been recalled in 2023. The truck only has 110k miles and I have faithfully had routine maintenance. For an expensive $9,500 engine to fail at only 110,000 miles is not right. I will try to upload a picture of the invoice in a separate email. I hope this information will be part of the second investigation of the initial recall and also the present lawsuit against Honda in the state of California, thus sufficing as more evidence that the 2018 Ridgeline should be included in a Second Recall.Read more

I am filing a complaint regarding Consumer Portfolio Services (CPS) and the reporting and collection of a repossession deficiency balance related to my 2021 Ford EcoSport, VIN [XXX] and Ford Motor Company. ... te these known defects, CPS is attempting to collect a large deficiency balance following repossession and is reporting the account negatively on my credit report. I dispute the validity and fairness of the alleged deficiency balance because the vehicle’s value was negatively impacted by manufacturer-related defects and safety concerns. I also question whether the vehicle was sold in a commercially reasonable manner and whether the resale value properly accounted for the vehicle’s defective condition. Ford was notified and did nothing to resolve any matter. The vehicle stalled on th interstate in [XXX] while traveling to vacation. Ford has not reimbursed me all of my money.
