The Integrated Charging Control Unit (ICCU) failed on my 2023 Hyundai Ioniq 5. The vehicle stopped charging prematurely overnight via AC Level 2 charging and then refused to charge again at all. The ICCU is responsible for maintaining the 12V battery by topping it up during normal operation. No warning or check engine light came on, I was only made aware when I attempted L2 charging and the car wo
... uld refuse to take the charge. I understand that with a failed ICCU, if I'm unaware of the failure (e.g., relies solely on DC fast charging which bypasses the ICCU), the 12V battery can deplete without warning while the vehicle is in motion, potentially causing sudden loss of vehicle systems including power steering, brakes, and other safety-critical electronics. The vehicle was parked at the time charging failure was discovered. No injuries occurred.Read more

On Saturday 16th Oct 2026 around 10 AM EST, while driving down the ramp from Parking Floor 2 to Parking Floor 1 in the JFK Terminal 4 Short Term Parking garage (John F. Kennedy International Airport, New York), the Tesla vehicle suddenly accelerated forward very fast on its own without any input on the accelerator pedal. Either I was not pressing the gas pedal at the time, or I was slowing pressin
... g as the car was going down on ramp. The car surged ahead with significant force and struck a fence at the bottom/end of the ramp area. There were two passengers in the vehicle. No other vehicles were involved. The impact caused: My Tesla Model Y to sustain severe front-end damage and appear to be a total loss. Injuries to both passengers. Minor scratches on my hands. Sequence of Events: I was driving the vehicle at low speed down the ramp from Parking Level 2 to Level 1 in JFK Terminal 4 Short Term Parking. Without pressing the accelerator, the car unexpectedly accelerated. Braking did not prevent the impact. The vehicle came to a stop only after hitting the fence. Today, I have reported this as a sudden/unintended acceleration event to Tesla and requested a full vehicle data log review. I would like Tesla to investigate following. 1. Why the car suddenly accelerated on very speed? 2. Why the steering got locked when it happened? 3. Since it is the closed parking lot, why Tesla did not stop automatically before hitting fence? I have been driving for more than 27 years, I never had any accident, and I never got any tickets in last 27 years.Read more

Reporting a severe hydraulic/mechanical braking defect on a 2023 Nissan Altima with only 26,820 miles. Vehicle experiences persistent 'spongy' brake pedal feel, premature metal-on-metal rotor gouging, and severely darkened/degraded brake fluid, indicating a systemic hydraulic component failure or electronic brake-dragging defect. This is a severe safety hazard causing unpredictable stopping dis
... tances. Authorized dealer (Coral Springs Nissan) refused warranty coverage prior to the May 21, 2023 expiration date and attempted to extort a signed liability/warranty waiver under duress to release my vehicle, which has been documented in writing on the service order and on video footage. ***Manufacturer is actively concealing this safety defect to avoid a mandatory safety recall. I own an identical second 2023 Altima with no such issues, proving a manufacturing defect on this vehicle. I paid $63k for both cars brand new. The only difference is the color. This issue began 2023 just five months after purchase. I called Coral Springs Nissan who told me this was not the brakes but electrical components that are used now. I trusted what they told me. It made sense. How could breaks be like that on a new car? What they needed to tell me was get to the nearest Nissan immediately because they were aware they've been having cars with break issues especially between 2021 and 2023. DO YOU UNDERSTAND THESE ARE BRAKE ISSUES? And what is this I was told that they recently got a judge to sign off and block any more class action lawsuits?? If this was some kind of air-conditioner component and still it wouldn't be fair for people who are trusting in quality. But these are brake issues. These are safety issues! The car stopped at a light on a busy street in [XXX] (in the middle lane), then the engine stopped, and my wife could not restart it. I went as fast as possible and met her. I believe it is the stop-start capability, but I can't say for sure. We
... have also had bad engine oil burning problems and electrical problems. All of these (I now know, but did not before) are recurring problems for 2019 Tiguans. The cylinder head was rebuilt last year, associated with a similar instance. I am now fully aware that the 2019 Tiguans are known to experience all of these problems, but when I brought my car in on previous occasions, that never got mentioned. I am told XYZ is an issue, then billed for repairs. The car was originally leased, and when I purchased it at the end of the lease, I also purchased a full premium warranty. When the issue happened in May of 2025, the policy eventually covered this, but it took over a month of justifications and records before the third-party insurance company agreed to cover the repairs. All of this said, I am no longer comfortable that this car is safe for my wife to drive, as the issues that are repeating are known issues, and yet Volkswagen seems to sweep these under the rug from both a manufacturer and dealership perspective. I apologize if this does not meet the NHTSA bar of a safety hazard. It does reduce the ability for hands free navigation and acts as a distraction for drivers. Toyota Rav4s have very poor GPS tracking which makes the directions in the integrated head unit and guage cluster very distracting because they are consistently incorrect. There are dozens, maybe 100+, reports online of people having sim
... ilar issues. Toyota dealership has looked at it many times, escalated for troubleshooting, and as of today they say there is no fix. I have many videos to demonstrate the issue is not a fluke. There are no warning signs and the GPS will just start giving poor directions. The "danger" is in the fact that the only workaround now is to engage with a phone or other device while driving. In theory a human can rise above this and be smarter than the car but isn't that true of many safety components like non-functioning wipers(drive slow) or blind spot protectiong(check your mirror). The car is giving bad directions that the user is relying on as a navigational aid.Read more

As a loyal GM customer who has purchased three Cadillac Escalades and had them exclusively serviced at Cadillac dealerships, , I am writing regarding my 2023 Escalade, equipped with the 6.2L V8 (L87) engine, affected by the recall (N252494000) impacting approximately 597,630 vehicles . I recently took my vehicle to a Cadillac dealership for the recall inspection, where it passed, received a switc
... h to o@-40 Mobil 1 Supercar oil, and was granted an extended warranty of 10 years or 150,000 miles from the purchase date. Despite this , I am deeply troubled by GM's handling of the recall, which has caused significant unease, undermined my trust, failed to honor my loyalty, and imposed unfair burdens. I respectfully request clarification and action on the following issues: Inadequate Recall Remedy and Impact on Gas Mileage - The remedy for vehicles that pass inspection- switching from 0W-20 to higher-viscosity 0W40 oil- raises serious doubts about its effectiveness in addressing a manufacturing defect in the crankshaft and/or connecting rods, which GM acknowledges can lead to engine failure and increase crash risk. Many owners, including myself, question whether thicker oil is a permanent solution or a temporary workaround. Additionally, the switch to 0W-40 is likely to reduce fuel efficiency due to increased engine drag, imposing an unaddressed cost in deduced gas mileage. This is especially concerning for me, as I rely on my Escalade for annual drives from [XXX] to [XXX] for the winter months. Can GM provide technical evidence proving the oil change fully resolves the defect, and will the company compensate owners for the loss in fuel economy? Inconsistent Communications Regarding the Extended Warranty - The dealership assured my vehicle is "fine" after passing the inspection, yet GM is providing an extended warranty of 10 years or 150,000 miles. This contradiction - claiming the engine is defect-free while extending warranty coverage - suggests GM anticipat INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INRead more

Technicians at Bobby Rahal BMW explicitly instructed me to return the car to BMW of Pittsburgh because they suspected the original VANOS safety recall repair was botched. On October 28, 2024, I returned the vehicle to BMW of Pittsburgh to address this issue. Their technicians opened the engine block assembly a second time, yet failed to correct the defect. Following this second visit, the check engine light continued to come on intermittently. The vehicle began suffering rough idling, difficulty starting, and dangerous stalling events during operation. Cycling the ignition on and off provided only temporary relief. The check engine light has now become permanently illuminated. The safety hazards have severely escalated into a protective engine "limp mode," causing violent drivetrain shuddering and a catastrophic loss of driveability. BMW of Pittsburgh refuses to honor the recall workmanship, demanding a predatory $6,000 out-of-pocket teardown fee to bypass their liability. This caused the vehicle behind me to nearly rear-end me, creating a high-risk traffic situation. These events occurred with no obstacle or reason that would justify emergency braking. In addition, the vehicle developed a coolant leak around 20,000 miles, raising concerns about overall reliability and potential related system issues. May 11, 2026 approx 9:35am. I was in a flat parking area at a gas station waiting for a friend. There were 2 other cars parked near me. No one was in them. Neither were moving. I backed out of my space into a large, empty, open area to turn my car around to have a better view. Because the audi pre-sense chronically malfunctions, I always turn it off before I drive. But I left it on this t
... ime because I was only driving about 15 feet in a parking area. I backed out, then stepped on the brake to turn the wheel. While I was AT A FULL STOP, the seatbelt motor started up loudly and grabbed my chest with the seatbelt, slamming me back against the seat. No one was there. No cars were moving. I was at a stop. When the seatbelt let me go, I noticed a red lighted image of some sort on the screen. I believe there was a red star-shaped image and some text indicating a collision. Then the image disappeared. I turned off pre-sense and backed into my parking space. It was painful and frightening - especially as I have osteoporosis. This car has previously slammed the brakes on an empty road while I was driving 45-50 mph. Dealer says there are no codes or errors and I need to clean the sensors. I bought it new. Likewise, the rear cross-traffic, pedestrian detection etc. rarely works and sometimes fires for no reason. If the brakes slam when there's a car behind me, I'll be rear-ended. If the seatbelt slams while I'm on the road, I'll certainly swerve from the discomfort and being startled. Very dangerous. NOTE: My previous report seems to be removed from this website. This is a well-known and widely reported issue: 2013 Hyundai Sonata Hybrid car, which has recently experienced the following dashboard alert: "Hybrid system warning! Safely stop and do not drive." The car repair shop indicates that there are no codes saved when this happens. This is apparently a documented issue, where the codes are not saved, and difficult to diagnose by repair shops and dealers. It could lead to the vehicle completely shutting down while driving at highway speeds within traffic, which could cause a serious accident involving multiple vehicles and people. Why is this not recalled by the dealer and fixed? This has been reviewed by the dealer and by an independent repair shop. On January [Insert Date], 2026, my 2024 Ford Escape suffered a sudden turbocharger failure that saturated the vehicle’s internal HVAC system with toxic ethylene glycol coolant. The vehicle’s ventilation system subsequently pumped these toxic chemical vapors directly into the enclosed passenger cabin. The inhalation of these toxic chemical vapors posed an immediate, life-threatening risk to my
... pregnant wife and myself (a patient with moderate persistent asthma), backed by formal medical warning letters from our OB/GYN and Pulmonologist. When I refused to take delivery of the un-remedied vehicle, a Ford corporate representative mandated that I drive it anyway, stating that if I suffered a respiratory crisis while operating the vehicle, I should simply "call the ambulance". This ultimatum caused an immediate severe hypertensive crisis (160/112 BP), requiring emergency medical transport to CHI Health Immanuel and cardiac evaluation. This represents a critical component failure causing toxic cabin air contamination, compounded by a complete failure of the manufacturer’s remedy. Ford Engineering and Woodhouse Ford Service admitted in writing that they have NO scientific test to verify cabin air safety and NO official technical procedure to decontaminate the internal HVAC housing following this chemical saturation. Ford’s only attempt at a remedy was an external spot clean and a simple filter replacement, leaving toxic residual chemicals inside the blower housing. Ford's directive to operate a chemically contaminated vehicle without any scientific validation of air safety constitutes an extreme, ongoing non-operational safety defect that risks public safety. I was driving my 2024 Toyota Corolla SE gas model (with the convenience package) on the interstate in [XXX] during stop-and-go traffic. Traffic had been moving slowly for about 15 minutes, and I had Brake Hold turned on, cruise control was off, Proactive Driving Assist was on. I was behind a semi truck and was moving very slowly, getting ready to stop. I had maybe 3 or 4 car lengths between me an
... d the semi so they could see me in their mirrors. As I was about to stop, the brake pedal suddenly went all the way to the floor, the car jerked to a stop, and the electronic/emergency parking brake applied on its own. I put the car into Park and then back into Drive, and the problem did not happen again during that drive. There were no warning lights, dash lights, or warning messages before this happened, and there were also no warning lights or dash lights afterward. The issue happened suddenly with no prior symptoms that I noticed. I believe the brake system, Brake Hold system, electronic parking brake system, or a related brake/vehicle control system malfunctioned. This put my safety and the safety of others at risk because the car unexpectedly stopped abruptly in interstate traffic. Since I was in heavy stop-and-go traffic and other vehicles, there was a risk of being rear-ended, causing a crash, or becoming disabled in traffic. I took the vehicle to a Toyota dealership after the incident. They were not able to reproduce the problem, but they did find stored code C14FE14. They told me they checked the brake system, cleared the code, tested the steering angle sensor connector, and said everything tested within specification. They also performed a multipoint inspection and said the battery, brake pads, and other inspected items tested well. To my knowledge, the vehicle has only been inspected by the Toyota dealership/service center. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The Jayco manufacturer cut structural support members (“X” supports) during assembly, allegedly to provide plumbing clearance. On multiple occasions (four) my 2025 Toyota Grand Highlander Hybrid Max has failed to accelerate when I push the gas pedal. Once in traffic, which was extremely scary and a huge hazard.
