On [XXX], my 2017 Land Rover Range Rover HSE Td6 diesel (VIN: [XXX] ) suffered a complete, catastrophic engine failure without warning at approximately 70 mph on [XXX] . My wife and our two infant sons (ages [XXX] and [XXX] ) were passengers. There were no prior warning lights or symptoms. The engine stopped instantaneously, eliminating power steering and power brakes while I was overtaking a sem
... i-truck in the passing lane. We narrowly reached the shoulder safely. The odometer read 79,018 miles — inside both limits of the federally mandated 8-year/80,000-mile emissions warranty under 40 CFR §85.2103. The vehicle was 7 years old at time of failure. Hoehn Land Rover Carlsbad diagnosed complete engine failure caused by timing chain guide failure — a known defect pattern in the Ingenium diesel engine. The service manager stated failures like this "aren't that rare" with these engines. Repair estimate: $28,000–$30,000. Jaguar Land Rover North America (Case #[XXX]) denied the warranty claim entirely, offering $800, without ever evaluating the claim under the active federal emissions warranty. Their denial cited only the expired extended CPO warranty and a maintenance gap — neither of which constitutes valid grounds for denying an emissions warranty claim under 40 CFR §85.2103(d), as the manufacturer cannot demonstrate the maintenance gap caused a documented design defect. This failure pattern is documented in JLR Technical Service Bulletins LTB00735 and SSM74184. Land Rover Korea issued a recall for the same timing chain failure in 2020. No equivalent recall was issued in the United States for identical vehicles with identical engines. JLR has refused all requests for review or reconsideration for 24 months. Daytime running lights have both failed within the last 79 days. Passenger side failed at ~32660 miles o/a 05 Mar 26. It was replaced under warranty. Driver side failed around 36234 miles on 22 May 26 (5-days out of warranty). It is pending a shop appointment for replacement. This is not a “wear and tear” issue, it is a product quality control issue. It is not a simple plug and play re
... pair. It is a costly repair due to the design of the headlamp assembly. I have owned vehicles since 1988 and last changed a headlamp or taillight bulb in the early 2000’s and that was on a MY93 vehicle. I have also owned two other Land Rovers MY07 LR3 from 2008-2011 and MY14 LR4 from 2014-2022 and did not have to replace a single bulb in the vehicle. Point is that technology and quality have improved over the years and these headlamp/DRL assemblies should not fail with less than 36500 miles on the vehicle. This is a known issue that warrants further investigation. Jaguar Land Rover has issued a Special Service Message 75479 concerning the known issue with the Defender DRL. My DRLs failed in similar fashion described in the SSM. Inoperable DRL reduces safety due to lower visibility since they should operate during normal driving conditions. This issue is dangerous since there is no indication on the dashboard or other warning unless the headlights are manually inspected while the vehicle is operating. Additionally, inoperable DRL leads to a failed State vehicle safety inspections.Read more

I was operating a 2024 Kia Telluride and was fully stopped in traffic when my vehicle was rear-ended by another vehicle. Immediately after the impact, my vehicle experienced apparent unintended forward propulsion/movement despite my braking attempts. Following the collision, the vehicle continued moving/driving forward for approximately 20 seconds and did not respond normally to brake input. The
... behavior created a loss-of-control situation and significantly increased the risk of secondary collisions, injury to vehicle occupants, nearby motorists, and pedestrians. I was unable to immediately stop the vehicle despite attempting to do so. The exact failed component has not yet been definitively identified; however, the incident appears potentially related to malfunction or unintended operation of one or more of the following systems: * drivetrain/transmission engagement, * electronic throttle control, * braking system, * powertrain control module, * or Advanced Driver Assistance Systems (ADAS). The vehicle should be inspected for: * Event Data Recorder (EDR) data, * diagnostic trouble codes, * throttle/brake command activity, * drivetrain engagement data, * and post-collision electronic system behavior. The vehicle is available for inspection upon request. At this time, the issue has not yet been reproduced intentionally. The vehicle has not yet been fully evaluated or confirmed by a Kia dealer or independent repair facility for the specific unintended movement condition. The vehicle is expected to undergo inspection and diagnostic review as soon as the Progressive adjuster issues the acceptance/approval letter. The vehicle will then be observed by insurance representatives, and collision repair personnel, as part of the accident and claim process. My car caught on fire on [CCC] and it's completely burned down, a total loss. Following is information about my car and some summary of what happened. Car information: - Mazda CX5 2024 - Bought in June or July 2024(car payment only made 22 times so far) - Odometer 24,445mi(as of 05/15/2026) What happened: - In [XXX] Friday afternoon, I was in Bellevue and noticed an engine overheat warning. So,
... I stopped immediately to cool down and called Mazda Dealership(Lee Johnson Mazda) to schedule a service. - Then after 20-30 minutes, I started the car again and the warning was gone. So I drove slowly and got on [XXX] to head home in Kirkland. I was already driving slowly. - After a few minutes, overheat warning as well as engine warning started firing and noticed that smoke coming out of bonnet. - So, I pulled over on the shoulder near [XXX] and called roadside assistance. But even before I get connected to the roadside service, engine caught on fire and I had to call 911. - The car is completely burned down and it's a total loss. - State patrol wrote me a "Exchange of Information" and "Tow/Impound and Inventory Record". Why I am submitting this form: - This car is less than 2years old and has been seeing Mazda service regularly. The odometer checked by dealership on 05/15 is 24,445mi which is slightly more than 24,000 which is stated in the Lemon LAW. But still, the car is pretty brand new and around 24,000 ish. So, I'm wondering if this still qualifies for Lemon Law. - Even before this happened, I had a service history for engine warning and get the engine checked on 05/15/2026 which is exactly a week ago before the accident. Also, they said the engine thermostat had an issue and replaced and covered by Mazda's warranty. - Overheat and engine warning was on and off. As soon as I pull over, it caught on fire. - This happened on [XXX]. It was very dangerous and lucky that we pulled over on right before it caught on fire INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

This vehicle is experiencing an aggressive, heat-accelerated hardware degradation of the central capacitive screen assembly commonly identified as ghost glitching, culminating in a total crash of the primary graphic interface and touch controller hardware. While your customer service infrastructure routinely classifies display anomalies as low-priority convenience or entertainment glitches, an obj
... ective systems analysis reveals that Honda has centralized critical safety telemetry, emergency communications, and vehicle calibration gateways into a single, unbacked hardware node. Consequently, the catastrophic failure of this infotainment module directly compromises the operational safety of this vehicle, presenting an immediate, unpredictable hazard not only to myself but to the general driving public sharing the roadway. The systematic failure modes of this defect are detailed as follows: 1 Multi-Module Network Failure and Primary Gauge Cluster Blindness The failure of the central display audio unit is not isolated to an entertainment screen. It acts as a network parasite that actively destabilizes the vehicle's primary Driver Information Interface DII on the steering wheel instrument cluster. Toggling the steering wheel controls to view Audio or System telemetry on the primary driver's dash gauge results in an immediate, total blackout of the left digital dashboard display. Because the central infotainment unit's microprocessor is frozen, degraded, or locked up, it fails to respond to high-priority internal Controller Area Network CAN bus data requests from the instrument cluster. This creates a severe data bottleneck that blinds the operator to system telemetry directly behind the steering wheel while traveling at highway speeds. Furthermore, while the vehicle's secondary hardwired communication circuit allows background audio streaming and steering wheel track-skipping overrides to function, the manual physical volume controls, dashboard knob,Read more

I am writing to formally report a critical safety defect regarding the OnStar Telematics Control Module in my 2022 Chevrolet Tahoe High Country VIN: [XXX] . A few days later we found a 2nd tow truck that agreed to bring the vehicle several hundreds of miles back to our home city. This car is a 2020 model, and has around 55,000 miles. It has been meticulously maintained by a certified Mercedes mechanic. There were no issues with the car and no indication that there might be a problem. Our mechanic has thoroughly inspected the vehicle and said that the engine has completely seized and the only option is to replace it. We have no idea why this happened. If requested, the vehicle is available for inspection.Read more

The issue involves the second and third-row power folding seat control system in my 2026 Hyundai Palisade Calligraphy Hybrid following Recall 296. Before the recall update, the vehicle included driver-controlled power folding seat functions through infotainment screen controls and one-touch seat folding operation. After Recall 296 was performed, Hyundai disabled or significantly altered these fun
... ctions due to safety/compliance concerns involving the seat folding system. The affected component is available for inspection upon request. The vehicle has already been inspected and updated by Hyundai under the recall process. My concern is that the recall confirms there was a safety-related issue involving the power folding seat mechanism and occupant protection. While Hyundai modified the system to address safety concerns, I believe NHTSA should continue monitoring whether the revised operation fully resolves the underlying safety risks and whether similar concerns may affect additional vehicles or model years. There was no crash, injury, or fire in my case. No warning lamps or error messages appeared before the recall update. The operational changes became noticeable after the recall remedy/software update was completed. The issue has been confirmed by Hyundai through Recall 296 and Hyundai Motor America’s response regarding the intentional operational changes made to the vehicle.Read more
