The starter on a new 2026 Chevrolet Equinox RS failed at approximately 6,500 miles. While the vehicle was being driven home during evening rush hour, it shut off at a red light and would not restart. The vehicle was stranded in the left lane of a four-lane highway. It could not be moved because the engine would not start, and the vehicle would not even roll in neutral, as the wheels were locked an
... d would not turn. The disabled vehicle blocked a live travel lane in heavy traffic. The driver was a [XXX] new driver. Being stranded in a moving traffic lane during rush hour created a serious risk of a rear-end collision to the occupant and to surrounding vehicles forced to maneuver around the disabled car. Police responded because the vehicle was creating an active roadway hazard and arranged for a tow. The tow operator had to drag the immobilized vehicle onto the flatbed because the wheels would not turn. The failure was confirmed by an authorized Chevrolet dealer, which diagnosed a failed starter and replaced it under the manufacturer's warranty. There were no warning lamps, messages, or symptoms prior to the failure. The vehicle shut off and failed to restart without any advance indication. The vehicle has been repaired and is available for inspection. The failed starter was replaced by the dealer. I am submitting this complaint after learning through a recent TechCrunch article regarding the NHTSA investigation PE26004 into Rivian rear suspension / rear toe link failures that my incident appears substantially similar to the failures currently under investigation. On March 2, 2024, at approximately 1:29 AM, my son [XXX] was driving my 2023 Rivian R1S (VIN [XXX] ) in [XXX] . While making a
... normal right turn from [XXX] onto [XXX] and accelerating normally afterward at approximately 30 mph, the vehicle suddenly suffered what appeared to be a catastrophic rear suspension failure. According to the driver, the left rear portion of the vehicle suddenly jolted downward, a loud mechanical breaking sound was heard, and the vehicle immediately became uncontrollable. The rear suspension appeared to collapse instantly. The vehicle then began sliding uncontrollably across the roadway and crossed into the opposite traffic lane. The driver attempted to release the accelerator and correct steering input, but the vehicle did not respond normally. Steering control was effectively lost and the vehicle could not be controlled. The vehicle ultimately impacted a tree on the left rear/driver-side rear area before coming to rest perpendicular to the roadway. Importantly, the loss of control occurred BEFORE impact with the tree. The impact was a consequence of the apparent suspension failure and resulting loss of vehicle control, not the initiating cause of the event. There was no rain and road conditions were normal and dry. Driver-side airbags deployed and Rivian’s emergency response system automatically contacted the occupants after the crash. Police responded to the scene within minutes and a traffic crash report was generated. The vehicle had approximately 7,862 miles at the time of the incident and remained under factory warranty. The vehicle had never undergone any prior suspension repair, suspension service, or suspension INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6Read more

Component Affected: ENGINE AND ENGINE COOLING Description of the Defect & Incident: My 2024 Nissan Rogue equipped with the 1.5L VC-Turbo engine is exhibiting severe progressive mechanical failure, matching the symptoms described in NHTSA Campaign Numbers 26V080000 (Engine Bearing Seizure) and 26V081000 (Throttle Body Gear Failure). The vehicle is currently experiencing excessive engine shaking, a
... violently rough idle, abnormal mechanical noise from the engine bay, and a dangerous drop in motive power/acceleration. History of the Issue & Dealer Denial: I originally identified and proactively reported an abnormal engine noise to an authorized Nissan dealership in May 2024. The service department issued a "could not duplicate" finding and turned me away. In June 2024, I provided a digital audio/video recording of the symptom at that time. Following the closure of the initial Nissan dealership, I transferred the care and servicing of my 2024 Nissan Rogue to Future Nissan of Roseville. I explicitly reported the recurring, abnormal, loud engine noise and excessive engine shaking with no remedy offered. Following the official February 2026 recall announcements, the symptoms have severely worsened, progressing to violent engine shaking. I returned to the dealership for the recall remedy. The dealership service department is actively denying recall coverage, claiming the current failure is an unrelated "secondary issue" rather than a consequence of the recalled components. Because Nissan's own NHTSA filings explicitly state that these defects are progressive and give advanced warning through "unusual noises or vibration/rough running" before complete engine failure, the dealership's refusal to honor the recall is a violation of federal safety standards. The alleged secondary damage is direct consequential damage from a primary safety defect that I documented within the initial warranty period. Nissan must take responsibility for this failure.Read more

EHICLE: 2023 Kia Sorento, VIN [XXX] , currently 48,000 miles. Vehicle has never been in a collision. DEFECT: MASTER POWER WINDOW SWITCH / WIRING HARNESS — OCCUPANT EGRESS HAZARD The driver's master power window switch fails intermittently, causing simultaneous loss of all window operation and electric door lock function. When this occurs, occupants cannot operate any window or unlock any door el
... ectrically until the condition resets, typically by opening and closing the driver's door. The condition was first reported to Arapahoe Kia at 24,859 miles on 11/08/2024 (RO 208018) and again at 25,050 miles on 11/15/2024 (RO 208449). On the 11/15/2024 visit, the dealership confirmed the defect on customer-supplied video and performed a full diagnostic including pin tension testing on connector D01, power, ground, B-CAN continuity testing, and a wiggle test of the harness. The dealership's written conclusion on the repair order: "we've determined that the harness is probably not the issue and it will need a new master switch." The master switch was not ordered or installed, and the vehicle was returned with the defect unresolved. The condition continues to occur intermittently as of the current 48,000-mile reading. SAFETY IMPLICATION: This defect creates an occupant entrapment and egress hazard. In a fire, submersion, or crash event where the mechanical door release is inaccessible, inoperative, or where occupants are incapacitated, simultaneous loss of window operation and electric door lock function eliminates secondary egress options. The defect is intermittent and unpredictable. The vehicle remains within the 5-year / 60,000-mile basic warranty period. Very loose and clanking around.” Despite verifying this severe manufacturing safety defect, Service Manager April McCall formally refused to submit a corporate Goodwill or Warranty Extension claim to FCA unless I paid out-of-pocket to replace three CV axles, which she claimed were "loose and dry." To verify this claim, the vehicle was immediately taken to a secondary licensed automotive facility (Dr. Auto LLC, Invoice #XXX) on May 27, 2026. The independent inspection confirmed that the dealership’s diagnosis was fraudulent. All CV axle boots are completely intact and sealed, meaning the internal grease has not escaped. The front axles have zero play, and the rear axles exhibit only normal, minor factory play. The dealership intentionally used a loaded 4-post lift to misdiagnose the vehicle, leveraging the high-frequency vibrations of the failing central PTU to fabricate secondary, non-covered user-pay repairs. This is a deliberate corporate tactic used to deflect financial liability for a known federal safety defect onto the consumer. I am requesting that the NHTSA review these attached service documents as evidence of predatory dealership behavior surrounding the Cherokee PTU failures. Hello, I was driving home from work with my six-month old in the car and the engine stalled while on the Belleville Turnpike. It seized and I was almost rear-ended by a tractor trailer hauling lumber to the lumber yard just a few streets away. I couldn't move the car and was afraid of how to even attempt to secure my car as my daughter was strapped in her car seat. An extremely nice man pulled ov
... er and began to flag vehicle to go around me. By this time, I had only my hazard lights on. I was frightened beyond belief having gone through that experience. The car was towed to the dealership who confirmed it was catastrophic engine failure(misfire of a cylinder). They stated initially with my 3rd party warrant that it would be covered and in their words " it was a good thing I had a warranty". Fast forward to two weeks after I check to see what is going on. Things have changed. Now, because I am missing one oil change receipt, they need to be paid $1400, to further corroborate the cause of the engine failure. I haven't paid and honestly don't plan on paying them anything. To date, I have spent over $2,000 on car rental fees since my car is now sitting at the Global Hyundai Dealership.Read more

While driving my 2023 Nissan Rogue on Interstate xxx from [XXX] to [XXX] with my xxx daughter in the vehicle, the vehicle suddenly displayed multiple warnings including collision assist/sensor malfunction warnings, start/stop system failure warnings, and “check owner’s manual” messages. Immediately afterward, the vehicle experienced a sudden loss of power and refusal to accelerate. At the
... time of failure, I was traveling approximately 78 mph in the fast lane of the interstate. The vehicle rapidly lost speed and dropped into what appeared to be limp mode, eventually slowing into the upper 50 mph range despite pressing the accelerator. I had to fight across multiple lanes of freeway traffic while unable to maintain highway speed. This created an extremely dangerous situation due to surrounding high-speed traffic and limited safe shoulder access in a remote desert area. The vehicle became unsafe to continue driving and required a tow of over 70 miles to the nearest Nissan dealership. The tow expense alone exceeded $600. Prior to this incident, the vehicle had already experienced a separate unexplained no-power/no-start event. At that time, the battery, starter, and alternator reportedly tested normal. The vehicle remains within the manufacturer’s 5 year/60,000 mile powertrain warranty and currently has active recalls associated with the vehicle. A Nissan case has been opened and the vehicle is pending dealer diagnosis and inspection. After posting publicly about this incident in Nissan owner groups, I identified numerous other owners reporting nearly identical symptoms involving warning light cascades, sudden loss of acceleration, limp mode events, and highway safety concerns. This incident created a serious safety hazard involving sudden highway deceleration and loss of motive power during interstate travel. I am writing to report an unresolved high-voltage safety defect involving the Motor Power Inverter Module on my 2019 Honda Clarity Plug-In Hybrid. The vehicle's high-voltage hybrid propulsion inverter is emitting a severe, high-pitched mechanical squealing noise during acceleration and regenerative braking. The high-voltage inverter serves as the foundational link regulating current between the lit
... hium-ion battery and the electric drive motor. A failing inverter module presents an immediate, unreasonable safety hazard. Internal failure, thermal runaway, or component degradation within the inverter stack creates an imminent risk of sudden high-voltage isolation fault. This will result in an immediate vehicle stall and a total loss of propulsion while driving at highway speeds, leaving the vehicle stranded in traffic with no power to accelerate out of harm's way, creating a severe crash hazard.Furthermore, an authorized Honda dealership—Visalia Honda (Groppetti Automotive) in California—has failed to properly diagnose, isolate, or resolve this high-voltage drivetrain defect despite the vehicle being brought in specifically for this issue. Despite the component being legally covered under California’s mandated 15-year/150,000-mile extended emissions and hybrid warranty, the dealership has taken no constructive corrective action, allowing a known, physically degraded high-voltage component to remain active on public roadways.Federal intervention and a formal technical investigation into Honda's clarity hybrid propulsion inverter systems are required to address this vehicle stalling hazard before an electrical failure results in a highway collision.Read more

I am filing this complaint regarding an active safety recall on my 2024 Toyota Tundra involving manufacturing debris left in the V35A 3.4L twin-turbocharged V6 engine during production. This recall has been open for multiple years and spans over 250,000 vehicles across the 2022–2024 Tundra, Lexus LX 600, and 2024 Lexus GX 550. The manufacturing debris causes damage to the engine’s main bearing
... s, resulting in engine knocking, rough running, failure to start, and sudden loss of motive power while driving at speed — a condition Toyota itself acknowledges increases the risk of a crash. Toyota deployed a remedy consisting of full engine replacement; however, Toyota has since acknowledged that the replacement engines are experiencing the same failures, meaning there is currently no confirmed, working fix. Despite this, Toyota continued selling new vehicles with this known defect. Remedy deadlines have been repeatedly promised and repeatedly missed, with the latest expected resolution pushed to July/August 2026 — a deadline that has already slipped multiple times. As an owner, I am left driving a vehicle that Toyota acknowledges is dangerous, with no confirmed timeline for a legitimate repair, and suffering significant financial harm due to the ongoing depreciation and diminished resale value caused directly by this unresolved defect.Read more

In 2025 I took my truck in for a recall reference 24S37 which was a software update to prevent intermittent failure of my output shaft sensor in my transmission. My sensor is now failed and FORD is claiming that there is no remedy for coverage as I already did the software update but my truck is still experiencing problems

Frame cracked and suspension issues.
Incident date: May 27, 2026
While driving, the vehicle suddenly began running rough, losing power, and displaying engine warning lights. Due to the severity of the issue, the vehicle was towed to a GMC dealership for diagnosis. The dealership diagnosed lifter failure and a cracked cylinder head. The dealership also documented coolant intrusion, oil/coolant contamination, and evidence of internal engine damage. Repairs were
... attempted, including replacement of the cylinder head and lifter. After the repairs were completed, the vehicle never left the dealership because it continued misfiring on the same cylinder. GM later determined the engine had a failed piston ring and scored engine block requiring complete engine replacement. The vehicle has approximately 55,000 miles, has been properly maintained, and was purchased new. This catastrophic engine failure resulted in loss of power and major internal engine damage at relatively low mileage. I also opened a case directly with GM Customer Assistance regarding this failure. The case involved multiple escalations after the initial repair attempt failed and the vehicle ultimately required full engine replacement. GM ultimately advised that only partial assistance toward the engine replacement would be provided, leaving the customer responsible for a significant portion of the repair cost. I am submitting this complaint because this vehicle appears consistent with the ongoing NHTSA investigation involving GM 6.2L L87 engine failures and loss of motive power.Read more
