After stopping at a red light behind a few cars in a left turn lane, the Auto Idle Stop (AIS) system turned off the engine. A moment later the light turned green and I let my foot up from the brake pedal. The engine failed to start. The hazard lights turned on automatically. I reapplied pressure to the brake and pressed the Engine Start/Stop button, which had no effect. Electrical systems remained
... active (i.e. the car did not turn off completely), but the engine also didn't start. I ensured the vehicle was in park and stepped out to verify the fuel door was closed (I had just filled the tank) and check the battery connections. Cars were going around us on the passenger side, and oncoming traffic was very close to me on the driver's side. I was concerned for myself and my passengers. No one was injured, nor was there a collision. After returning to the driver's seat, I tried the Engine Start/Stop button again. This time the car shut down. I waited a minute, then started the car. The engine started successfully. The vehicle is available for inspection. The problem has not been reproduced. We have been disabling AIS since the incident, so it's unlikely to happen again. The problem has not been confirmed by a dealer or service center, nor has it been inspected by anyone yet. I noticed the engine being slightly slower to restart after AIS stopped it in the few weeks leading up to the incident. There were no other symptoms I noticed.Read more

At approximately 87,000 miles, my 2021 GLE350 (M264 2.0T engine) experienced a severe engine misfire resulting in a flashing check engine light, rough idle, engine shaking, significant loss of power, and near-stall condition. The vehicle was taken to Speedhaus Automotive (Newbury Park, CA) where all four ignition coils and spark plugs were replaced. Total repair cost was approximately $635. This f
... ailure pattern is consistent with the documented M264 exhaust valve seat wear issue acknowledged by Mercedes-Benz technicians. A Mercedes dealer service report on an identical 2021 GLE350 explicitly states: "COMMON ISSUE FOR EXHAUST VALVE SEATS TO WEAR — KNOWN ISSUE WITH 260 AND 264 MOTORS." Mercedes-Benz has extended the warranty on the M260 engine (transverse variant) for this identical failure mode but has not extended it for the longitudinally-mounted M264 despite the same defect being present. Repair costs for full cylinder head failure on the M264 are being quoted at $6,000–$13,000. I am requesting NHTSA investigate whether a safety recall is warranted for the M264 engine across all affected GLE350, GLC300, C300, and E350 vehicles.Read more

Initial Breakdown: July 2024, my 2021 Kia Soul (under 40,000 miles) idled roughly and stalled while in motion. It restarted, but as a precaution, I towed it to Kia Greenbrier Chesapeake while it was operable. After diagnostics, the vehicle would no longer start. Warranty Denial: The dealer blamed engine sludge from two missed oil changes in 2022 and quoted $10,000 for a new engine. In August 2024
... , I appealed the decision but was denied (Exhibit A & B). There were no recalls active for the 2021 model at this time, but there were recalls for the 2018-2020 models for identical symptoms. Third-Party Repair: I paid $3,017 out of pocket to an independent shop for a new timing assembly to make the vehicle safe and drivable again (Exhibit C). Because my daughter is disabled and requires transportation, getting repairs was imperative. A new valve kit, fresh synthetic oil, and filter service were completed by the third-party repair shop. I also returned to Kia Virginia Beach for several subsequent oil changes (Exhibit D). In April 2025, Kia Corporate issued an official safety recall for my exact vehicle (Exhibit E). Before that, in March 2025, I attempted to have the newly issued recall (25V-099) performed, but the dealer turned my vehicle away, claiming a lack of parts. Final Failure: In May 2025, the night before a rescheduled recall appointment, the car became permanently inoperable (at only 47,500 miles). Due to extreme financial hardship and health concerns, I am without my vehicle and without an option. Complaint: The Kia dealership refused to perform the safety recall remedy, verbally citing the same 2022 maintenance gaps from the previous year. The dealership violated federal recall regulations for a known defect. I was left stranded, resulting in a financial charge-off of my auto loan.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The Engine Control Module (ECM) is malfunctioning, triggering persistent P0101 codes related to the Mass Air Flow (MAF) sensor. This specific issue is addressed in Nissan Technical Service Bulletin DE97AA, which recommends an ECM reprogramming. The component is still installed in the vehicle and is a
... vailable for inspection. ⸻ • How was your safety or the safety of others put at risk? When the vehicle is at a complete stop and I attempt to accelerate, it becomes jumpy and unresponsive, refusing to accelerate properly. This creates a serious safety hazard, especially when trying to merge into traffic or cross intersections, as I am unable to match the speed of other vehicles. This increases the risk of being rear-ended or causing an accident due to unexpectedly slow acceleration. ⸻ • Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. An independent mechanic diagnosed the issue and confirmed the code P0101. After reviewing Nissan’s official TSB DE97AA, they advised me to go to a Nissan dealership to have the ECM reprogrammed as outlined in the bulletin. ⸻ • Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle was inspected by an independent mechanic, but not yet by the manufacturer. The dealership is aware of the issue but has refused to perform the repair without payment. ⸻ • Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The Check Engine Light is consistently triggered by code P0101, and the traction control warning light also comes on when the issue is active. The symptoms appeared recently and continue to recur even after the code is reset by standard non-dealership methods like unplugging the battery.Read more

Dear Sir or Madam: I am submitting a safety complaint regarding my 2022 Audi Q3 involving water intrusion from the sunroof. At approximately 41,000 miles, approximately a month ago, water began leaking through the front edge of the sunroof despite the sunroof never being opened prior to the issue. The leak was not caused by clogged drains or lack of maintenance. Water entered through the overhe
... ad console, resulting in failure of the interior lighting system causing lights to flicker on/off and water dripping directly onto me, the driver, while operating the vehicle. These issues created a significant distraction and potential safety hazard, especially during night driving. The dealership diagnosed the issue and confirmed that the repair required modification of the sunroof assembly and track to correct a water intrusion problem. This was not a routine maintenance issue. The total cost of repair was $1,259.18, which I paid out-of-pocket. Additionally, the service advisor at the dealership stated that this is a frequent problem seen on Audi Q3 vehicles and expressed surprise that it has not already been subject to a recall. Given that this issue involves water intrusion affecting electrical components and actively interfering with safe vehicle operation, I believe this may represent a broader design or manufacturing defect impacting vehicle safety. I respectfully request that NHTSA review this issue for potential investigation and possible recall action. Thank you for your attention to this matter.Read more

I purchased a 2021 Tesla Model Y (VIN: [XXX] ) used directly from Tesla. I am the first retail owner outside of Tesla. I am currently experiencing a knocking noise from the driver side suspension. Tesla’s response does not address that these are federal safety recalls, not warranty repairs. Federal law requires recall repairs to be performed free of charge regardless of warranty status, age, or mileage. Tesla has also failed to provide written documentation confirming whether these recalls were completed prior to selling me the vehicle. Instead, I have been redirected to a self-service VIN lookup tool and a general data privacy request form — neither of which confirms when, where, or whether the recall work was actually performed. On May 23, 2026 I was driving family and friends around San Francisco, CA in my 2017 Honda Pilot (48K miles). We were driving up multiple steep hills, up to a 20% grade in some places with a full car of 7 people. The car handled it well. However, as we neared my home 25 miles away, the car started shaking and the check engine light started flashing. We searched online and the advice was to pull
... over immediately to prevent the car from overheating and causing permanent, expensive damage. We eventually made it home, shaken, and didn't touch the car for a couple days to let it rest. I learned that there is a warranty extension "Warranty Extension TSB 20-100" for what seems like this exact problem concerning the fuel injectors. The spark plugs have been replaced in the last year, which eliminates that as a potential issue. The next week, I took the car to Honda San Carlos where it's been maintained most of its life. I informed them of the symptoms and that the car is eligible for this warranty extension. They tested it and found nothing. I told them that testing it in the shop or driving it around a few minutes on flat ground with one person is not going to replicate the driving conditions we encountered but they didn't seem to care and I had to pay $250 anyway. I took this to another dealership and the guy said it sounded exactly like an Warranty Extension TSB 20-100 issue. They tested it and also could not find anything. I called Honda Corporate and they will also do nothing. This is clearly a safety issue as this problem will happen again at some point. I have a family and young children and do not want to be stuck on the freeway as a road hazard to ourselves and other drivers. Honda has clearly identified this as a problem yet they won't authorize the repair. They are waiting for this to happen again before they'll fix it, and who knows if this ticking time bomb will occur in a safe place or a dangerous one.Read more
