My Transmission failed on me driving on the highway.
Incident date: Oct 10, 2024
Engine has failed and is unrepairable. The car has been maintained well according to the manufacturer’s guidelines with regular oil changes. It has only 94,875 miles. The mechanic said that there is probably a piston gasket failure leading to oil leak into the engine. This appears to have been reported with this year/model vehicles. **Report to NHTSA – 12-Volt Battery Issue in Toyota BZ4X** **Vehicle Information:** - **Make & Model:** Toyota BZ4X - **Issue:** 12-Volt Battery Drain **Description of the Problem:** I have been experiencing repeated issues with the 12-volt battery in my Toyota BZ4X. If the vehicle is not driven for one or two days, the battery depletes, requiring a jump-start to operate. I have t
... aken the car to the dealership for inspection on two separate occasions—once in November and again recently. On both visits, the dealer was unable to identify any issues. However, despite these inspections, the problem persists. Most recently, after taking the vehicle to the dealership, the car once again failed to start the following day and required another jump-start. **Safety Concern:** This recurring battery failure presents a significant safety risk, particularly if the vehicle becomes inoperable in a remote location or in an emergency situation. The inability to start the car reliably raises concerns about potential hazards, especially in extreme weather conditions or areas with limited access to assistance. **Request for Investigation:** I urge the NHTSA to investigate this issue further, as it affects the reliability and safety of the vehicle. A resolution is necessary to ensure that the 12-volt battery functions as intended without leaving drivers stranded.Read more

The only symptom at first was rough shifting. Transmission no longer knows what gear to be in. For example it will move to tenth at 25mph or drop to first gear at 50mph. Adaptive learning computer has been reset multiple times. A warning light started coming on intermittently ("wrench" symbol) after changing fluid. This happened on 2/18/2023. After I charged my car at a local supercharging station, I drove on the local roads for about 2 miles when the car started shutting down. I could barely steer, and the computer monitor stated that the brakes were turning off, and other parts, like cameras, were losing functionality. I pulled over into the local park. I was frightened that my car was no longer functi
... onal and that I could be in grave danger driving again. I tried to call Tesla but no one answered. I had to go through the app. My dad came and we found a Youtube video that said to walk out of the car to let it restart. Looks like it happened to another owner. We did that and the car started functioning again. Not sure why it did this after a charge. But it happened again 3 mos later when I was on the freeway. Luckily, I knew what to do from prior experience. But I am mindful of this whenever I supercharge my car. I wait after charging to drive. This shouldn't be the case and Tesla should tell people of this issue. I tried to tell Tesla service when I went in for another repair but they ignored my comments.Read more

NHTSA recall campaign numbers are below. ... positions. Every week of OTA updates has had vehicles whose owners have reported them as undrivable afterward (~2%-5% usually). Windows are 48 hrs long, car is unusable for the entire window. Partial updates are expected, and another 48 hrs may be required to complete. Complete updates (correct sounds and all), per owner surveys, have been under 30%. One set of numbers shared with FOA, and then shared with owners, was that only ~10% of the fleet (~300) was updated after 5 weeks of attempts and over 800 VINs submitted for attempt. There are only ~20 US service people available for the entire country, and only ~5 travel nationwide to fix our vehicles when a car is undrivable. [XXX] Recall rates shared in the email announcement, repeated for your reference here. Up to $175 for a water pump replacement and all door handle checks $85 for the replacement of a faulty non-NFC door handle (so any door handle that is not the driver’s door handle), $100 for the replacement of the NFC’ed, driver’s door handle. $225 for a manual 2.2 update regardless of the starting software version, or $375 for all three (manual 2.2 update, water pump replacement, and all door handles checked). There was also oil left on the roadway that could be a hazard to other drivers. We had the vehicle towed to our mechanic. He confirmed that the engine had blown and would need replaced. He informed us that this vehicle has a known oil overconsumption issue. GM has settled on a class action lawsuit involving this make and model for the 2010 to 2013 model years. There is also a class action lawsuit currently in process out of Missouri against GM for the model years 2014 to 2017. I contacted GM and was told there was no active recall on my vehicle. I inquired about help getting the issue resolved and was told they were unable to help due to the vehicle not being diagnosed at a GM dealership. O have contacted GM dealerships and am having a hard time finding one that is willing to work with me on getting this issue resolved. This vehicle never warned of low oils or oil pressure. The engine did not make any kind of noise to indicate an issue. There was nothing indication anything was wrong with the vehicle until the vehicle went into low power mode and the engine was blown. A police report was not filed and insurance has not been contacted due to no accident being caused by the incident. Pedestrian Warning Sounds Gone I am writing to express my deep concerns regarding the ongoing issues that are being seen with the NHTSA recall campaign numbers 24V865000 and 24V623000. Following an attempted OTA software update for the safety recall, I have noticed that the pedestrian alert noises are missing, which poses a significant safety risk not only for those around the car but also for the
... occupants inside the vehicle. This situation is unacceptable and raises serious concerns about the vehicle’s safety features. Moreover, it has come to my attention that a concerning number of owners have reported their vehicles as undrivable following OTA updates, with 2%-5% of vehicles experiencing such issues weekly. The current 48-hour update window renders my car unusable, and now I require another session, an additional 48 hours to complete the update only adds to my frustration. Based on owner surveys, it appears that complete updates, including the correct sounds, have been successfully implemented in less than 30% of vehicles attempted. Given these concerns, I urge you to look into the situation with my vehicle and the entire process. I should not have to pay out of pocket for the safety recall and restored function of pedestrian alert noises, nor the cost of a rental car. Thank you for your attention to this critical matter.Read more
