12/2020 First the TPS sensor failed at 1,000 miles and almost caused an accident. Would not go over 25mph, was shifting hard, and the service engine light was on. Verified and replaced at dealership. Second, the rear view camera stopped working while backing up and almost caused an accident - no back windows - major safety issue. Replaced by me as they have no replacement for it even though it

Verified and replaced at dealership. No warnings, no lights Fourth, the transmission bolt was found loose, hard shifts, and has an internal breakdown at 71,000 miles. Verified first by independent shop - they tightened the bolt for me and is now sitting at the dealership waiting on another transmission that has been verified and approved under warranty. The contact owns a 2016 Kia Soul. The contact stated that an abnormal, knocking sound would emit upon depression of the accelerator pedal as a check engine light appeared on the instrument panel. The vehicle was initially taken to an independent mechanic where the diagnosed the vehicle with a defective rod knock sensor; he was then advised to no longer operate the vehicle. The vehicle was later
... towed to a dealer where they discovered that engine rod#4 had become defective which led to engine failure. Upon investigation, the contact discovered NHTSA Campaign Number: 19V120000 (Engine, Engine and Engine Cooling) which he linked to his failure. The manufacturer was then notified of the failure and the contact was informed that the vehicle was already serviced for the recall prior to him owning the vehicle. The vehicle was not repaired and remained in his possession. The failure mileage was approximately 90,000.Read more

I own a 2021 Ford Explorer ST (3.0L EcoBoost) with 47,680 miles. The vehicle is exhibiting a severe metallic knocking noise at idle, a flashing Check Engine Light, and a dangerous loss of motive power while driving. A certified Ford dealership technician (Kendall Ford) diagnosed the vehicle with catastrophic engine failure, requiring a full engine replacement estimated at $13,000.My vehicle is exh
... ibiting the exact mechanical failure described in Recall 24S55 (NHTSA Campaign 24V635000) regarding brittle engine intake valves. Although my vehicle shares the same build year, engine type, and failure symptoms as the recalled units, Ford states my specific VIN is not currently included in the 'repair' population. The sudden loss of engine power while operating at highway speeds creates an immediate crash hazard. I request that the NHTSA investigate whether the scope of Recall 24S55 should be expanded to include this production range.Furthermore, Ford offered a 'goodwill' assistance of approximately $3,762, which I believe is an admission of a known manufacturing defect. At the time of this failure, the vehicle was only 17 days past the 5-year warranty period and well under the 60,000-mile limit. Despite this premature failure and the safety risk, Ford refuses to cover the full replacement, leaving me with a $9,000+ bill for a defect already acknowledged in 90,000 other units.Read more

Toyota dealership records document abnormal tire wear including feathering and cupping. Recent service documentation states the alignment could only be adjusted “to best ability.” A prior independent inspection identified possible lower control arm bushing concerns. Despite this, the vehicle was reportedly operating “within factory specifications.” The condition has persisted through multiple sets of tires and repeated service visits. The ongoing steering pull, vibration, and instability create a safety concern during highway driving because the vehicle does not consistently track straight and requires repeated steering correction. The upper grille and hood trim assembly on my 2018 Toyota Tundra 1794 Edition failed and partially detached from the vehicle due to apparent mounting clip and retainer failure. This failure created a significant safety risk because the panel could have fully de

The detached component also created concerns regarding possible interference with the front grille assembly and Toyota Safety Sense components located behind the grille area. Prior to the failure, there were no warning lamps, dashboard messages, or other electronic warnings indicating a problem. There were also no visible signs of impact damage or collision that would explain the failure. The condition appears to have occurred during normal vehicle operation. The vehicle currently has approximately 76,900 miles. After researching the issue, I identified multiple reports from other Toyota Tundra owners describing similar failures involving the upper grille garnish, mounting tabs, and retaining clips on this generation of Tundra vehicles. At this time, the issue has not yet been formally inspected by Toyota, law enforcement, insurance representatives, or an independent repair facility. Dealer confirmation is currently UNKNOWN. However, based on visual inspection, the mounting clips and retainers appear to have failed under normal operating conditions.Read more

SUMMARY OF SAFETY HAZARD: I am reporting a catastrophic and unpredictable loss of motive power in a 2023 Volkswagen Taos (4Motion with DQ381 DSG Transmission). The vehicle presents a critical electronic sensor failure within the transmission mechatronics unit that causes the vehicle to enter an immediate "Limp Mode" without warning. This has resulted in several near-miss incidents where the veh
... icle became immobilized in active traffic and on high-speed roadways. TECHNICAL DATA OF FAILURE: Factory-level ODIS diagnostic logs (recorded Feb 3 and March 17, 2026) reveal that the transmission’s "Actual position clutch 1" reports a constant value of -0.69 bar. In an automotive hydraulic system engineered for positive pressure, a reading of -0.69 bar is a physical impossibility (a vacuum state). This indicates a total failure of the pressure sensor/TCU logic, leading to the unintentional disengagement of the primary drive clutch. DECEPTIVE REPORTING RISK: Authorized service centers have been documented falsifying the safety status of this vehicle. Despite possessing internal diagnostic logs showing the -0.69 bar vacuum failure, a dealership issued a final Service Order certifying "No issues found" and stating "Actual values match." This reporting practice is forcing a vehicle with a known, documented powertrain failure back onto public roads, creating a severe life-safety risk for occupants and other motorists. The vehicle is currently grounded to prevent a collision.Read more

I am writing to bring to your attention a safety issue regarding Audi windshields that warrants further investigation. I own a 2022 Audi SQ5, purchased in December 2021 from Audi Wilmington. I currently reside in [XXX] where my vehicle has been exclusively serviced by Audi Sarasota since April 2023. Over the past two years, I have experienced four (4) windshield cracks resulting from minor road
... debris (1/26/24, 5/22/24, 2/27/25, and 5/1/26). In each instance, a small stone—rather than significant impact—caused an immediate, large starburst crack that was not repairable, requiring full replacement. I have attached my repair records including the latest with pictures. In my XXX years of driving all makes and models of vehicles prior to this Audi, I have only had one windshield incident and it was small and able to be repaired by Safelite. In researching this issue, I found that numerous Audi Q5/SQ5 owners report similar experiences with windshields cracking from minimal debris, and in some cases without a clear point of impact. This suggests the possibility of a broader pattern that may merit closer review. (Google Search: “Audi Q5 issues with cracked windshield from minor road debris”) I have reached out to Audi to bring this to their attention. Their reply was the following: "Our windshields are designed and tested to meet all applicable safety requirements, including Federal Motor Vehicle Safety Standard (FMVSS) 205. This standard ensures that automotive glass provides appropriate strength, impact resistance, and safety performance under normal driving conditions." I respectfully request that the NHTSA review this matter to determine whether a design review or other corrective action, including a potential recall, is warranted. Beyond the financial burden to customers, this represents a broader safety concern. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

After researching it I found that many other people have had the very same issue with no help from Ford. This should be a recall as this is an internal issue with the short block and something that should have been disclosed before purchasing this model/engine. The only repair that will fix the core of the issue is replacing the entire short block which costs thousands of dollars, way more than the cost of the entire vehicle at this point. Without fixing the issue, the engine will fail. Again, this should be something Ford should pay to repair, not any customer who came into this blind.Read more

I am filing this complaint regarding multiple serious safety defects in my 2021 Subaru Outback purchased in July 2021, currently at 48000 miles THERMO CONTROL VALVE FAILURE (26,307 miles) While traveling in the center lane on [XXX] southbound in [XXX] the evening of 1/6/2024 my vehicle suddenly experienced a complete loss of control. It seemed that the car was accelerating and braking at the s
... ame time without driver input making it impossible to operate safely. It started shaking and several lights were illuminated on the dashboard, power steering was not working. The car coasted to the right shoulder and starlink was used to call for help. The car was towed to the dealership for repairs. The diagnosis was a failed thermo control valve and related sensor circuit. There were no dashboard lights before the incident that fore shadowed this event. This failure rendered the vehicle completely inoperable at highway speed and posed and extreme risk to the safety of the driver, passenger and other motorists. I am aware that Subaru has extended the warranty on the thermo controls valve to 15 years/150000 miles for 2020-2022 Outback’s which suggests that Subaru had knowledge of the defect. Since the incident several other premature “wear” defects have presented under 48000 miles on a 2021 vehicle: - 41,402 miles: driver side rear strut replaced - 41,402 miles: rear brake pads and rotors - 41,402 miles: driver side front window motor, regulator and run Chanel - 41808 miles: rear knuckle assembly, rear wheel bearing and backing plate replaced - 45,768 miles: front brake pads and and rotors replaced - 48,000 miles: complete exhaust system - 48,000 miles: passenger side front window motor, regulator and run chanel This vehicle has proven to be uncharacteristically unreliable for a Subaru Outback. The intended use for this vehicle is for my children to drive to college nearly 600 miles from home in the fall. This car is not appropriate for this purpose. I am reporting a serious safety defect involving repeated engine stalling and failure to restart associated with the auto idle stop/start system on a 2025 Acura MDX. The vehicle was leased in December 2024. Beginning in late summer and early fall of 2025, the vehicle intermittently shut off at stops and failed to restart when releasing the brake. This resulted in the vehicle becoming unresponsive
... in active traffic conditions, creating a significant safety hazard. The vehicle was presented to an authorized dealer for this issue on multiple occasions: August 15, 2025 September 9, 2025 October 31, 2025 After repeated unsuccessful attempts to diagnose and repair the condition, the dealer ultimately replaced the starter on October 31, 2025. This repair did not permanently resolve the issue. In April 2026, the vehicle again began exhibiting hesitation and delayed engine restart following auto idle stop events. On April 30, 2026, the vehicle stalled again, demonstrating that the defect persists despite prior repair efforts. This condition is intermittent and unpredictable, which increases the risk, as the vehicle may fail to restart when entering or crossing intersections or when moving with traffic. The inability of the vehicle to reliably restart creates a clear safety concern. Given the repeated repair attempts and recurrence of the defect, this appears to be a systemic issue rather than an isolated component failure.Read more

Unresolved Safety Defects & Request for Buyback/Replacement - 2024 Dodge Hornet GT Dear NHTSA I am writing to formally demand a resolution regarding a persistent and life-threatening safety defect in my vehicle, purchased from Gary Barbera Autoland. Since the time of purchase, this vehicle has experienced repeated electrical malfunctions causing a total loss of power, including the failu
... re of steering and braking systems while in motion. To date, the vehicle has been returned for service seven times over the past 2.5 years for this identical issue. The dealership has failed to rectify the defect and is now citing an expired mileage warranty. However, the records clearly indicate that this safety defect was reported and remained unresolved while the vehicle was well within the warranty period. Under the Pennsylvania Lemon Law and the federal Magnuson-Moss Warranty Act, a manufacturer is required to repair a vehicle within a reasonable number of attempts (typically three). This vehicle is currently unsafe for myself, my disabled mother, and the public. I am requesting that Stellantis the Manufacturer initiate a buyback or replacement of this vehicle immediately. Please provide a written response within [5-7] business days outlining your intent to resolve this matter. Thank you Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Meanwhile I look up code and see hundreds of post for same issue, bad actuator replace. 28 years as a diesel mechanic for the military, so I found the part online and ordered. Replaced the actuator, drove about 50 miles, light comes back on. Finally get my appointment with dealer and they say my turbo is bad, they want $8k to fix. I ordered the turbo and replaced it myself along with another actuator, test drove. Light still comes on but have not lost power yet?? Dealer is not helping. This is a major issue that is all over the internet! The truck just goes to limp and your in the middle of traffic. If this has not caused an accident it’s only a matter of time before it does. The dealer ship just wants to replace big parts for lots of money. I’m already $2500 into this and still having an issueRead more

On April 27, 2026, my 2025 Hyundai Santa Fe experienced an intermittent electrical/system malfunction affecting multiple vehicle systems at the same time. Multiple orange warning lights/messages appeared on the dash, including driver-assistance warnings and the master warning triangle. At the same time, the front turn signal stopped working, causing rapid flashing; the power locks stopped working;
... the windows would not roll up or down; and the rear liftgate would not open using the key fob or exterior liftgate button. The vehicle/component is available for inspection upon request. This created a safety and access issue. The malfunction affected exterior signaling, locks, windows, and rear cargo access. My dog was in a crate in the rear cargo area, and I could not open the liftgate normally to access him. The failure of locks/windows/liftgate could create a serious emergency risk if a person, pet, or cargo needed to be accessed quickly, and the turn signal failure affected my ability to safely signal to other drivers. The issue lasted approximately 30–40 minutes and did not immediately clear after turning the vehicle off and back on. It later returned to normal on its own. This has happened more than once. The vehicle was taken to a Hyundai dealership for inspection. The dealer has been working with Hyundai Techline, but as of now they have not been able to reproduce the issue or find active diagnostic codes. I provided video evidence of the dash warnings from the incident. The dealer advised that scans currently show systems operating as designed because the issue is intermittent and not occurring during inspection. There were no prior symptoms before the failure. The warning lamps and messages appeared at the time the malfunction occurred. Seek reimbursement for rental car fees and diagnostic fee.Read more
