On November 26, 2025, while traveling in stop-and-go freeway traffic, I experienced a severe braking failure that led to a rear-end collision. Despite applying maximum force to the brake pedal, the vehicle did not decelerate as expected. Objective technical evidence from the Bosch CDR report (Case #1105768) confirms a critical system failure: Extreme Driver Input: At -0.5 seconds before impact, D
... river Applied Brake Pedal Pressure reached 10,935 kPa, representing a near-maximum emergency braking effort. Inadequate Response: Despite this peak pressure, the Vehicle Indicated Speed decreased only from 24 MPH to 22 MPH in the final 0.5-second interval. Data Discrepancy: The vehicle decelerated by 12 MPH (from 36 to 24 MPH) between -1.0s and -0.5s under lower pressure (7,695 kPa). However, when brake pressure was increased to its peak at -0.5s, deceleration effectively stalled. Electronic Anomaly: The Traction Control System (TCS) was reported as "Active" throughout the braking event, indicating potential improper electronic intervention. This vehicle has a history of significant electrical failures, including a total power loss on 10/15/2024 that required a high-voltage battery replacement. The failure of the braking system to respond to maximum driver input poses a catastrophic safety risk. I am requesting an investigation into the 2024 Cadillac LYRIQ's brake-by-wire and electronic stability control integration.Read more

Incident date: Feb 2, 2026
The vehicle experiences a severe loss of power, placing it into a reduced-power or limp mode that prevents the vehicle from exceeding approximately 10 mph. The issue has occurred repeatedly despite multiple repair attempts. The battery was replaced initially, after which the vehicle broke down while still at the dealership. The service department removed the new battery & reinstalled the original
... battery, at which point the vehicle temporarily operated normally. On a subsequent visit for the same reduced-power condition, the dealership replaced the throttle body chamber and multiple gaskets. This repair resolved the issue only for a short period of time. During a third service visit, I paid approximately $3,000 for additional repairs; however, the same issue occurred again, & I was unable to safely leave the dealership parking lot. The vehicle is currently available for inspection, testing, & servicing upon request. All prior inspections & repair attempts are documented in dealership service records. My safety & the safety of others were placed at significant risk because the vehicle unexpectedly loses power while being driven. The vehicle abruptly slows & cannot exceed 10 mph. This occurred while driving on the highway during heavy 5:00 p.m. traffic, as well as during normal roadway driving. A sudden and severe reduction in speed in traffic creates a high risk of rear-end collisions or other serious accidents, particularly if surrounding drivers are unable to react in time. Yes. The dealership where the vehicle was purchased is the same dealership that has serviced the vehicle on all three occasions. Each time, the dealership has inspected the vehicle and represented that the issue was repaired. However, the problem continues to occur immediately after service, including instances where the vehicle could not be driven out of the dealership parking lot. Any qualified service center test-driving the vehicle would be able to observe & reproduce. Yes check engine lightRead more

The A/C evaporator core in my 2020 Dodge Charger Hellcat has failed twice within a short period of ownership. The first failure occurred at approximately 20K miles in 2022, when the air conditioning suddenly stopped blowing cold air. The dealership diagnosed a leaking evaporator core and replaced it under warranty. On [XXX] at approximately 50k, the exact same failure occurred again — loss of r
... efrigerant and no cooling. The vehicle again required replacement of the evaporator core, a repair that involves removing the entire dashboard and is extremely labor-intensive and expensive. This appears to be a recurring defect rather than normal wear. I have learned that many other Dodge Charger and Challenger owners report identical evaporator failures, often multiple times, suggesting a possible design or manufacturing flaw with the HVAC evaporator used in these vehicles. Loss of A/C may seem like a comfort issue, but in extreme heat it can create a driver-safety risk due to overheating, window defogging problems, and driver fatigue. I live in Arizona where daytime temperatures regularly climb above 80 degrees, even in the winter. A major component like an evaporator should not repeatedly fail in a late-model vehicle. I am requesting that NHTSA investigate whether 2019–2022 Dodge Charger/Challenger vehicles are experiencing an abnormal rate of evaporator core failures that may warrant a recall, extended warranty, or technical service bulletin. Repairs Performed: • Evaporator core replaced in 2022 at Bill Luke Chrysler / Dodge in Phoenix, AZ • Evaporator core replaced again on February 4th, 2026 at AAA Auto Repair in Phoenix, AZ • Refrigerant leak confirmed both times • Dash removal required for repair Supporting Documents Available: Repair orders, dealership invoices, diagnostic reports INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Shortly after purchase, the vehicle began experiencing ongoing mechanical and electrical issues that required multiple repair attempts. The vehicle was taken to the dealership for service on the following dates: •November 29, 2024 •December 2, 2024 •December 16, 2024 •December 23, 2024 •January 6, 2025 In total, the vehicle was out of service for 21 days in December and 28 days in Januar
... y. Despite repeated repair attempts, the issues were not resolved. Due to the excessive time out of service and repeated unsuccessful repairs, I submitted a request for Volkswagen to repurchase the vehicle. This request was denied. The vehicle was returned to me on February 1, 2025. On February 9, 2025, while parked in my driveway and not in operation, the vehicle caught fire. I was later informed that the fire originated in the dashboard and was electrical in nature. This incident presented a significant safety risk. Had the fire occurred while the vehicle was in operation or spread to my residence, it could have resulted in serious injury, loss of life, or significant property damage. I am reporting this incident due to concerns that the electrical failure and resulting fire may indicate a broader safety defect affecting this vehicle model. I respectfully request that NHTSA review this matter to determine whether further investigation or corrective action is warranted in the interest of public safety.Read more

My 2014 f150 xlt Ecoboost 4x,4 was experiencing symptoms directly related to Recall 24S37 / 24V444 (erratic/harsh shifting while on the highway). P0606, P0607 (PCM Processor Faults) P0706, P0707 (Transmission Range Sensor issues) P1702-00 (TRS Circuit Intermittent) P1921-00 (Transmission Range Signal issues) This was following the software update in the last recall. Plus the truck would not start
... despite a brand new battery. I purchased a new lead frame and was told by Ford to take it to Conklin Automotive in Newton Ks so I had it towed Jan 7th. My truck is still there. They replaced the lead frame but released it to me without a diagnosis, without replacing transmission fluid due to a stripped dipstick they were unable to remove, without trying the many options for removal or refilling it and without checking to see if the lead frame fixed the main issue. I had said from the beginning they needed to check the neutral safety switch and fuses. Finally by end of Jan he checked and found a corroded fuse and the truck started but because of refusal to refill the fluid they didn't check the lead frame. So I have had to rent cars for a month resulting in financial distress and return the truck to a different shop to be fixed. Ford refuses to honor the recall even tho it was extended to my year and model of truck, and refuses to help because Conklin, where they told me to go, is not an authorized dealership. I've attached all the correspondence with them and Ford.Read more

SEE FULL COMPLAINT NOTES IN ATTACHED PDF; SUMMARY HERE (you do not provide sufficient space to provide all info you request) I own a 2019 Ram 1500 4WD Limited (not eTorque). On multiple occasions, the vehicle has unexpectedly shifted out of Drive into Neutral while driving. When this occurs, a warning chime dings and a message on display directs me to “Shift to Park”. The vehicle immediately l
... oses forward power (because it has shifted itself to Neutral), until I come to a complete stop, shift to Park, and then shift back to Drive to move forward. Usually the engine stays on, but on at least one occasion the engine turned off and I had to restart. This has happened at least five times total, including three times within the past month. The events have occurred while decelerating to a stop, braking but not stopping, and accelerating from a slow down. Each time, I experienced a sudden and unexpected loss of propulsion, requiring me to stop the vehicle in traffic, shift fully into Park, and then reselect Drive before the vehicle would move again. These incidents have occurred in active traffic situations and have placed me and others at risk of a collision due to the sudden loss of power (SEE DETAILS IN PDF). I previously brought the vehicle to a dealer for this issue after it had just occurred (~1 year ago, before the recent increase in frequency); however, the dealer stated that the vehicle did not throw a code, or no code was stored. They could not replicate the problem and no repair was made. There are at least 55 NHTSA complaints filed that appear the same as my issue; none appear to have been resolved by dealers or RAM. This appears to be a systemic safety defect involving the powertrain and electronic shifting system. The unexpected loss of propulsion in traffic presents a serious risk of injury or death. I am submitting this complaint due to the ongoing and unresolved safety concern. I am scared to drive my family in my vehicle until it is known to be fixed.Read more

I am writing to formally request that the National Highway Traffic Safety Administration review and consider an investigation into a potential safety-related defect affecting 2017 Dodge Grand Caravan vehicles. My vehicle recently experienced a failure of the engine oil filter housing, which cracked and leaked oil and required an expensive repair. According to the repair facility, Pro Auto Service
... , this failure is not isolated. I was advised that the shop has repaired a significant number of identical failures over the past year, to the extent that replacement parts have at times been difficult to obtain. The repair facility also indicated that many similar failures have occurred in recent weeks, coinciding with extreme cold weather conditions. The oil filter housing is a critical engine component responsible for maintaining proper oil circulation and pressure. Failure of this component can lead to oil loss, reduced lubrication, engine damage, or potential engine shutdown if not immediately addressed. Because this defect directly impacts engine operation, it presents a powertrain-related safety concern. Based on the frequency of reported failures, the consistency across the same model year, and the apparent correlation with cold weather conditions, this issue appears to be the result of a design or material defect rather than normal wear and tear. I respectfully request that NHTSA review this matter for possible investigation and recall consideration to protect vehicle owners from unexpected engine failure and costly repairs. Thank you for your attention to this concern and for your continued commitment to vehicle safety.Read more

When driving recently, the car suddenly began showing a warning message “check electrical system” and would not go beyond 25mph. I was on the way home but still far, and had to turn on my hazard lights to alert other drivers. Eventually the car escalated the warning to “pull the vehicle over” and implied that the main battery was having an issue. I had to have th car towed to a dealer
... (closest is 10 miles) who eventually diagnosed the issue as a blown ICCU. My understanding is this is a critical component to the charging infrastructure and must be fully operational for the car to safely drive. Hyundai repaired the part and I had the car back in a few days, but I could get no assurances on what happened or whether or not it would occur again. The car has 14,000 Miles in the Washington DC area. It seems incredibly unsafe that this component could blow at any time. I also understand there are many other owners with this exact issue.Read more

The manufacturer however, is saying it’s out of warranty and that’s absurd because now we all have to pay $2000+ to have our entire roof repai
... nted. I have read that some people have had it repainted and it’s peeling again. That is a major problem. Acura needs to remedy this quickly before they lose customers. And I really love my Acura and I would love to buy another one in the future, but I’m counting on this one lasting a very long time! Won’t last that long, however, if the paint peels off continuously. I’ve included below in the attachments the preliminary repair cost of this that I had quoted when I first discovered the issue in October. 2. My battery kept draining two years ago, even though I got a new battery – – twice within the same month – – and the dealership in Birmingham, King Acura, was very thorough and very kind and generous, and the only thing that they could figure out after I had read numerous reviews from consumers online and suggested it to them was that the Bluetooth was draining my battery. They told me I could replace it for $3500 or have them disconnect it permanently. I chose to have them disconnected permanently because I didn’t want to pay $3500 for a Bluetooth system that was working but caused a random side effect that affected my whole car. Can you please help me with these issues as I am a big fan of Acura and I would like to see Honda make good on this. Thank you.Read more

During the incident in question, the vehicle’s lane‑assist system malfunctioned in a manner I have never previously experienced. As I approached an area with multiple lane markings, the system appeared to become confused—possibly due to low‑light conditions—and abruptly pulled the steering wheel with significant force. This deviation was severe enough that I temporarily lost control, cau
... sing the vehicle to drift toward the left-side ditch. When the steering suddenly released and became limp, I attempted to correct the trajectory, but the response window was too narrow to avoid impact. The vehicle struck a wooden signpost, resulting in damage to the right headlight assembly and the complete detachment of the right fog light. No lane‑departure warnings were issued at any point. Under normal circumstances, the vehicle provides audible alerts regardless of whether lane assist is active, and I am familiar with the areas where such alerts typically occur. Additionally, the forward‑collision warning system failed to activate, despite its usual tendency to trigger even in non‑hazard situations. This inconsistency has been present since I acquired the vehicle. Prior to this event, I had also been experiencing intermittent electrical issues. I attempted to seek service at the nearest Kia dealership; however, reaching it requires traveling through a national forest during winter conditions, which is unsafe unless absolutely necessary. I attempted to file an incident report with local law enforcement, but was informed that a report could only be taken if another motor vehicle was involved. I then attempted to file a report with the DMV, but the online system requires an Oregon‑issued driver’s license. I currently hold a valid Oklahoma license and have been unable to update it due to financial constraints and an anticipated move later this year. I have experienced neck pain between my shoulders since the incident.Read more

FORMAL COMPLAINT Safety Recall 94B (NHTSA Campaign No. 24V-838) I hereby submit this complaint regarding the failure to properly complete Safety Recall 94B (NHTSA Campaign No. 24V-838) involving the HCU/ABS module on my 2018 Dodge Durango. On December 23, 2025, I contacted an authorized Dodge dealer located at 5404 S. Illinois Route 31, Crystal Lake, IL 60012, to remedy the HCU/ABS defect under
... the recall. I was informed that the recall repair would be performed free of charge and an appointment was scheduled for December 29, 2025. At the scheduled visit, the vehicle was returned to me in less than 30 minutes without any recall repair being performed. Instead, I was told that paid diagnostics were required and that the estimated repair cost could range from $3,000 to $5,000. I was also informed that the recall repair could not be completed because the HCU/ABS module was not in stock. The dealer later stated that the module would be ordered, but the delivery timeline was repeatedly postponed. On January 27, 2026, the vehicle was returned to me with the same HCU/ABS-related malfunctions. The dealer stated that Recall 24V-838 / FCA 94B had been closed and refused to replace the HCU/ABS module under the recall. Although FCA systems reflect the recall as “completed,” the safety defect has not been remedied. The vehicle continues to display ABS/ESC warnings, and diagnostic trouble codes related to the braking and electronic stability control systems remain present, confirming that the defect described in the recall persists. Pursuant to 49 U.S.C. § 30118(b) and 49 CFR § 573.6(c)(10), when a safety defect persists or reoccurs after recall repairs, the manufacturer is required to implement additional corrective measures, including repeat repair or replacement of the affected component or system. Closing a recall while the defect remains unresolved does not satisfy federal requirements. I respectfully request regulatory review and appropriate action to ensure that thiRead more
