On February 11, 2026, the 2025 Tesla Model Y "Vision AutoPark" system initiated an uncommanded high-speed reverse acceleration into a stationary yellow concrete bollard in a parking lot. The vehicle's vision-only parking assist hardware failed to detect a clearly visible, fixed obstruction and displayed a clear path on the system interface immediately before impact. The vehicle closed the distance to the bollard in under one second, exceeding human reaction time and making manual braking intervention impossible before impact. This failure mode — autonomous acceleration toward a stationary object without detection or warning — creates a severe crush hazard for pedestrians and property. The vehicle and its onboard data logs are available for inspection upon request. High-definition video evidence of the failure has been preserved showing the system ignoring the visible obstacle. The problem has not been confirmed by a dealer. The manufacturer's service center refused to inspect the vehicle or review data logs on the date of the incident despite an immediate in-person request. The vehicle has not yet been inspected by insurance, police, or the manufacturer. I have also been waiting several months for reimbursement of the almost $500 paid for the temporary repair, with no resolution. I am concerned that I was charged for a temporary repair related to a serious safety failure and that continued delays in both reimbursement and permanent repair place me at ongoing risk. The Air conditioning mixing flap failed. The is a technical bulletin out for this SIM 64 02 21. The mixing flap separated from the heater housing which is located under the dash board. It went to the local Chrysler Dealership; they replaced the Master Cylinder. I picked it up 12/10 drove it 5 miles home and parked it. I got in the vehicle the next morning to go to work and on my way to work all the same dash lights came on and I had a complete loss of Brake function. It was towed back to the dealership again; this time they replaced the ABS module. I picked it up on 12/30 drove 5 miles home, parked it, tried to leave 3 hours later and couldn't push the brake pedal far enough to get the van started. When I did finally get it started all the same dash lights were on and the brakes were not functional. It was towed back to the dealership again and this time they replaced the wiring Harness. I picked it up 1/16. Drove it minimally over the weekend, on 1/19 the front-end collision warning lights came on and the check engine light came on while driving 70 mph. The brakes didn't lose function this time, however. The following day I was sitting in line at an elementary school waiting to pick up my daughter when all the same dash lights came on again and I had a complete loss of brakes, it was towed back to the dealership, where it still is. There was contact made with Chrysler engineering during the 2nd repair. The dealership has been able to recreate the problem more than once since they have had it. This put myself and my children a significant risk of injury, and other cars if there was an accident.Read more

I then contacted my landlord and super since the vehicle was parked in a location where it was visible to my building cameras but no one was near my vehicle during the time the rear windshield had broken. I proceeded to do my research online and discovered that this is a constant occurrence with Acura RDX models from years 2012-Current. The issue is with the defroster when the vehicle engine starts. I took my vehicle to Paragon Acura where they proceeded to take pictures of the vehicle and the damage and replaced the vehicle with an aftermarket windshield, stating the issue is with the windshield which is incorrect, the issue is with the defroster. They assured me this will not happen again due to them using aftermarket glass and this is a one time goodwill repair. Glass is still in my trunk door as I was unable to leave the vehicle for cleaning. I explained to Acura this can not happen due to having 3 young children who place their school bags in the trunk of the car, it could have shattered while they were putting their bags in the trunk as they do daily. ... owever, they still cannot give me an eta for parts and repair. They also do not have rentals or loaners and there are no car rentals in Raton, NM. Neither the dealership or Ford Customer Service have authorized a rental for me yet. I am a full time RV'er with my travel trailer parked in Las Cruces, NM for the winter. I returned here 1/14 by renting a Uhaul van which was ridiculously expensive and have been here in Las Cruces since then, with no means of transportation. The dealership initially told me the last one of these recalls took 5 weeks for parts. When the parts fail the rear axle disengages from the drive line. You cannot move, and in park the truck rolls. This recall has been going for a couple of years and there are no replacement parts available. I will need transportation to move my trailer to a new job in Colorado before the end of April. Key Details Regarding the Problem: Cause: Moisture from the PCV system and ambient humidity builds up in the CAC, which can freeze during cold weather. Affected Models: 2018-2022 Chevy Equinox and other GM vehicles with small-displacement turbocharged engines. Symptoms: Reduced power, stalling, rough idle, and engine light in cold, humid, or snowy conditions. This submission supplements my prior NHTSA ODI complaint regarding the same vehicle and loss-of-motive-power condition. On Nov 8, 2025, the vehicle experienced a loss of propulsion at highway speed, creating an immediate safety risk. ... stored diagnostic trouble codes P0A3F-00 and P0A43-00 were retrieved, relating to drive motor position sensor / resolver signal loss or degradation. These fault codes are associated with loss of propulsion and indicate a propulsion control signal-path fault, rather than a condition resolved by software calibration alone. The vehicle remains available for inspection and is out of service at a dealership due to unresolved safety concerns. Since my prior ODI complaint, I provided formal written notice to the manufacturer describing the incident, the retrieved fault codes, and the safety implications, and requested a written response. Receipt was confirmed by USPS certified mail on January 6, 2026. No written response was provided, and repeated attempts to contact the manufacturer through customer service channels were not returned. The manufacturer authorized only a recall firmware update and declined to authorize or perform the intermittent wiring and resolver diagnostics identified in its diagnostic procedures for these fault codes. The firmware update does not inspect, repair, or replace the resolver, wiring, or related hardware indicated by the stored codes, and does not determine whether the underlying propulsion fault has been corrected. As a result, the vehicle has been out of service for more than two months, and the risk of another loss-of-propulsion event remains unresolved. This update documents the manufacturer’s non-response after notice and reliance on a software-only remedy that does not address the indicated propulsion system fault.Read more

While driving my 2013 F150 Eco boost is started shifting erratically. While at 60 MPH the truck would down shift on it's own to 1st gear and my truck would skid. Luckily I did not crash but it's dangerous and it's my only transportation. I took it to Ford and they said I needed a $8K transmission replacement. I ended up taking to a transmission shop for a rebuild but shortly after if did it again
... . The transmission shop replaced the valve but said Ford needed to reflash the TCM. When I asked for them to follow recall 19S07 they said I would need to bring it in and pay them another $400 for a diagnostic and they couldn't just reflash it. Now I'm in a position where I have to drive what feels like a timebomb and I have no way out of it.Read more

Each thermostat failure resulted in abnormal engine temperature behavior and warning indicators. Thermostat failure directly compromises the engine cooling system and creates a risk of overheating, sudden engine shutdown, loss of propulsion, and engine damage. These conditions present a clear safety hazard, particularly if failure occurs at highway speeds, in traffic, or during extreme temperatures. A thermostat is a non-consumable, safety-critical component that should not require multiple replacements in a vehicle of this age and mileage. The fact that the failure occurred again after repair suggests that the defect may be systemic and potentially affects other 2020 Dodge Journey vehicles equipped with the same thermostat design or housing.
