We were driving on a 2 lane stretch of highway under construction. There were concrete barriers at the edge of the left and right lanes. There were no shoulder pull off areas or highway exits. We had been driving at highway speeds for 30 minutes. For about the next 10 minutes, there was heavy traffic congestion. Traffic was periodically stopped in both lanes and generally moving at less than 30
... MPH. We were in the left lane when we got an audible chime followed by a dash board message, “Stop Safely - Vehicle will Shut Off Soon”. I was hoping the warning would give me enough time to get off the highway. I used my turn signal to change lanes but the traffic in the right lane would not let me in. I pushed on the gas and there was no engine response and the check engine light was on. I put my hazard lights on and rolled to a stop. The vehicle was blocking the entire left lane and there was no where else to go. We needed a tow. Roadside assistance instructed us to call 911. The vehicle is being repaired under warranty. The engine and transmission are available for inspection upon request to the rental car owner. My family and the other highway drivers safety was put at risk. It is my understanding that over 60,000 HEV Pacifica vehicles were recalled between 2017-2023 for this issue. The corrective action was not to mechanically engineer the problem out of the vehicle but to install a software update to provide the dash board audible and verbal warnings. The 2024 models come with the software mod and so there is no current recall for this years model. I would argue the recall fix is inadequate. There are many roadways where it is not possible with the warning time provided to “Drive to a safe location” and safety continues to be compromised. Verbally filed with NHTSA, ID11598224 due to not having a VIN for the web site entry. My intent is not to duplicate the report, but to provide a more accurate 1st hand account with this submission.Read more

I purchased my 2017 in 2023. Within a few months, problems began. When we brought the van back to the Chrysler dealer to have it checked with their OBD2 code reader, multiple codes pulled up… First thing “needing” to be replaced was the hood sensor because the display said the hood was open (even though I was simply stopped at a red light). That repair was done (I believe) in April of 2024 at the Chrysler dealer (and the auto start/stop light came back on a week later, both batteries testing fine independently of one another). Next thing “needing” to be replaced was the accelerator pedal module because my van would start shaking, not accelerate, and go into (what I now know is) limp mode with all warning lights displaying on almost every right hand turn. This absolutely puts my safety and the safety of my four children at risk when it suddenly decides to not accelerate!! That repair was done in June 2024 at the Chrysler dealer. Next thing to happen is the check engine light illuminating. We checked the coolant and it was almost empty! This should NOT be happening! We filled with coolant, but have to do this every few days now. The check engine light has come back on and is still producing many codes, the worst of which is p0303. This code has pulled up every single time we’ve had the van scanned at the dealer. The dealer has had my vehicle on MULTIPLE occasions for MULTIPLE days each time and NEVER ONCE told me the severity of this problem. Their words: We just go down the list, starting at the top. P0303 was not at the top, and now I have to pay the price for their incompetency! My van would not start and had to be towed to the dealer yesterday, 7.17.2024. Keep in mind FCA had my information (email, phone & address) either from me emailing or speaking with a customer rep. It's been over 3 years since last heard from FCA. On my side I no longer have ability to contact them directly since they became Stellantis. 49 U.S.C. § 30166(f); 49 C.F.R. §§ 573.6(c)(10); 577.5(a), 577.7(c)(1), & 49 U.S.C. § 30165(a)(1); 49 C.F.R. § 578.6(a) NHTSA fined FCA/Stellantis > $100 million for 'not' making an effort to send recall notifications of any type of recalls directly to owners of their vehicles within a reasonable amount of time also to remedy the issue. If they don't comply $7,000 per violation & only remedy is replacing the vehicle in question. It's exact issue! Been over 7 years waiting within a reasonable remedy, it never happened. This is a safety issue. I was assured it'd be fixed, never was. I'm putting my life at risk & others driving.
