A vehicle was taken to a collision repair facility following minor front-end damage. At intake, the repair facility documented visible exterior damage but did not perform or document an underbody inspection, which is necessary to assess high-voltage battery integrity in electric vehicles. After the vehicle entered teardown, the repair facility later claimed that the high-voltage battery was damag
... ed and recommended that the vehicle be declared a total loss. This determination was made without documented evidence of required OEM diagnostic procedures and without initial intake documentation supporting battery damage. The sequence and timing of documentation raises concern that the battery damage determination was made after teardown rather than through proper inspection protocols. A subsequent manufacturer-level evaluation did not support the claim of battery damage. This situation presents a safety concern regarding whether proper high-voltage battery inspection and handling procedures are being followed by collision repair facilities. Misidentifying or improperly handling high-voltage battery conditions in electric vehicles can pose safety risks, including potential thermal events, improper classification of vehicle safety, or unsafe return to service. This complaint is submitted to request review of whether proper safety and OEM procedures were followed in evaluating the high-voltage battery, and whether this reflects a broader safety issue in electric vehicle collision repair practices.Read more

On the morning of April 28, 2026, at approximately 7:30 AM, I discovered that the front panel of the panoramic sunroof on my 2015 Mercedes-Benz GLA45 AMG had spontaneously shattered while the vehicle was parked at my residence in Meridian, Idaho. When I went out to the vehicle in the morning, I found glass fragments distributed across the rear seat area, the rear floor, and my child's car seat. L
... ooking up, I saw that the front panoramic glass panel had shattered. My husband and I conducted a complete walk-around inspection of the vehicle exterior. We found no evidence of any external impact: no damage to the roof surface, body panels, surrounding trim, or any other glass on the vehicle. There was no debris (rocks, branches, hailstones, construction materials, or any other potential impact source) on top of, beneath, in or around the vehicle. The rear panoramic glass panel was intact. Glass fragments were present both inside the cabin and on the exterior surface of the roof and a few fragments were found on the roadway. The car seat where glass fragments fell is regularly occupied by my [XXX] daughter. On this particular morning, she was not in the vehicle only because of a coincidental change in our family's schedule that day, but typically she gets into her carseat independently, buckles herself, and I check her buckles. Thankfully, she was not injured but her car seat now has glass fragments in the seat. I am filing this complaint to document the incident in support of NHTSA's ongoing collection of safety data for consumers. My 2025 Volkswagen Taos is exhibiting active symptoms of a known safety recall involving electrical system malfunctions. The vehicle experienced multiple failures including the instrument cluster going blank, screens shutting off, and a false start condition where the dashboard illuminated with multiple warning lights. The vehicle became inoperable and required towing to a Volkswagen dealership.
... The dealership confirmed that the symptoms are consistent with the current recall but stated there is no available remedy at this time. They insist the vehicle be returned to me after only charging the battery, without any repair addressing the recall condition. I refused. I do not consider the vehicle safe to operate given the ongoing electrical failures and lack of an available fix. This complaint is being filed because a recalled vehicle is actively experiencing the defect, and the manufacturer is unable to provide a repair or safe interim solution.Read more

2022 Ioniq 5 SE. ICCU failed April 2026 with around 29325 miles. Original owner. Purchased June 2022. I had the original Recall 272 software update performed back in February 2025, but the hardware still failed a year later. **Timeline of Failure:** 1. **Phase 1 (The Trigger):** Level 2 charging stopped unexpectedly before reaching the limit. It tripped my house breaker and unfortunately kille
... d my Level 2 EVSE (Grizzl-E) with a stuck relay. Didn't think too much of it because I had to be somewhere. Car started up normally and I was able to drive throughout the day with no problems. 2. **Phase 2 (The Warning):** The next day, I attempted Level 1 charging. It immediately tripped the breaker. I tried another outlet on a different breaker; the breaker didn't trip, but the car failed to initiate a charge after three attempts. 3. **Phase 3 (Dashboard Error):** After third failed attempt with the L1, I took a peek inside and saw the dashboard had a yellow triangle. With the charger still plugged in, I sat in the driver's seat and turned the car on. The **"Check EV System"** error appeared. I turned off the car and went inside to call the dealer. I called the dealer and they said I can have it towed there. They also scheduled an appointment for the next morning. I went back to car to attempt to position the car to make it easier for the tow truck, but the check ev error was not there when I turned on the car. It said ready and I was able to drive around the block a few times so I figured I'll just try to drive to the dealer. I drove the car to the dealer the next morning. It took **one week** to get my car back. **Diagnostics & Parts Replaced:** The dealer confirmed the following codes and performed a full hardware replacement: * **DTC P1E1300:** AC Power Module Failure * **DTC P1A9096:** DC/DC Converter Fault * **ICCU Part:** `36400-1XAA0-AQQH` * **HV Fuse:** `375F2-GI040-QQH` * **Coolant:** Install of Pink Antifreeze (`00232-19098`)Read more

My Yukon Denali XL GM engine that was put in on 07/2022 has already failed. It failed as I was driving down highway at 65 mile per hour. It failed on 04/12/2025. Now the engine that I paid thousands of dollars for has already failed and has no compression in cylinder 4. I have researched this and
... can see GM is currently in litigation over car owners reporting engine failure due to either liftors or cams. My car should be considered in this class action lawsuit against GM. It's wrong for a company to take so much money from owners and use an engine that GM knew had issues and was faulty to place it into my vehicle only to have it fail and now make my vehicle undriveable and I am told it's another $8,000 to repair it. GM should address this issue with my vehicle and replace this engine that I had put in in 07/2022. GM was aware of this engine problem with my model car Yukon. GM knew this particular engine that they used in my vehicle on 07/22 (A GM engine) had the issue that GM is being sued for and sure enough the engine has now failed in my Yukon. Please hold GM accountable for this and include my car in this lawsuit.Read more

I received the recall notification for Campaign 25V-302 regarding the Curtain Airbag (CAB) defect, which per the manufacturer's notice may fail to retain sufficient pressure during deployment, increasing the risk of occupant ejection through a side window during a crash. The notification informed us that no parts were available at that time. Many months later we received another notification that
... parts were now available. Upon contacting the dealer, I was informed that parts were not available. I have been waiting for parts to become available for an extended period. Most recently, the dealer scheduled a repair appointment for Tuesday of this week. Approximately 30 minutes before the scheduled appointment, the dealer called to cancel, stating that the parts were still not available. A new appointment was scheduled for the following week. As of today, the dealer has confirmed parts are still not available. Throughout this period, I have been required to continue operating the vehicle, including with my family as passengers, with a known, unremedied ejection-mitigation defect. The manufacturer has not offered a loaner vehicle or any alternative transportation, and no firm parts availability date has been provided. Concurrent stranding failure: Separately, today (23rd April), the vehicle failed to start in my driveway. Symptoms included: interior electronics (infotainment, lights) powered on, but the start button produced no response, the brake pedal would not depress (electronic brake booster inoperative), and the steering wheel remained locked. No error message was displayed. The vehicle was ultimately determined to have a dead 12V battery, the second battery failure on this vehicle, with the previous battery having been replaced under warranty by the dealer in August 2025 (only 8 months prior to this failure). The dealer has declined to investigate whether a parasitic electrical draw or other underlying defect is causing repeat battery failures.Read more

After deploying the second-row Stow 'n Go captain's chair seat from its stowed position in the floor, the integrated headrest fails to lock in the upright, passenger-safety position. The headrest remains folded forward or flops forward under normal conditions and cannot be secured in the upright position required for safe occupancy of the seat. The root cause, as identified by dealership technicia
... ns and documented by numerous owners across multiple model years (2017–2022), is failure of a small plastic component within the headrest actuator cable assembly. This cable mechanism is designed to automatically fold the headrest during stowing and return it to the locked upright position when the seat is deployed. When the plastic cable end or the locking mechanism fails, the headrest cannot return to or maintain the upright, locked position. The cable itself is not sold separately by Chrysler/Stellantis, with dealers quoting $500–$1,000+ for repair requiring full seat frame or headrest replacement. This constitutes a safety hazard because: (1) a headrest in the forward/folded position provides no rear-impact protection to an occupant, increasing the risk of whiplash or cervical spine injury in a collision; (2) the condition prevents safe installation of child booster seats in the affected seating position; and (3) the defect is a known, widespread, and persistent design flaw affecting multiple model years with no manufacturer recall or remediation issued. This issue has been reported by a large number of owners on owner forums and acknowledged by Chrysler customer care representatives, yet no recall, technical service bulletin fix, or goodwill repair program has been established. I am filing this complaint to support an investigation and potential recall.Read more

I keep getting check engine lights and they have repaired serval of the items and I did get a letter in the mail saying the battery would now have a lifetime warranty if it threw a certain code and I had two codes come up and some people told me to replace my 12v battery and it would probably clear up. so reset the codes and put new battery in and ran fine for a week or so (I don't drive much) the
... n when I got in the other day and had it fully charged and it showed on screen only 69% and I thought that was strange then the check engine light came on again showing code P0A0A high voltage system interlock circuit so I am really worried the battery is having a problem and needs replaced. Will it be covered under the lifetime warranty? I see on safety recalls that these vehicles were recalled??Read more

Vehicle shut off 2 times driving down the road prior to 6000 miles being on what was a vehicle we purchased new from Chrysler. Dealer diagnosed as a faulty transmission that was replaced under warranty.

When driving on highway, electrical system started beeping and showing various signals, could no longer keep speed or accelerate. Had to put on warning lights and coast to side of road with four small kids in car. Was able to pull off to side and slowly get to safe location by resting vehicle in between events. When fully charged, didn’t seem to have issue so was able to drive to dealer. Thought
... was related to engine switching from electric to gas power. Dealer checked and performed two software updates. On way home from dealer, same problem occurred on highway. Super dangerous to loose all acceleration or same speed on highway! I got a video of second event as I was a passenger and my husband was driving. But as can see in video the car has a charge so the incident is not related to the switch from electric to gas power. Now we are very concerned whether Chrysler has the means to fix this at all and or whether the dealership will be actually able to identify the issue given the car was literally being driven home from the dealer. Attaching photos but have a full video. First time happened on April 4, 2026. Second on April 16, 2026Read more

We bought our van new in 2019. It has always made a grinding noise when making hard turns. The dealer said it was normal. Flash forward to a few weeks ago when we hit 105k miles. We started noticing more grinding noises and sensations when turning the wheel. Additionally, we noticed a tugging sensation on the wheel when going over 60 mph. It felt like the lane assist was turned on, but it wasn’t
... . We took our van to our trusted auto repair garage. They did a series of tests which included isolating the sound of clicking and grinding to the electronic steering gear. We were told that this part would cost $5400 to replace. We believe that this part is going bad too early in the life of our van and would like the NHTSA to investigate.Read more

My 2017 Chrysler Pacifica experienced repeated engine timing and oil system failures. The vehicle first developed a check engine light and engine timing code (P000D – camshaft position slow response). The vehicle was inspected by a dealership service department. Shortly after, a major engine oil leak was discovered from the oil cooler / oil filter adapter assembly. The vehicle required repair o
... f the oil cooler assembly and associated seals. After that repair, the vehicle continued to experience engine timing issues. The dealership later confirmed failure of the cam phaser components associated with the engine timing system and performed a repair. Within days of the repair, the check engine light and engine timing code returned again. The failure involves the engine valve timing system and oil pressure dependent components based on Autozone code check. Improper camshaft timing can affect engine operation, cause loss of power, rough running, or possible engine stall while driving, which creates a safety risk in traffic. The issue has been inspected and confirmed by a dealership service department. The vehicle is available for inspection upon request.Read more
