... is was due to an injector breaking off inside the engine, sending metal fragments into the camshaft, which subsequently locked the motor. This created an extremely dangerous situation as the vehicle became unresponsive and unstable at high speed. Just two days prior to this incident, the vehicle had been serviced by Quantrell Cadillac in Lexington, KY, where a recall repair involving the engine was performed. Despite that service, a catastrophic failure occurred almost immediately afterward. After the breakdown, Quantrell Cadillac attempted repairs over the course of three weeks. These repairs included replacing the injectors, camshaft, and other components, before eventually replacing the entire long block. However, upon picking up the vehicle, it is exhibiting the exact same failure symptoms again—making a popping noise, stalling the engine, and becoming unsafe to operate. This vehicle is clearly not safe to drive and poses a serious risk to myself, my passengers, and others on the road. There have already been multiple recalls related to this issue, indicating a broader systemic problem. We are requesting the NHTSA’s immediate assistance to investigate this safety issue and help us obtain a replacement vehicle that meets all applicable federal safety standards. This level of repeated engine failure & instability is completely unacceptable for any vehicle—especially one with such recent manufacturer service and recall repairs. I had to manually push my car to chihuahua tires, where they told me it was my starter, so they fixed my starter, and then told me it’s my gasket. Now apparently my head gasket is gone, and it’s going to take thousands of dollars that I don’t have. I’m already paying off the $800 for this starter repair that I didn’t really need :/ and I’m still playing for a car note on a car I’m not even driving! I can’t get to work, I can’t make money, this is depressing! I got this car because they said at Carfect that my jeep was perfect and in great condition. I am filing a complaint regarding a serious and widespread defect in the Uconnect 8.4 infotainment system installed in my 2019 Jeep Grand Cherokee Limited and many other FCA/Stellantis vehicles. The infotainment system controls multiple functions beyond entertainment, including navigation, Bluetooth phone connectivity, climate control, backup camera, and access to vehicle settings. Malfunctions cause driver distraction and interfere with safe vehicle operation, which could easily lead to an accident. When I contacted Jeep/Chrysler Customer Care, I was informed this is a known issue. My research confirmed that FCA/Stellantis has issued an internal service bulletin acknowledging the defect, but they refuse to issue a formal recall. This appears to be a deliberate decision to avoid the significant cost of replacing defective units across multiple models, leaving many unsafe vehicles on the road. This problem is widespread and well-documented among owners. It is unacceptable that Stellantis treats it only as a service bulletin issue when the defect directly affects safety. Drivers must be able to rely on the infotainment system to respond correctly, especially since it integrates with systems used while driving. A delaminated screen dripping glue and producing ghost inputs is a hazard, not a minor inconvenience. I am requesting that NHTSA investigate this defect and require FCA/Stellantis to take responsibility through a formal recall and replacement program to protect the safety of drivers, passengers, and the public.Read more

I would like to bring up a serious safety issue with this vehicle (2024 Chrysler Pacifica Select PHEV) WITH 6997 miles on the odometer. As I was driving, at approximately 11:15AM, I received a message on the dashboard that read, “Stop Safely!! Vehicle will shut off in < 2 minutes”. For my own safety as well as other drivers on the road, I tried maneuvering the vehicle to the side of the road,
... however the vehicle shut off within 15-30 seconds. The vehicle did not move at all. The serious safety concern I have is “What if the vehicle had stopped running on the major highways full of traffic”? Please note, this is also a well-known issue starting from the 2021 Chrysler Pacifica models. Additionally, a similar incident occurred to another similar make model in Sept. 2024 with VIN # [XXX] . It seemed like, when I received the message, the vehicle's computer system interlocks fuel supply and chokes up the brakes to the wheels so the vehicle is completed locked up in one position and it does not do rolling stop to safely pull over to the side of the road. This is very dangerous because if a semi-truck with 80,000 lbs. load is coming behind you, you will NOT be able to move your vehicle to the side. This can potentially kill all the occupants in the vehicle. This vehicle has been towed to the dealer to troubleshoot/resolve the issue. I have had to replace the Driver's Side sun visor on my truck twice due to failure of the bracket holding the visor in place. The most recent time this happened was this month (August 2025). ... ing lights, and nothing recorded in the vehicle’s logs. This occurred less than 48 hours after the vehicle had received recall-related software updates and routine service at an Audi dealership. Prior to that service, the vehicle had no history of mechanical or safety issues. The following day, the keyless entry and anti-lockout system failed. I placed my toddler in his rear-facing car seat behind the front passenger seat and set our baby bag—with the key fob inside—on the rear seat. After closing the door and walking to the driver’s side, the car unexpectedly locked. The fob and my child were both inside. The outdoor temperature was over 90°F. I attempted to open the vehicle and then to break the window with tools, without success. After approximately seven minutes, I broke the driver’s side window with a sledgehammer and removed my child from the car. The car alarm eventually activated, but no lock-in event, key fob error, or warning was recorded. The vehicle was inspected by the dealership twice and Audi Corporate engineers once. Audi confirmed faults in the emergency call module, comfort system, rear lid sensor, and parking sensors. However, they could not reproduce or explain the braking event or lock-in failure. Audi confirmed that the vehicle should not be able to lock with the key inside, and they do not know why it did and are unable to guarantee the safety feature will work in the future. The vehicle has not been driven since. The failure of both safety function and system visibility placed my child in danger. I am requesting a safety investigation. ... at he had to repair this “Subaru manufacturing defect”. I had to get it repaired. I checked to see if there was a NHTSA recall on my Outback year and there was none, only prior years. Yet when I went online searching more, I found an article that stated that Subaru was going to recall the 2012 Subaru as well? The repair shop owner said to us that my wife is a "gentle driver" and that was good, because, if she had to apply the brakes very hard to avoid a collision, they probably would have failed. Please understand that I read the recall for previous years of Outbacks and other Subaru models, and my Subaru Outback had the exact same defect. I did contact Subaru of America and they responded that my Outback does not have a recall, only prior years. Unbelievable!!! The exact same make and model with the exact same defect and they just blow me off! I was lucky that we did not reach the point of a serious accident. I hope that NHTSA will evaluate my, and I am sure other safety issues with Subaru brakes, and take some action to give other owners the opportunity to make this very serious safety matter right, and avoid an accident. The contact resumed driving; however, the failure recurred, causing the contact to pull over to the side of the road several times. The contact was able to drive to the residence. The contact stated that upon starting the vehicle later, the vehicle made an abnormal grinding sound and failed to respond upon depressing the accelerator pedal. An independent mechanic diagnosed the vehicle at the residence. The mechanic determined that the transmission was slipping and might need to be replaced. The contact was referred to the dealer for assistance. Several unknown dealers were contacted and provided estimates for the transmission replacement. The vehicle was not repaired. The manufacturer was notified of the failure and informed the contact that the vehicle was not covered under warranty. The failure mileage was approximately 105,000.Read more

I was slowing down, foot off of throttle, to make a left turn. After completing turn and wanting to speed up, I had no throttle response for 2 or 3 seconds. Then all of a sudden back to normal. There was no accident. This has happened before, at least 3 times. Same/similar circumstances and result. I have not taken it to the dealer yet after reading other owners doing so and was not resolved.

I recently saw that this make and model of car was recalled about a year ago because the car could stall in the rain due to unsealed battery compartments, which is exactly what my wife experienced. Vehicle is equipped with aftermarket Fuel Off Road D679 REBEL wheels that use spacers, sold with the pre-owned vehicle by a licensed dealer. ... six lug nuts were very loose/near removal and the wheel could be moved laterally by hand. Suspected contributors: (1) misalignment of polyurethane spacer during re-installation, leading technician to believe wheel fasteners were “torqued-to-spec,” and (2) interaction between spacer/wheel seating surfaces and the hub during movement. All components are available for inspection upon request. Video of the condition and photographs of the spacer are available; the dealer reports relevant in-shop CCTV exists. Safety risk: Loss of clamp load during vehicle operation created sufficient lateral wheel movement to be felt as wobble/thumping of steering wheel, with risk of wheel separation/loss of control. Reproduction/confirmation: The dealer confirmed the issue, and reported findings to customer. According to the Service Manager, the post-incident follow-up inspection (Aug 20) found the front-right spacer had shifted off-center during the tire rotation service, allowing the wheel to reach false-secure position and the fasteners to reach a proper torque without properly seating. This led to clamp-load loss as the spacer re-seated in subsequent vehicle operation, particularly as speeds increased. Inspections performed: The dealer inspected the vehicle on Aug 20 after towing back to the Jeep dealership. Customer has notified manufacturer of this incident and is awaiting response. Warnings/symptoms: No vehicle warnings. Around 1 mile into the drive from dealership, a slight wobble and then a repeating thump developed, that worsened by arrival at destination, prompting inspection.Read more

The internal short is causing excessive current draw, resulting in loss of driver assistance features. The latest software has revealed the underlying root issue within the car computer.” This indicates that the vehicle was operating with a latent defect that Tesla’s diagnostic update exposed. The update did not cause wear-and-tear failure; it surfaced a hidden safety defect inside Tesla’s own hardware. Why This Is a Safety Concern Loss of traction control, stability control, and steering assist creates a significant risk of loss of vehicle control. If this had occurred while driving at speed, the sudden loss of these systems could have led to an accident.
