When accelerating to pass another car in front of us on a 2 lane highway we experienced a loss of power and all instrumentation flashed on/off/on, then it posted a check engine light and stated that auto start stop wasn't working. Power came back and we finished the pass but it could have resulted in a head on collision.

The contact owns a 2020 Ram 3500. The contact stated that while driving at various speeds, the vehicle made an abnormal ticking sound. The vehicle was taken to the dealer, where it was diagnosed with high-pressure fuel pump failure, but the vehicle was not repaired at the time. The contact stated that upon pulling into a parking lot, the vehicle stalled. The vehicle failed to restart and was towed
... to the dealer, where it was diagnosed and determined that the high-pressure fuel pump had imploded, causing damage to the fuel system. The dealer related the failure to NHTSA Campaign Number: 21V880000 (Fuel System, Diesel), and the vehicle was repaired; however, the contact later became aware that the dealer had replaced the high-pressure fuel pump with the same part. The contact stated that upon taking the vehicle to an independent mechanic to be serviced, the mechanic informed the contact that the high-pressure fuel pump installed on the vehicle was not the recall part, but the same defective part. The mechanic recorded the high-pressure fuel pump and part number, and the vehicle was taken back to the dealer, where it was confirmed that the recall part had not been installed on the vehicle. The manufacturer was notified of the failure and referred the contact to the dealer for assistance. The contact stated that the dealer delayed performing the recall repair free of charge, which required the contact to pay for the replacement of the high-pressure fuel pump with the correct part. The vehicle was repaired. The contact stated that the manufacturer failed to complete the recall repair free of charge. The failure mileage was approximately 38,000.Read more

The shift to park in my 2018 GMC Acadia was faulty. On many occasions and randomly, the car would be in park but would not turn off, saying I needed to shift it to park in order to turn the car off. This has left me unable to turn my car off resulting in dead batteries and missed appointments and being late for work. This defect is well documented by GM in TSBs PIT5616A, 18-NA-297, and 19-NA

This problem has been on going and as a teacher I cannot continue to be late for school because my car, though in park, would not turn off. On [XXX], I was driving my recently purchased 2018 Tesla Model S (VIN: [XXX], mileage approx. [30,536]). The vehicle was purchased directly through Tesla’s app in late August 2025 and delivered to me the same day of the incident. Approximately 4.5 hours after delivery, while driving with my son, white smoke began to come out of the cabin air vents. Within minutes, the smoke turned black and rap
... idly filled the interior of the car. I immediately pulled over and evacuated the vehicle with my son. Shortly after, the car caught fire and was completely destroyed. No modifications had been made to the vehicle, and it was sold to me as inspected and certified by Tesla. The sudden appearance of smoke from the vents and rapid escalation to a full vehicle fire posed a severe danger to me, my child, and others on the road. I am reporting this to NHTSA to document the incident and to request that the agency investigate whether Tesla Model S vehicles have underlying defects that may cause dangerous cabin fires shortly after use. The salespeople are very unprofessional and don’t take the wellbeing and safety of their customers or the surrounding public seriously. To even get the parking brake lowered, my husband had to disassemb
... le the plastic manifold, second the car was lowered to the point where it scraped the road whenever we were turning on an incline, finally there was some mechanical issue with the gear shift where we weren’t able to reverse. When we got back from the test drive the salesperson explained to us that they knew of this issue and that there’s a trick to it. This trick wasn’t mentioned to us at the start of the test drive, nor did the salesperson go with us to explain it to us on the road. This is very dangerous, what if there was an emergency and we needed to reverse. Not knowing this trick not only puts our lives in danger but also all the other drivers in the area. I’m not a car person, but even I know that a car should be able to go forwards and backwards.Read more

On [XXX], while driving at approximately 50 mph on [XXX], the vehicle experienced a sudden and complete loss of power accompanied by severe shuddering. Multiple warning lights illuminated on the dashboard, including the check engine light. The vehicle would momentarily regain power before failing again. This created a significant safety hazard, as I was unable to maintain speed or accelerate safel
... y in traffic. The vehicle was immediately taken to Team Gillman & then transferred to Sterling McCall Acura located at Houston, TX. The dealership diagnosed a catastrophic internal engine failure, originating from the bottom end, requiring a full engine replacement. The vehicle's mileage at the time of failure was approximately 142,250 miles. This critical failure occurred without any prior warning. I am aware of NHTSA Investigation Number ODI PE25008 regarding faulty connecting rods in certain Acura and Honda vehicles, which can lead to sudden engine failure. While my vehicle's VIN was not included in the initial recall population, its symptoms and failure mode are identical to those described in the investigation. I requested that Acura address this known defect through a goodwill repair, given the clear correlation to an ongoing federal safety investigation. This request was formally denied June 2025. Given that the NHTSA has expanded its investigation to include additional vehicles and model years, including that of my MDX, I am filing this complaint to ensure my vehicle and its specific failure data are included in the official NHTSA record. Urge the agency to compel American Honda Motor Co. to expand any subsequent recall to include my vehicle and others with identical failure patterns. The sudden nature of this engine failure presents an unacceptable safety risk to the driver, passengers, and the public. Thank you for your attention to this matter. Vehicle Make: Cadillac Vehicle Model: Escalade Model Year: 2022 Component(s) Affected: Engine, Fuel System, Powertrain, Vehicle Stability Control Incident Date: [Insert Date of Incident] Mileage at Time of Incident: [Insert Approximate Mileage] Summary of Complaint: While driving my 2022 Cadillac Escalade at approximately 70 mph on the highway, the vehicle suddenly lost all power and stability. Th
... is was due to an injector breaking off inside the engine, sending metal fragments into the camshaft, which subsequently locked the motor. This created an extremely dangerous situation as the vehicle became unresponsive and unstable at high speed. Just two days prior to this incident, the vehicle had been serviced by Quantrell Cadillac in Lexington, KY, where a recall repair involving the engine was performed. Despite that service, a catastrophic failure occurred almost immediately afterward. After the breakdown, Quantrell Cadillac attempted repairs over the course of three weeks. These repairs included replacing the injectors, camshaft, and other components, before eventually replacing the entire long block. However, upon picking up the vehicle, it is exhibiting the exact same failure symptoms again—making a popping noise, stalling the engine, and becoming unsafe to operate. This vehicle is clearly not safe to drive and poses a serious risk to myself, my passengers, and others on the road. There have already been multiple recalls related to this issue, indicating a broader systemic problem. We are requesting the NHTSA’s immediate assistance to investigate this safety issue and help us obtain a replacement vehicle that meets all applicable federal safety standards. This level of repeated engine failure & instability is completely unacceptable for any vehicle—especially one with such recent manufacturer service and recall repairs. When this stall occurs, the entire computer system & all safety features are shut down and collision avoidance is also disabled. Have viideoRead more
