After dealer (worldwide motors Ltd also call (Paragon Acura) 33-40 57th street, Woodside, NY 11377)replaced front and rear brake pads and rotors, my vehicle experienced unintended acceleration while stopped. While idling at a red light with my foot firmly on the brake pedal, the vehicle suddenly moved forward approximately 10 feet on its own. This was a serious safety concern and nearly caused a c
... ollision or pedestrian injury. I immediately returned the vehicle to the dealer. The dealer stated they could not duplicate the issue and said the vehicle was operating normally. No warning lights were present. The incident occurred immediately after brake service. I am concerned this may involve the braking system, brake booster, brake switch, or electronic throttle interaction. This behavior is not normal and poses a safety risk.Read more

My 2018 f150 has a 10r80 transmission. I was driving through heavy traffic on US Route 93 southbound, moving at about 60MPH when the transmission suddenly lost all power. Because there was no left-side shoulder (I was in the left lane), I needed to throw on my hazard lights and make my way to the right shoulder across 4-lanes of traffic. Multiple near collisions as I had no propulsion to do this.

In December 2024, at under 50,000 miles, my 2021 Chevrolet Colorado suffered complete brake failure caused by vacuum pump failure. The vacuum pump failure led to irreparable internal engine damage, requiring complete engine replacement under warranty. Sudden, complete brake failure in traffic could have led to a serious accident causing injuries or death to myself and/or others. The component was
... inspected by the manufacturer and the service advisor at the dealership stated this was "kind of a known issue on Colorados". There were no warning lamps, messages, or symptoms of the problem prior to the failure. This vacuum pump failure is documented in GM Technical Service Bulletin #21-NA-268, which addresses vacuum pump failures on other GM vehicles. The identical failure mode in my 2021 Colorado demonstrates this is a systemic manufacturing defect, not an isolated incident.Read more

In April 2022, I purchased a brand-new 2022 Yukon Denali from Roberts-Robinson Chevrolet GMC in Excelsior Springs, MO. Shortly thereafter, the first engine failure occurred in October 2022. The vehicle was towed to GMC AutoNation in Golden, CO, where it remained until repairs were completed in January 2023. Soon after the vehicle was returned, the second engine failure occurred in February 2023.
... The vehicle was again towed to GMC AutoNation, and repairs were not completed until April 2023. At that time, given the repeated failures, I worked with GMC and Roberts-Robinson Chevrolet GMC to replace the vehicle entirely. I took delivery of a new 2023 GMC Yukon Denali as a resolution. On October 8, 2025, the engine on the 2023 Yukon Denali also failed, marking the third major engine failure I have experienced across these vehicles. The vehicle was towed again to GMC AutoNation in Golden, CO, where it remained for two months. I picked up the "repaired" vehicle on Tuesday, December 9, 2025. This pattern of failures has caused significant disruption and distress. I have spent countless hours coordinating tows, securing rental vehicles, and seeking reimbursement for out-of-pocket expenses. More importantly, these repeated engine failures, all of which occurred while I was driving 65 MPH on I-70 through the Colorado mountains, have created extremely dangerous situations for me and my loved ones. Navigating heavy traffic during sudden engine failure and waiting on the roadside for assistance has been frightening and unacceptable for a vehicle of this caliber. Overall, this experience has been incredibly stressful and disappointing.Read more

A structural chassis component (V-brace) was permanently removed from my vehicle by Tesla Service during a service visit. The removal was performed under a Tesla-described engineering retrofit (Tesla Service Article 9188600). The component was not reinstalled or replaced, and Tesla stated that once removed it cannot be reinstalled. The service visit was originally initiated due to a persistent rat
... tle/noise from the vehicle The safety concern arises from: •A permanent modification to a structural chassis component, performed without my authorization. •Removal of a component originally installed for structural reinforcement and stability, creating uncertainty regarding vehicle rigidity, crash behavior, and handling. •The continued presence of an unresolved structural noise/rattle, which can be distracting to the driver and may indicate an underlying structural or mechanical issue. The vehicle was sold and delivered with this component installed, and its removal was not disclosed or approved prior to service. The noise/rattle was present prior to the service visit and prompted the service appointment. The structural component removal occurred during the service visit. The problem was reproduced and confirmed by Tesla Service and they: •Acknowledged the noise/rattle concern. •Performed a structural retrofit involving removal of the V-brace. •Marked the concern as “resolved”, despite the noise still being present. •Later stated the noise is considered “normal”, even though it is not present in all Model Y vehicles, including the Juniper variant. No warning lights or system messages appeared. The only symptom was a persistent rattle/noise originating from the vehicle chassis The vehicle was inspected and modified by Tesla Service, acting on behalf of the manufacturer. The modification was performed under a Tesla engineering directive. No independent service center, insurance representative, or law enforcement agency has inspected the vehicle.Read more

1. failure appears to involve the vehicle’s electrical control and communication systems, specifically (BCM), (TIPM), and/or CAN bus network. The malfunction caused multiple vehicle systems to activate or fail simultaneously, including windshield wipers, warning indicators, and the transmission shift interlock. The vehicle has been at a repair facility since the incident and remains available f
... or inspection upon request. Nothing has been removed/discarded w/o documentation. 2. The malfunction created a serious safety risk while the vehicle was in operation. All warning lights illuminated at once, the windshield wipers activated without driver input and would not turn off, and the vehicle experienced difficulty shifting and limited drivability. The unpredictable activation and loss of control over critical systems impaired the driver’s ability to safely operate the vehicle and created a risk of becoming stranded or disabled in traffic, posing a danger to both the driver and other road users. 3. Yes. The vehicle was taken immediately to an independent service center following the incident and has remained there due to the severity of the malfunction. The service center confirmed abnormal electrical behavior and initiated further diagnostic evaluation. 4. The vehicle has been inspected by an independent service center. The manufacturer has not yet conducted a direct inspection. No police inspection was performed, as no collision occurred. 5. No warning symptoms were observed prior to the failure. The malfunction occurred suddenly and without warning on August 8. At the time of the incident, all warning lights illuminated simultaneously, and abnormal vehicle behavior began immediately.Read more

Subject: Safety Complaint – 2025 Jeep Wrangler 4xe Unexpectedly Shifted Into PARK While in Traffic Vehicle: 2025 Jeep Wrangler 4xe Mileage at Time of Incident: 1300 Date of Incident: [XXX] Location: Ohio I am submitting this complaint to formally report a serious and dangerous safety defect involving my 2025 Jeep Wrangler 4xe. While approaching a red traffic light at normal driving speed, the
... vehicle suddenly and without any warning shifted itself into PARK. I did not press the park button, and no alerts or warnings were displayed prior to the event. The vehicle abruptly decelerated, leaving me momentarily without power in active traffic. I had to quickly react, restart, and shift back into gear. This incident was completely unexpected, and placed me and others at immediate risk of a rear-end collision. This behavior is unacceptable in a modern vehicle and represents a clear safety hazard, particularly in traffic conditions where sudden loss of propulsion can result in serious injury or death. There were no dashboard warnings, messages, or indications that the vehicle was about to enter PARK. The vehicle was functioning normally immediately before the incident. I am requesting: 1.A full investigation into this incident 2.Written documentation of any known defects, recalls, or software issues related to unintended gear shifts in the Wrangler 4xe 3.A permanent fix, not a temporary reset or software update without explanation 4.Documentation placed in my vehicle service record acknowledging this safety issue I am also submitting this complaint to the National Highway Traffic Safety Administration (NHTSA) due to the severity of the safety risk. This incident has caused me to lose confidence in the safety and reliability of this vehicle, and I expect this matter to be treated with urgency. Incident Description (Firestone Destination LE3 – P235/65R17) I purchased four new Firestone Destination LE3 tires (size P235/65R17, DOT IW2 8ILE31) and within the first week three of them developed noticeable bulges in the sidewalls. I returned to the tire service center, and they replaced the three tires due to the bulging. The following week, new bulges appeared again in the sidewalls on mul
... tiple replacement tires, for a total of six affected tires. Sidewall bulging creates a risk of sudden tire failure and loss of vehicle control, and I do not feel the tires are safe to drive on. One service center stated this was “normal from driving on roads,” but I disagree due to the number of tires affected in such a short time. The condition has been repeated and confirmed by multiple service centers. The tires remain available for inspection upon request. There were no warning lamps or messages before the bulges appeared. The issue began immediately after installation of the first set, and the problem persisted with the replacement set. In my assessment, the repeated sidewall bulging indicates a potential structural or manufacturing defect.Read more

Incident date: Dec 15, 2025
on 11/1/25 we were driving our Jeep and it malfunctioned while at a stop light. We heard a loud "clunk" and the car would barely go and if we tried to put in park, it would continue to roll. We were on top of a hill so thankfully we didnt roll back or into traffic. We could not drive the car and had to have it towed since it was not safe to drive. Our Jeep is at an authorized Jeep dealer since 11/
... 3/25 and we were told the issue was a power transfer unit that would be covered on the recall but the part was on backorder until 12/2. Jeep would only cover $50/day for a car rental, so we had to rely on borrowing cars from others. Once 12/2 came, we reached out to the dealership and they said the part ship date was moved to 12/18! This is not a timely manner. We have made at least 3 calls to Jeep over the past week, and they keep telling us since 12/10/25 that they will let us know in 2 days...2 days pass and they transfer our case. Another two days pass and still no call from them so I called them back and they have yet to call me. We had our Jeep in the week of 11/17 or 11/24 for inspection and it passed. We noticed some lagging when we were going up hills in the 1-2 rpm range but mechanic did not notice it. Between the inspection and when it finally broke down, no lights came on and there were no warning messages. Not until the car broke down on 11/1.Read more

Vehicle: 2016 Honda CR-V Mileage: ~133,120 miles Component: Driver-side seat belt retractor The driver-side seat belt retractor failed during normal use and became mechanically locked and unusable. There was no crash, no collision, no misuse, no airbag deployment, and no SRS warning light at the time of failure. This failure rendered the primary occupant restraint inoperable. I requested coverage
... under Honda’s Seat Belt Limited Warranty, which states that seat belts that fail to function properly during normal use are covered. Honda denied warranty coverage for the driver-side seat belt retractor based on internal component classification, not because the seat belt functioned properly. The warranty denial was final. I was informed that the failure would not be covered under the seat belt warranty and that any assistance would only be discretionary goodwill, not warranty coverage. I am submitting this complaint because a primary safety restraint failed without a triggering event, and warranty coverage was denied despite normal-use failure. I believe the consumer-facing warranty language does not clearly disclose that a seat belt can fail mechanically yet still be excluded from coverage based on internal classificationRead more
