With this 2025 Lexus NX350 Premium AWD, the front windshield glass is BADLY distorted. Looking at the road ahead while seated behind the steering wheel, objects aren't clear, they tend to be wobbly as there is a MAJOR safety issue here with the glass itself. My dealer told me that the other 2025 Lexus NX models they checked all have the same distorted windshield problem as mine does. Its so bad, I

On December 24, 2025, the contact's 2023 Chevrolet Malibu was involved in a severe side-impact collision. The vehicle was struck on the rear passenger side by a full-size pickup truck traveling at high speed (est. 60 mph) that failed to stop at a red light. The impact caused the contact's vehicle to spin approximately 180 degrees and strike a concrete median sideways, generating high lateral G-for
... ces. FAILURE 1: AIRBAG NON-DEPLOYMENT. Despite the severity of the impact and the violent spin into a curb (rollover trip point), the side curtain airbags and front seat-mounted side impact airbags failed to deploy. The failure of the restraint system caused the driver to be thrown against the driver-side door structure, striking their head on the window and B-pillar. The driver sustained a concussion, cervical strain, and lumbar strain. Medical records confirm the injuries are consistent with impact against the vehicle interior due to non-deployment. FAILURE 2: EMERGENCY TELEMATICS MALFUNCTION. Immediately post-collision, the driver attempted to manually activate the OnStar/SOS emergency buttons. The system failed to connect to an advisor. Instead, the vehicle's infotainment screen malfunctioned, opening a voice-to-text transcription application. The system then bypassed the vehicle's internal telematics module and attempted to route the emergency call to the owner's personal cell phone, indicating a complete failure of the vehicle's emergency communication hardware. This resulted in a 34-minute delay in coordinating professional assistance. Contact notes; while this vehicle currently shows 0 open recalls, the failure mechanism (sensor non-response during side impact) mimics the defect described in NHTSA Campaign 22V923000 (GM Recall N222392300), where improper welding of the front impact bar causes crash sensors to fail. The vehicle is available for inspection. Police and medical reports confirm the incident details. No warning lights were illuminated prior to crash.Read more

While the vehicle was parked and not in operation, the passenger-side active head restraint suddenly deployed forward on its own. There was no collision, no braking event, no external force, and no warning lamps, messages or other systems of the problem or that it was going to occur. The deployment was unexpected and presents a risk of head or neck injury, as the active head restraint is designed
... to move only during a rear-impact event. This appears to be a known defect, as the manufacturer previously issued Warranty Extension X88 related to unintended Active Head Restraint deployment on certain 2014–2015 Dodge Durango vehicles. A Dodge dealership refused to repair the defect, stating the warranty extension expired, and quoted over $800 for repair. The dealership did not provide a written denial or repair order. This condition involves a federally required safety system and constitutes a safety defect.Read more

Vehicle experienced critical ignition system failure while parked - the engine would not turn off despite repeated attempts over 20 minutes. The vehicle has a communication failure between the ignition switch and the Body Control Module/Keyless Access Unit, creating a stuck electronic latch circuit (B12D3 latch error). This is consistent with Honda TSB 20-116. The keyless start system warning appe

Unable to shut down the vehicle through normal means, I was forced to manually pull the fuel pump fuse from the fuse box and then disconnect the battery while the engine was still running. This created an extremely dangerous situation requiring me to work in the engine compartment with live electrical systems. The vehicle lacks proper failsafe shutdown mechanisms. This communication failure and stuck ignition circuit poses serious safety risks including fire hazard, carbon monoxide poisoning if in enclosed space, and electrical system damage. No officially stated emergency stop solutions had worked for this vehicle included the triple press and long press.Read more

My 2022 had a new crate engine put in at 26,000 miles in approx June 2025 (told me it would be 2 days but in less than 24hrs the service advisor called and told me it was completed) and from the second I picked up my truck and started it at the dealership, I heard a loud, fast ticking noisy sound coming from under the hood so I popped my hood and it sounded like a diesel engine and as I started d
... riving I felt a weird popping/vibrating noise when turning either right or left at slow speeds and it felt like there were no shocks or motor mounts or something was bent and the rear end was skipping and making a loud thud when slightly stepping on gas pedal and Toyota said it's all normal. It also lost all power while I was driving 2 days after I picked it up from engine replacement along with the continuous surging forward when taking off from stop sign or red light. My front seat is cracked and they refused to fix it under warranty, my brakes squeal so bad and they said they ordered parts under TSB but it will be months before they come in, it's now been 7 months and still nothing. I will not drive further than 10 miles from my home Every time I get in this truck and put it in D, I fear for my life and when I put it in P, I breathe a sigh of relief. AutoNation Toyota in Weston, Fl replaced my engine and I truly believe that whoever put my new engine in, didn't put it back together correctly and damaged the suspension/steering system as well as the new ticking engine that is so rough. Component/System Involved: Driver-side rear seat back latch / locking mechanism. The rear seat back would not latch or lock in the upright position. The vehicle and seat are available for inspection upon request. Description of the Incident: During normal use of the vehicle, the driver-side rear seat back unexpectedly moved forward because it was not latched. At the time of the incident, we were
... not aware that the seat back would not latch or could not be secured in the upright position. The condition was discovered only after the incident occurred. Safety Risk: Because the driver-side rear seat back was not latched, the seat back or attached child seat moved forward and struck a child passenger seated in the rear. This created a serious risk of injury and represents a failure of a safety-related interior component. Confirmation / Reproduction: After the incident, attempts were made to secure the driver-side rear seat back. The seat could not be latched under any condition. A Hyundai dealership later inspected the vehicle, confirmed there was an issue, and ordered parts. A dealer representative stated on video that a component appeared to be missing. Inspections: The vehicle has been inspected by a Hyundai dealership. No collision occurred. The vehicle has not been modified. Warnings or Prior Symptoms: There were no warning lights, messages, or indicators. No prior warning or indication of the seat latch failure was known before the incident. Cause (Best Assessment): UNKNOWN. Based on post-incident inspection and dealer findings, the condition appears consistent with a defective, improperly installed, or missing rear seat latch component from the factory.Read more

Unintended acceleration. When you stop the truck, and have the brake on, the rpms go down to idle ~650. As soon as you lift the brake, without touching the accelerator, the vehicle starts ramping up the rpms and moving. On level ground it will go to 1000 easily before you have to stop it. It will pull up a grade even while towing my boat. If you're in a line a traffic, say at a traffic light
... , and the light changes, traffic moves and the car ahead takes off their brakes, and you do same, the truck starts moving faster than the vehicle ahead without touching anything. It's idling too fast without the brake and doesn't stay at idle once the brake is off before you push the accelerator. First the dealer told me everything was "Normal" - it's supposed to do that. I've been driving vehicles since 1962, so I think I know what I'm talking about. They told me to call Toyota. Toyota started a case. They took it in for an factory inspection because I think this might be related to a non highway collision with a bldg, I had shortly after I got it, I'm not sure. I put into park in a parking space but apparently it didn't engage, only into reverse. I was looking for something and the truck started backing up and hit the gas station bldg close behind. The anti-collision was off, because you have to turn it off to tow. And I hadn't turned it back on yet. This truck has way too many audible alarms and very annoying, especially when backing or exit the vehicle. I don't need to know that I'm out of the vehicle.Read more

The loss of propulsion occurred on a busy city roadway and created an immediate safety risk to me and surrounding motorists due to the sudden inability to maintain speed or power, with a foreseeable risk of collision. The problem has been confirmed by the dealership. After subsequent inspection and teardown, the dealer documented catastrophic internal engine damage, including cylinder wall scoring and a failed head gasket. The dealership acknowledged that deeper inspection during the prior internal engine repair would have revealed the need for engine replacement. The vehicle has been inspected by the dealership multiple times and by the dealership’s insurer. The manufacturer (Stellantis) has opened a customer care case. The vehicle is currently available for inspection and has been removed from service due to safety concerns. Prior to the failure, the vehicle displayed warning indicators and messages at the time of the incident, including limp mode activation and critical alerts immediately before and during the shutdown. No warnings were provided to me at the time the vehicle was returned from repair regarding unresolved internal engine risk or potential loss of propulsion.Read more

Supplemental Safety Description I am submitting this supplemental statement to provide a complete and consolidated description of the ongoing safety concern affecting my vehicle. Vehicle Behavior and Safety Symptoms During normal driving conditions, the vehicle consistently exhibits directional instability, including pulling to one side and difficulty maintaining straight-line tracking. Steering c
... ontrol feels unstable, and the vehicle experiences abnormal harshness and vertical movement over the road surface. The vehicle often feels difficult to control and demonstrates behavior consistent with lateral misalignment, including a crab-walking sensation. During rapid left and right steering inputs, the vehicle exhibits abnormal and asymmetric body movement. One side of the vehicle body rises excessively while the opposite side compresses, resulting in excessive body roll and uneven suspension response. This abnormal dynamic behavior creates a strong perception of instability and increases the risk of loss of control or rollover during steering maneuvers. Additionally, overall traction appears reduced. Under wet or slippery road conditions, the wheels are more prone to slipping, further compromising vehicle stability and steering control. Repair History and Dealer Involvement The vehicle has been presented to four different Toyota authorized dealerships for the same steering and suspension-related concerns. Multiple wheel alignments, steering calibrations, inspections, and road tests were performed. Despite these repeated repair attempts, the condition persists. Each dealership reported that no abnormal condition was found and stated that the vehicle was operating normally. In at least one instance, a dealership declined to continue further repair attempts while the safety-related condition remained unresolved.
