Incident Description (in my own words): We were driving southbound on [XXX] from New Hampshire back to Boston in our Tesla Model Y. Traffic became increasingly congested and slowed to a crawl. My wife was driving, and I was in the front passenger seat working on my laptop. As traffic built up, my wife became momentarily distracted and did not apply the brakes in time. ... vision, the vehicle ahead of us—a truck—suddenly filling the windshield. At that point, I heard the forward collision warning alert, but it activated extremely late, essentially at the moment of impact. The vehicle did not automatically brake or stop, despite the low speed, close proximity, and clear visibility of the truck ahead. We collided with the truck in front of us. Damage was minimal (license plate damage and a small dimple on the front of the vehicle), but the safety concern is significant. Both of our Teslas are configured with forward collision warning set to “Early.” Under normal circumstances, this alert triggers frequently, sometimes conservatively. In this incident, however, the warning activated far later than expected, and there was no automatic braking intervention prior to impact. The front camera appeared clean and unobstructed. I am filing this complaint because this behavior undermines my confidence in the vehicle’s collision avoidance and automatic emergency braking systems. I am also filing a similar complaint for our Tesla Model 3. I am now anxious about whether the software will perform as expected in situations where driver attention lapses briefly—precisely the scenario these safety systems are designed to mitigate. I am requesting that NHTSA review this issue, as the system did not behave in a manner consistent with its intended safety function. Despite this, suspension overheating warnings and intermittent system failure notifications continued to appear. On December 23, 2025, while driving at moderate speed in clear conditions, the vehicle suddenly and uncorrectably migrated off the roadway. Adaptive steering correction, braking intervention, and lane assist failed to engage. At the time of the incident, the display actively showed service notifications indicating suspension system failure and overheating. The vehicle veered into a ditch, resulting in a significant drop and requiring specialized heavy-duty extraction. This defect has impaired the vehicle’s handling, stability, and safety from delivery onward, with no resolution after attempted repair. I am writing about my recent engine failure on my 2016 Chevy Silverado High Country. This is a 4x4 with the 6.2L motor. My engine has failed at only 66,790 miles due to the lifter issue that seems to be common with these motors. I noticed a loud ticking from my motor that was getting worse. I took it into A1 Automotive Georgetown, TX. They advised me to contact a dealership for Warranty repair or
... replacement as they noted it was coming from the valves. I contacted Don Hewlett Chevrolet; they advised me due to the date of the truck being made in 2016 it is no longer covered in warranty and the advisor said they do not see engines with lifter issues. I did standard service with an oil and transmission change using the recommended 0W-20W oil. The next day the ticking was very bad and turned into what I would describe as ‘jack hammer’ noises. Then I lost power while driving and the dashboard lights came on. I got towed back to A1 Auto; they drained the oil and noticed a lot of metal fragments and advised me that theck engine will need to be replaced. The engine was removed and we found the pushrod on the passenger side was sticking out to the side of the rocker arm presumably due to a lifter issue. I now have a new engine with the ‘delete kit’ to remove the 4-cylinder shutoff that has been linked to lifter failures. This has been a very unnecessary expense for something that shouldn’t happen. The motor only having 66,790 miles and I am the only owner, I have always taken very good care of it, and even use only premium gas. I have gotten all the oil changes on time and most of them have been done by the dealership at Don Hewlett. Sometimes the speedometer will say 0mph even tho the car is moving. Sometimes I am able to pull over and turn my car off and back on and the lights will go away, but often times this doesn’t solve the problem. In fact, it feels like it only exacerbates the problem because when I shift it into park and then attempt to turn off the vehicle, it says “Shift to Park” even though it IS parked. In fact, this morning when the vehicle wouldn’t start after I tried to start it with the button, I attempted to turn the vehicle off (because the radio would come on and I can see lights on the dashboard, but the vehicle itself won’t start) and it said “Shift to Park” even though it is parked and has been since last night. I contacted the nearest Chevy dealership and they informed me that they have dealt with plenty of these complaints lately. I have not had an inspection completed by the dealership because my extended warranty is up and I do not have the funds to pay for a diagnostic inspection.Read more

When I left the dealership the day I bought it, the vehicle drastically depreciated once I cleared their mailbox and to this point, GM hasn't offered any assistance with offsetting my trade difference. I also got 0% financing with GM that isn't currently being offered if I were willing to take the massive loss on my trade in. Here are just a few of the issues that I'm dealing with. The blind spot warning light stays on for no reason. Multiple updates have been done, and modules were changed and the issue remains. At night the warning light shines right in my face. I've tried adjusting the mirror completely outward and it's still an issue. I try avoiding driving at night all together because it makes it very unsafe. If I do have to drive in the dark, I have to use my arm to try blocking the light from my face, but my arm isn't big enough to block all of it. The entire dash has gone black leaving me with no speedometer or gauges. It did come back on, and the dealership ran updates. Most recently, while driving the car will slam on the brakes and put the red triangles on the windshield for no reason. I've had vehicles behind me that almost hit me because it is a drastic drop in speed. Like brake checking. Sometimes the adaptive cruise will start slowing me down like when a car is in front of you only there's nothing there. Dealership doesn't have a solution for these issues. I've also been places and come back out to the car to find that the rear hatch has opened on its own. The dealership ran updates to hopefully prevent this from reoccurring.Read more

Physical inspection shows progressive circumferential rotor banding, pitting, and corrosion on all four wheels that does not clear with normal driving. This indicates the brake pads are failing to make uniform contact with the rotors, compromising the integrity of the vehicle's primary safety system. The dealer has acknowledged the symptoms but has declined Integrated Brake (IB) Module–related repair, stating the VIN is not currently flagged for recall. However, these symptoms are materially consistent with the failure modes described in NHTSA recalls 24V-104 and 24V-697 involving the Integrated Brake Module on this platform. Despite three documented repair attempts (October 2025 – December 2025), the manufacturer has failed to rectify the underlying mechanical defect. Now I see many people have been having trouble with this CVT transmission around the same mileage or earlier. My registration is due this month and cant renew it do my vehicle being undrivable. Anecdotally (online forums and other resources) and despite the reported backorder, the recalls are being completed in a timely fashion in parts of the country. Some recalls apparently are being completed even on trucks where the bolts have yet not failed. Obviously parts are available because Ford is selling new trucks which presumably are not subject to the recall. In December, we were 14,000+ "in line" for the parts according to Ford service. Apparently we moved up about 1,000 places in one month. At that rate, we think it is unacceptable to not have full use of our truck until 2027, are frustrated to have to continue to pay insurance and licensing costs, and are very disappointed at the prospect of canceling summer plans again.Read more
