-ICCU failure, identified by dealer service department as the cause of catastrophic failure. -The car suffered a catastrophic failure on a 2-lane country road in Princeton NJ on [XXX] at approximately [XXX] at night in 20 degree temperatures. Every function except blinkers (temporarily functional) ceased to work. I waited in the car, without heat, for an hour before service arrived. The car could
... not be moved and was left in the road until 3:00 am, when the towing company I paid towed the car to my driveway a mile from the failure site. -My safety was at severe risk while I waited in the car. -Dealer replaced ICCU assembly with the same part, with the possibility of repeated failure. -Warning messages appeared somewhat over a mile before catastrophic failure. The car slowed to 11 mph. -Hyundai has known of the potential for catastrophic ICCU failure in their electric cars since 2022. I took possession of this 2025 model in March 2025. I was never informed of any recall, although it appears that a recall was issued at the end of 2024. -Hyundai has denied compensation for irresponsibly, with knowledge of the potential failure, subjecting me to a hazardous, life-threatening situation. -Anecdotally, of the 4 Ioniq owners I know, 3 have experienced ICCU failure. have a 2019 Ram four-door. I had it for about 6 months when I got this alert for the first time. I was coming to a stop light, and the truck dings at me, gives me the error code "Shift to P then Desired Gear" and shifts itself into neutral. Once I come to a stop, I can then put it back into Drive and continue driving. This only happened once then so I let it slide at a quirk. Fast forward to now
... . The error code comes up at least 4 times per week, sometimes multiple times in a day. Always happens when I'm coming to a stop. I do notice that the truck seems to shift very hard when accelerating from a full stop or slowing to stop. This has almost got me killed twice now. I have taken to dodge multiple times with no resolution. Please helpRead more

Electric Power Steering system issue after battery died and had to be jump started.
Incident date: Jan 24, 2026
I am reporting a serious nighttime visibility and driver-assistance safety issue with my 2025 Honda Passport. During evening driving, approximately 60% or more of the upper portion of the windshield appears blacked out or severely dimmed, significantly impairing forward visibility. This condition worsens on downhill grades, where an even greater portion of the windshield darkens. Visibility improv
... es when high beams are manually activated; however, driving continuously with high beams is unsafe and inappropriate. This creates an unreasonable risk of collision, particularly on unlit roads and downhill grades, as pedestrians, road curvature, and hazards are difficult or impossible to see without improper use of high beams. In addition, multiple ADAS systems operate erratically. Adaptive cruise control delays braking on downhill grades until the vehicle accelerates approximately 9–10 mph above the set speed, delays acceleration after braking and resuming, and disengages with minor steering inputs. Automatic high beams operate inconsistently. These issues persist despite recalibration. The condition has existed since early ownership and continues at approximately 20,000 miles despite multiple service visits. A Honda field technician confirmed all systems were calibrated but did not perform a nighttime drive. A dealership shop foreman conducted a nighttime drive but did not share findings, and I was told he was unavailable and had already spent “enough time” on the vehicle. My initial service record included the notation “do not diagnose.” I was initially told the dealership had never encountered this issue; later I was told they had seen it on other vehicles but did not know how to fix it. A dealership-recommended nighttime comparison drive with a 2026 Honda Passport did not exhibit these issues. I also regularly drive a 2024 Honda CR-V that shows none of these nighttime visibility or ADAS problems.Read more

Problem Description The core issue stems from a production fault in the battery cells that can cause increased self-discharge. In a high-voltage battery pack, which operates like a chain of interconnected modules, a single faulty module with elevated self-discharge becomes the “weakest link.” This leads to: • Degradation of the overall usable capacity. • Reduced driving range and power l
... imitations. • Potential cascading failures affecting additional modules over time. • Warning indicators, such as a yellow electrical system fault in the instrument cluster (fault code P0BBD00: Hybrid Battery Pack Voltage Variation Exceeded Limit). • Infotainment messages like “Electronic system does not work correctly. Please go to workshop.” • In severe cases, unexpected shutdowns or failure to pass technical inspections, potentially rendering the vehicle non-roadworthy under local regulations. The fault is linked to specific battery module part numbers from LG, including: • 0Z1.915.592.G/H/J • 0Z1.915.599.G/H/J/P These were produced through the end of 2023 (up to December 4, 2023) and include configurations like 12S2P (small capacity) and 8S3P (large capacity), meaning all MEB battery setups could be impacted. Volkswagen does not publicly disclose full VIN ranges, exact affected counts, or the precise technical cause of the manufacturing fault, but internal escalation procedures exist to identify additional defective batches.Read more

Safety defect in my 2021 Honda CRV. Until 11/23/2025 the rearview camera had perfect view. 11/24/2025, camera view became obstructed by the camera Lense black rim in the permanent housing. On 11/28/2025 saw my local mechanic. Told after visual inspection that the lense had slipped down in the housing. This would have to be repaired by Honda. Appt.11 AM on 12/18/2025 at Crown Honda Pinellas Park
... Fl. I viewed Honda manual for 2021 Honda CRV camera problems. Had to be taken to a Honda dealership for repair. Honda forums, and Honda repair technician blogs stated backup camera lense is a defect, has to be repaired by Honda. At Crown Honda on 12/18/2025 at 10:45 AM, I was checked in by a manager. This guy ran the outside car take in. He looked at the problem in driveway. After 15 min, I was told its “Normal” he said “I looked at the same year in lot and it looks the same.” I told him the camera lense housing is loose and needs repair,“ no its not” he said. I explained the backup camera lense dropped further since 11/24/2025 and almost resulted in a pedestrian injury twice. Backing out of my drive way and in a shopping center parking lot. Both times from the rear right side of the car, a known blind spot in Honda CRVs. Both times the camera did not see the cross-traffic movement. I insisted a mechanic look at the car. Another shop manager comes and says its normal. I showed both of them the proof it is not normal, my obstructed view is a safety concern. At 12:07 they finally agreed to have mechanic # 971716A look at it. The second manager says “nothing is wrong.” I said, “if an injury occurs which is a result of this obstructed view, I will hold you/him accountable.” I said” check the housing holding the camera” he said “No, and the truth is; not enough people are complaining for Honda to pay for this repair”. I am truly concerned this is a serious safety hazard that can result in injury or death due to an obstructed view of rear camera.Read more

After 38,000 miles and 8 years of normal service (mix of city, highway, and 10% towing), my 8L90 transmission began to shudder occasionally. The problem first became apparent when driving slightly uphill, on cruise control at about 50 mph, or while slightly accelerating on a flat road, with no load in the box, and no trailer. It felt like I was driving across a rough patch of asphalt or rumble s
... trips. Every half mile or so, it would shudder. Slowing down or speeding up stopped the rumble. The problem proved not to be caused by tires, U-joints, or AFM. The problem in fact was caused by defective GM transmission fluid, which was sold as "sealed for life", and rated for 200,000 miles, as factory-installed. That proved to be BS. If not repaired, this problem would lead to transmission lockup and/or lack of propulsion, which could contribute to a vehicle crash. The problem was solved by installing AMSOIL Signature Series Fuel-Efficient 100% Synthetic Automatic Transmission Fluid, Product Code: ATLTP-CA. If GM properly cared for their customers, to make up for false advertising at the time of purchase, GM should publish and pay for GM Service Bulletin #20-NA-142, for all 8L90 owners, instructing owners to replace their transmission fluid every 5 years or 40,000 miles, whichever comes first.Read more

Vehicle: 2008 Ford Escape VIN: [XXX] Component: Steering – Electric Power Steering Assist Recall Involved: 14S05 / NHTSA 14V-284000 Description of the Safety Problem: My vehicle is experiencing a recurrence of electric power steering assist failure after a prior safety recall repair. This 2008 Ford Escape was subject to Safety Recall 14S05 (NHTSA Campaign 14V-284000), which addresses sudden los
... s of electric power steering assist due to a torque sensor fault. This condition can occur while driving and results in significantly increased steering effort, creating a safety risk. Ford records show the recall was marked “completed” on November 22, 2014. In a written response dated December 22, 2025, Ford Motor Company confirmed that the recall remedy consisted only of software reprogramming of the Power Steering Control Module (PSCM) and Instrument Panel Cluster (IPC). No mechanical components (including the torque sensor or steering column) were replaced. In 2025, the same steering assist failure recurred, resulting in heavy steering effort and renewed safety concerns. This indicates that the recall remedy did not correct the underlying defect. Ford Motor Company has acknowledged in writing that: The recall was software-only The recall is closed No current programs provide coverage for the recurring steering failure Ford denied further assistance strictly on eligibility grounds and did not dispute the recurrence of the defect or its safety implications. Because this is a recurrence of the same safety-related defect addressed by the recall, I am concerned that the recall remedy was inadequate, and that other vehicles repaired under this recall may also experience recurrence after software-only updates. I am submitting this complaint to request NHTSA review of: The adequacy of Recall 14S05’s remedy Whether a software-only repair sufficiently addresses torque sensor failure The safety implications of recurring steering assist loss after recall completion Loss of INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIORead more

While driving, the vehicle began experiencing repeated electrical failures including blown fuses, inoperative headlights and brake lights, and the transmission becoming locked in PARK. When the fuse blows, the gear shift cannot be moved out of PARK and the ignition key cannot be removed, leaving the vehicle stuck and unable to be driven or safely turned off. The brake lights and daytime running li
... ghts also stop working, making the vehicle unsafe to operate in traffic. This condition has been occurring repeatedly and requires frequent fuse replacement just to move or shut off the vehicle. An emergency override hole on the shifter had already been exposed, indicating the problem existed before and the vehicle was previously being forced out of PARK manually. A tow truck driver had to use this override to move the vehicle. The airbag warning light is also illuminated, indicating the safety restraint system may not function properly in a crash. These failures create a serious safety risk due to loss of exterior lighting, inability to shift, inability to turn the ignition off, and possible airbag malfunction. This failure is consistent with a known defect involving the Daytime Running Light (DRL) module overheating and causing electrical shorts, as documented in manufacturer recall and technical service bulletins for this vehicle. The problem has been reproduced multiple times and remains unresolved.Read more

The affected components are available for inspection upon request. Repair
... was quoted but not performed. This condition involves a safety-critical suspension component and may affect vehicle handling, stability, and braking performance, particularly during turning or emergency maneuvers. This issue follows a prior safety-related brake failure at approximately 17,000 miles. At that time, a brake system warning light illuminated, and the condition was confirmed and repaired by the dealer. Both incidents involve safety-critical systems occurring at unusually low mileage. No warning lights or messages were observed prior to the suspension issue. The vehicle has not been involved in a collision and has not been inspected by police or insurance representatives.Read more

While driving at normal road speed, the vehicle suddenly displayed multiple warnings at the same time indicating that AdvanceTrac (electronic stability control), traction control, and ABS were disabled, and the vehicle entered a reduced functionality state. These warnings appeared simultaneously without any hard braking, loss of traction, or driver input at the time. The warnings and system fault
... s then cleared in a cascading manner over a short distance, with each system returning one after another while continuing to drive, without shutting off the engine. After approximately a few thousand feet, all warning messages disappeared and the systems appeared to return to normal operation on their own. This has occurred more than once since ownership. The first occurrence happened at approximately 50 mph. The second occurrence happened at approximately 20 mph and occurred only seconds before an emergency braking event in the rain to avoid a collision. At the time the warnings appeared, the vehicle had temporarily lost ABS, stability control, traction control, and related braking assistance systems, creating a serious safety risk during an actual emergency maneuver. After the events, an OBD-II scan was performed and no stored fault codes or diagnostic trouble codes were present. However, for the next several engine on/off cycles, the steering and traction control behavior felt abnormal, as if the systems were only partially engaged, even though no warning lights or messages were displayed. No permanent warning lights remained illuminated afterward. The problem is intermittent and difficult to reproduce. No collision occurred, no repairs have yet been performed, and the cause is currently unknownRead more
