The fuel injectors in my vehicle failed,this failure caused the catalytic converter to become damaged, andHonda accepted responsibility for the faulty fuel injectors. Honda has refused to cover the damage to the catalytic converter. Because the catalytic converter failure was reportedly caused by faulty fuel injectors—components for which Honda accepted responsibility—I believe the manufacturer should cover all resulting damage, regardless of warranty status. I request that the National Highway Traffic Safety Administration: Investigate whether similar fuel injector failures are occurring in Honda vehicles. Review whether these failures are causing catalytic converter or other engine damage. Determine whether the manufacturer is appropriately addressing these failures.Read more

When it happened, a brake light appeared on my dash with the message "electric brake failure" and "speed limited". I slammed the Brake pedal to the floor repeatedly with nothing happening. NHTSA Complaint Text (Copy/Paste) Vehicle: 2020 Ford F-250 Super Duty 6.7L Power Stroke Recall: 24S78 – Powertrain Control Module (PCM) Software Update Complaint: Ford has issued Recall 24S78 for the high-pressure fuel pump defect on my 2020 F-250, but the company has not provided an actual remedy. The recall shows as open on my VIN, and Ford’s documentation says a PCM software update is the f
... ix, but Ford released the software and then pulled it back within a week. My dealership confirmed Ford removed the remedy from their system, so they cannot perform the recall at all. This leaves my truck exposed to a known safety defect with no available repair. The high-pressure fuel pump failure this recall addresses can cause sudden loss of power, engine stall, or complete shutdown while driving. Until a working remedy is released, I’m driving a truck that Ford has admitted has a serious safety defect, but they have not provided any fix or timeline. I am requesting NHTSA investigate why the remedy was withdrawn, why no replacement remedy has been provided, and why Ford is leaving owners with a known safety risk for an extended period. This is an expensive vehicle, and the defect can cause catastrophic engine damage and create a hazardous situation on the road. Owners deserve an effective recall remedy that is actually available. Over the past few years, I have experienced sporadic issues with my transmission. I am aware of an investigation into 2015-2017 F-150's with 6-speed 6R80 automatic transmissions which is what I have. ... at it's just a matter of time. I have experienced slipping out of 6th gear or what I would describe as the transmission "hunting" to find a shift point between 4th and 6th. I first became aware of this while trailering an RV on the highway using cruise control in the summer of 2023. Periodically, I would notice a "lurching" sensation as the vehicle seemed to have difficulty maintaining 6th gear. As time has gone on, I've had this happen even when not trailering but when going up hills at times (transmission under load). Additionally, I have experienced sudden loss of power under acceleration like entering an on-ramp to a highway. Twice (at least) I have had this happen, the check engine light will begin flashing and I lose the ability to accelerate. After stopping, shutting off the vehicle and then starting again, the CEL turns off and I can resume driving. I have brought this issue up with my local dealer twice, but they have not been able to diagnose anything because of how sporadic the problem is.Their advice was to simply wait until the problem got worse and more predictable. Since my F-150 is not a daily driver for me, it has largely just sat in my garage since 2023 and rarely used. Due to the poor underbody shielding on these vehicles, the entire hydraulic system (lines, fitting, pumps, etc) of the Active Cornering Enhancement (ACE) system had severely corroded and must be replaced. If this system fails, you lose your air suspension and makes the car unsafe in turning while driving causing you to lose control over your vehicle. My independent service center quote is for $1

... lear that the EGR valve failed. Unplugging the valve and replacing the fuse has returned the vehicle to normal operations, but it's now out of compliance with local emissions regulations. GM has failed to properly produce replacement parts. As of Feb 11, 2026, a local GM dealer in Bloomington, IN quoted a backorder and unclear wait time on a replacement part and quoted me at $2,000+ just for the part. If not for my quick thinking, this part failure could have overheated my vehicle causing extensive damages. GM has failed to produce enough replacement parts and given the safety issues associated with a blown fuse and overheating, NHTSA should order a recall on this part and force GM to replace the parts for owners. This is a widespread problem documented among Volt owners, and the only stopgap issue is to drive the vehicle out of compliance with local emissions rules.Read more

On October 29, 2025, my Infiniti QX50 was repaired at Naples Infiniti after the engine under shield detached while driving, ripping off the passenger-side wheel well liner. The technician stated the failure was likely caused by missing bolts from a prior oil change approximately two weeks earlier. Based on that representation, I authorized the repair. That explanation later proved to be incorrect.
... On January 18, 2026, the entire under shield detached again while driving, once more tearing off the wheel well trim and additional underbody plastic components. This second incident also caused a visible gouge in a new tire. This was the second identical failure, creating a significant and ongoing safety hazard. Subsequent research shows a well-documented pattern affecting 2019–2021 Infiniti QX50 vehicles, where underbody components detach while driving. This strongly suggests a design or materials defect, not isolated maintenance error. On January 19, 2026, while driving to Naples Infiniti to drop the vehicle off following the second failure, additional plastic components detached from the vehicle, nearly causing an accident. Photographs are available documenting the damage and debris. I immediately reported this to Infiniti, and they deny any wrongdoing or vehicle design defects.Read more

On 1/27/26 the vehicle began shuddering and lost power while I was driving on the highway. As I attempted to get to a gas station off of the next exit, it intermittently lost power and began smoking under the hood. It seemed to be overheating but there is no temperature gauge on the display. I had it towed to my service center again. At that time, four spark plugs and an ignition coil were replaced. After these repairs, I have continued to have significant power issues, the engine warning light has come back on, and the "Service Brake Assist" warning light has come on. Additionally, the instrument panel frequently goes blank or displays what can best be described as "static" and the A/C and heat do not work, making it difficult to defrost and defog the windshield. The wiper fluid will not come out even though the reservoir is full. Due to prohibitive diagnostic fees at the dealership, which I was told would not be waived or reimbursed, I have continued to utilize my independent service center to maintain basic drivability. My service center has advised me that the engine may fail completely within a short time frame. I have reported these issues to the manufacturer and have been assigned a case number. I'm concerned that this may indicate a potential safety defect and/or premature engine failure affecting my vehicle and others of the same model. On Sunday, 02/08/2026, I parked briefly to drop something off. When I tried to restart the car, it was clicking but would not start. An engine light came on, and a flashing message appeared saying “Park.” I initially thought it was a battery issue, but the warning light remained on. The next morning, the car started, but the engine was locked and the vehicle would not move forward or backward.

I restarted the vehicle, and the message on the screen cleared and the vehicle was engaged. I then dropped the vehicle off at the dealership. After inspection, they told me the front passenger seat weight sensor needed to be replaced, which is very expensive ($1,300) When I researched the, Honda has already recalled their vehicles for this same issue, and I am not sure I need to pay all this money for something that is caused by an identified defect of the vehicle system. The driver's iPhone detected the crash and automatically contacted the emergency contact and the state police. The driver sustained injuries to the knee, head, neck, and back and was transported to the hospital for medical assistance. There was no airbag deployment, fire, or police report filed; however, a police officer and the state police were present at the scene and moved the vehicle before photos could be taken. The vehicle was towed to an impound lot and later to an auto body shop. The local dealer was contacted and confirmed there were no recalls applicable to the vehicle. According to the inspection findings, excessive carbon buildup caused the piston oil control rings to become stuck, allowing oil to bypass and resulting in damage to the cylinder walls. The inspector determined that the required repair is full engine replacement. This failure was not caused by misuse, neglect, or lack of maintenance. The vehicle has been inoperable and in the repair shop since approximately August 2025. At the time of sale and shortly thereafter, water was present inside the trunk area, and the vehicle continued to exhibit water intrusion. The water intrusion appears to be associated with defective weatherstripping or sealing, which allowed moisture to enter the vehicle. On March 19, 2024, the dealership returned the vehicle to me with visible mold present inside the vehicle. The mold condition was observable at the time it was returned to consumer use. The vehicle was not professionally remediated for mold contaminated before being released back to consumer possession. A professional mold inspection later identified over 100,000 mold spores and at least ten (10) different types of mold inside the vehicle. Although the source of water intrusion was reportedly repaired on October 25, 2024, the vehicle remains inoperative and unsafe due to mold contamination. The mold condition persists despite repair attempts. I remain in possession of the vehicle, which cannot be safely driven or occupied. The manufacturer was notified of the water intrusion and mold contamination; however, the safety issue remains unresolved. At the time of purchase, the mileage was 125,647; however, the contact found a repair bill from January 10th, 2018, that indicated that the mileage as 233,045. Upon researching the issue Gm has recognized and released a special coverage to fix the issue. N252508341. However my vehicles VIN does not pull up the coverage. All the characteristics of the coverage seem to fit the model and issues I have. It’s the same year, model, engine, etc. I’ve attached a DOT report of the special coverage. I’m hoping this will help fix my vehicle and others with this same issue. Although it’s not specifically listed as a safety recall, I feel the cause of the sudden power reduction is known by Gm and warranty issued and the problem should be resolved for all vehicles with this issue.
